[Drug-induced interstitial lung diseases].

Drug-induced infiltrative lung disease may manifest as variable clinical radiological patterns, including subacute or chronic interstitial pneumonia, pulmonary fibrosis, eosinophilic pneumonia, organising pneumonia, pulmonary edema, or sarcoidosis. A large amount of drugs have been incriminated, including those used in cardiovascular diseases (amiodarone, statins and angiotensin converting enzyme inhibitors), antibiotics (minocycline, nitrofurantoin), most of anticancer drugs (and especially chemotherapy and chest radiation), treatment of rheumatoid arthritis, as well as more recent drugs. [Epidemiology of interstitial lung diseases].

Diffuse interstitial lung diseases (DILD) cover more than 200 conditions classified in 4 groups: secondary DILD; sarcoidosis; particular DILDs and idiopathic interstitial pneumonias. Overall, these diseases have a prevalence of 60-80/100,000 and an incidence around 30/100,000. Sarcoidosis and idiopathic pulmonary fibrosis are the 2 most frequent diseases, accounting together for more than 50% of all cases, followed by DILD related to connective tissue disease and to immunologic lung diseases. The incidence of different causes of DILD depends on epidemiologic factors: age, sex, race, smoking ha

Smoking-related interstitial lung diseases.

Interstitial lung diseases (ILDs) encompass diverse clinicopathological disease entities ranging from idiopathic interstitial pneumonia (IIP) to interstitial pneumonia accompanied by collagen vascular diseases and diseases related to smoking, drug reactions, occupational environments, infections, and malignancies. Our focus is on the smoking-related interstitial lung diseases. Specifically, we focus on reports suggesting that chronic smoking is deeply involved in the disease pathogenesis and on reports suggesting that chronic smoking may influence the clinical course in terms of either disease

[Environmental exposome and interstitial lung diseases].

Interstitial lung diseases (ILDs) encompass a heterogeneous group of pulmonary disorders whose prevalence is increasing, partly driven by the environmental exposome. Fine particulate matter (PM2.5), air pollutants, bioaerosols, and occupational exposures (such as silica, asbestos, and organic dust) play a major role in the initiation, progression, and exacerbations of ILDs. These agents induce oxidative stress, inflammation, and fibrotic activation. Conditions such as asbestosis, anthracosilicosis, idiopathic pulmonary fibrosis, sarcoidosis, and hypersensitivity pneumonitis illustrate the inte

Home monitoring in interstitial lung diseases.

The widespread use of smartphones and the internet has enabled self-monitoring and more hybrid-care models. The COVID-19 pandemic has further accelerated remote monitoring, including in the heterogenous and often vulnerable group of patients with interstitial lung diseases (ILDs). Home monitoring in ILD has the potential to improve access to specialist care, reduce the burden on health-care systems, improve quality of life for patients, identify acute and chronic disease worsening, guide treatment decisions, and simplify clinical trials. Home spirometry has been used in ILD for several years a

The genetics of interstitial lung diseases.

Interstitial lung diseases (ILDs) are a set of heterogeneous lung diseases characterised by inflammation and, in some cases, fibrosis. These lung conditions lead to dyspnoea, cough, abnormalities in gas exchange, restrictive physiology (characterised by decreased lung volumes), hypoxaemia and, if progressive, respiratory failure. In some cases, ILDs can be caused by systemic diseases or environmental exposures. The ability to treat or cure these ILDs varies based on the subtype and in many cases lung transplantation remains the only curative therapy. There is a growing body of evidence that bo

Acute exacerbations of interstitial lung diseases.

PURPOSE OF REVIEW: This review aims to highlight recent advances in pathogenesis, clinical presentation and treatment of interstitial lung diseases (ILDs). RECENT FINDINGS: Acute exacerbation is increasingly recognized as a major complication in the course of idiopathic pulmonary fibrosis. It is precipitated by a variety of intrinsic and extrinsic factors. Moreover, acute exacerbation is an apparently equally frequent event in hypersensitivity pneumonitis and collagen-vascular diseases associated ILDs, especially the rheumatoid pulmonary fibrosis. Treatment of acute exacerbations is unsatisfa

Inhalation therapies in interstitial lung diseases.

INTRODUCTION: Fibrosing interstitial lung diseases (ILDs), including Idiopathic Pulmonary Fibrosis (IPF) and Progressive Pulmonary Fibrosis (PPF), impose substantial clinical and societal burdens due to irreversible lung function decline, limited therapeutic options, and poor survival. Current oral antifibrotics, while effective in slowing progression, are hampered by severe gastrointestinal side effects and high discontinuation rates, emphasizing the need for better-tolerated treatments. AREAS COVERED: This review summarizes the therapeutic limitations of existing antifibrotics, emphasizing

Idiopathic interstitial lung diseases - treatment options.

