[Lobar emphysema. Important differential diagnosis of obstructive lung diseases in childhood].

The lobar emphysema is characterised by a lobe-restricted overinflation combined with dislocation of the neighbouring organs. Surgical treatment is indicated because of the high mortality by waiting and conservative treatment. Differential diagnosis has to exclude other causes for overinflation as atelectasis with compensated emphysema, diaphragmatic hernia, and stenotic changing of the tracheobronchial tree and others. In the course of a retrospective study of 6350 thoracotomies (1978-1988) out of 198 patients with congenital malformations of the lungs 5 (2.5%) children less than 5 years were

Rheology predicts sputum eosinophilia in patients with muco-obstructive lung diseases.

BACKGROUND: Mucus is known to play a pathogenic role in muco-obstructive lung diseases, but little is known about the determinants of mucus rheology. The purpose of this study is to determine which sputum components influence sputum rheology in patients with muco-obstructive lung diseases. METHODS: We performed a cross sectional prospective cohort study. Spontaneous sputum was collected from consecutive patients with muco-obstructive lung diseases. Sputum rheology was assessed using the Rheomuco® rheometer (Rheonova, Grenoble); the elastic modulus G', viscous modulus G″, and the critical stre

Trypsin-Like Proteases and Their Role in Muco-Obstructive Lung Diseases.

Trypsin-like proteases (TLPs) belong to a family of serine enzymes with primary substrate specificities for the basic residues, lysine and arginine, in the P1 position. Whilst initially perceived as soluble enzymes that are extracellularly secreted, a number of novel TLPs that are anchored in the cell membrane have since been discovered. Muco-obstructive lung diseases (MucOLDs) are characterised by the accumulation of hyper-concentrated mucus in the small airways, leading to persistent inflammation, infection and dysregulated protease activity. Although neutrophilic serine proteases, particula

[The role of viruses in the pathogenesis of obstructive lung diseases].

Viral infections, especially recurrent viral infections in childhood are a well-known risk factor of bronchial hyperresponsiveness and the development of asthma and COPD. The aim of this review was description of the possible role of viruses in the pathogenesis of obstructive lung diseases. Even though a lot of mechanisms in which viruses induce asthma and obstructive lung disease remain unclear, the role of viruses seems to be undeniable. Respiratory syncytial virus--a common cause of childhood bronchiolitis--is a risk factor for the development of atopy, asthma and allergy. The risk is incre

The Impact of Lung Function on Sleep Monitoring in Obstructive Sleep Apnea Associated with Obstructive Lung Diseases: Insights from a Clinical Study.

Background/Objectives: Obstructive sleep apnea (OSA) and obstructive lung diseases (OLD) are common and interdependent respiratory disorders, where one condition may contribute to the development and worsening of the other (OLDOSA syndrome). The term OLDOSA syndrome includes two different conditions: Overlap syndrome (OVS: OSA + chronic obstructive pulmonary disease, COPD) and Alternative Overlap syndrome (aOVS: OSA + Asthma). Data on the interactions between lung function and respiratory monitoring during sleep in OLDOSA patients are few and controversial. Our study aims to evaluate the impac

The lung proteome in HIV-associated obstructive lung disease.

RATIONALE: Obstructive lung disease is increasingly common among persons living with HIV (PLWH). There are currently no validated biomarkers that identify individuals at risk of developing obstructive lung disease (OLD), and specific mechanisms contributing to HIV-associated OLD remain elusive, independent of smoking. We sought to identify biomarkers and biological pathways associated with OLD using a broad proteomic approach. METHODS: We performed tandem mass tagging and mass spectrometry (MS) analysis on bronchoalveolar lavage fluid samples from persons living with HIV with OLD (n=26) and w

Burden, clinical features, and outcomes of post-tuberculosis chronic obstructive lung diseases.

PURPOSE OF REVIEW: Post-tuberculosis lung disease (PTLD) is an increasingly recognized and debilitating consequence of pulmonary tuberculosis (PTB). In this review, we provide a comprehensive overview of PTLD with airflow obstruction (PTLD-AFO), focusing on its burden, pathophysiology, clinical manifestations, diagnostic methods, and management strategies. RECENT FINDINGS: The relationship between PTLD and airflow obstruction is complex and multifactorial. Approximately 60% of the patients with PTLD have some spirometric abnormality. Obstruction is documented in 18-22% of PTLD patients. The h

[The stomach and duodenum condition in patients with chronic obstructive lung diseases].

Examining the gastroduodenal mucosa (GDM) in 61 patients with chronic obstructive pulmonary diseases (COPD) showed changes in the areas studied in all cases. GDM atrophy increased with the severity of COPD. Helicobacter pylori are essential in developing gastric pathology in patients with mild bronchopulmonary disease. The severity of atrophic gastritis increases with the longer duration of COPD and with more severity of hypoxia and bronchial obstruction.

Lung function and onset of cardiometabolic diseases in the longitudinal Burden of Obstructive Lung Disease study.

