Chromoblastomycosis due to Fonsecaea pedrosoi: an old wine in a rare bottle.

Chromoblastomycosis is a chronic subcutaneous mycosis commonly caused by Fonsecaea, Phialophora, and Cladophialophora spp. Out of these, Fonsecaea pedrosoi is the most common etiological agent, implicated in 70%-90% of the cases reported worldwide. The histopathological diagnosis of chromoblastomycosis is based on visualization of medlar or sclerotic bodies in the tissue. These sclerotic bodies divide by planar division. Rarely, budding is seen in these sclerotic bodies. As this entity can be confused with phaeohyphomycosis, it is important to be aware of such a presentation also. We report tw

True multifood allergy in a 4-year-old child: a case study.

BACKGROUND: Food allergy is quite common in children, but it usually trends to improve with ageing. When an individual has specific IgE to a large variety of foods (multifood allergy) the clinical picture may be of remarkable severity and the avoidance of the offending foods may lead to severe dietary impairment. We describe a case of a child with ascertained multifood allergy. METHODS: The prick-by-prick testing with fresh foods and serum IgE tests were used to evaluate the patient's sensitivity to food allergens. The clinical effect of sensitizations were evaluated by DBPCFC which was carri

Chromoblastomycosis in a resident of a leprosarium.

Chromoblastomycosis is caused by dematiaceous fungi. It develops after inoculation of the organism into the skin. The lesion begins as a pink, scaly papule or warty growth. We report a case of chromoblastomycosis occurring in a multibacillary leprosy patient, who had already been released from treatment (RFT). The diagnosis was confirmed by the presence of sclerotic bodies (Medlar bodies/copper penny bodies). Systemic antifungal treatment has been found effective. The case is being reported in view of the association of two diseases and the dramatic clinical response to systemic treatment with

Lymphangitic chromoblastomycosis.

Chromoblastomycosis (CM), a chronic subcutaneous mycosis, is caused by several dematiaceous fungi, the most common being Fonsecaea pedrosoi. It usually occurs in the lower extremities following traumatic implantation of the organisms. We are reporting a case of chromoblastomycosis on the right lower limb in a sporotrichoid pattern caused by F. pedrosoi. The pattern was probably due to lymphatic spread that seems to be one of the rare presentations. The histopathology showed typical muriform or medlar bodies both intracellularly and extracellularly within the granuloma. Culture revealed sporula

Sinonasal chromoblastomycosis of the maxillary sinus.

A unique case of chromoblastomycosis, a chronic subcutaneous fungal infection, involving the maxillary sinus-a site rarely affected by this organism. The case highlights the diagnostic challenge posed by its non-specific clinical features, often mimicking chronic rhinosinusitis or neoplastic lesions. Histopathological examination revealed characteristic sclerotic (Medlar) bodies, confirming the diagnosis. Surgical debridement combined with prolonged antifungal therapy led to successful resolution. This report emphasises the importance of considering rare fungal aetiologies like chromoblastomyc

Caroli's syndrome leading to splenic artery aneurysm: a rare presentation.

Splenic artery aneurysm is a rare disorder (0.7%) that arises mainly as a sequlae to portal hypertension. Other causes of splenic artery aneurysm are atherosclerosis, arterial wall injury due to trauma, pancreatitis, and medial dysplasias of the wall. However, though Caroli's disease is known to cause portal hypertension, the rise of vascular pressure leading to aneurysm is not yet reported (extensive Medlar search failed to reveal any publication). Every effort should be made to diagnose this condition as early as possible because 25% of ruptured splenic aneurysms are fatal. A unique case of

Frostbite: review and current concepts.

BACKGROUND: A literature review was conducted to analyze developments in the epidemiology, pathogenesis, treatment, and prevention of frostbite injury. Increased participation in outdoor activities, as well as the epidemic of homelessness, makes knowledge of the treatment of frostbite crucial for physicians in both rural and urban areas. METHODS: A literature search, using the key words "frostbite" and "cold," was done using MEDLINE and Index Medicus. This search focused on the epidemiology, pathogenesis, treatment, and prevention of frostbite. RESULTS: Research done during the past 15 years

Syndrome--a changing concept.

Syndrome is one of the oldest terms in the medical vocabulary. Traditionally, the term has been used mainly as a designation for complex medical entities, such as multiple abnormalities, that are characterized by clusters of concurring symptoms, usually three or more. During the mid-twentieth century, the meaning and the use of the term were altered. First to take place was an attempt to eliminate physicians' names from syndrome nomenclature, resulting in a significant increase in the use of descriptive designations in proportion to eponyms. But the trend was counterbalanced by the creation of

An Atypical Case of Exophiala Bergeri Chromoblastomycosis in an Immunosuppromised Individual.

