Volume controlled apparatus for neonatal tidal liquid ventilation.

Conventional gas ventilation is often unsuccessful for premature neonatal patients suffering from respiratory distress syndrome (RDS). For such patients, liquid ventilation (LV) with perfluorocarbon (PFC) liquids has been proposed. By eliminating the air-liquid interface in saccules (the premature gas exchange structures), where scarce or absent surfactant production exists, pulmonary instability is avoided, lung compliance is improved, and atelectatic saccules are recruited, ultimately lowering the saccular pressure. Tidal LV involves administrating a liquid tidal volume to the patient at eac

Liquid ventilation: from experimental use to clinical application.

Despite advances in perinatal care of preterm infants, complications of prematurity are still common. If new conventional ventilatory interventions fail, ECMO is the only alternative method, but is a complex, invasive and costly technique and difficult to apply to small infants. Perfluorocarbon liquid ventilation is a promising technique. It has been demonstrated to be effective in experimental models, in mature and immature animals. However, its role in the management of human neonates, infants, older children and adults with acute respiratory failure is still not established. Both liquid ven

Ventilator-Induced Lung Liquid and Alveolar Rupture.

Ventilation is an essential function of life, and one of the first to be replicated by artificial means. Annually, it is estimated that 15 to 20 million patients worldwide are intubated and receive invasive mechanical ventilation (MV). However, MV is a non-physiologic intervention and frequent complications are associated with its use, including extravascular lung liquid, impaired cardiac performance, and alveolar rupture. Research shows that injurious MV can cause or aggravate lung damage and initiate an intense inflammatory response, contributing to multiple organ dysfunction and poor outcom

Liquid ventilation: new frontiers in the treatment of ARDS.

At our institution, PLV continues to be used as an investigational therapy for patients with ARDS who meet the protocol criteria. Since this is the beginning of the first nonneonatal clinical trial of PLV, results of this therapy are not yet available. Adverse effects on hemodynamic parameters have not been documented in the current literature for patients treated with liquid ventilation. In addition, further clinical studies using PLV therapy will be necessary to determine whether it will significantly change the morbidity and mortality of ARDS in infants or adults. Other questions to be answ

Pneumomediastinum: elucidation of the anatomic pathway by liquid ventilation.

Partial liquid ventilation is a new technique to improve oxygenation in patients with severe acute respiratory distress syndrome. In a patient with status asthmaticus and tension pneumothorax treated with subsequent liquid ventilation, radiopaque perfluorocarbon was identified along brochiovascular structures, in the mediastinum, and in the retroperitoneum. Perfluorocarbon outlined on CT and chest radiography the anatomic pathway by which spontaneous pneumomediastinum develops following alveolar rupture, as described earlier by histopathologic study in animals. This represents the radiopaque e

A new paradigm for lung-conservative total liquid ventilation.

BACKGROUND: Total liquid ventilation (TLV) of the lungs could provide radically new benefits in critically ill patients requiring lung lavage or ultra-fast cooling after cardiac arrest. It consists in an initial filling of the lungs with perfluorocarbons and subsequent tidal ventilation using a dedicated liquid ventilator. Here, we propose a new paradigm for a lung-conservative TLV using pulmonary volumes of perfluorocarbons below functional residual capacity (FRC). METHODS AND FINDINGS: Using a dedicated technology, we showed that perfluorocarbon end-expiratory volumes could be maintained be

Imaging Assessment of Partial Liquid Ventilation in Bronchopulmonary Dysplasia.

Partial liquid ventilation is proposed as an alternative ventilation strategy to reduce surface tension, increase alveolar recruitment, and decrease inflammation. Studied in acute respiratory distress and other indications, liquid ventilation is being revisited for infants with bronchopulmonary dysplasia. Perfluorooctyl bromide used for liquid ventilation is radiopaque, allowing radiographic visualization of lung liquid ventilation patterns that may provide additional insight into pulmonary pathophysiology. Current protocols utilize reduced liquid dosing, resulting in unique imaging features.

Pulmonary pathology of patients treated with partial liquid ventilation.

Initial trials of partial liquid ventilation (PLV), which is gas ventilation of perfluorocarbon-filled lungs, are underway in patients with severe respiratory failure. We report the first study of the effects of the perfluorocarbon, perflubron, on the lung. Necropsies were conducted in nine patients (seven adults and two neonates; mean adult age, 31 +/- 5 yr) managed with PLV (average number of doses, 4 +/- 1). All of the patients required extracorporeal life support. The patients had pneumonia with the acute respiratory distress syndrome (six patients), trauma/capillary leak syndrome (one pat

High-frequency oscillatory ventilation with partial liquid ventilation in a model of acute respiratory failure.

OBJECTIVE: To determine whether there is an improvement in oxygenation when partial liquid ventilation and high-frequency oscillatory ventilation are combined in the treatment of acute lung injury, compared with high-frequency oscillatory ventilation alone. DESIGN: Controlled animal trial. SETTING: Research laboratory in a university setting. SUBJECTS: Ten 3-kg piglets. INTERVENTIONS: Anesthetized piglets underwent high-frequency oscillatory ventilation, with mean airway pressure of 20 cm H2O, before induction of acute lung injury with repeated saline lavage. When PaO2 values were < 100 to

Partial liquid ventilation in acute salt water-induced lung injury.

