Living-unrelated donors yield higher graft survival rates than parental donors.

BACKGROUND: Living-unrelated donors (LURD) have been shown to yield kidney graft survival rates equivalent to that of related donors. Here, we show that in three diseases, LURDs supply grafts that survive at a higher rate than parental or offspring donors. METHODS: We analyzed 111,643 first adult kidney transplants from the United Network for Organ Sharing database performed between 1991 and 2003 by Kaplan-Meier curves, log-rank tests, and Cox proportional hazard tests. RESULTS: Five-year kidney graft survival rates in patients receiving grafts from LURD, parental, and offspring donors were

Living-donor kidney transplantation: a review of the current practices for the live donor.

The first successful living-donor kidney transplant was performed 50 yr ago. Since then, in a relatively brief period of medical history, living kidney transplantation has become the preferred treatment for those with ESRD. Organ replacement from either a live or a deceased donor is preferable to dialysis therapy because transplantation provides a better quality of life and improved survival. The advantages of live versus deceased donor transplantation now are readily apparent as it affords earlier transplantation and the best long-term survival. Live kidney donation has also been fostered by

Management of urolithiasis in living kidney donors.

BACKGROUND: Kidney donors with asymptomatic stones were  previously excluded from the kidney donation list because of a potential increased morbidity risk for both the recipient and the donor. Currently, recent studies tend to consider these risks as overestimated. AIM: The aim of this study was to analyze our experience in the management of urolithiasis in potential donors. METHODS: We conducted a retrospective analysis during the period (2008-2015). We included donors with urilithiasis or a family history of urolithiasis whom had urinary biochemical analysis of urolithiasis. We identified

Acute dialysis risk in living kidney donors.

BACKGROUND: Reduced kidney function confers a higher risk of acute kidney injury at the time of an inciting event, such as sepsis. Whether the same is true in those with reduced renal mass from living kidney donation is unknown. METHODS: We conducted a population-based matched cohort study of all living kidney donors in the province of Ontario, Canada who underwent donor nephrectomy from 1992 to 2009. We manually reviewed the medical records of these living kidney donors and linked this information to provincial health care databases. Non-donors were selected from the healthiest segment of th

Living kidney donation: the outcomes for donors.

During the past decade, the number of transplantation from living kidney donors has substantially increased worldwide. The rate of increase varies from one country to another. The risk of unilateral nephrectomy to the donor includes perioperative mortality and morbidity plus the long-term risk of living with a single kidney. The rate of perioperative mortality and morbidity is about 0.03% and 10%, respectively. More attention is required to prevent serious complications of laparoscopic donor nephrectomy. A grading system in recording perioperative complications is necessary for making it avail

[Surgical complications of nephrectomy in living donors].

Renal transplantation from a living donor is now considered the best treatment for chronic renal failure. We reviewed the operative complications in 38 living related donor nephrectomies performed at our institution over the past 14 years. The mean age of our donors was 30 years old with age range between 18 and 58 years old and female predominance (55.2%). These swabs were realized by a posterolateral lumbar lombotomy with resection of the 11 third. The left kidney was removed in 34 donors (90%), surgical complications were noted in 39.4% of the cases: one case of wound of inferior vena cava

Reasons for not accepting living kidney donors.

Organs were donated by a small number of relatives (54/146=37%), genetically related or familial, who volunteered to give a kidney to a relative or spouse at our institution in 1991-1994. The most common reason for not accepting them was an immunological incompatibility (30 cases), followed by a diagnosis of hypertension and/or renal disease (24 cases). Other medical contraindications-including heart/lung disease, obesity, latent diabetes, and hepatic disease-were found in 14 potential donors. One woman (22 years of age) was judged too young for donation. Five ESRD patients died before the inv

Should Jehovah's Witnesses Be Living Kidney Donors?

AbstractTransplant programs can be reluctant to accept Jehovah's Witnesses as living kidney donors, because Jehovah's Witnesses refuse blood transfusions based on their religious beliefs. This article explores the ethical issues surrounding Jehovah's Witnesses as living kidney donors. I argue that, in light of the low risk of kidney donation for healthy patients and recent advances in the safety of bloodless surgery, healthy Jehovah's Witnesses meet the standard used to evaluate other prospective organ donors. Therefore, as a matter of equality, patient autonomy, and respect for altruism, Jeho

Evaluation of older living donors and outcomes.

PURPOSE OF REVIEW: The widening gap between the demand for and supply of kidneys for transplantation has intensified the need to expand the living donor pool. At the same time, a growing proportion of individuals aged 65 years and older are being waitlisted for kidney transplantation. Living donor kidney transplantation remains the optimal therapy for end-stage kidney disease, and older living donor candidates represent a promising but underutilized resource. Although older donors typically present with greater medical complexity, their lifetime risk of developing end-stage kidney disease is c

Hypertension and obesity in living kidney donors.

