In vivo mammillary body volume deficits in amnesic and nonamnesic alcoholics.

BACKGROUND: Neuropathological studies use the presence of mammillary body (MB) pathology as a cardinal, diagnostic feature of Wernicke's encephalopathy (WE) in neuropsychiatric diseases, most notably alcoholism. Although Korsakoffs Syndrome (KS), which is marked behaviorally by dense global amnesia, is a typical sequela of WE, it remains controversial whether these two conditions necessarily co-occur and whether MB pathology is therefore a diagnostic requisite for KS. METHODS: We investigated these issues by examining, in vivo, 24 nonamnesic alcoholics (ALC), 5 amnesic alcoholics (KS), and 51

Physiological characteristics of neurons in the mammillary bodies align with topographical organization of subicular inputs.

The mammillary bodies (MBOs), a group of hypothalamic nuclei, play a pivotal role in memory formation and spatial navigation. They receive extensive inputs from the hippocampus through the fornix, but the physiological significance of these connections remains poorly understood. Damage to the MBOs is associated with various forms of anterograde amnesia. However, information about the physiological characteristics of the MBO is limited, primarily due to the limited number of studies that have directly monitored MBO activity along with population patterns of its upstream partners. Employing larg…

Collateral Projections Innervate the Mammillary Bodies and Retrosplenial Cortex: A New Category of Hippocampal Cells.

To understand the hippocampus, it is necessary to understand the subiculum. Unlike other hippocampal subfields, the subiculum projects to almost all distal hippocampal targets, highlighting its critical importance for external networks. The present studies, in male rats and mice, reveal a new category of dorsal subiculum neurons that innervate both the mammillary bodies (MBs) and the retrosplenial cortex (RSP). These bifurcating neurons comprise almost half of the hippocampal cells that project to RSP. The termination of these numerous collateral projections was visualized within the medial ma…

A dorsal subiculum-medial mammillary body pathway for spatial memory.

The dorsal subiculum (dSub) and medial mammillary body (MM) are essential components of the extended hippocampal system, heavily implicated in spatial memory processes. However, the exact function of the prominent dSub-to-MM projection has remained ambiguous. Here, integrating c-fos mapping, fiber photometry, optogenetic manipulations and behavioral paradigms, we found that the monosynaptic connection between dSub and MM, rather than collaterals to other regions, is not only involved in spatial memory retrieval but also highly vulnerable to perturbations on activity level in both directions. Mammillary Body Atrophy in Temporal Lobe Epilepsy With Hippocampal Sclerosis.

BACKGROUND AND PURPOSE: We aimed to determine 1) the frequency of mammillary body (MB) atrophy in patients with temporal lobe epilepsy (TLE) and hippocampal sclerosis (HS), 2) the clinical significance of MB atrophy, and 3) the association between MB atrophy and volume changes in other subcortical limbic structures. METHODS: We enrolled 69 patients with pathologically confirmed TLE with HS, who underwent a standard anterior temporal lobectomy, as well as 40 healthy controls. We used the FreeSurfer deep-learning tool of U-Net to obtain the volumes of the subcortical limbic structures, includin

Mammillary body injury in neonatal encephalopathy: a multicentre, retrospective study.

BACKGROUND: The mammillary bodies (MBs) have repeatedly been shown to be critical for memory, yet little is known about their involvement in numerous neurological conditions linked to memory impairments, including neonatal encephalopathy. METHODS: We implemented a multicentre retrospective study, assessing magnetic resonance scans of 219 infants with neonatal encephalopathy who had undergone hypothermia treatment in neonatal intensive care units located in the Netherlands and Italy. RESULTS: Abnormal MB signal was observed in ~40% of infants scanned; in half of these cases, the brain appeare…

Mammillary body and fornix injury in congenital central hypoventilation syndrome.

Congenital central hypoventilation syndrome (CCHS) is accompanied by reduced ventilatory sensitivity to CO2 and O2, respiratory drive failur … campus and anterior thalamus, are damaged in CCHS. Other memory formation circuitry, the fornix and mammillary bodies, have not been evaluated. We collected two high-resolution T1-weighted image series from 14 CCHS and 31 control subjects, using a 3.0-Tesla magnetic resonance imaging scanner. Image series were averaged and reoriented to a standard template; areas c …

Mammillary body and cerebellar shrinkage in chronic alcoholics with and without amnesia.

