Ruptured Lung Abscess Into the Pleural Cavity: The Significance of Imaging and Medication Compliance.

A lung abscess is defined as a necrotizing infection with a pus-filled cavity. The infection can be primary in a previously normal lung or secondary to a pre-existing condition such as bronchial obstruction or immunosuppression by HIV or steroid use. Abscesses are more common in men aged 54-74 years, with a history of smoking, alcohol or other sedative use, and immunodeficiency. We present a case of a 65-year-old male with a past medical history of chronic obstructive pulmonary disease (COPD) who presented two months following a necrotizing pneumonia infection and was diagnosed with an acute p

Impact of corticosteroids on improving lung compliance in a patient with pulmonary microscopic polyangiitis.

Microscopic polyangiitis (MPA) is a small-vessel vasculitis characterised by systemic infiltration, with a primary focus on the renal and pulmonary systems. One of its more lethal pulmonary manifestations is diffuse alveolar haemorrhage (DAH), although the spectrum of lung pathology in MPA is vast and calls for immediate immunosuppressive therapy. Our case looks at an older woman initially presenting with MPA-induced rapid progressive glomerulonephritis. However, she gradually develops an array of vasculitis-related pulmonary insults that necessitate endotracheal intubation, and following a sh

Reduction of lung parenchyma compliance as a factor in the genesis of pleural effusion in neoplasms.

It is well known that bronchial obstruction causes an increase in negative pressure in the pleural cavity and is considered to be one of the factors giving rise to pleural fluid. We suggest that a reduction in the compliance of the lung parenchyma, whatever its cause (apart from bronchial obstruction), may also lead to an increase in negative pressure in the pleural cavity and may contribute, together with the other known factors, as an additional mechanism in the appearance of pleural fluid. We believe that this factor is valid in neoplastic pleurisy and may also apply in pleurisy, and possib

Relationship between Static Lung Compliance and Extubation Failure in Postoperative Cardiac Surgery Patients.

BACKGROUND: Static lung compliance, which is seriously affected during surgery, can lead to respiratory failure and extubation failure, which is little explored in the decision to extubate after cardiac surgery. OBJECTIVE: To evaluate static lung compliance in the postoperative period of cardiac surgery and relate its possible reduction to cases of extubation failure in patients submitted to the fast-track method of extubation. METHODS: Patients undergoing cardiac surgery using cardiopulmonary bypass (CPB) at a state university hospital admitted to the ICU under sedation and residual block w

Tidal lung hysteresis to interpret PEEP-induced changes in compliance in ARDS patients.

BACKGROUND: In ARDS, the PEEP level associated with the best respiratory system compliance is often selected; however, intra-tidal recruitment can increase compliance, falsely suggesting improvement in baseline mechanics. Tidal lung hysteresis increases with intra-tidal recruitment and can help interpreting changes in compliance. This study aims to assess tidal recruitment in ARDS patients and to test a combined approach, based on tidal hysteresis and compliance, to interpret decremental PEEP trials. METHODS: A decremental PEEP trial was performed in 38 COVID-19 moderate to severe ARDS patien

Exogenous surfactant improves lung compliance and oxygenation in adult rats with meconium aspiration.

We studied the effects of exogenous surfactant on lung function and morphology in an adult rat model of severe meconium aspiration syndrome. Animals ventilated with 100% oxygen received 4-6 ml of human meconium (25 mg/ml) intratracheally. After 30 min, lung-thorax compliance had decreased by > 30% and arterial PO2 was < 10 kPa. Animals were then treated with no material (MECO group), 0.9% NaCl (MECO-saline group), natural porcine surfactant (NPS group) at a dose of 100 mg/kg, or modified porcine surfactant at a dose of either 100 (MPS100 group) or 200 mg/kg (MPS200 group) and were ventilated f

Improved lung compliance following indomethacin therapy in premature infants with persistent ductus arteriosus.

In the course of a double-blind controlled study of intravenous indomethacin therapy in premature infants with patent ducts arteriosus (PDA), dynamic lung compliance (CL) was determined in 11 infants (six control, five indomethacin) who were not on assisted ventilation during the study period. The clinical, biochemical and laboratory data before the study were comparable between the groups. Following therapy with indomethacin there was a significant decrease in left atrial/aortic root ratio (LA/Ao), left ventricular end-diastolic dimension (LVEDD) on echocardiogram, and an increase in tidal vo

Lung compliance in newborns with patent ductus arteriosus before and after surgical ligation.

Pressure volume curves of the lungs were determined in 10 premature infants (mean gestational age 31.4 weeks, mean birth weight 1,260 g) before and after surgical ligation of a patent ductus arteriosus (PDA). 7 infants who had low compliance initially showed a significant improvement in lung compliance after surgery, while 3 infants whose compliance was close to normal before surgery had a decrease after ligation. In conclusion, a PDA with left to right shunt is frequently associated with a decrease in lung compliance that improves after ligation. Measurement of lung compliance in infants with

Prognostic Influence of Lung Compliance in Patients with Cardiogenic Shock and Invasive Mechanical Ventilation.

BACKGROUND: There is limited data regarding the influence of lung compliance on the outcome of patients with cardiogenic shock (CS). Thus, a registry study was conducted to assess the prognostic influence of lung compliance in invasively ventilated patients with CS. METHODS: Hospital records for consecutive invasively ventilated CS-patients from June 2019 to May 2021 were collected into a prospective registry. Our study evaluated the prognostic influence of lung compliance on 30-day all-cause mortality. Statistical analyses comprised t-tests, analysis of variance (ANOVA), Kruskal-Wallis-tests

Mechanical power normalized to lung-thorax compliance indicates weaning readiness in prolonged ventilated patients.