Over the past few decades, the shortage in the kidney donor pool as compared to the increasing number of candidates on the kidney transplant waitlist led to loosening of kidney donors' acceptance criteria. Hypertension and obesity represent risk factors for chronic kidney disease, both in native kidneys and those in kidney transplant recipients. While great progress has been made in kidney transplantation from living donors to benefit the recipient survival and quality of life, progress has been slow to fully risk-characterize the donors. This review critically reassesses the current state of

GFR Assessment of Living Kidney Donors Candidates.

Living kidney donation provides the best outcomes (survival, cost, and quality of life) of all renal replacement modalities. Living kidney donors, on the other hand, are at the increased risk of end-stage kidney disease (ESKD) after donation compared with healthy nondonors for multiple possible reasons. Extensive predonation screening is required to assess eligibility for donation to avoid the rejection of suitable candidates and minimize acceptance of donors with increased risk of ESKD. The association between the lower predonation glomerular filtration rate (GFR) and increased ESKD risk in d

[Outcome of living kidney donors for transplantation].

Nowadays, several treatments exist to treat terminal chronic renal failure. Best results for the recipients are obtained with kidney transplantation concerning mortality and quality of life. Transplantation is also the cheaper option for society. Living kidney donation raises the issue of the becoming of the donor, an absolutely healthy subject who gets to a surgical procedure. The becoming of living kidney donors has been compared with the one of controls subjects in several studies. The evaluations focused on the complications of nephrectomy in the short and long-term: kidney failure, hypert

Abdominal Aortic Calcification in Living Kidney Donors.

OBJECTIVE: This study assesses the association between abdominal aortic calcification (AAC) and renal function of living kidney donors and evaluate AAC as a surrogate marker for nephrosclerosis. METHODS: Between January 2010 and March 2013, 287 donors who underwent living donor nephrectomy were enrolled. We analyzed computed tomography angiographies and quantified AAC scores by calculating the Agatston score for the abdominal aorta. The donors were stratified into the non-AAC group (AAC score = 0; n = 238) and the AAC group (AAC score >0; n = 49). The relationship between AAC and perioperativ

Postoperative Psychiatric Complications in Living Liver Donors.

BACKGROUND: To understand the impact of psychologic variables on donor quality of life, we studied long-term data on postoperative psychiatric complications in living liver donors. This study is a focused psychological investigation of diagnoses, treatments, and long-term clinical courses of living liver donors with psychiatric complications. METHODS: Of the 142 donors who underwent live-donor liver transplantation at Nagoya University Hospital between April 2004 and July 2014, we investigated those without a history of mental illness who had developed such illness after transplantation and r

Living kidney donors: current state of affairs.

Living kidney donation continues as the cornerstone of transplantation. In order to determine with ever-renewing assurance that living donation is safe for the donor, we need to periodically review the literature, review the United Network for Organ Sharing database for donor characteristics that may put them in danger, and scour databases for donors starting dialysis and/or listed for transplant. Additionally, we must encourage financing studies that follow large diverse cohorts of donors over their entire lifetimes in order to detect key characteristics that influence outcomes. Currently, it

Hypercoagulability risks among adult living liver donors.

BACKGROUND: Review of the literature is limited with respect to thrombotic risk in the living liver donor. This study examines inherent coagulable changes that occur as a result of the surgery. MATERIALS AND METHODS: At our center, we have performed 353 orthotopic adult liver transplants in the past 4 years. Of these, 20 were adult-adult right lobe living donor transplants. All living donors are alive and doing well. Of these, eight living donors were followed preoperatively and postoperatively monitoring protein C, protein S, antithrombin III, and factor VIII levels. Levels were checked at 4

Effects of schistosomiasis on living kidney donors.

Twenty living kidney donors with schistosomiasis were compared with 20 uninfected donors for a mean follow-up period of 42 months (range 12-62). All patients with schistosomiasis had been treated preoperatively with antischistosomal chemotherapy. None of the donors developed any appreciable change in mean systolic or diastolic blood pressure during the follow-up period, though one infected and two uninfected donors had traces of protein in the urine. One uninfected donor developed microscopic haematuria. The two groups has similar reductions in renal function after unilateral nephrectomy. The

Living-donor hepatectomy.

BACKGROUND: Organ transplantation from deceased donors is still far below the need. Because of this deficiency, liver transplantations are performed mostly from live donors in many transplant centers in our country. Living-donor liver transplantation (LDLT) has evolved dramatically over the past decade. The aim of this study was to present our clinical experience with living-donor hepatectomy. METHODS: We retrospectively analyzed all patients who underwent donor hepatectomy between March 2000 and September 2010. We reviewed demographic data, operation type, operation and cold ischemia times,

Living donor transplants.

The number of living donor transplants reported to the UNOS Scientific Renal Transplant Registry has increased from 1,810 in 1988 to 2,861 in 1994. Nearly all 250 United States transplant centers have reported living donor transplants. Graft survival rates were uniformly high for recipients of living donor transplants. One- and 5-year survival ranged from 95% and 84% for 3,653 HLA-identical sibling transplants to 89% and 69% for 1,981 offspring-to-parent transplants. Transplant half-lives ranged from 10 years for 360 transplants from nonspouse unrelated donors to 22 years for 3,653 transplants
