Microbiologic survey of long-term care facilities.

BACKGROUND: We undertook a microbiologic survey of long-term care facilities to categorize bacteria found in cultures of residents. Culture and sensitivity data were collected on 566 samples from indwelling bladder catheters, percutaneous gastrostomy tubes, nares, stool, wounds, pressure ulcers, and tracheostomies in 25 Nebraska and Iowa facilities. Information was also collected on resident factors (eg, presence of indwelling urinary catheter, prior antibiotic administration) and institutional variables (eg, number of beds, nosocomial infection rates). RESULTS: There were 478 gram-negative i

[Modern migraine therapy-interdisciplinary long-term care].

Migraine has a very high lifetime prevalence with a severe illness-related burden. As a result, extensive long-term and regular treatment is required, which cannot be covered solely by neurologists. This is particularly the case for the long-term monitoring of migraine, which often takes place over several decades. The diagnosis is made using the diagnostic criteria of the International Headache Society (ICHD-3) based on the clinical phenotype. Owing to often complex neurological symptoms, a detailed weighing up of the differential diagnoses is required, which calls for specialist neurological

Long term outcome of primary care depression.

We conducted a follow up over 23 years of depressed patients originally presenting to general practice in 1981 and studied in detail at that time. Aims were to assess the long term course and outcome of depression in primary care. Patterns of recovery and recurrence of major depressive episodes, together with other aspects of course, treatment and current state, were assessed at interview. 78% (129) of the original sample were traced to current general practice and outcome data obtained on 54. One third had a prior history of depression. Interview data were obtained on 37 patients. Time to rec

Pain-related health care costs for long-term care residents.

BACKGROUND: We tested for differences in direct health care costs among long-term care (LTC) residents age 65 and older with clinically significant pain (CSP) and with no pain or non-daily mild pain (NP/NDMP). We are not aware of any other large scale investigation that examined the cost of pain in LTC environments. METHODS: Population-based administrative health data from Saskatchewan, Canada for 2004 to 2015 were used to compare direct health care costs for CSP and NP/NDMP groups up to one year after admission to LTC. Total accumulated costs for hospitalization, physician services, LTC, and

Long-run consequences of informal elderly care and implications of public long-term care insurance.

We estimate a dynamic structural model of labor supply, retirement, and informal caregiving to study short and long-term costs of informal caregiving in Germany. Incorporating labor market frictions and the German tax and benefit system, we find that in the absence of Germany's public long-term insurance scheme, informal elderly care has adverse and persistent effects on labor market outcomes and, thus, negatively affects lifetime earnings and future pension benefits. These consequences of caregiving are heterogeneous and depend on age, previous earnings, and institutional regulations. Policy

[Diseases requiring severe-level care certification for long-term care insurance].

Objectives Extending health expectancy is important; however, health and welfare programs in local areas and communities present their own issues. This study aimed to identify main diseases requiring severe-level care certification for long-term care insurance based on information gathered from opinion papers prepared by primary doctors. Methods Data were obtained for 4,089 patients aged ≧65 years who were certified for long-term care insurance for the first time between April 1, 2013, and March 31, 2015. Their disorders were categorized into the groups used in the Comprehensive Survey of Livin

Priorities for long-term care: comments from board-and-care residents.

In these times of shrinking human services resources, establishing clear service priorities becomes all the more necessary. This is particularly true for the care of chronically mentally ill persons, whose many needs place high service demands on the care system. As a group these patients rarely have a say in the establishment of service priorities. To obtain a better idea of their perceived needs, we asked 278 randomly selected, mentally disabled residents in 30 Los Angeles board-and-care homes about their current life conditions and problems. Their responses to open-ended questions about rec

Implementing an Integrated Palliative Care Pathway in Long-term Care Facilities.

BACKGROUND: Long-term care facilities (LTCFs) care for residents with frailty, dementia, and progressive illness, yet palliative care needs are often recognized late. The Modena Local Health Authority developed a pathway to improve identification and integration with the Local Palliative Care Network. MEASURES: Baseline measures from seven LTCFs and a province-wide survey of 53 LTCFs included SPICT-estimated palliative care needs, individualized palliative care plans, place of death, and data completeness. After implementation, measures included resident characteristics, diagnoses, vital stat

Four Dimensions of Sustainability in Long-Term Care.

Sustainability became a critical concern in the care sector during the COVID-19 pandemic. This article defines and discusses the economic, social, environmental, and technological dimensions of sustainability in long-term care (LTC) in the pandemic context to expand the sustainability discourses in aging and care research. Economic sustainability shifted focus beyond expenditure control to encompass adequacy and stability in LTC financing and service delivery systems. Addressing moral distress of staff in care provision and social cohesion inside care-providing organizations were emphasized du

Long-Term Care of the Liver Transplant Recipient.

Reflecting continued enhancements of surgical techniques and immunosuppression, liver transplant recipients are experiencing increased longevity. Once patients have successfully navigated the perioperative and early posttransplant period, their survival depends on management of immunosuppression to minimize systemic side effects that will contribute to the development of medical comorbidities while avoiding rejection. This includes mitigating cardiovascular risk factors and other components of the metabolic syndrome. The concept of transplant survivorship emphasizes a holistic approach to impr

Primary care management of Long-Term opioid therapy.

The United States underwent massive expansion in opioid prescribing from 1990-2010, followed by opioid stewardship initiatives and reduced prescribing. Opioids are no longer considered first-line therapy for most chronic pain conditions and clinicians should first seek alternatives in most circumstances. Patients who have been treated with opioids long-term should be managed differently, sometimes even continued on opioids due to physiologic changes wrought by long-term opioid therapy and documented risks of discontinuation. When providing long-term opioid therapy, clinicians should document o

Reimagining Family Involvement in Residential Long-Term Care.

Although descriptions of family involvement in residential long-term care (RLTC) are available in the scientific literature, how family involvement is optimized in nursing homes or assisted living settings remains underexplored. During the facility lockdowns and visitor restrictions of the COVID-19 pandemic, residents experienced social deprivation that may have resulted in significant and adverse health outcomes. As with so many other critical issues in RLTC, the COVID-19 pandemic has magnified the need to determine how families can remain most effectively involved in the lives of residents.

Tuberculosis outbreak in a long-term care facility.

OBJECTIVES: Tuberculosis (TB) was a major cause of morbidity and mortality in Canada early in the 20th century. Elderly populations in Canada remain at high risk for TB disease. Elderly patients may present atypically, with the result that many active cases can remain undiagnosed. We present an outbreak of TB that occurred in a Residential and Long-Term Care (LTC) facility in Ontario. METHODS: Case finding was carried out through the conventional concentric circle approach. Three rounds of tuberculin skin testing were conducted at 8-12 week intervals. Laboratory analysis was conducted at the

[Long-term care services in Spain: an overview].

To date, both in Spain and virtually all the other European Union (EU) countries, dependency has been seen to be a fundamentally private problem to be dealt with by the family concerned. In this way, whether through informal carers or contracted professionals, in the domestic environment or in care homes, it is the dependent person themselves and their families who currently bear the majority of the costs. In light of this, current concern lies in the social change that is coming on, mainly the accelerated aging process and the increased participation of middle-aged women in the labour market,