Mammillary body and cerebellar atrophy have been described as postmorten neuropathologic markers of Korsakoff's syndrome. This study examined whether shrinkage in the mammillary bodies and cerebellum is present consistently in amnesic chronic alcoholics during life and whether the degree of abnormality in these patients differs from that in nonamnesic alcoholic and healthy controls. The severity of shrinkage in the mammillary bodies, cerebellar hemispheres, and cerebellar vermis visualizable on MRI scans was rated on a three-point scale in 33 chronic nonamnesic alcoholics, 9 amnesic alcoholics…

Atrophy and sponginess of the mammillary bodies with neuronal sparing: not only inactive Wernicke's encephalopathy.

Two cases are reported in which old anoxic/ischemic damage to the Ammon's horns was associated with atrophy and sponginess of the mammillary bodies but with sparing of the neuronal perikarya. The lesions were unilateral in one case and bilateral in the other. The changes in the mammillary bodies closely resembled those seen in inactive Wernicke's encephalopathy but they were considered to be due to transneuronal atrophy following loss of hippocampal afferent fibres. Consequently, this condition may present a problem in diagnostic neuropathology. However, there was no reactive astrogliosis in t…

Hemosiderin Detection inside the Mammillary Bodies Using Quantitative Susceptibility Mapping on Patients with Wernicke-Korsakoff Syndrome.

Hemorrhage inside the mammillary bodies (MMBs) is known to be one of the findings of Wernicke encephalopathy. Brain MRI of two patients with Wernicke-Korsakoff syndrome (WKS) demonstrated high susceptibility values representing hemosiderin deposition in MMBs by using quantitative susceptibility mapping (QSM). QSM provided additional information of susceptibility values to susceptibility-weighted imaging in diagnosis of WKS.

Hippocampal, fornix, and mammillary body atrophy in patients with mesial temporal sclerosis.

This study investigates structural changes in the hippocampus, fornix, and mammillary bodies in patients with mesial temporal sclerosis (MTS), examining their relationship to clinical characteristics and association with memory performance. We conducted a retrospective study involving 104 patients with unilateral MTS and 47 age-matched healthy controls. Automatic segmentation of MRI scans was performed to calculate volumes, and atrophy was assessed relative to average values in the control group. Volume differences between patients and controls as well as associations of circuit atrophy with c…

Isolated Mammillary Body Infarct Causing Global Amnesia: A Case Report.

OBJECTIVES: Mamillary bodies play an important role in human memory and emotions. Vascular lesions causing an isolated mammillary body lesion without affecting the surrounding structures are very rare. METHODS: A 53-year-old male was brought to the emergency department with acute-onset memory problems suggestive of partial anterograde and retrograde amnesia. RESULTS: Magnetic resonance imaging revealed an isolated left mammillary body infarct sparing adjacent structures. CONCLUSION: Mamillary bodies play an intrinsic role in memory formation and retrieval rather than acting as relay-only st

Normative mammillary body volumes: From the neonatal period to young adult.

The mammillary bodies may be small, but they have an important role in encoding complex memories. Mammillary body pathology often occurs following thiamine deficiency but there is increasing evidence that the mammillary bodies are also compromised in other neurological conditions and in younger ages groups. For example, the mammillary bodies are frequently affected in neonates with hypoxic-ischemic encephalopathy. At present, there is no normative data for the mammillary bodies in younger groups making it difficult to identify abnormalities in neurological disorders. To address this, the prese…

Mammillary body regulates state-dependent fear by alternating cortical oscillations.

State-dependent memory describes a phenomenon that memory will be efficiently retrieved only when the brain state during retrieval matches the state during encoding. While a variety of psychoactive drugs, such as ethanol, cocaine, morphine and NMDA receptor antagonists, are able to induce state-dependent memory, the biological hallmark of brain state and neural mechanism of its regulation are still unknown. In this study, we found that MK-801 enhanced delta oscillations in awake mice, representing a drug-induced brain state, in which fear memory could only be successfully retrieved when the sa

Perioperative Mammillary Body Injury on MRI in Neonates with Severe Congenital Heart Disease.

BACKGROUND: Neonates with critical congenital heart disease (CCHD) are at risk for long-term cognitive impairments, including memory deficits. We aimed to assess the incidence and timing of mammillary body (MB) injury, critical memory structures, in neonates with CCHD and explore its association with cognitive outcome at 2 years of age. METHODS: We included neonates who underwent surgery with cardiopulmonary bypass in the first 6 weeks of life. Pre- and postoperative cerebral magnetic resonance imaging were reviewed for MB injury, defined as signal change on T1-, T2-, or diffusion-weighted im
