Probable initial pulmonary lymphangioleiomyomatosis and mediastinal lymphangioleiomyoma.

A 68 year old woman was submitted to a mediastinal lymphangioleiomyoma resection found in a follow-up study of lower left lung resection due to bronchiectasis complicated by chylothorax. This led to a revaluation of the pulmonary specimen that revealed, in addition to inflammatory bronchiectasis, small spindle cell nodules in the lung parenchyma, similar to minute pulmonary meningothelial-like nodules, but with smooth muscle actin immunohistochemical positivity. The possibility of initial pulmonary development of lymphangioleiomyomatosis is discussed.

[Extra-pulmonary lymphangiomyomatosis presenting as an exudative enteropathy with a rapid fatal course].

We report a case of lymphangiomyomatosis (LAM) which occurred in a twenty six year old woman, presenting with protein losing enteropathy and pancreatic fibrosis due to lymphostasis. Despite having no pulmonary lesion, the patient died quickly. The proliferation of smooth muscle involved the mesenteric, peripancreatic lymphatic channels but also the submucosal lymphatic vessels of the duodenum. Only one other case of LAM presenting with protein losing enteropathy has been reported, in which there were no abnormalities in the intestinal tractus or in the pancreas.

Pulmonary lymphangioleiomyomatosis followed by a localized retroperitoneal lymphangioleiomyoma.

Lymphangioleiomyomatosis is a rare disease that affects females of reproductive age. Microscopically, it is characterized by abnormal proliferation of immature smooth muscle-like cells that grow diffusely in the lung. Extrapulmonary manifestations in the mediastinum, peritoneum and pelvic lymph nodes are uncommon. We here describe a patient who initially presented with pulmonary lymphangioleiomyomatosis and subsequently developed a localized retroperitoneal mass. Pathologic examination showed that the mass was a lymphangioleiomyoma. The result of the immunohistochemical study was the same as t

[Pulmonary lymphangioleiomyomatosis].

The second and third cases of this rare entity reported on in Czechoslovak literature were observed in two females, one 59, the other 24 years old. In both, there was typical diffuse pulmonary involvement with honeycomb transformation, recurrent spontaneous pneumothorax, and cor pulmonale decompensation. In the older patient, there was, at the same time, pronounced paraoesophageal node leiomyomatosis. In the younger one, there were associated renal and hepatic angiomyolipomas such as are found in tuberous sclerosis.

Bilateral ovarian lymphangioma (lymphangioleiomyoma).

Lymphangiomas of the ovary are rare and are usually unilateral. We present a 50-yr-old patient who presented with irregular bleeding secondary to multiple uterine leiomyomas who was found to have bilateral ovarian lymphangiomas. There was no evidence of pelvic lymphatic obstruction or of lymphadenopathy, and this appeared to exclude the possibility of acquired lymphangiectasia. The ovarian tumors were associated with a prominent smooth muscle cell component that partly surrounded many of the dilated vascular spaces to the extent that the diagnoses of lymphangioleiomyoma and lymphangioleiomyoma

[Anesthetic management of a patient with diffuse pulmonary hamartoangiomyomatosis associated with spontaneous pneumothorax].

We reported successful anesthetic management of a 34 year-old woman with diffuse pulmonary hamartoangiomyomatosis associated with spontaneous pneumothorax. She had recurrent pneumothorax and chest X-ray film showed reticulo-granular shadow, and chest CT revealed multiple bullae. Induced by thiopental and succinylcholine, anesthesia was maintained safely with enflurane in oxygen. Nitrous oxide was used after thoracotomy. This disease accompanies multiple bullae in the lung and some patients have complications such as tuberous sclerosis or hemangioma of the kidney. Therefore, we have to pay atte

[High resolution CT of interstitial lung diseases].

High-resolution CT images were assessed in 39 patients with confirmed interstitial lung disease. 22 patients with fibrosing alveolitis, 12 patients with lymphangitic spread of tumour and 5 patients with lymphangioleiomyomatosis of the lung were included. In fibrosing alveolitis the dominant features on HR-CT were honeycomb cysts with concomitant fibrosis in a predominantly peripheral subpleural distribution. In contrast to this the patients with LAM showed cystic lesions without any predominance--throughout the lung fields and without any fibrotic changes. In lymphangitic spread of tumour, pol

Lymphangioleiomyomatosis. A case report with an immunohistochemical study.

A case of pulmonary lymphangioleiomyomatosis is presented, which occurred in a 50-year-old woman suffering from severe dyspnea of 7 year's duration and with a previous history of diseases (polycystic ovaries and endometrial adenocarcinoma) usually related to hyperestrogenism. The patient showed no signs or symptoms of tuberous sclerosis, a condition often associated with pulmonary lymphangioleiomyomatosis. Both the radiological pattern and histological findings had previously been interpreted at other hospitals as consistent with either nodular interstitial fibrosis with emphysematous changes

Primary retroperitoneal PEComa: an incidental finding.

