Multi-System Adaptation to Confinement During the 180-Day Controlled Ecological Life Support System (CELSS) Experiment.

Confinement experiments are essential to prepare long-term space exploration. The 180-day Chinese CELSS (Controlled Ecological Life Support System) study is unique in its design, including a closed-loop system and mid-mission simulation of Mars-like day-night cycle of 24 h 40 min for 36 days (days 72-108). Our aim was to study physiological and psychological consequences of this confinement in four healthy volunteers (one female). CELSS platform consisted of six interconnected modules including four greenhouses. Life support systems were controlled automatically. Body composition, fluid compar

A Machine Learning-Based Clinical Decision Support System to Improve End-of-Life Care.

End-of-life care (EoLC), such as advance care planning, advance directives, hospice, and palliative care consults, can improve patient quality of life and reduce costs, yet such interventions are underused. Machine learning-based prediction models show promise in identifying patients who may be candidates for EoLC based on increased risk of short-term (less than 1 year) mortality. Clinical decision support systems using these models can identify candidate patients at a time during their care when care teams can increase the provision of EoLC.Evaluate changes in the provision of EoLC with imple

Quality of life of people living with chronic hepatitis B: The role of social support system.

People living with chronic hepatitis B (PLCHB) are recommended to follow a lifelong monitoring regimen and face increased risk of liver cancer. Additionally, PLCHB frequently encounter stigma and discrimination, and relationship disruptions because of their chronic hepatitis B (CHB). Social support plays a key role in coping with chronic illnesses; however, this is inadequately assessed for PLCHB. This study aims to assess the physical, social, and mental impacts of living with CHB, the strategies PLCHB utilize to cope with their disease, and how social support-or lack of-impacts their journey

Mistriaged Advanced Life Support Patients in a Two-Tiered, Suburban Emergency Medical Services System.

INTRODUCTION: Emergency medical services (EMS) systems exist to provide prehospital care in diverse environments throughout the world. Advanced Life Support (ALS) services can provide advanced care including 12-lead electrocardiogram (ECG), endotracheal intubation and parenteral medication administration. Basic Life Support (BLS) can provide basic care such as splinting, wound care and cardiopulmonary resuscitation. ALS can release patients to BLS for transport to the hospital, and this is an area of high risk. Our study examines patients who were triaged and admitted to a critical care locati

Successful management of fulminant pulmonary embolism using a novel portable extracorporeal life support system.

A 46-year-old man presented to the emergency room with pain in his left leg, dyspnea, and general cyanosis. During examination he collapsed and required resuscitation. Under suspicion of pulmonary embolism, a new portable "click 'n run" extracorporeal life support system (LIFEBRIDGE-B(2)T [Medizintechnik AG, Ampfing, Germany]) was implanted by the femoral vessels under resuscitation within 15 minutes of presentation. The patient was stabilized, despite severe decompensation (pH, 6.8), and could be transferred for a computed tomographic scan, which confirmed massive pulmonary embolism. Still co

Computational analysis of the three-dimensional hemodynamics of the blood sac in the twin-pulse life-support system.

Blood flow in the twin-pulse life-support system (T-PLS) pulsatile blood pump was simulated using a three-dimensional rigid body-fluid-solid interaction model. This model can delineate the blood flow in the T-PLS resulting from operation of a moving actuator. The numerical method used in this study was a commercial finite element package called ADINA. We used a contact and fluid-solid interaction model to compute the blood hemodynamics in the sac. Blood flow is generated by the motion of the actuator, which strongly interacts with the solid material surrounding the blood. To obtain basic bioen

First Use of a Novel Extracorporeal Life Support System: Successful Application in Tracheoesophageal Fistula Repair.

Extracorporeal life support, commonly referred to as extracorporeal membrane oxygenation (ECMO), is indicated when conventional medical and surgical measures fail to support a patient during cardiac or respiratory failure. Increased use of ECMO in recent years has led to innovation that has improved safety in appropriate candidates. This has resulted in the application of novel approaches to complex surgical problems. Herein, we describe a simple, novel, and new-to-market ECMO circuit used for successful perioperative veno-venous ECMO support of a patient undergoing complex repair of a tracheo

Supporting Parental Decision-Making After Life-Limiting Fetal Diagnoses: The Role of Perinatal Hospice and the NOVA-L Decision Support System.

Background: Prenatal diagnosis of life-limiting fetal conditions often leads to counseling focused primarily on therapeutic abortion. Perinatal hospice has emerged as an alternative model of care for families who choose to continue the pregnancy. This paper has two primary aims. First, it discusses structured perinatal hospice programs and their role in supporting parental decision-making after such diagnoses, with attention to ethical and emotional complexities. Second, the paper introduces NOVA-L (Navigating Options & Vital Assistance for Life-limiting conditions), a conceptual Decision Supp

Social support systems as determinants of self-management and quality of life of people with diabetes across Europe: study protocol for an observational study.

BACKGROUND: Long-term conditions pose major challenges for healthcare systems. Optimizing self-management of people with long-term conditions is an important strategy to improve quality of life, health outcomes, patient experiences in healthcare, and the sustainability of healthcare systems. Much research on self-management focuses on individual competencies, while the social systems of support that facilitate self-management are underexplored. The presented study aims to explore the role of social systems of support for self-management and quality of life, focusing on the social networks of p

Transferring an extremely premature infant to an extra-uterine life support system: a prospective view on the obstetric procedure.

To improve care for extremely premature infants, the development of an extrauterine environment for newborn development is being researched, known as Artificial Placenta and Artificial Womb (APAW) technology. APAW facilitates extended development in a liquid-filled incubator with oxygen and nutrient supply through an oxygenator connected to the umbilical vessels. This setup is intended to provide the optimal environment for further development, allowing further lung maturation by delaying gas exposure to oxygen. This innovative treatment necessitates interventions in obstetric procedures to tr

Clinical decision support system for quality of life among the elderly: an approach using artificial neural network.

BACKGROUND: Due to advancements in medicine and the elderly population's growth with various disabilities, attention to QoL among this age group is crucial. Early prediction of the QoL among the elderly by multiple care providers leads to decreased physical and mental disorders and increased social and environmental participation among them by considering all factors affecting it. So far, it is not designed the prediction system for QoL in this regard. Therefore, this study aimed to develop the CDSS based on ANN as an ML technique by considering the physical, psychiatric, and social factors.

Factors affecting survival after prehospital asystolic cardiac arrest in a Basic Life Support-Defibrillation system.

OBJECTIVES: Previous studies have shown a low but meaningful survival rate in cases of prehospital cardiac arrest with an initial rhythm of asystole. There may be, however, an identifiable subgroup in which resuscitation efforts are futile. This study identified potential field criteria for predicting 100% nonsurvival when the presenting rhythm is asystole in a Basic Life Support-Defibrillation (BLS-D) system. METHODS: This prospective cohort study, a component of Phases I and II of the Ontario Prehospital Advanced Life Support (OPALS) Study, was conducted in 21 Ontario communities with BLS-D
