Lower Extremity Cast Application.

UNLABELLED: This chapter will focus on lower extremity casting. Casting is widely recognized as the best treatment for many pediatric lower extremity fractures, unlike those in adults. Due to children's remodeling potential, the displacement threshold for surgery is often higher than that for skeletally mature patients, and immobilization in a cast with proper molding may be the only treatment needed.Lower extremity casts can be used to maintain the alignment of nondisplaced fractures. They are also utilized after closed reduction of a fracture, with appropriate molding to prevent any further

Lower extremity entrapment neuropathies.

Entrapment neuropathies are frequently encountered by rheumatologists, not only because they are common but also because of their association with certain rheumatological and systemic disorders. Recognizing entrapment neuropathy early can help avoid progressive neurological deficits, as well as facilitate appropriate treatment measures, which can effectively minimize a patient's symptoms. Entrapment neuropathies may be distinguished from other musculoskeletal causes of lower extremity pain by identifying characteristic patterns of weakness and/or sensory loss, so a focused bedside neurological

Lower extremity nerve trauma.

Peripheral nerve injuries of the lower extremity (LE) are frequently encountered in orthopaedic practice. They can be traumatic or iatrogenic. Proper and timely diagnosis and treatment are the keys to optimizing outcomes. This paper reviews and discusses the basic anatomy and physiology of nerve injury and the current literature on the incidence, pathogenesis, diagnosis, management and outcomes of sciatic, femoral, peroneal, and tibial nerve injuries. The purpose of this review is to suggest a protocol for evaluation and management of LE nerve injuries.

Recurrent lower extremity pseudocellulitis.

The term "Pseudocellulitis" can be used to describe an uncomplicated nonnecrotizing inflammation of the dermis and hypodermis from a noninfectious etiology. Chemotherapeutic agents have been associated with a variety of cutaneous reactions, including radiation recall dermatitis, hypersensitivity reactions, and erysipeloid reactions. Gemcitabine (2,2-difluorodeoxycytidine) is currently being used for treatment of a variety of solid malignancies, including carcinoma of the lung. The dermatitis involved with gemcitabine is typically a radiation recall reaction whereby an inflammatory reaction occ

Outpatient lower extremity infusions.

The considerable development of ambulatory surgery has led to an increase in the number of lower extremity procedures performed in an outpatient setting. More recently, the availability of disposable pumps has allowed us to extend the indications of continuous nerve blocks for ambulatory post-operative pain management. Indications for lumbar plexus continuous blocks include anterior cruciate ligament (ACL) reconstruction and patella repairs as well as frozen knee, whereas continuous sciatic blocks are indicated for major foot and ankle surgery. Different modes of local anaesthetic administrati

Functional outcome after major lower extremity amputation: a survey on lower extremity amputees.

The post-operatve course of amputees is poorly documented. This cross-sectional survey was unertaken to determine functional outcomes of 213 patients who had undergone either a below-knee or above-knee amputation from 2000 to 2002 in a state-hospital setting. The study comprises a self-constructed questionnaire and interview conducted by phone. Of the 213 amputees, 41 out of 61 documented telephone numbers of the patients were useful for contact. Only 30 amputees were available for the study as the remaining 11 had passed away. Included in the questionnaire was the modified Barthel Index, a me

Chronic Lower Extremity Wounds: Updates on Lower Extremity Reconstruction in Clinics in Plastic Surgery.

Chronic lower extremity (LE) wounds represent a growing source of morbidity, limb loss, and health care expenditure, driven by rising rates of diabetes, vascular disease, and advanced age. Effective management requires a comprehensive understanding of underlying wound pathophysiology, etiology, and patient risk factors. This article reviews the assessment and multidisciplinary management of chronic LE wounds, emphasizing infection control, vascular optimization, and biomechanical correction. Institutional outcomes using a coordinated "vasculoplastic" approach demonstrate that complex limb salv

Extremity preservation in traumatic and nontraumatic lower extremity defects.