Since critical respiratory muscle workload is a significant determinant of weaning failure, applied mechanical power (MP) during artificial ventilation may serve for readiness testing before proceeding on a spontaneous breathing trial (SBT). Secondary analysis of a prospective, observational study in 130 prolonged ventilated, tracheotomized patients. Calculated MP's predictive SBT outcome performance was determined using the area under receiver operating characteristic curve (AUROC), measures derived from k-fold cross-validation (likelihood ratios, Matthew's correlation coefficient [MCC]), and

Cardiopulmonary bypass does not induce lung dysfunction after pulmonary thrombarterectomy: role of pulmonary compliance.

OBJECTIVES: Pulmonary endarterectomy is a heavy surgical procedure that is performed under cardiopulmonary bypass (CPB) and aimed to cure postembolic pulmonary hypertension. Reperfusion oedema is both the hallmark of successful surgical procedure and the most frequent postoperative complication. Post-CPB lung dysfunction was not mentioned in any report. We undertook a study to determine whether post-CPB lung dysfunction was present in these patients. METHODS: In a retrospective cohort study with matching on some baseline covariates, we selected 41 patients who had undergone pulmonary endarter

Level of Education Matters in Regard to Participants' Compliance With Screening in the National Lung Screening Trial.

PURPOSE: The success of cancer screening depends on patient adherence to the screening program. The purpose of this study is to assess how the level of education might affect participants' compliance with screening in the National Lung Screening Trial (NLST). MATERIALS AND METHODS: Secondary data analyses of the participants in the NLST were performed. A total of 50,104 participants were included in this study. Participants who enrolled in the trial but refused the initial screening were compared with those who completed the screening. A multivariate logistic regression model was used to asse

Implementation of a pulmonary thromboembolism prophylaxis program in Chinese lung surgery patients: compliance and effectiveness.

BACKGROUND: Most studies on prophylaxis against pulmonary embolism (PE) after lung surgery have come from the West. Whether such prophylactic programs can be successfully developed in China has not been fully studied. METHODS: A prospective observational trial included 581 Chinese patients receiving lung resection surgery between August 8 and September 12 of 2017. The Caprini score was assessed on the first postoperative day (POD1). For PE prophylaxis, patients with a low score (0-4, n=55) received early ambulation, and those with a high score (≥5, n=526) received early ambulation combined wi

The Response of Dorsal Lung Compliance to Prone Positioning Differs by Body Mass Index in ARDS.

BACKGROUND: Excess adipose tissue compresses dorsal lung regions that PEEP may not fully reverse in the supine position. Prone positioning, in combination with PEEP, can unload the lung, but whether regional responses differ by obesity is unknown. METHODS: We measured regional lung compliance using electrical impedance tomography in 14 mechanically ventilated subjects with COVID-19 ARDS (6 with BMI ≥35 kg/m2 and 8 with BMI <30 kg/m2). Measurements were taken during decremental PEEP titration in supine and prone positions, following recruitment maneuvers. Regional compliance was derived from p

The role of total static lung compliance in the management of severe ARDS unresponsive to conventional treatment.

A group of 36 patients with severe adult respiratory distress syndrome (ARDS) meeting previously established blood gas criteria (mortality rate 90%) became candidates for possible extracorporeal respiratory support [low frequency positive pressure ventilation with extracorporeal CO2 removal (LFPPV-ECCO2R)]. Before connecting the patients to bypass we first switched the patients from conventional mechanical ventilation with positive end expiratory pressure (PEEP) to pressure controlled inverted ratio ventilation (PC-IRV), and then when feasible, to spontaneous breathing with continuous positive

Premature monochorionic monoamniotic twins have lower lung compliance at birth than matched dichorionic diamniotic twins.

BACKGROUND: Premature infants are born with immature lungs that demonstrate abnormal pulmonary function with differences in passive respiratory system compliance and resistance, and functional residual capacity. To our knowledge, no studies have evaluated differences in neonatal pulmonary function based on the type of twin gestation, or chorionicity. Given the effect of chorionicity on outcomes, we aimed to study the effect of twin type, monochorionic monoamniotic (MCMA) vs dichorionic diamniotic (DCDA), on neonatal early pulmonary function tests. METHODS: In this prospective cohort study, 5

Mechanical power normalized to lung-thorax compliance predicts prolonged ventilation weaning failure: a prospective study.

BACKGROUND: Mechanical power (MP) of artificial ventilation, the energy transferred to the respiratory system, is a chief determinant of adequate oxygenation and decarboxylation. Calculated MP, the product of applied airway pressure and minute ventilation, may serve as an estimate of respiratory muscle workload when switching to spontaneous breathing. The aim of the study was to assess MP's discriminatory performance in predicting successful weaning from prolonged tracheostomy ventilation. Congenital diaphragmatic hernia: lung compliance after antenatal tracheal obstruction or surgical correction of the defect.

Fetal lambs with diaphragmatic herniae (CDH) created surgically at 73 days' gestation were subjected to three different forms of intrauterine correction: silastic patch correction of the diaphragmatic defect plus an abdominal patch at 101 days gestation; an intrathoracic "silo" at 101 days; and a tracheal "plug" obstruction at 101 or 129 days. At 143 days' gestation (term 145-149 days), the lambs were delivered by caesarean section and ventilated for 30 min before undergoing respiratory compliance measurements. These results were compared to those of normal lambs and animals with uncorrected h
