Impaired folate metabolism reshapes auditory response profiles and impairs loudness perception in MTHFR-deficient mice.

Folate metabolism, regulated by methylenetetrahydrofolate reductase (MTHFR), is crucial for proper neurodevelopment, and disruptions-whether due to genetic polymorphisms or maternal nutritional deficits-have been linked to cognitive and behavioral impairments. Notably, MTHFR-deficient mouse models display altered social interaction and auditory communication, hinting at disruptions in auditory-related circuits and prompting the question of whether impaired folate metabolism might also affect sound processing and perception. Here, using two-photon calcium imaging, we show that MTHFR deficiency

The Interaction Between Vowel Quality and Intensity in Loudness Perception of Short Vowels in Mongolian.

PURPOSE: This study investigated the influence of vowel quality on loudness perception and stress judgment in Mongolian, an agglutinative language with free word stress. We aimed to explore the effects of intrinsic vowel features, presentation order, and intensity conditions on loudness perception and stress assignment. METHOD: Eight Mongolian short vowel phonemes (/ɐ/, /ə/, /i/, /ɪ/, /ɔ/, /o/, /ʊ/, and /u/) were recorded by a native Mongolian speaker of the Urad subdialect (the Chahar dialect group) in Inner Mongolia. The short vowels were paired under different intensity conditions. Native

Relative loudness perception of low and high frequency sounds in the open and occluded ear.

A comparison of published equal loudness contours indicates that different shapes are obtained at a comfortable level when the measurements are done in an occluded ear than when they are done in an open ear, even though all measurements are expressed as dB SPL at the eardrum. This paper presents the result from a loudness balancing test which confirms this observation. Eleven normal-hearing listeners balanced the level of a 500- and a 3000-Hz octave band babble-noise to the level of a 1500-Hz octave band babble-noise. The balancing test was completed in open and occluded ears using a loudspeak

Positron Emission Tomography Imaging Reveals Auditory and Frontal Cortical Regions Involved with Speech Perception and Loudness Adaptation.

Considerable progress has been made in the treatment of hearing loss with auditory implants. However, there are still many implanted patients that experience hearing deficiencies, such as limited speech understanding or vanishing perception with continuous stimulation (i.e., abnormal loudness adaptation). The present study aims to identify specific patterns of cerebral cortex activity involved with such deficiencies. We performed O-15-water positron emission tomography (PET) in patients implanted with electrodes within the cochlea, brainstem, or midbrain to investigate the pattern of cortical

On Balancing Sharpness of Excitation Patterns, Loudness, and Potential Effects on Speech Perception with Cochlear-Implant Current-Focussing Strategies.

PURPOSE: Cochlear implant (CI) users struggle to understand speech under noisy conditions. This has long been attributed to channel interactions: a given neuron not only responds to electrical stimulation from the closest electrode but also is sensitive to stimulation from electrodes located further away. Shaping the stimulation voltage into a very sharp profile (e.g. using tripolar instead of the more common monopolar stimulation) has been proposed as a solution, only with limited success. We argue this is due to several factors: the strong correlation between envelopes of adjacent CI channel

Children's speech perception and loudness ratings when fitted with hearing aids using the DSL v.4.1 and the NAL-NL1 prescriptions.

This paper reports speech and loudness measures on a group of children in a double-blind cross-over trial comparing the NAL-NL1 and DSL[i/o] prescriptions. Twenty-four children with hearing impairment were fitted with digital WDRC hearing aids at each site (Australia, Canada). Speech recognition was measured for nonsense syllables and for the 50% correct threshold for sentence recognition in noise. Loudness ratings for sentences were made on a 7-point scale. Measures were made at fitting and repeated following 8-week trials. Fitting orders were randomized and counterbalanced. Significant diffe

An investigation of input level range for the nucleus 24 cochlear implant system: speech perception performance, program preference, and loudness comfort ratings.

OBJECTIVE: Cochlear implant recipients often have limited access to lower level speech sounds. In this study we evaluated the effects of varying the input range characteristics of the Nucleus 24 cochlear implant system on recognition of vowels, consonants, and sentences in noise and on listening in everyday life. DESIGN: Twelve subjects participated in the study that was divided into two parts. In Part 1 subjects used speech processor (Nucleus 24 SPrint trade mark ) programs adjusted for three input sensitivity settings: a standard or default microphone sensitivity setting (MS 8), a setting t

Effects of phase duration and pulse rate on loudness and pitch percepts in the first auditory midbrain implant patients: Comparison to cochlear implant and auditory brainstem implant results.

The auditory midbrain implant (AMI), which is designed for stimulation of the inferior colliculus (IC), is now in clinical trials. The AMI consists of a single shank array (20 contacts) and uses a stimulation strategy originally designed for cochlear implants since it is already approved for human use and we do not yet know how to optimally activate the auditory midbrain. The goal of this study was to investigate the effects of different pulse rates and phase durations on loudness and pitch percepts because these parameters are required to implement the AMI stimulation strategy. Although each

Using auditory reaction time to measure loudness growth in rats.

Previous studies have demonstrated that auditory reaction time (RT) is a reliable surrogate of loudness perception in humans. Reaction time-intensity (RT-I) functions faithfully recapitulate equal loudness contours in humans while being easier to obtain than equal loudness judgments, especially in animals. In humans, loudness estimation not only depends on sound intensity, but on a variety of other acoustic factors. Stimulus duration and bandwidth are known to impact loudness perception. In addition, the presence of background noise mimics loudness recruitment; loudness growth is rapid near th

Loudness summation of equal loud narrowband signals in normal-hearing and hearing-impaired listeners.