Perivascular epithelioid cell neoplasm (PEComa) is a rare type of tumour, and primary retroperitoneal PEComa is rarer still. Although pulmonary lymphangioleiomyomatosis (LAM), angiomyolipomas and clear cell 'sugar' tumours of the lung are well described, relatively little is known about other members of the PEComa family. We describe a case of an asymptomatic retroperitoneal PEComa, lymphangioleiomyoma type, which appeared in a previously healthy middle-aged woman as an incidental finding, in a CT scan performed in the context of spontaneous pneumothorax. The patient underwent surgical excisio

Lymphangiomatosis: a differential diagnosis of lytic lesions of the temporal bone.

OBJECTIVE: We report a histologically proved case of lymphangiomatosis of the skull, involving the temporal bone and presenting as multiple lytic bone lesions. METHOD: A case report and a review of the literature concerning lymphangiomatosis are presented. RESULTS: Lymphangiomatosis affecting bones is a rare disorder caused by a congenital malformation of the lymphatic system, resulting in diffuse proliferation of the lymphatic channels and involving bones, parenchymal organs and soft tissue. Involved bones show massive osteolysis and progressive, localised bone resorption. CONCLUSION: Lymp

Recurrent chylothorax in a woman with primary amenorrhoea.

A 47 year old woman receiving oestrogen replacement therapy for primary amenorrhoea presented with recurrent chylothorax. The clinical and radiological features of her illness were characteristic of pulmonary lymphangio-leiomyomatosis, a rare hormone-dependent disease. The condition was treated with pleurodesis and withdrawal of oestrogen therapy.

[Mediastinal-pulmonary lymphangioleiomyomatosis. The anatomicoclinical aspects].

The lymphangioleiomyomatosis is a rare disease which has a severe course and prognosis and occurs exclusively in females. The symptomatology is not characteristic and the radiological picture is that of a reticular and micronodular diffuse interstitial process. The morphological examination is the only which elucidates the diagnosis. Two cases are reported, the diagnosis of which was established postmortem. In the first case, the site was predominantly pulmonary; in the second case, in addition to the lung, the mediastinal lymph system was also strongly damaged. The morphological changes are c

Diagnostic approach to pulmonary lymphangioleiomyomatosis. Case report.

A diagnosis of lymphangioleiomyomatosis is to be considered in women with pleural chylous effusion when plain chest films show diffuse interstitial changes, possibly associated with cystic-like spaces and overinflation. The CT, showing the presence of a retroperitoneal mass, can ultimately help to outline the diagnosis of this syndrome as in the personal case observed in a postmenopausal woman. A presumptive pre-surgical biopsy diagnosis can be useful for studies of hormone-receptors and may be clinically relevant.

Angiomyolipoma of kidney.

Angiomyolipoma of the kidney is a clonal neoplasm, apparently part of a family of neoplasms derived from perivascular epithelioid cells. Early angiomyolipomas are small nodules composed of HMB-45-reactive spindle cells in the renal capsule, cortex, or medulla. Most angiomyolipomas are asymptomatic, and they are more common than previously appreciated, approaching 13 per 10,000 adults. They are much more prevalent in patients with tuberous sclerosis, where they often are accompanied by cysts and occasionally by renal cell carcinoma. That they are rarely diagnosed before puberty in patients with

Broncheoalveolar carcinoma associated with pulmonary lymphangioleiomyomatosis and Tuberous Sclerosis Complex: case report.

Lymphangioleiomyomatosis (LAM) is a rare disorder that predominantly affects the lung parenchyma of young women and it's characterized by pulmonary cyst. Tuberous sclerosis complex (TSC) is a rare genetic disorder presenting with hamartomas and neurologic symptoms. The two renal pathologies most commonly seen in TSC are angiomyolipomas and cysts; less commonly, TSC co-exist with polycystic kidney disease. In this report is described an uncommon case of a patient with broncheoalveolar carcinoma, pulmonary LAM and TSC with polycystic kidney disease.

Clinical and histological findings of generalised lymphangioleiomyomatosis are demonstrated. A 40-year old woman died within a few weeks because of massive pulmonary involvement. Tumour mass in the area of the left ovary together with enlarged lymph nodes suggested a metastasising ovarian cancer.

Multiple unilateral renal angiomyolipomas with regional lymphangioleiomyomatosis.

We present an unusual case of multiple unilateral renal angiomyolipomas occurring together with lymphangioleiomyomatosis of the regional lymph nodes. The association of these 2 hamartomatous lesions has been described only rarely but lends support to the concept that they represent a forme fruste of the tuberous sclerosis disease complex.

Early chylopneumothorax in a patient with pulmonary lymphangioleiomyomatosis.

A 50-year-old woman was admitted with the complaint of cough and dyspnea on exertion for the previous two months. A radiograph of the chest showed a right-sided hydropneumothorax, which was proven to be a chylous effusion by lipoprotein electrophoresis and was very refractory to tetracycline-pleurodesis. The chylopneumothorax was cured by ligation of the thoracic duct and surgical pleurodesis. After an open lung biopsy, lymphangioleiomyomatosis (LAM) was diagnosed histopathologically with smooth muscle nodules scattered throughout the lungs, obstructing the small airways, venules and lymphatic
