Transient increase of P-glycoprotein expression in endothelium and parenchyma of limbic brain regions in the kainate model of temporal lobe epilepsy.

Several recent studies have shown that the multidrug transporter P-glycoprotein (PGP) is over-expressed in endothelial cells from brain blood vessels of patients with refractory temporal lobe epilepsy (TLE), suggesting that altered drug permeability across the blood-brain barrier (BBB) may be involved in pharmacoresistance to antiepileptic drugs (AEDs). Furthermore, over-expression of PGP has been found in astrocytes of epileptogenic tissue. However, it is not known in which regions of the temporal lobe PGP over-expression occurs and whether the over-expression is a result of uncontrolled seiz

Ophelia-like Paraneoplastic Limbic Encephalitis with Haemorrhagic Temporal Lobe Involvement and Cauda Equina Dysfunction in Classical Hodgkin Lymphoma: A Case Report.

Ophelia syndrome is a rare paraneoplastic limbic encephalitis associated with classical Hodgkin lymphoma (cHL), most often with antibodies against metabotropic glutamate receptor 5 (mGluR5). We describe a 19-year-old man with newly diagnosed cHL who presented with generalised seizures, cognitive dysfunction, spastic paraparesis, cauda equina-related autonomic dysfunction, and a 35 × 31 mm haemorrhagic inflammatory lesion in the right temporal lobe. Brain biopsy showed dense intravascular and perivascular inflammatory infiltrates without neoplastic cells. Cerebrospinal fluid demonstrated pleocy

Preoperative limbic cortical thinning in mesial temporal lobe epilepsy surgical candidates is associated with pre-existing and de novo postoperative depression.

OBJECTIVE: Depression is common in mesial temporal lobe epilepsy (mTLE), but its neuroanatomical basis-and overlap with major depressive disorder (MDD)-remains poorly defined. We investigated whether the cortical and subcortical alterations reported in MDD are also present in mTLE and leveraged the frequent occurrence of depression after epilepsy surgery to test whether such changes predate the onset of post-surgical depression. METHODS: Eighty-nine mTLE patients scheduled for anterior temporal lobectomy were classified as never depressed (NoD, n = 42), depressed before surgical evaluation (P

Prevalence of Temporal Lobe Epilepsy (TLE) Subtypes and Response to Resective Surgery in Patients with Presumed TLE Undergoing Limbic and Paralimbic Network Exploration with Stereo-Electrodes.

Background/Objectives: Temporal lobe epilepsy (TLE) responds well to surgical treatment, although a considerable percentage of patients experience seizure recurrence after resection. Relapse from the contralateral mesial temporal lobe, extratemporal lobe epilepsy mimicking TLE, or temporal plus epilepsy might account for surgical failures. Methods: We included patients with a pre-implantation hypothesis suggesting TLE, who underwent stereo-EEG (SEEG) evaluation at our institution and had an individual SEEG exploration paradigm with at least twelve stereo-electrodes placed to sixteen brain regi

Limbic system abnormalities associated with mesial temporal sclerosis: a model of chronic cerebral changes due to seizures.

In a landmark 1966 study, widespread cerebral abnormalities were discovered at histopathologic examination of autopsied brains from patients with long-standing temporal lobe epilepsy. In the brains diagnosed with mesial temporal sclerosis (MTS), these abnormalities involved not only the hippocampus but also other structures of the limbic system and medial temporal lobe. Many of these changes are evident at magnetic resonance imaging. Because many of these abnormalities are found within limbic structures along a known neuroanatomic pathway (the Papez circuit), they suggest a pattern of cerebral

Anterior cingulate cortex involvement in non-paraneoplastic limbic encephalitis.

BACKGROUND: Non-paraneoplastic limbic encephalitis is characterized by attention deficit, loss of emotion control, and impaired memory. Viral infection can cause acute encephalitis in children, occasionally exhibiting clinical features of limbic dysfunction. However, how viral infection affects the limbic system remains to be elucidated. CASE DESCRIPTION: A 5-year-old Japanese boy was admitted to our hospital because of high fever and status epilepticus. After seizures were controlled by diazepam, he exhibited attention deficit, loss of emotion control, and impaired memory, suggesting acute l

Limbic encephalitis occurring in association with Alzheimer's disease.

