Role of Bridge Contraception in Postpartum Long-Acting Reversible Contraception and Sterilization Fulfillment Rates.

OBJECTIVE: To estimate the association of bridge contraception with interval long-acting reversible contraception (LARC) and sterilization fulfillment rates. METHODS: This is a secondary analysis of a retrospective single-center cohort chart review study examining 1,851 postpartum women who requested LARC or sterilization after discharge. Bridge contraception was requested by 597 of these women. Primary outcomes included LARC or sterilization fulfillment, time to fulfillment, postpartum visit attendance, and pregnancy within 365 days of delivery. RESULTS: The rate of LARC or sterilization fu

Inequalities in short-acting reversible, long-acting reversible and permanent contraception use among currently married women in India.

BACKGROUND: In India, the usage of modern contraception methods among women is relatively lower in comparison to other developed economies. Even within India, there is a state-wise variation in family planning use that leads to unintended pregnancies. Significantly less evidence is available regarding the determinants of modern contraception use and the level of inequalities associated with this. Therefore, the present study has examined the level of inequalities in modern contraception use among currently married women in India. METHODS: This study used the fourth round of National Family He

Immediate Postpartum Long-Acting Reversible Contraception Access and Child Maltreatment Rates.

Policy changes that improve access to immediate postpartum long-acting reversible contraceptives (IPP LARC carveouts) were examined as a potential prevention measure against parent-perpetrated child maltreatment, neglect and physical abuse of children under five. Generalized differences-in-differences was applied to state-year outcome trends from the National Child Abuse and Neglect Data System child file. Findings suggest IPP LARC carveouts did not yield significant reductions in substantiated parent-perpetrated physical abuse (ATT = -22.9% or 49.97 fewer cases per 100,000 children under 5, p

Inpatient Adolescent Sexual Health Services and Long-Acting Reversible Contraception Training.

OBJECTIVE: Inpatient settings provide unique opportunities to deliver sexual health services, such as long-acting reversible contraception (LARC), including intrauterine devices and contraceptive implants, to adolescents. This study aimed to (1) characterize general inpatient screening and management of sexual health services by region and (2) assess clinician preferences about inpatient LARC training. PATIENTS AND METHODS: We conducted a convergent parallel mixed-methods study with a national online survey and semistructured interviews among pediatric residents, fellows, advanced practice pr

Familiarity and acceptability of long-acting reversible contraception and contraceptive choice.

BACKGROUND: Contraceptive choice is a preference-sensitive decision that is affected by contraceptive attributes, patient experience, and reproductive history. Familiarity with and acceptability of specific contraceptive methods may influence patient decisions. OBJECTIVE: The purpose of this study was to describe the acceptability of and previsit familiarity with long-acting reversible contraception (intrauterine devices and contraceptive implants) compared with depo-medroxyprogesterone acetate and oral contraceptive pills in women seeking contraceptive care and to investigate the relationshi

Promotion of Long-Acting Reversible Contraception Among Adolescents and Young Adults.

INTRODUCTION: Intrauterine devices and contraceptive implants are recommended as first-line contraceptives by health care professional societies. However, uptake among US women lags substantially behind other developed countries. Little information is available on the extent to which clinicians document discussion about long-acting reversible contraception (LARC) in this patient population. We sought to determine the frequency with which clinicians document LARC discussion with eligible women aged 14 to 25 years in a training clinic and evaluate factors associated with LARC discussion and upta

Changes in body composition in women using long-acting reversible contraception.

OBJECTIVE: Users of hormonal long-acting reversible contraception (LARC) report weight gain as a side effect, but few studies have assessed body composition change among LARC users. We evaluated weight and body composition of healthy women using the levonorgestrel intrauterine system (LNG-IUS), copper intrauterine device (copper IUD) or etonogestrel implant (ENG implant). We hypothesized that weight gain and body composition over 12 months would not differ between copper IUD, LNG-IUS and ENG implant users. STUDY DESIGN: We performed a prospective cohort study of a subgroup of women enrolled i

Increasing Use of Long-Acting Reversible Contraception to Decrease Unplanned Pregnancy.

Unintended pregnancy remains high in the United States, accounting for one-half of pregnancies. Both contraceptive nonuse and imperfect use contribute to unplanned pregnancies. Long-acting reversible contraception (LARC) have greater efficacy than shorter acting methods. Data from large studies show that unplanned pregnancy rates are lower among women using LARC. However, overall use of LARC is low; of the reproductive age women using contraception, less than 10% are LARC users. Barriers include lack of knowledge and high up-front cost, and prevent more widespread use. Overcoming these barrier

Factors influencing the provision of long-acting reversible contraception in California.

