Identifying and de-implementing low-value care in primary care: the GP's perspective-a cross-sectional survey.

OBJECTIVE: General practitioners have an important role in reducing low-value care as gatekeepers of the health system. The aim of this study was to assess the experiences of Dutch general practitioners regarding low-value care and to identify their needs to decrease low-value primary care. DESIGN: We performed a cross-sectional study. PARTICIPANTS: We sent a survey to 500 general practitioners. SETTING: Primary care in the Netherlands. PRIMARY AND SECONDARY OUTCOMES: The survey contained questions about the provision of low-value care and on clinical cases about lumbosacral spine X-rays i

Use of low-value cancer care in practices acquired by private equity firms.

Private equity acquisition of medical practices has been associated with increased prices and use of lucrative treatments. Using Medicare data from 2015 to 2021, we compared the prevalence of low-value cancer treatments between private equity-owned and non-private equity-owned practices. The 5 low-value treatments were (1) granulocyte colony-stimulating factors for low-risk chemotherapy, (2) denosumab for castration-sensitive prostate cancer, (3) nab-paclitaxel for breast or lung cancer, (4) the addition of bevacizumab for ovarian cancer, and (5) biologic agents with biosimilar alternatives. [Gender Bias in Low-Value Clinical Practices in Primary Care: A Scoping Review].

Low-value clinical practices (LVPs) in primary care are not sex- or gender-neutral, yet the evidence on this bias remains scattered. Following a scoping review per PRISMA-ScR and JBI, with a PCC-structured question, we searched PubMed/MEDLINE, Embase, CINAHL and Web of Science (1999-2026), complemented by Spanish foundational primary-care safety studies (APEAS, EVADUR). After independent screening (agreement 81.1%) and a 2016-2026 window, 250 documents were included. Evidence concentrated in inappropriate psychotropic prescribing (n=181) and excessive diagnostic testing (n=124), with scarce re

Low-Value Emergency Care and Racial and Ethnic Differences Across US Children's Hospitals.

OBJECTIVES: To examine (1) differences in low-value care (LVC) for common emergency conditions by race and ethnicity in US children's hospitals and (2) the association between hospital characteristics, including LVC rate, and the magnitude of within-hospital LVC differences by race and ethnicity. We hypothesized smaller LVC differences in hospitals with lower overall LVC rates. METHODS: We performed a cross-sectional study of children younger than 18 years discharged from the emergency department with asthma, bronchiolitis, headache, or minor head injury in the Pediatric Health Information Sy

Reducing Low-Value Delirium Care Using Behavioral Design in the Electronic Medical Record.

Delirium is associated with increased morbidity, mortality, and healthcare costs. Despite best practice guidelines recommending non-pharmacological treatment strategies, delirium is frequently sub-optimally managed with reliance on antipsychotics or benzodiazepines. To optimize delirium management at our institution, we implemented a clinical decision support tool embedded within our Epic electronic medical record (EMR). Our team developed an Our Practice Advisory (OPA) triggered by a positive delirium screening result to alert hospitalists to patients at high risk for delirium. An accompanyin

Increased spending on low-value care during the COVID-19 pandemic in Virginia.

Characterizing the value and equity of care delivered during the COVID-19 pandemic is crucial to uncovering health system vulnerabilities and informing postpandemic recovery. We used insurance claims to evaluate low-value (no clinical benefit, potentially harmful) and clinically indicated utilization of a subset of 11 ambulatory services within a cohort of ∼2 million Virginia adults during the first 2 years of the pandemic (March 1, 2020-December 31, 2021). In 2020, low-value and clinically indicated utilization decreased similarly, while in 2021, low-value and clinically indicated utilization

Identification of low-value practices susceptible to gender bias in primary care setting.

BACKGROUND: Data on overuse of diagnostic and therapeutic resources underline their contribution to the decline in healthcare quality. The application of "Do Not Do" recommendations, in interaction with gender biases in primary care, remains to be fully understood. Therefore, this study aims to identify which low-value practices (LVPs) causing adverse events are susceptible to be applied in primary care setting with different frequency between men and women. METHODS: A consensus study was conducted between November 1, 2021, and July 4, 2022, in the primary care setting of the Valencian Commun

Low-value injury care in the adult orthopaedic trauma population: A systematic review.

OBJECTIVES: Fifteen potentially low value practices in adult orthopaedic trauma care were previously identified in a scoping review. The aim of this study was to synthesise the evidence on these practices. METHODS: We searched four databases for systematic reviews, randomised controlled trials (RCTs), cohort studies and case series that assessed the effectiveness of selected practices. Methodological quality was evaluated using the Measurement Tool to Assess Systematic Reviews version 2 (AMSTAR-2) for systematic reviews and the Critical Appraisal Checklist for Case Series. We evaluated risk o

Health Care Innovation for Low-Resource Settings: The Value of Local Immersion and Partnership.

Health Care Innovation is the creation, development, and translation of new and better solutions to health care challenges. At its core, this endeavor does not require extending the frontiers of science or the creation of new fundamental technologies. Rather, it is primarily focused on the use of existing science and established technologies in the design of new solutions to problems in health care. Successfully innovating for low- and middle-income countries (LMICs) requires a needs and stakeholder-driven approach to enable development and adoption of available, accessible, and acceptable sol

Use of High- and Low-Value Care Among Traditional Medicare Beneficiaries With and Without Medigap.

