[Ultrasound diagnosis of periventricular leukomalacia].

Periventricular leucomalacia (PVL) is the most common form of hypoxemic brain damage in infants. In PVL, there is usually a preceding history of asphyxia or hypoxia. This results in hypoxic-ischaemic damage to the region of the brain around the lateral ventricles, a particularly vulnerable area as it is in a border zone between arterial supplies. PVL is usually first recognised as an echodense heterogenic flare in the anterior and posterior periventricular areas on brain ultrasound scan. This must clearly be differentiated from the typical halo seen in newborns. The formation of pseudocystic p

Periventricular leukomalacia associated with hypocarbia.

We here report a case of periventricular leukomalacia (PVL) associated with hypocarbia which remained even after extubation. The patient had no risk factors affecting PVL development other than hypocarbia. We consider that the irregular tachypnea, which remained after extubation might be attributable to overdriving of ventilation of central neurogenic origin. Our patient's clinical course suggests that sodium bicarbonate drip infusion is a very effective way to alter the set point of respiratory neuronal drive of the patient with central neurogenic hyperventilation.

Periventricular leukomalacia: risk factors revisited.

The dramatic improvement in neonatal care during the last decade did not succeed in reducing the incidence of periventricular leukomalacia (PVL), suggesting that prenatal events may be the main target for PVL prevention. The studied cohort included 753 very preterm infants born between 24 and 32 weeks of gestational age, admitted to the intensive care unit and surviving at least 7 days; 69 (9.2%) of these infants had a diagnosis of cystic PVL. The highest PVL frequency was observed among the infants born at 28 weeks of gestation (16%). Inflammatory prenatal events occurring during the last day

Intrauterine tachycardia and periventricular leukomalacia.

We describe a preterm infant of 32 weeks' gestation with hydrops fetalis due to intrauterine supraventricular tachycardia. On the second day of life, cranial ultrasound showed a mainly right-sided periventricular leukomalacia already with porencephalic cysts. These findings were confirmed by autopsy. An association of intrauterine tachycardia with periventricular leukomalacia must be assumed.

Radiologic diagnosis of periventricular leukomalacia.

Fifteen infants with clinical evidence of periventricular leukomalacia, i.e. spastic diplegia or quadriplegia and premature birth were studied. CT scans of the brain demonstrated: 1) reduction in quantity of periventricular white matter, 2) deep and prominent sulci which abutted the ventricles without interposed white matter and 3) ventriculomegaly with irregular outline of the lateral ventricles. The location and severity of abnormalities on CT scans correlated well with neurologic handicap and the known anatomic location of periventricular leukomalacia, thus demonstrating the diagnostic valu

[Leading etiologic factors in periventricular bleeding and periventricular leukomalacia].

Prenatal, intrapartum and postnatal factors were investigated in less than 32 gestational age preterm infants known to have periventricular haemorrhage or periventricular leukomalacia. The diagnoses were established on cranial ultrasonography and/or autopsy. Factors associated with periventricular haemorrhage were: postnatal severe hypercapnia, acidosis and hypoxia. In the periventricular leukomalacia group the incidence of antepartum and intrapartum hypoxia and/or ischaemia (antepartum haemorrhage) were significantly higher than in the control group.

Periventricular leukomalacia is decreasing in Japan.

Periventricular leukomalacia is recognized as the leading cause of cerebral palsy in preterm infants. To clarify the prevalence of periventricular leukomalacia and cerebral palsy in Japan, a nationwide survey was performed. The prevalence of periventricular leukomalacia in the group of surviving preterm infants of gestational ages less than 33 weeks born in 2007 was 2.7% (78/2883) on ultrasound diagnosis, and 3.3% (92/2824) on magnetic resonance imaging. The prevalence of cerebral palsy was 4.3% (125/2883) on clinical diagnosis. In our previous study, the prevalences of periventricular leukoma

[Diagnosis and prognosis of periventricular leukomalacia].

