In a prospective study we investigated the results of 18 patients with Kienböck's disease stage II as defined by Decoulx, treated with transposition of the pedicled pisiform. In eight cases of minus variance of the ulna, a radius shortening osteotomy was performed. There was an average follow-up of 30 months, X-ray investigations were done every six months after operation. 17 patients had less pain, 14 patients showed an improved range of motion of 30 degrees. Magnetic resonance imaging proved vitalizing of the pisiforme in 16 cases.

Bilateral palmar instability of the wrist with subluxation between capitate and lunate bone. A stable wrist requires intact ligaments as well as a normal shape and size of carpal bones. While the reasons for bony changes are mainly posttraumatic, degenerative diseases and congenital laxity of ligaments as well as traumatic ruptures may cause an insufficiency of the ligaments. The following article deals with a theoretical discussion of carpal instability considering diagnosis, progress, development, and treatment of congenital bilateral palmar carpal instability which caused painful subluxatio

Between 1971 and 1982 tendon interposition arthroplasty in combination with denervation of the wrist joint was performed on 55 patients with either Kienböck's disease, irreducible lunate dislocations with or without scaphoid dislocation, or scaphoid non-union with a small proximal fragment. Follow-up was possible on 38 patients (69%). In two third of the cases good and very good results are achieved with this operative technique. The significant points were the arthrosis in the wrist joint and the complete obliteration with tendon material of the cavity left by the extirpated carpal bone. Ulna

Hydroxyapatite deposition disease (HADD) is a mostly uniarticular, self-limiting condition caused by deposition of hydroxyapatite (HA) crystals in tendons or in the peritendinous soft tissues. Commonly, the glenohumeral joint is affected. More rarely, the HA depot can be cause of a carpal tunnel syndrome due to an acute inflammatory reaction and space-occupying soft tissue oedema. We report a case of acute HA depot located at the volar site of the right wrist with affection of the deep flexor tendons and intraosseous migration into the lunate bone in a 50-year-old female. There are two main go

Kienböck's disease is a progressive condition characterized by avascular necrosis of the lunate, leading to carpal collapse, chronic pain, and functional impairment if left untreated. Management remains challenging due to variability in disease presentation and the absence of standardized treatment algorithms, particularly regarding surgical options. Among these, vascularized bone grafts (VBGs) have gained increasing attention for their ability to restore blood supply, promote osteogenesis, and preserve carpal architecture. This review provides a comprehensive synthesis of the anatomical basis

PURPOSE: Kienböck disease, characterized by avascular necrosis of the lunate bone, remains difficult to treat. This study developed a 3D-printed Bgh/GelMA/CMCS (BGC) hydrogel scaffold loaded with bone mesenchymal stem cells (MSC-EVs) for lunate bone repair.
METHOD: MSC-EVs were isolated using ultracentrifugation, and incorporated into BGC lunate-like scaffolds via 3D-printing. Scaffold properties, including surface morphology, biomechanical strength, degradation, and EV release, were investigated. Rabbit lunate defects were treated with these hydrogel scaffolds, and their structure and stabil

Fractures of the lunate bone are rare and usually treated by prolonged immobilization. We report a fracture of the dorsal pole of the lunate bone detaching the scapholunate ligament. The clinical presentation was equivalent to an acute disruption of this ligament. Percutaneous screw repair of the bone fragment was performed under wrist arthroscopy. The patient kept an analgesic splint for five days and early mobilization was provided. After 6 weeks, the patient had returned to his previous level of activity with normal wrist motion and full strength. Screw fixation under arthroscopy is minimal

Fourty patients with limited carpal fusion have been retrospectively reviewed. The aim of this study was to compare the results of four bone fusions (30 wrists) versus capitolunate fusion (11 wrists). Follow-up averaged 30 months with a range of 15-96 months. Twelve patients presented SLAC-wrist (scapho-lunate advanced collapse) and fourteen with SNAC-wrist (sapho-non union advanced collapse). There were seven cases of primitive wrist arthritis, one mid-carpal instability, one sequella of Fenton's syndrome, one Preiser's disease and two Kienbock's disease. All 40 patients were evaluated by the