OBJECTIVE: Loudness perception of binaural broadband signals, e.g. speech shaped noise, shows large individual differences using frequency-dependent amplification which was adjusted to restore the loudness perception of monaural narrowband signals in hearing-impaired (HI) listeners. To better understand and quantify this highly individual effect, loudness perception of broadband stimuli consisting of a number of spectrally separated narrowband components which where individually adjusted to equal loudness is of interest. DESIGN: Based on categorical loudness scaling, the loudness of an equal

Spectral and binaural loudness summation of equally loud narrowband signals in single-sided-deafness and bilateral cochlear implant users.

Recent studies on loudness perception of binaural broadband signals in hearing impaired listeners found large individual differences, suggesting the use of such signals in hearing aid fitting. Likewise, clinical cochlear implant (CI) fitting with narrowband/single-electrode signals might cause suboptimal loudness perception in bilateral and bimodal CI listeners. Here spectral and binaural loudness summation in normal hearing (NH) listeners, bilateral CI (biCI) users, and unilateral CI (uCI) users with normal hearing in the unaided ear was investigated to assess the relevance of binaural/bilate

Perceived auditory dynamic range is enhanced with wrist-based tactile stimulation.

Tactile stimulation has been shown to increase auditory loudness judgments in listeners. This bias could be utilized to enhance perception for people with deficiencies in auditory intensity perception, such as cochlear implant users. However, several aspects of this enhancement remain uncertain. For instance, does the tactile stimulation need to be applied to the hand or body, or can it be applied to the wrist? Furthermore, can the tactile stimulation both amplify and attenuate the perceived auditory loudness? To address these questions, two loudness-matching experiments were conducted. Partic

Perception Versus Reaction: Comparison of Tinnitus Psychoacoustic Measures and Tinnitus Functional Index Scores.

Purpose Psychoacoustic characteristics of tinnitus include its loudness and pitch. These characteristics are commonly measured and reported; however, it has not been shown that they are associated with the impact, or bothersomeness, of tinnitus. This study addressed this question by determining correlations between measures of tinnitus loudness, tinnitus pitch, and functional effects of tinnitus. Method Tinnitus loudness matches, pitch matches, a numeric rating scale (NRS) of tinnitus loudness, and responses to the 25-item tinnitus functional index (TFI) were obtained from 223 participants who

The Effect of Pulse Shape and Interphase Gaps on Speech Perception and Perceived Sound Quality in Electrical Hearing.

BACKGROUND: Stimulation with triphasic pulses has been shown to reduce the occurrence of unwanted facial nerve stimulation (FNS) with cochlear implants (CIs). However, there is little data available on how different pulse shapes affect the hearing outcome with electrical hearing in general. The aim of the study was to evaluate the effects of different stimulation pulse shapes on speech perception in noise, as well as loudness perception and subjective sound quality. METHODS: Twenty experienced cochlear-implant users not suffering from FNS participated in a prospective single-visit study. Base

The Relationship Between Physical Activity and Tinnitus Loudness and Severity: A Cross-Sectional Study.

INTRODUCTION: Tinnitus is characterized by the perception of sound in the absence of an external stimulus and affects about 14.4% of the adult population. Psychological co-morbidities such as stress, anxiety, and depression can largely influence the patient's perception of tinnitus loudness and severity. Research has shown that these psychological conditions improve when patients are more physically active. To date, however, it is unclear if physical activity also affects tinnitus loudness and severity. Therefore, this study aims to uncover the relationship between physical activity and tinnit

Effect of Tinnitus Habituation Therapy on Auditory Abilities.

Introduction  Research with tinnitus patients presenting with hearing loss and normal hearing subjects have verified that tinnitus interferes with auditory processing. One treatment option for this symptom consists of tinnitus habituation therapy. Objective  To determine the influence of tinnitus habituation therapy on auditory abilities, on the self-perception of tinnitus regarding loudness and discomfort, and its impact on the quality of life. Methods  A quasi-experimental study was conducted with 19 individuals with tinnitus. Audiological tinnitus and auditory processing evaluations were pe

Cochlear implant and hearing aid: a new approach to optimizing the fitting in this bimodal situation.

More and more patients with residual hearing on the contralateral side are becoming candidates for cochlear implants (CI) surgery due to increasing CI. The major benefits of regular binaural hearing are spatial hearing, localization, and signal source discrimination in both quiet and noisy surroundings. In most of the reports, hearing aid fitting was carried out without balancing both the devices. Twelve children and eight adults with residual hearing on the non-operated side were binaurally fitted. Our fitting procedure for the hearing aid was based on the desired sensation level [i/o] method

Comparison of Tinnitus Loudness Measures: Matching, Rating, and Scaling.

Purpose Chronic tinnitus ("ringing in the ears") is a phantom auditory perception with no cure. A goal of treatment is often to reduce the loudness of tinnitus. However, tinnitus loudness cannot be measured objectively. It is most commonly assessed by obtaining a loudness match (LM) with a pure tone and by using a numeric rating scale (NRS). Constrained loudness scaling (CLS) is a more recent measure of tinnitus loudness that utilizes auditory training of a fixed loudness scale to guide tinnitus loudness judgments. The purpose of this study was to compare results using these 3 measures of tinn

Perceptual Distortions in Aided Hearing-Impaired Listeners: Psychophysical Evidence of Excessive Loudness Growth and Altered Timbre Representation.

Hearing aid users often report that some everyday sounds are abnormally intrusive or salient. This issue may stem from distorted coding of sound features due to peripheral impairments and/or hearing aid processing. This study investigates whether aided hearing-impaired (aHI) listeners experience (1) abnormal loudness processing and (2) distorted timbre perception, both of which may contribute to this problem. In Experiment 1, we measured loudness functions using sounds varying in four timbre dimensions (brightness, spectral flux, attack time, roughness) in normal-hearing (NH) and aHI listeners