Idiopathic interstitial pneumonias (IIPs) represent a heterogeneous group of rare lung diseases characterized by distinct radiological and histological phenotypes. The new 2025 classification provides a more detailed definition of individual entities, including the incorporation of idiopathic eosinophilic pneumonias and rare forms, without yet altering current therapeutic approaches. The most common form is idiopathic pulmonary fibrosis (IPF), where antifibrotic therapy with pirfenidone and nintedanib has been shown to improve prognosis. Recent clinical trials (FIBRONEER, TETON) suggest furthe

Management of Fibrosing Interstitial Lung Diseases.

A proportion of patients with interstitial lung diseases (ILDs), including the ILDs that are commonly associated with autoimmune diseases, develop a progressive fibrosing phenotype characterised by worsening of lung function, dyspnoea and quality of life, and early mortality. No drugs are approved for the treatment of ILDs other than idiopathic pulmonary fibrosis (IPF). At present, immunomodulatory medications are the mainstay of treatment for non-IPF ILDs. However, with the exception of systemic sclerosis-associated ILD, the evidence to suggest that immunosuppression may preserve lung functio

Macrophage polarization in interstitial lung diseases.

The role of bronchoalveolar lavage fluid (BALf) examination in differential diagnosis of interstitial lung diseases (ILD) was established. Currently, functional polarization into M1 (pro-inflammatory) and M2 (anti-inflammatory) subpopulations is emphasized. The aim of our study was to compare the proportion of M1 and M2 in BALf of patients with different ILD. BALf samples were collected from 75 ILD patients: sarcoidosis (SA, 36), hypersensitivity pneumonitis (HP, 10), non-specific interstitial pneumonia (NSIP, 8), idiopathic pulmonary fibrosis (IPF, 6) and other ILD (15). Phenotyping was perfo

[Radiation diagnosis of interstitial lung diseases].

The authors analyzed the results of complex radiation study of 1192 patients with various interstitial lung diseases (ILD). An algorithm of differential diagnosis of diffuse and disseminated lesions of the lung was derived by using the findings. The X-ray morphological principle underlies the algorithm. The level of structural lesions of the lung and tissue reactions and the presence of granuloma shadows are the governing criteria for evaluating the gross structure in the differential diagnostic complex. Great emphasis is laid on the significance of computed tomography (CT), high performance C

Clinical features of interstitial lung diseases.

OBJECTIVES: Interstitial lung diseases (ILD) are heterogeneous groups of disorders that involve the interstitium of the lung. Lung biopsy is mandatory in most cases of ILD for diagnosis. In Korea, a few clinical data about ILD were analyzed on the basis of pathologic proof. Thus, we analysed the clinical profiles of patients with ILD who had lung biopsy in a tertiary university hospital. METHODS: Clinical and pathologic data concerning 100 patients who had open lung biopsy (OLB) and/or transbronchial lung biopsy (TBLB) were prospectively analysed. Two patients were excluded because one patien

Lung transplantation for interstitial lung diseases and pulmonary hypertension.

Lung transplantation (LTx) is an established therapeutic option for patients with various end-stage lung diseases. Presently the worldwide procedural frequency is ∼ 3,200 per year. Unfortunately, the shortage of donor organs leads to approximately every sixth patient in Western countries dying before a donor organ is available. The most frequent underlying clinical indications for LTx are emphysema, pulmonary fibrosis, and cystic fibrosis. Worldwide, a recent analysis of all recipients reported that 23% had idiopathic pulmonary fibrosis (IPF) and 3% had pulmonary artery hypertension. In experi

Lung cancer and interstitial lung diseases: a systematic review.

Interstitial lung diseases (ILDs) represent a heterogeneous group of more than two hundred diseases of either known or unknown etiology with different pathogenesis and prognosis. Lung cancer, which is the major cause of cancer death in the developed countries, is mainly attributed to cigarette smoking and exposure to inhaled carcinogens. Different studies suggest a link between ILDs and lung cancer, through different pathogenetic mechanisms, such as inflammation, coagulation, dysregulated apoptosis, focal hypoxia, activation, and accumulation of myofibroblasts as well as extracellular matrix a

[Disseminated interstitial lung lesions in Lung Diseases Clinic patients].

The large number of diseases with interstitial lesions in the lungs needs numerous expensive and complicated examinations for differential diagnosis. These difficulties and increasing number of patients made us to look for possibilities to facilitate diagnosis. In 103 patients with diagnosed disease with disseminated lung lesions, the age and gender, vital capacity of the lungs, arterial blood gases pressure, the number of lymphocytes in peripheral blood were assessed. In sarcoidosis patients the subtypes of lymphocytes in peripheral blood and in bronchial washings were examined. It was establ