OBJECTIVE: To explore the use of wrist arthroscopy for treatment of aseptic necrosis of lunate bone and its curative effect.
METHODS: From March 2012 to January 2016, 12 patients with aseptic necrosis of lunate bone underwent necrotic lunate bone extirpation assisted by wrist arthroscopy, and the scaphocapitate joint treated with fusion-stabilization were selected. Among these patients, 10 patients were male and two patients were female. The age of these patients ranged from 25 to 42 years old, with an average age of 32 years old. These lesions were located in the right hand in eight patients

OBJECTIVE: Type II lunate bones have an "extra" facet that articulates with the hamate bone, which frequently leads to arthritis. Because the only prior studies, to our knowledge, on this common and clinically significant variant involved dissection of cadavers, we performed an MR imaging study of the type II lunate.
MATERIALS AND METHODS: We retrospectively reviewed MR images obtained at 1.5 T of 186 wrists for frequency of type II lunates, size of the extra facet, amount of hamatolunate apposition, and presence of hamate subchondral edema. Of the 186 wrists, 28 also had correlation with fin

BACKGROUND AND AIM OF THE WORK: The authors show their experience about six patients suffering of chronic scapho-lunate (S-L) dissociation treated with the Cuenod method modified by Saffar-Romano.
METHODS: Clinical assessment was performed in all patients and compared before and after surgery at follow-up measuring pain value through the VAS, ROM of the wrist with a goniometer, grip strength by Jamar test, functional outcome with the Italian version of the Disability of Arm, Shoulder and Hand (DASH) score. Radiological assessment consisted in measuring S-L angle and classifying radio-carpic a

We report the treatment of three patients with stage 3 Kienböck disease that had advanced after radial shortening (n = 2) or lateral wedge osteotomy (n = 1). We shortened the capitate and fused the capitate and hamate, and applied a vascularised bone graft taken from the distal dorsal radius to the lunate. This was successful in all three patients.

BACKGROUND: At present, conventional operations are weak for pain relief and restoring the carpal joint function in the treatment of avascular necrosis of the lunate bone.
CASE REPORT: Mirror image 3-dimensional (3D)-printing technology has rarely been reported for the treatment of lunate bone necrosis, The use of mirror 3D-printed lunar bone replacement and bone cement technology in the treatment of stage IIIb lunar bone necrosis has been rarely reported in previous literature. Personalized customized 3D-printed prostheses have become an effective solution for solving many complex cases, and

UNLABELLED: We report the outcomes of an arthroscopic-assisted minimally invasive technique to reconstruct the scapho-lunate ligament using a bone-ligament-bone graft in 11 patients (11 wrists). The mean follow-up time was 29 months (range 20 to 38). The preoperative mean wrist flexion, extension, grip strength and patient-rated wrist evaluation score values were 61°, 54°, 115 N and 54, respectively. The postoperative mean values were 64°, 58°, 142 N and 15, respectively. There were no statistical differences between the pre- and postoperative wrist flexion and extension, whereas changes in gr

Kienböck's disease is idiopathic osteonecrosis of the lunate, leading to its fracture and collapse. This study compares internal and external bone micro-architecture of normal and fractured lunates (Kienböck's), by using high-resolution three-dimensional (3D) micro-computed tomography (micro-CT) on the whole bone of the two lunate types, and histology. Fractured Kienböck-diseased lunates were obtained from patients undergoing proximal-row-carpectomy, while normal cadaveric lunates served as controls. 3D-micro-CT-imaging of control lunates revealed an encircling cortex surrounding trabecular bo

Objective Kienbock's disease is an unusual disorder caused by osteonecrosis and the collapse of lunate bone which leads to pain and a chronic decrease in wrist function. The treatments in this disease aim to relieve pain and maintain wrist function and movement. Various surgical procedures have been recommended for the subjects with Kienbock's disease; however, the main question posed here is which of the selected procedures are more successful in relief of the pressure applied on lunate. Methods and Materials Computed tomography (CT) scan images of a normal subject were used to create a thr

Kienböck, a German radiologist, described avascular necrosis of the lunate (Kienböck's disease) in 1910. The epidemiology and etiology are not well-known and always has been debated. A negative ulnar variance is considered as a predisposing factor for Kienböck's disease by the majority of the authors. The treatment depends upon the stage of the disease at the time of presentation and diagnosis. Radial shortening and lengthening of the ulna are biomechanically satisfactory procedures because they increase the load sharing of the ulna and result in decompression of the lunate. Revascularization
