The pulsed dye laser represents a new tool in the treatment of ureteral calculi. Laser energy channeled through a delicate optical fiber is used to disimpact and fragment calculi. Standard ureteroscopic instruments including graspers, baskets and dilatation balloons can be used with the laser system. Interchangeable optical fibers ranging from 200 to 400 micrometers deliver up to 120 mJ of energy to the stone's surface. For two weeks in 1989 an endourology clinic was held in upper Egypt. Eighty patients ranging in age from nine years to sixty-seven years old were evaluated and treated endoscop

PURPOSE: To report in literature the first case of fatal multi-organ embolization of ureteral stones fragments during laser lithotripsy.
CASE PRESENTATION: A tetraplegic 43-year-old woman was admitted to the hospital to undergo laser lithotripsy because of bilateral ureteral stones and right ureteral infected stent. During the removal of the right ureteral stent, the patient developed a sudden severe bradycardia followed by a reduction in the arterial oxygen saturation. In spite of a rapid and intensive medical intervention, the clinical picture did not improve; the woman was therefore transf

BACKGROUND: Holmium laser lithotripsy is the most common technique for the management of ureteral stone. Studies founded that holmium laser firing can produce heat which will cause thermal injury towards ureter. The aim of our current study is to explore factors affecting thermal effect of holmium laser during ureteroscopic lithotripsy.
METHODS: An in vitro experimental model is design to simulate the ureteroscopic lithotripsy procedure. Different laser power settings (10w (0.5JX20Hz, 1.0 JX10Hz), 20w (1.0 JX20Hz, 2.0 JX10Hz), 30w (1.5JX20Hz, 3.0 JX10Hz)) with various firing time (3 s, 5 s, 1

We report the preliminary results of laser lithotripsy of cystine stones accomplished in 2 patients with the use of a new method of photosensitization. Transurethral rigid ureterorenoscopy was performed with an 8.5F instrument to reach 0.8 and 1.8 cm. stones located in the right and left lumbar ureters, respectively. A 320 mu. fiber was then inserted and pushed into contact with the stone. Then, 400 preliminary impulses were released from a pulsed dye laser (504 nm) with energy output at 140 mJ. and at a rate of 5 Hz. without any visible modification of the stone structure. The saline solution

Three patients with symptomatic intra- and extrahepatic choledocholithiasis who were not good candidates for retrograde endoscopy, surgery, or extracorporeal shock wave lithotripsy (ESWL) were treated successfully with endoscopically guided tunable dye laser lithotripsy via a 12-F transhepatic sheath. There were no complications secondary to the use of the laser. On the basis of this initial experience, transhepatic laser lithotripsy is a technically feasible and safe alternative when choledocholithiasis cannot be managed with retrograde endoscopy, ESWL, or surgery. Its role in the management

PURPOSE OF REVIEW: To examine the evidence on contemporary lasers and provide a practical framework for platform selection, parameter optimization, and thermal safety based on the principles of smart lithotripsy.
RECENT FINDINGS: Holmium: yttrium-aluminum-garnet (Ho:YAG) remains versatile for fragmentation and dusting, whereas thulium fiber laser (TFL) provides efficient fine-particle dusting with limited retropulsion. Pulsed thulium: YAG (p-Tm:YAG) shows an intermediate profile between Ho:YAG and TFL, but clinical evidence on lithotripsy outcomes is limited. Higher power settings with Ho:YAG

Symptomatic prostatic calculi are rare occurrences with several management options, the most popular of which is currently transurethral laser lithotripsy. This is a generally well-tolerated procedure with minimal complications. To date, no reported episodes of steinstrasse at the urethral level following prostatic calculi lithotripsy have been documented to our knowledge. We report a unique case of acute urinary retention secondary to obstructive calculi fragments following a transurethral laser lithotripsy of large prostatic calculi, further complicated by stricture at the fossa navicularis.

BACKGROUND: Most retained gallstones can be extracted at the time of operative exploration or endoscopic retrograde cholangiopancreatography (ERCP). Infrequently, impaction or associated anatomic abnormalities may prevent their clearance. We assessed the efficacy of the holmium:YAG laser in managing retained biliary calculi that had proven refractory to the usual methods of extraction.
METHODS: Two patients with calculi impacted in the intrapancreatic common bile duct and one patient with residual stones in a nonfunctional gallbladder were treated with holmium:YAG laser lithotripsy. Two of th

An in vivo study was performed to evaluate the use of pulsed-dye laser beam for the fragmentation of salivary calculus. In accordance with the absorption and reflection spectra of sections of salivary stones, optimal fragmentation was achieved with a pulsed-dye laser with a pulse width of 1.4 microns and tuned to a wavelength of 504 nm. Further studies on particle size were conducted, and a new "sialoendoscope" technique was developed. Fifteen patients with sialolithiasis of the submandibular gland were treated. Under continuous endoscopic monitoring laser-induced shock wave lithotripsy was pe

We treated 225 patients with 227 ureteral calculi (5 steinstrasse) via the pulsed dye laser with 9.5F rigid, 7.2F semirigid and flexible, steerable ureteroscopes. The 222 calculi (excluding steinstrasse) were fragmented by laser alone in 141 cases (64%), laser and a basket in 30 (13%), and laser and extracorporeal shock wave lithotripsy in 33 (15%). The laser failed in 18 cases (8%). Of the 222 calculi 165 were impacted. Of 29 impacted upper ureteral calculi 15 were treated by laser disimpaction and fragmentation, 13 by laser disimpaction followed by extracorporeal shock wave lithotripsy and 1