Paraneoplastic limbic encephalitis is a rare cause of subacute dementia. A patient with limbic encephalitis and small-cell lung carcinoma is reported in whom the onset of subacute cognitive impairment was obscured by concurrent Alzheimer's disease. MRI revealed increased T2 signal in medial temporal lobes which corresponded to inflammatory pathology demonstrated at necropsy. High titres of antineuronal antibody (type II, anti-Hu) were present at death. Direct immunofluorescent staining of necropsy tissue revealed IgG bound to most remaining neurons in the temporal lobe. Antineuronal antibody s

Incidentally Detected Subclinical Limbic Encephalitis on 18F-Fluorodeoxyglucose Positron Emission Tomography/Computed Tomography in a Patient with Myasthenia Gravis and Sarcoidosis: A Rare Immunological Triad?

Limbic encephalitis is the inflammation of the medial temporal lobe structures due to direct infectious or indirect autoimmune etiology. Sarcoidosis is a rare cause of limbic encephalitis, has been reported by some authors. Association has also been reported between sarcoidosis, myasthenia gravis, and limbic encephalitis. We report an interesting case of 18F-fluorodeoxyglucose positron emission tomography/computed tomography demonstrating limbic encephalitis and sarcoidosis in a known case of myasthenia gravis.

Herpes zoster chronification to postherpetic neuralgia induces brain activity and grey matter volume change.

OBJECTIVE: Herpes zoster (HZ) can develop into postherpetic neuralgia (PHN), which is a chronic neuropathic pain (NP). Whether the chronification from HZ to PHN induced brain functional or structural change is unknown and no study compared the changes of the same brains of patients who transited from HZ to PHN. We minimized individual differences and observed whether the chronification of HZ to PHN induces functional and pain duration dependent grey matter volume (GMV) change in HZ-PHN patients. METHODS: To minimize individual differences induced error, we enrolled 12 patients with a transiti

Limbic encephalitis with involvement of prefrontal cortices and persistent amnesia.

OBJECTIVES: Most abnormalities on magnetic resonance imaging (MRI) in limbic encephalitis involve the medial temporal lobes and are associated with memory impairment. Recent neuroimaging studies have shown that not only the medial temporal lobes, but also the prefrontal lobes, contribute to cognitive functions. METHODS: We describe a case of limbic encephalitis with abnormalities in the medial temporal lobes on MRI and decreased accumulation of radionuclide in the prefrontal regions on SPECT. RESULTS: The patient had persistent retrograde amnesia, particularly affecting remotely acquired mem

Hurst Exponent Analysis of Resting-State fMRI Signal Complexity across the Adult Lifespan.

Exploring functional information among various brain regions across time enables understanding of healthy aging process and holds great promise for age-related brain disease diagnosis. This paper proposed a method to explore fractal complexity of the resting-state functional magnetic resonance imaging (rs-fMRI) signal in the human brain across the adult lifespan using Hurst exponent (HE). We took advantage of the examined rs-fMRI data from 116 adults 19 to 85 years of age (44.3 ± 19.4 years, 49 females) from NKI/Rockland sample. Region-wise and voxel-wise analyses were performed to investigate

EEG leading to the diagnosis of limbic encephalitis.

Limbic encephalitis is characterized by subacute onset of short-term memory loss, seizures, sleep disturbances, as well as psychiatric and behavioral symptoms. A subgroup is associated with voltage-gated potassium channel antibodies (VGKC-Abs). In many cases, brain magnetic resonance imaging (MRI) demonstrates hyperintense areas in the medial part of the temporal lobe. Also, pleiocytosis is frequently found. In this study, we describe a 69-year-old man with VGKC-Abs limbic encephalitis with generalized tonic-clonic seizures, increasing memory deficits, visual hallucinations, depression, and se