Chromoblastomycosis (CBM) is a rare chronic, granulomatous mycosis affecting the skin and subcutaneous tissue. It is a deep, slow growing, fungal infection that most often presents with epidermal changes that clinically appear as a hypertrophic and verrucous plaque. We describe an atypical presentation of an immunocompromised male patient, age 76 years, who presented with a 4-month history of painless subcutaneous firm nodules of the left fifth finger. The fungal culture grew Exophiala bergeri, and histopathology showed medlar bodies with lack of pseuodepitheliomatous hyperplasia or epidermal

Intracranial drainage versus extracranial shunt in the treatment of intracranial arachnoid cysts: a meta-analysis.

OBJECTIVE: To review the literature and analyze the efficacy and safety of two surgery procedures, intracranial drainage and extracranial shunt, for intracranial arachnoid cysts. METHODS: We searched the online Medlars, PubMed, and Cochrane Central electronic databases and collected studies of patients with intracranial arachnoid cysts treated with two surgical methods. RESULTS: The meta-analysis results shows that there were not statistically significant in clinical symptoms improvement, cyst reduction, the improvement of epilepsy, epidural hematoma, cerebrospinal fluid leak, and recurrence

Online databases that cover pharmaceutical topics are described and then illustrated with four sample searches. The online databases useful for searching pharmaceutical topics can be grouped into three categories: (1) databases devoted exclusively to drugs and the pharmacy profession, such as International Pharmaceutical Abstracts, Pharmaceutical News Index, and Ringdoc; (2) databases covering the clinical medicine literature, such as MEDLINE and its related files and Excerpta Medica; and (3) other scientific databases that include coverage of the pharmaceutical literature, such as BIOSIS Prev

A literature assessment of the use of miscellaneous topical agents, growth factors, and skin equivalents for the treatment of pressure ulcers.

BACKGROUND: Agents reviewed for this manuscript were both rare and popular topical treatments for pressure ulcers. OBJECTIVE: To review topical treatments for pressure ulcers and evaluate them based on available literature of controlled, blinded, and randomized trials. METHODS: A MEDLARS database (1966-1993) search and a thorough review of reference article lists of key articles produced over 100 manuscripts. Studies were considered for review if they were conducted on humans with chronic stage II to IV pressure ulcers, in a properly controlled and randomized fashion. RESULTS: The use of zi

Medical telecommunications--fundamental changes in the art and science of medicine.

The availability of easy to use microcomputers and medical information retrieval systems will result in fundamental and important changes in medical practice. The acquisition of information literacy and fluency will be mandatory techniques for the future practice of medicine and should be taught at the institutional level by faculty sophisticated in the use of information retrieval systems. The resulting improvements in the practice of medicine go beyond simple cost savings to fundamentally alter the ways patients and their disorders are looked upon and handled.

[Nizatidine: a meta-analytical study].

The medline database was applied to search all randomized clinical trials published from 1986 to 1992 concerning the use of Nizatidine, a powerful H2-antagonist, versus other treatments, including placebo, in the treatment of duodenal ulcer disease. Therapeutic efficacy of nizatidine is comparable to that of other H2-antagonists; with the use of meta-analysis, no significant differences were demonstrated, both in acute and long-term treatment of duodenal ulcer between nizatidine and ranitidine, but untoward side effects were less frequent with nizatidine than with ranitidine.

Perusing the literature: comparison of MEDLINE searching with a perinatal trials database.

The existence of a Register of Controlled Trials in Perinatal Medicine (National Perinatal Epidemiology Unit, Oxford, England) has offered an opportunity to assess the efficacy of online searching of MEDLINE, an example of a broad bibliographic database. Retrieval of all relevant randomized control trials (RCTs) in a given field is important in analyses in which results are pooled (meta-analyses). Reports of RCTs of prevention and treatment of neonatal hyperbilirubinemia and prevention of intraventricular hemorrhage (IVH) for the years 1966-1983 were sought in both the Register and MEDLINE fil

Case report: Fever- pneumonia- lymphadenectasis- osteolytic- subcutaneous nodule: Disseminated chromoblastomycosis caused by phialophora.

Chromoblastomycosis (CBM) is a chronic cutaneous and subcutaneous fungal infection caused by certain dematiaceous fungi (usually Fonsecaea, Phialophora, or Cladophialophora). Histologically, CBM is characterized by the presence of medlar bodies. However, the diagnosis is difficult because of the rarity of these pathognomonic presentations and the wide variety of presentations. Treatment of these infections is challenging as it lacks standardization. Herein, we report a case of chromoblastomycosis caused by Phialophora, in a 42-year-old immunocompetent male agriculturist from the humid and subt

Dietary treatments of obesity.

Dietary treatment of obesity is based on one or another of two premises: that the obese eat too much or that they eat the wrong things. The first is a tautology lacking explanatory power. The second is a meaningful and promising hypothesis but has yet to be effectively applied. At present, virtually all outpatient treatments of obesity, including behavior modification, are based on the first premise and consist of strategies for reducing the subject's caloric intake. Most such interventions produce short-term weight loss. Regain after the end of treatment remains the usual outcome. A survey of