BACKGROUND: Indications for reconstruction of the lower extremity range from posttraumatic defects to infections and tumors. Despite advancements in plastic surgery, flap surgery still poses a challenge. In this retrospective study local flap surgeries and microsurgical free flaps were assessed. Postoperative complications and limb preservation were analyzed. METHODS: This retrospective study included 187 patients who were treated at a university-affiliated tertiary care hospital. Defects were of traumatic (29.4%) and nontraumatic (70.6%) etiology. Limb preservation was determined during a 12

Lower Extremity Abnormalities in Children.

Rotational and angular variations of the lower extremities in children are common conditions seen in primary care. These visits are often due to parental concerns about the appearance of their child's lower extremities. Common variations include intoeing (metatarsus adductus, internal tibial torsion, and femoral anteversion); out-toeing (external tibial torsion, femoral retroversion, and pes planus); and angular variations (genu varum and genu valgum). History, particularly age of onset; duration; progression; appearance and impact; prenatal and birth history; nutritional deficiencies; growth

Aortoiliofemoral Lower Extremity CT Angiography.

CT angiography (CTA) of the aortoiliofemoral (AIF) arteries in the abdomen, pelvis, and lower extremities has become an invaluable tool in assessment of patients with peripheral arterial disease (PAD) and lower extremity trauma. AIF CTA provides rapid and comprehensive assessment of arterial inflow and outflow, guiding management of patients with chronic claudication and those with more acute manifestations, including atherothrombotic occlusion, embolic disease, or thrombosis of prior interventions such as bypass graft or stent placement. Careful attention to technique is critical in performin

Third Trimester Lower Extremity Lymphorrhea.

INTRODUCTION: Lower extremity edema is one of the most common complaints among pregnant patients. However, there is no literature mentioning weeping edema (i.e., lymphorrhea) in a pregnant woman who has no concordant underlying renal and/or cardiac pathology. There is also a lack of evidence and recommendations regarding the therapeutic benefit and safety profile of diuretic use to treat profound pregnancy-associated edema. Herein, we present the case of 32-year-old female who presented with a significant lymphorrhea during the third trimester without cardiac or renal comorbidity and was succe

[Lower-extremity artery disease (LEAD)].

Women have a risk of LEAD (lower-extremity artery disease)similar to men's risk. Symptoms are often absent, atypical or underestimated, leading to diagnosis in the most severe stages. Medical care is often less well adapted. In cases of revascularization, women have a higher morbidity rate than men, regardless of the severity grade and procedure chosen.

Metastatic Angiosarcoma of Lower Extremity.

Angiosarcoma (AS) is an uncommon malignant neoplasm characterized by rapidly proliferating, extensively infiltrating anaplastic cells derived from blood vessels. These are aggressive tumors and tend to recur locally, spread widely with high rate of lymph node and systemic metastases. They are more frequent in skin and soft tissue, head and neck being the most common sites. Here we report a case of metastatic AS affecting lower extremity in an elderly patient on a background of chronic lymphedema due to filariasis (Stewart-Treves syndrome).

Lower extremity injuries in snowboarders.

The popularity of skiing and snowboarding has increased tremendously over the past few decades. Whereas skiing has been an established winter sport for some time, snowboarding was created only in the 1960s. The great surge in popularity of snowboarding was sparked by its inclusion in the 1998 Winter Olympics. With increased participation in these winter activities has come a tremendous rise in injuries, despite equipment changes and promotion of safety behavior. Although participants in these sports share many common lower extremity injuries, the unique characteristics of skiing and snowboardi

Lower extremity injuries in snowboarding.

BACKGROUND: In snowboarding, the upper extremity is known as the most common injury site and little information is available for lower extremity injuries. Here, we aim to discuss lower extremity injuries during snowboarding. METHODS: We retrospectively analyzed the epidemiologic factors, injury types, and injury mechanisms for injured snowboarders (7,793 cases) between 2004-2005 and 2008-2009 seasons; information was gathered via questionnaires. Individuals were classified into a lower extremity injury group (961 cases) and a control group with other injuries (6,832 cases). RESULTS: The inci
