On Step 1 Mania, USMLE Score Reporting, and Financial Conflict of Interest at the National Board of Medical Examiners.

Though intended to inform a binary decision on initial medical licensure, the United States Medical Licensing Examination (USMLE) is frequently used for screening candidates for residency positions. Some have argued that reporting results as pass/fail would honor the test's purpose while preventing inappropriate use. To date, the USMLE's sponsor organizations have declined to make such a change. In this Perspective, the authors examine the history and mission of the National Board of Medical Examiners (NBME), trace the rise of "Step 1 mania," and consider the current financial incentives for t

Revisiting Retake Policy: Analyzing the Success Rates of Examinees With Multiple Attempts on the United States Medical Licensing Examination.

PURPOSE: A testing program's decision making on retake policy considers a number of factors, including fairness to examinees, examination security, examination purpose, and classification accuracy. For high-stakes licensure and certification examinations charged with protection of the public, this includes balancing fairness issues inherent in the potential for false negatives with the public protection need to minimize false positives. Since 2012, the United States Medical Licensing Examination (USMLE) has maintained a policy of ≤ 6 attempts on any examination component. The purpose of this s

Thirty Years of Perfusion Licensure in the United States: Exploring Current Concepts and Developing New Strategies for the Future.

Over the past 30 years, licensure and certification requirements for cardiovascular perfusionists have varied considerably in the United States. In addition to intraoperative perfusion for cardiac surgery, the role of cardiovascular perfusion has recently expanded to include extracorporeal membrane oxygenation and the management of organ recovery systems used for transplantation. Efforts by organizations such as the American Society of ExtraCorporeal Technology and the American Board of Cardiovascular Perfusion have led to the recognition of perfusion as a distinct allied health profession in

Use of a formative mock examination in predicting Australian general practice licensure examination performance.

The Royal Australian College of General Practitioners (RACGP) administers high-stakes summative licensure examinations for unsupervised independent Australian general practice. Examination failure can have adverse consequences including psychological stress and financial implications. In-training assessments as predictors of summative or licensure exam outcome are frequently administered as early-training stage formative assessments. Assessments more proximate to the time of licensure examinations may also be of utility. The Mock-AKT, a formative assessment, was available to registrars of an A

Medical gross anatomy as a predictor of performance on the USMLE Step 1.

Traditional predictors of medical school performance, such as Medical College Admission Test (MCAT) scores and grade point averages, are often used during the admissions process to help identify the prospective students who are most likely to complete the basic science portion of the curriculum successfully. Here we analyzed the admissions files and student records of 285 first-year medical students who matriculated at the University of California at Davis School of Medicine between 1999 and 2001 to determine if performance in medical gross anatomy is a similar, if not better, predictor of per

Obesity Coverage on Medical Licensing Examinations in the United States. What Is Being Tested?

UNLABELLED: Phenomenon. As one of the most common chronic disease affecting adults and children, obesity is a major contributor to noncommunicable diseases, both nationally and globally. Obesity adversely affects every organ system, and as such it is imperative that the United States Medical Licensing Examination (USMLE) adequately assesses students' knowledge about the science and practice of obesity management. The purpose of this study was to evaluate the coverage and distribution of obesity-related items on the three USMLE Step examinations. APPROACH: Examination items that included obesi

Artificial intelligence in medical ethics education: a descriptive study of eight models in multiple choice question generation.

INTRODUCTION: Integrating artificial intelligence (AI) into medical education poses barriers and opportunities for medical educators. One of those opportunities is content creation for medical ethics assessment. Assessing the performance of AI in generating multiple choice questions (MCQs) in ethics that aligns with United States Medical Licensing Examination (USMLE) is important. The present study evaluates the performance of eight AI models-GPT-4 Turbo, GPT-3.5 Turbo 0125, o1 Mini, o1 Preview, GPT-4, Claude 3.5 Sonnet, Claude 3 Opus, and Gemini-in regard to the relevance, clarity, and accura

Primary care performance of alternatively licenced physicians in Ontario, Canada: a cross-sectional study using administrative data.

OBJECTIVES: Medical Regulatory Authorities (MRAs) provide licences to physicians and monitor those physicians once in practice to support their continued competence. In response to physician shortages, many Canadian MRAs developed alternative licensure routes to allow physicians who do not meet traditional licensure criteria to obtain licences to practice. Many physicians have gained licensure through alternative routes, but the performance of these physicians in practice has not been previously examined. This study compared the performance of traditionally and alternatively licenced physician

Validity of admissions measures in predicting performance outcomes: a comparison of those who were and were not accepted at McMaster.

BACKGROUND: In typical validity studies, regression analyses are used to examine the relation between admissions measures and subsequent performance. This approach is problematic as it generally yields low correlation coefficients, which are difficult to interpret. Further, it leaves unanswered the question of how those applicants rejected by the process would fare had they been admitted. PURPOSE: This study examines the validity of the admissions measures used to assess non-cognitive qualities at McMaster's Medical School in a unique manner. METHODS: Three cohorts: (a) those offered an admi

Trends in Medical Student Publications in the Pass/Fail Licensing Examination Era.

OBJECTIVES: The transition of the US Medical Licensing Examination Step 1 to pass/fail scoring has intensified concerns about a research "arms race" among medical students seeking advantage in the residency application process. Over three graduating cohorts of medical students (classes of 2022-2024), the study aimed to determine whether medical students have increased participation in published scholarship and whether medical student publications exhibited diminishing quality, as measured by citations, authorship position, and publication type. METHODS: Data from a registry of recently enumer

The USMLE Step 1 Pass/Fail Reporting Proposal: The APDR Position.

BACKGROUND: The National Board of Medical Examiners (NBME) and the United States Medical Licensing Examination (USMLE) has convened a conference of "key stakeholders" on March 11-12, 2019 to consider reporting the results of the USMLE Step 1 as pass/fail. DISCUSSION: While the original purpose of the USMLE Step 1 was to provide an objective basis for medical licensing, the score is increasingly used in residency applicant screening and selection because it is an objective, nationally recognized metric allowing comparison across medical schools in and outside the United States. Excessive relia

Growth in basic science knowledge in first-year medical school and USMLE Step 1 results: a longitudinal investigation at one school.

PURPOSE: Our US medical school uses National Board of Medical Examiners (NBME) tests as progress tests during the pre-clerkship curriculum to assess students. In this study, we examined students' growth patterns using progress tests in the first year of medical school to identify students at risk for failing United States Medical Licensing Examination (USMLE) Step 1. METHOD: Growth Mixture Modeling (GMM) was used to examine the growth trajectories based on NBME progress test scores in the first year of medical school. Achieving a passing score on the USMLE Step 1 at the end of the second year

The Relationship Between Preclinical Grading and USMLE Scores in US Allopathic Medical Schools.

BACKGROUND AND OBJECTIVES: Pass/fail preclinical grading is associated with improved student well-being. Studies do not demonstrate differences in United States Medical Licensing Examination (USMLE) scores after adopting pass/fail grading, but were conducted at unrepresentative, highly selective institutions, and average scores increased overall during the study periods. We investigated the effect of preclinical grading on USMLE scores across 96 US medical schools. METHODS: Preclinical grading systems were identified with the 2017 Association of American Medical Colleges Medical School Admiss