Periventricular leukomalacia (PVL) is an important pathogenetic factor in cerebral palsy in premature infants. We studied 708 premature infants with gestational ages < 36 weeks, who had been admitted to the NICU of Matsudo City Hospital, examined by cranial ultrasonographic method repeatedly, and followed up for more than 1 year with cranial ultrasonography, magnetic resonance imaging (MRI), and CT scanning. We made a diagnostic protocol for PVL. All cystic lesions and periventricular echo densities (PVE) 3, and 14.9% of PVE 2 on ultrasonography were clinically diagnosed in PVL. Cranial MRI we

Risk factors for periventricular leukomalacia.

OBJECTIVE: To identify risk factors implicated in the development of periventricular leukomalacia (PVL) and to evaluate the possible association between PVL with neonatal morbidity. DESIGN: Retrospective case control study. SETTING: Medical records of neonates admitted to a University Hospital between January 2000 and December 2005. POPULATION: Sixty-nine neonates with PVL born at gestational ages from 24 to 34 weeks. Forty-three of these had a cystic form of PVL (cPVL), whereas 26 had transient periventricular echodensities (PVE). METHODS: Each case was matched for gestational age and yea

Periventricular leukomalacia and West syndrome.

The authors studied the clinical course and electroencephalograms (EEGs) of 27 patients with periventricular leukomalacia (PVL), to investigate the relation between PVL and West syndrome. Seven of the 27 patients with PVL developed WS; in all seven the PVL was severe. Bilateral parieto-occipital dominant irregular polyspike-and-wave (PO-polySpW) bursts were seen in eight of the 21 patients whose EEGs were recorded within the age of one year (corrected for preterm birth). All eight had severe PVL, and in six, bilateral PO-polySpW bursts were followed by hypsarrhythmia and spasms. None of the 12

[Imaging diagnosis of periventricular leukomalacia].

The neurodevelopmental prognosis of periventricular leukomalacia (PVL) has been predicted effectively by ultrasonography (US). US is the best imaging modality to identify both the early changes of PVL (echogenic phase) and the late changes (cystic phase). In cases with intraventricular hemorrhage, however, it was difficult to differentiate from periventricular hemorrhage in the echogenic phase by US. The PVL resolved to a discrete cyst separate from the ventricle, but periventricular hemorrhage developed a porencephalic cyst. Computed tomography is useful for distinguishing periventricular hem

Morphologic variations in periventricular leukomalacia.

Periventricular leukomalacia (PVL) usually is manifested as discrete foci of coagulation necrosis of the deep periventricular white matter in the human neonatal brain. During the examination of the brains of 116 infants utilizing an oil red O technic on gelatin-embedded frozen sections, 25 cases of PVL were found with typical foci of coagultion necrosis. Three morphologic varieties of the lesion could be demonstrated. In the first type, rather than being restricted to the periventricular zone, the discrete necrotic foci extended throughout the entire zone of cerebral white matter, even out to

[Prognosis of periventricular echodensities--its relationship with periventricular leukomalacia].

We studied periventricular echodensities (PVE) in 154 premature infants under 33 weeks of gestational age. PVEs were classified into four groups due to Pidcock's description, and its relation with periventricular leukomalacia (PVL) was examined as a cause of cerebral palsy. Twelve (11%) of 112 infants with PVE 2 and all of 10 infants with PVE 3 were developed cystic PVL. PVE 3 was therefore considered to be the same as the pathological PVL. Infants of PVE 2 - 3 groups were more premature and lower in birth weight, and they were more often complicated with respiratory distress syndrome, asphyxi

Periventricular leukomalacia in a neonatal calf.

A 10-day-old, Japanese Black, female calf had shown astasia since just after birth. Focal symmetrical periventricular malacic lesions of the cerebrum and suppurative arthritis of the left hip joint were observed in macroscopic examination. Histologically, the cerebral lesions were confirmed as periventricular leukomalacia (PVL). The location and histological features of the lesions were similar to PVL in humans, caused by neonatal ischemia/hypovolemia. This is the first report of PVL in a neonatal calf.
