Hemodynamic effects of ropivacaine and levobupivacaine intravenous injection in swines.

PURPOSE: To compare the hemodynamic effects following a toxic dose of either agent after intravenous injection in swines, as might accidenta … 20 and 30 minutes after intoxication. RESULTS: Hemodynamic repressions of acute intoxication with levobupivacaine were more important and more prolonged than those of ropivacaína. CONCLUSION: In pigs, levobupivacaine was shown to be more toxic than ropivacaine when the same large doses are injected intravenously.

Opioid consumption after levobupivacaine scalp nerve block for craniosynostosis surgery.

BACKGROUND AND OBJECTIVES: Craniosynostosis surgery is considered a very painful procedure due to extended scalp and periosteal detachment, and is associated with prolonged postoperative consumption of opioids and their side effects. In this observational descriptive case series study, we investigated perioperative opioid consumption in children undergoing craniosynostosis repair under general anesthesia when scalp nerve block with levobupivacaine was involved. METHODS: After standard anesthesia induction, scalp nerve block with levobupivacaine 2 mg/kg plus epinephrine 1:800,000 was performed…

Intravenous lipid emulsion for levobupivacaine intoxication in acidotic and hypoxaemic pigs.

Intravenous lipid emulsion is, in some countries, the recommended treatment for local anaesthetic toxicity. Systemic local anaesthetic toxicity results in hypoxaemia and acidosis, and whether this influences the effects of lipid therapy on drug concentrations and cardiovascular recovery is currently unknown. Twenty anaesthetised pigs were given a 3-mg/kg bolus of levobupivacaine followed by a five minute phase of hypoventilation and 1 mmol/kg of lactic acid in one minute. After lactic acid infusion, pigs were treated, in randomised order, with either 20% lipid emulsion or Ringer's acetate for …

An isobolographic analysis of diamorphine and levobupivacaine for epidural analgesia in early labour.

BACKGROUND: Few data describe the pharmacological interactions between local anaesthetics and opioids. The aim of this study was to measure the median effective concentration (MEC) of diamorphine and levobupivacaine when given separately and as mixtures for epidural analgesia, and determine whether the combination is additive or synergistic. METHODS: One hundred and twenty patients were enrolled in this prospective randomized, two-phase, double-blind study. In the first phase, 60 women were randomized to receive a fixed 20 ml volume of either levobupivacaine or diamorphine epidurally . Dosing…

Effects of levobupivacaine and ropivacaine on rat sciatic nerve blood flow.

BACKGROUND: Ischaemia is one of the causative mechanisms of peripheral nerve injury, a documented complication of regional anaesthesia. Local anaesthetics per se and/or vasopressor adjuvants may account for changes in peripheral nerve blood flow. The aim of this study was to test the effects of levobupivacaine and ropivacaine in a rat sciatic nerve model with respect to local blood flow and histopathological changes. METHODS: Forty-eight female Sprague-Dawley rats were anaesthetized for left sciatic nerve exposure. After baseline nerve blood flow measurement with a laser Doppler flowmeter, 0.…

Comparison of levobupivacaine versus ketamine and levobupivacaine for ultrasound-guided adductor canal block in patients undergoing unilateral total knee replacement: A randomized, double-blinded study.

BACKGROUND AND AIMS: Ultrasound-guided adductor canal block (ACB) has been increasingly used for postoperative analgesia in total knee replacement (TKR) surgery patients. We hypothesized that the addition of ketamine to levobupivacaine in ultrasonograsonography (USG)-guided ACB would result in prolonged and better postoperative analgesia in patients undergoing TKR. MATERIAL AND METHODS: This randomized prospective study was conducted on 90 patients undergoing unilateral TKR. In Group LK (n = 45), levobupivacaine 0.25% plus ketamine 1 mg/kg (total volume 20 ml) was given, and in Group L (n = 4…

Comparative Analysis of Duration of Postoperative Analgesia between Levobupivacaine and Levobupivacaine with Clonidine after Ultrasound-Guided Transversus Abdominis Plane Block in Patients Undergoing Lower Segment Cesarean Section.

BACKGROUND: Transversus abdominis plane (TAP) block in cesarean section is carried out by local anesthetics such as bupivacaine or ropivacaine with a limited duration of analgesia. The addition of adjuvants such as clonidine and dexmedetomidine has increased the duration of postoperative analgesia. AIMS SETTINGS AND DESIGN: The aim of this study was to compare the duration of postoperative analgesia between clonidine and levobupivacaine (0.25%) versus levobupivacaine (0.25%) alone in the bilateral TAP block after lower segment cesarean section (LSCS). MATERIALS AND METHODS: In this prospecti…

Relative potencies of bupivacaine, levobupivacaine, and ropivacaine for neonatal spinal anaesthesia.

BACKGROUND: Comparing the relative potency of new local anaesthetics such as levobupivacaine and ropivacaine with bupivacaine by the minimum local analgesic concentration model has not been described for neonatal spinal anaesthesia. This information is important to compare agents and to determine the most effective spinal dose. METHODS: We performed a two-stage study to determine the ED50, the ED95, and the relative analgesic potency of isobaric spinal bupivacaine, levobupivacaine, and ropivacaine in infants. In phase 1, 81 infants were randomized in a Dixon-Massey study to describe the minim…

Comparison of Epidural Bupivacaine, Levobupivacaine and Dexmedetomidine in Patients Undergoing Vascular Surgery.

INTRODUCTION: Levobupivacaine is the s-isomer of racemic Bupivacaine. It is less cardio, neurotoxic and equally potent local anaesthetic compared to its racemate. It is known to cause less Depression of myocardial contractility. Dexmeditomidine when used via epidural route has synergistic effect with local anaesthetics. Majority of patients presenting for vascular surgery are elderly and have associated co-morbidities like diabetes, hypertension, and coronary artery disease. We intend to study safety and efficacy of epidural Levoupivacaine and Dexmedetomidine in this group of patients. MATERI…