INTRODUCTION: Previous population-based studies, mainly from high-income countries, have shown that a higher forced vital capacity (FVC) is associated with a lower risk of developing cardiometabolic diseases. The aim of this study was to assess the longitudinal association between spirometry measures and the onset of cardiometabolic diseases across sites in low-income, middle-income and high-income countries. METHODS: The study population comprised 5916 individuals from 15 countries participating in the Burden of Obstructive Lung Disease baseline and follow-up assessments. Postbronchodilator

Exacerbation Risk by Chronic Proton Pump Inhibitor Use in Obstructive Lung Diseases.

BACKGROUND: Previous studies have shown inconsistent results regarding the use of proton pump inhibitors (PPIs) and the risk on exacerbations in patients with chronic obstructive airway diseases (COADs). RESEARCH QUESTION: Is long-term PPI use associated with exacerbation risk? STUDY DESIGN AND METHODS: Using Belgian nationwide claims-based data of adult patients receiving long-term medication for COADs between 2017 and 2022, we investigated the association between PPI use and exacerbation risk using multivariable Cox models. Based on defined daily doses (DDDs) dispensed during the prior yea

Predictive factors of obstructive sleep apnoea in patients with fibrotic lung diseases.

BACKGROUND AND AIM: Several studies reported a high prevalence of Obstructive Sleep Apnoea (OSA) in patients with Idiopathic Pulmonary Fibrosis (IPF) or restrictive end-stage lung disease (ESLD). Besides the known risk factors for OSA like high Body Mass Index (BMI), reduced static and dynamic volumes for IPF patients and reduced DLCO and low minimal O2 saturation during sleep for ESLD patients were associated with higher Apnoea-Hypopnoea Index (AHI). The aim of our study was to determine potential predictive factors of OSA in patients with Fibrotic Lung Diseases (FLD). MATERIALS AND METHODS:

Airway obstruction in interstitial lung disease.

There is no question that most interstitial lung diseases result in structural alteration of the small airways as well as the alveoli. These structural changes of the airways produce airflow abnormalities that, depending on their extent and severity, are reflected in a variety of tests of pulmonary function. However, in most situations, obstructive lung disease rarely dominates the clinical picture. Airflow limitation may be the predominant defect in patients with Wegener's granulomatosis, lymphangioleiomyomatosis, and chronic eosinophilic pneumonia. Concomitant risk factors such as cigarette

Bronchodilator therapy in obstructive lung disease.

In recent years the management of patients with obstructive lung disease has been greatly simplified by the introduction of highly selective long-acting beta adrenergic agonists, slow release theophylline preparations and locally active anticholinergic agents. The beta adrenergic compounds are the most potent and many feel that they should be the first line therapy for acute episodes of airway obstruction. Methylxanthines continue to be used effectively in chronic disease and the improved bloodlevel time profiles of theophylline that are now available from sustained-release formulations provid

Management of chronic obstructive lung disease.

The management of chronic nonspecific lung disease (CNSLD) is a complex matter, influenced by different etiopathogenetic and physiopathological mechanisms. The most significant of these mechanisms have been revealed. Of the different function examinations the respiratory gas analysis appears to be particularly useful in obtaining detailed information. However, the choice of medication still remains of utmost importance. This paper presents a review of the selection criteria for antibiotics, bronchodilator drugs, theophylline derivatives, beta-sympathicomimetics, atropine corticoids, desensitiz

Ventilatory strategies in obstructive lung disease.

Chronic obstructive pulmonary disease (COPD) is characterized by expiratory flow limitation (EFL) due to progressive airflow obstruction. The various mechanisms that cause EFL are central to understanding the physiopathology of COPD. At the end of expiration, dynamic inflation may occur due to incomplete emptying the lungs. This "extra" volume increases the alveolar pressure at the end of the expiration, resulting in auto-positive end-expiratory pressure (PEEP) or PEEPi. Acute exacerbations of COPD may result in increased airway resistance and inspiratory effort, further leading to dynamic hyp

Acute exacerbations of obstructive lung disease.

Preview Because they are rapidly effective, inhaled beta2 agonists are the cornerstone of therapy in patients having an acute exacerbation of obstructive lung disease. In addition, inhaled anticholinergics, methylxanthines, systemic corticosteroids, and antibiotics are useful in appropriate situations. The authors describe assessment of these patients (who may be critically ill at presentation), provide recommendations for conventional therapy, and summarize alternative methods.

Obstructive lung disease in Behcet's syndrome.

Obstructive lung disease in two patients with Behcet's syndrome is presented. In both cases increased airways resistance, impaired PEF and FEV1.0 were found. In addition, diffusion capacity was slightly decreased in spite of enlarged total lung capacity and increased residual volumes. Both patients had mild hypoxaemia despite slight hyperventilation; a normal chest X-ray, and a decreased serum complement C3 level. Corticosteroids had a beneficial effect on VC in one of the two cases but failed to have any significant effect on the impaired ventilatory function in the other.