BACKGROUND AND OBJECTIVES: Salt-water aspiration results in pulmonary oedema and hypoxia. We tested the hypothesis that partial liquid ventilation has beneficial effects on gas exchange and rate of survival in acute and extended salt water-induced lung injury. METHODS: Anaesthetized, ventilated rats (tidal volume 6 mL kg(-1), PEEP 5 cmH2O) received a tracheal salt-water instillation (3%, 8 mL kg(-1) body weight) and were randomly assigned to three groups (n = 10 per group). While lungs of Group 1 were gas-ventilated, lungs of Group 2 received a single perfluorocarbon instillation (30 min afte

Liquid ventilation in an infant with persistent interstitial pulmonary emphysema.

We present the case of a full term infant affected by diffuse persistent interstitial pulmonary emphysema (PIPE), who was treated with partial liquid ventilation (PLV) after the failure of conventional management. PIPE is a lethal chronic lung disease of unclear pathogenesis. Clinical history, radiological and histological findings confirmed the diagnosis in our patient. PLV applied for 48 hours resulted in a significant improvement in the infant's respiratory function and was not associated with adverse effects. We concluded that PLV could be effective in prolonging the survival of infants wi

Acute respiratory distress syndrome: CT findings during partial liquid ventilation.

To describe the computed tomographic (CT) appearance of perflubron-filled lungs during partial liquid ventilation to treat acute respiratory distress syndrome, scans of the thorax were obtained in nine patients between June 1994 and December 1995. The distribution of perflubron was gravity dependent in four patients, with a mean interval of 6.25 days between scanning and perflubron administration; was patchy in four patients (mean interval, 16 days); and was homogeneous in one patient (interval, 3 days). Extraparenchymal perflubron was seen in intrathoracic lymph nodes (n = 4), supraclavicular

Partial liquid ventilation and nitric oxide in congenital diaphragmatic hernia.

PURPOSE: In congenital diaphragmatic hernia (CDH) there is immature lung development with a resulting clinical picture of pulmonary hypoplasia, surfactant deficiency, and pulmonary hypertension. Pulmonary hypoplasia and surfactant deficiency both have been successfully treated using partial liquid ventilation (PLV). Pulmonary hypertension associated with CDH has proven difficult to treat, but inhaled nitric oxide, which is a potent highly selective pulmonary vasodilator, may have potential. The aim of this study was to assess PLV in CDH and to document the effect of nitric oxide when administe

Partial liquid ventilation in newborn patients with congenital diaphragmatic hernia.

The authors evaluated the safety and efficacy of liquid ventilation with perfluorocarbon in four newborns with congenital diaphragmatic hernia and severe respiratory failure, who were on extracorporeal life support (ECLS). After 2 to 5 days on the ECLS, perflubron was administered into the trachea until the dependent zone of the lung was filled. The first dose was 6 +/- 1 mL/kg (range, 5 to 8 mL/kg). Gas ventilation of the perflubron-filled lung was performed (partial liquid ventilation). The administration of perflubron was repeated daily for 5 to 6 days, with total cumulative doses of 36 +/-

Effect of liquid properties on swallowing and ventilation coordination in rats.

BACKGROUND: One aspect of rehabilitation in swallowing disorders is to change liquid properties. The objective of our study was to test how liquid properties could improve oropharyngeal dysphagia and swallowing and ventilation coordination in an animal model. METHODS: Forty-two healthy male rats were distributed in six groups, including a control group. Rats were deprived of water for 24 h and then each group was administered liquid with different properties: tap water, sugar water, sparkling water, salt water, cold water, and acidic water. Rats were studied without and with oropharyngeal dys

A comparison of surfactant delivery with conventional mechanical ventilation and partial liquid ventilation in meconium aspiration injury.

The objective of this study was to compare surfactant (SF) distribution and physiological effects after standard SF delivery during conventional mechanical ventilation (CMV) with that using partial liquid ventilation (PLV). A model of meconium aspiration syndrome (MAS) was developed using two groups of adult rats (n = 14). After meconium instillation of 2.5 ml kg(-1) (20% v/w), SF/CMV: (n = 7) CMV and SF/PLV: (n = 7) PLV, received 14C-labeled surfactant (4 ml kg(-1)) delivered intratracheally in four aliquots over 20 min in both groups. Pleural liquid and kinetic friction coefficient of mesothelium after mechanical ventilation.

Volume and protein concentration of pleural liquid in anesthetized rabbits after 1 or 3h of mechanical ventilation, with alveolar pressure equal to atmospheric at end expiration, were compared to those occurring after spontaneous breathing. Moreover, coefficient of kinetic friction between samples of visceral and parietal pleura, obtained after spontaneous or mechanical ventilation, sliding in vitro at physiological velocity under physiological load, was determined. Volume of pleural liquid after mechanical ventilation was similar to that previously found during spontaneous ventilation. This f