OBJECTIVES: To quantify the additional health care spending associated with Medigap coverage among traditional Medicare (TM) beneficiaries and assess whether this spending is disproportionately allocated to high-value versus low-value services. STUDY SETTING AND DESIGN: We conducted a repeated cross-sectional study. DATA SOURCES AND ANALYTICAL SAMPLE: We analyzed TM beneficiaries with and without Medigap from the 2013-2021 Medical Expenditure Panel Survey. Inverse probability of treatment weighting (IPTW) was applied to balance observed covariates between TM beneficiaries with and without Me

Use Of High- And Low-Value Health Care Among US Adults, By Income, 2010-19.

Health care payment reforms in the US have aimed to encourage the use of high-value care while discouraging the use of low-value care. However, little is known about whether the use of high- and low-value care differs by income level. Using data from the 2010-19 Medical Expenditure Panel Survey, we examined the use of specified types of high- and low-value care by income level. We found that high-income adults were significantly more likely than low-income adults to use nearly all types of high-value care. Findings were consistent across age categories, although differences by income level in

Reducing unwarranted variation in low-value health care in Norway: description of the NOR programme.

Multiple advice-based interventions designed to reduce the use of low-value procedures, i.e. procedures that are medically unnecessary because they provide minimal or no net health benefit, have so far demonstrated limited impact on utilisation rates. The underlying assumption for such initiatives is that increased knowledge, together with data demonstrating unwarranted variation, are sufficient to change clinical practice. Notably, this has not been the case. Numerous studies have documented complex organisational, professional, and patient-level barriers to change. Therefore, we advocate for

Potentially low-value operative care in abdominal trauma: A retrospective National Trauma Data Bank study.

BACKGROUND: We currently lack national data on adherence to American College of Surgeons recommendations on nonoperative management for hemodynamically stable adults with solid-organ injuries. We aimed to estimate the incidence and interhospital variation in potentially low-value operative management for adults with blunt solid abdominal organ injuries. METHODS: We included adults with blunt solid-organ injury eligible for nonoperative management (grades I-IV spleen and liver and grade I-III kidney, hemodynamically stable on arrival, and no blood products used in the emergency department) who

Telemedicine Adoption and Low-Value Care Use and Spending Among Fee-for-Service Medicare Beneficiaries.

IMPORTANCE: Low-value care is a persistent problem with direct and cascading harms. Telemedicine is now commonly used and may reduce low-value testing by introducing barriers to completing tests at a given visit or expand opportunities for low-value testing by contributing to higher visit volumes. OBJECTIVE: To quantify the association between telemedicine adoption and low-value testing among fee-for-service Medicare beneficiaries. DESIGN, SETTING, AND PARTICIPANTS: In this cohort study using 100% fee-for-service Medicare claims data, US health systems were divided into quartiles based on 20

Regional variation in low-value musculoskeletal surgery: a nationwide study from the Finnish Care Register.

BACKGROUND AND PURPOSE: Healthcare systems globally are grappling with resource constraints and rising costs. Concerns have been raised about "low-value" care, which consumes healthcare resources without benefiting patients. We aimed to examine regional differences in common low-value musculoskeletal surgeries in Finland and explore explanatory factors behind the variation. METHODS: Using data from the Finnish Care Register for Health Care, surgeries conducted from 2006-2007 compared with 2020-2021 were analyzed across 20 hospital districts. Selected surgeries (acromioplasty, rotator cuff rep

Adherence of Spanish pediatricians to "do not do" guidelines to avoid low-value care in pediatrics.

UNLABELLED: The main objective of this study was to analyze the degree of knowledge and compliance of Spanish pediatricians with the "do not do" recommendations of the Spanish Association of Pediatrics. A nationwide cross-sectional, descriptive study was carried out using a 25-item questionnaire among Spanish pediatricians. Univariate, bivariate, and multivariate analyses were performed. A total of 1137 pediatricians participated in the study. Most of them were women (75.1%), older than 55 (28.3%), worked in specialized care (56.9%), with public financing (91.2%), and had been working for more

Medicare Accountable Care Organizations Are Not Associated With Reductions in the Use of Low-Value Coronary Revascularization.

BACKGROUND: Because specialty care accounts for half of Medicare expenditures, improving its value is critical to the success of Medicare accountable care organizations (ACOs) in curbing spending growth. However, whether ACOs have reduced low-value specialty care without compromising use of high-value services remains unknown. METHODS AND RESULTS: Using national Medicare data, we identified 2 cohorts: beneficiaries for whom the value of coronary revascularization is lower (those with ischemic heart disease without angina, congestive heart failure, or recent admission for acute myocardial infa

Impact of a G-CSF Policy to Reduce Low-Value Care on Guideline Adherence and Mortality.

PURPOSE: Practice guidelines recommend the prophylactic use of granulocyte colony-stimulating factors (G-CSFs) in patients with high risk of febrile neutropenia, but evidence suggests that G-CSFs are frequently overused. The objectives of this study were (1) to determine the prevalence and prescribing patterns of G-CSF and (2) to evaluate the impact of a program initiative on G-CSF prescribing patterns, adherence to guidelines, and mortality. METHODS: In this retrospective cohort study, data were used from the electronic health records of patients with metastatic colorectal cancer who receive

Barriers and facilitators to reducing low-value care for the management of low back pain in Iran: a qualitative multi-professional study.

INTRODUCTION: Low back pain (LBP) is a prevalent musculoskeletal disorder with a wide range of etiologies, ranging from self-limiting conditions to life-threatening diseases. Various modalities are available for the diagnosis and management of patients with LBP. However, many of these health services, known as low-value care (LVC), are unnecessary and impose undue financial costs on patients and health systems. The present study aimed to explore the perceptions of service providers regarding the facilitators and barriers to reducing LVC in the management of LBP in Iran. METHODS: This qualitat