OBJECTIVE: To assess long-acting reversible contraception (LARC) beliefs and practices among site directors who represent the family planning services delivered in their practices. METHODS: Medical directors from 1,000 sites listed in the Family Planning Access Care and Treatment program (California's family planning Medicaid program) provider database were mailed a survey in the fall of 2011 regarding their LARC beliefs and practices. Participants responded by mail, online, or telephone. Data on family planning clients served and LARC dispensing were obtained from administrative claims data.

Mental health diagnoses and early removal of long-acting reversible contraception.

BACKGROUND: Having a mental health diagnosis is associated with contraceptive non-adherence and user-related contraceptive failures of short-acting methods. There is a lack of research on the relationship between mental health diagnoses and early discontinuation of highly effective long-acting reversible (LARC) methods such as the intrauterine device (IUD) and subdermal implant (SDI). METHODS: Using a Primary Care and Obstetrics and Gynecology Patient Data Registry, we conducted a cross-sectional analysis of the relationship between any mental health diagnosis (any anxiety disorder or depress

Prevention of unintended pregnancy: a focus on long-acting reversible contraception.

This article summarizes the literature regarding the epidemiology and prevention of unintended pregnancy in the United States. Because of the Affordable Care Act and its accompanying contraceptive provision, there is a need for more primary care clinicians to provide family planning services. Office-based interventions to incorporate family planning services in primary care are presented, including clinical tools and electronic health record use. Special attention is paid to long-acting reversible contraceptive methods (the subdermal implant and intrauterine devices); these highly effective an

Profile of Long Acting Reversible Contraceptive Users.

BACKGROUND: Long acting reversible contraceptive methods are safe, effective and their uptake is integral to increasing modern contraceptive prevalence rate. Their utilization is influenced by women's reproductive health conditions and socio-economic background. This study aimed to explore the characteristics of women of reproductive age who chose these long-term contraceptive options. METHODS: A cross sectional study using a convenient sampling method was conducted among 74 women of reproductive age group (15-49 years) over a three-month period at Kathmandu Medical College, after obtaining e

Long-acting reversible contraceptives: intrauterine devices and the contraceptive implant.

The provision of effective contraception is fundamental to the practice of women's health care. The most effective methods of reversible contraception are the so-called long-acting reversible contraceptives, intrauterine devices and implants. These methods have multiple advantages over other reversible methods. Most importantly, once in place, they do not require maintenance and their duration of action is long, ranging from 3 to 10 years. Despite the advantages of long-acting reversible contraceptive methods, they are infrequently used in the United States. Short-acting methods, specifically

Trends in Continuation of Long-Acting Reversible Contraception Among Adolescents Receiving Medicaid.

PURPOSE: Despite increasing use of long-acting reversible contraception (LARC) among U.S. adolescents, there is limited literature on factors affecting intrauterine device (IUD) or subdermal implant use. This study aimed to describe statewide rates, and associated patient and provider factors of adolescent IUD or implant initiation and continuation. METHODS: This retrospective cohort study used N.C. Medicaid claims data. 10,408 adolescents were eligible (i.e., 13-19 years, female sex, continuous Medicaid enrollment, had an IUD or implant insertion or removal code from January 1, 2013, to Octo

Successful Provision of Long-Acting Reversible Contraception in a Sexual Health Clinic.

BACKGROUND: Women who attend sexual health clinics are at high risk for sexually transmitted infections and unintended pregnancy. Long-acting reversible contraceptives (LARC) are very effective contraceptive methods, but the provision of LARC in such clinics is not well described in the literature. METHODS: We conducted a retrospective chart review of women who presented to Denver Sexual Health Clinic for any reason and received family planning services between April 1, 2016, and October 31, 2018. We assessed demographic and clinical factors associated with contraceptive method received and c

Cost Minimization Analysis of Same-Day Long-Acting Reversible Contraception for Adolescents.

IMPORTANCE: Long-acting reversible contraception (LARC) is considered first-line contraception for adolescents but often requires multiple clinic visits to obtain. OBJECTIVE: To analyze Indiana Medicaid's cost savings associated with providing adolescents with same-day access to LARC. DESIGN, SETTING, AND PARTICIPANTS: An economic evaluation of cost minimization from the payer's (Medicaid) perspective was performed from August 2017 through August 2018. The cost model examined the anticipated outcome of providing LARC at the first visit compared with requiring a second visit for placement. Th
