Nine males with sleep apnea DOES syndrome and three males with sleep apnea DIMS syndrome were treated with prosthetic mandibular advancement (PMA). The method uses a prosthesis, which is designed to advance the mandible 3-5 mm to prevent upper airway occlusion during sleep. The apnea index in the obstructive-type apnea and the percentage of time spent in obstructive apnea decreased significantly with PMA. Although the apnea index showed merely a tendency to decrease in central apnea (p < 0.1), the percentage of time spent in central apnea decreased significantly with PMA. A marked improvement

Background: Most Obstructive Sleep Apnoea (OSA) treatments use cross-sectional examination of the Upper Airways (UA) to determine decreasing gap and UA length. Surgery is detrimental to all OSA patients, stressing the need for better assessment. Objective: This study integrates Computational Fluid Dynamics (CFD) with physical model validation to improve OSA prediction and turbulence model accuracy and dependability. Methods: The k-omega SST turbulence model is used to analyse OSA using CFD. SLS is used to build a physical model of the UA for CFD simulations. The UA's physical model is then com

Obstructive sleep apnoea (OSA) is a disorder characterised by complete or partial occlusion of the upper airway during sleep. Muscles relax during sleeping and collapse into the airway, closing the throat and prohibiting air flowing into the lungs. Different solutions have been adopted to manage the pathology to improve the life quality of affected patients. Mandibular advancement devices (MADs) are proven to be a compliant and successful therapy in the forward repositioning of the mandible to increase the upper airway volume. However, this method has some long-term adverse events that may aff

PURPOSE: Numerous studies have shown that tooth size is an important key to ideal occlusion. Bolton (Angle Orthod 28:13, 1958; 48:504, 1962) described a constant ratio between the widths of the upper and lower teeth that must be present to achieve an optimal occlusion. The purpose of this study was to determine the incidence of Bolton discrepancies in patients with Class II malocclusion scheduled for mandibular advancement surgery.
PATIENTS AND METHODS: This study included 126 patients (40 male, 86 female) with Class II malocclusion who had at least a mandibular advancement as part of their s

Intraoral vertical ramus osteotomy (IVRO) is an effective surgical procedure that is used for the correction of mandibular prognathism. However, application of IVRO for mandibular advancement has been limited because of the instability of the proximal segments caused by the gap between the distal and proximal segments. We report a case of unilateral mandibular advancement with bilateral IVRO for the correction of facial asymmetry. This case shows possible application of bilateral IVRO for unilateral mandibular advancement without any means of fixation.

The purpose of this study was to compare the effects of a modified Herbst appliance (mHA) and a muscle relaxation appliance (MR) on nocturnal breathing and body movement activity in patients with obstructive sleep apnoea syndrome (OSAS). To increase the airway space posterior to the tongue base without severely affecting the craniomandibular joint, the mHA was adjusted to anchor the mandible at 50% of maximum protrusion. MR producing an occlusal coverage but no protrusion served as a control appliance. All-night static charge-sensitive bed (SCSB) and finger oximeter recordings were done to six

OBJECTIVE: Mandibular advancement devices (MADs) are a recognized alternative to continuous positive airway pressure (CPAP) for treating obstructive sleep apnea (OSA), though patient selection is still challenging. Tailored MAD guided through drug induced sleep endoscopy (DISE) with a mandibular titratable positioner (SAM) offers a stable and reproducible simulation of MAD effects during DISE, potentially enhancing outcomes after this DISE-SAM protocol. We aim to assess its positive predictive value (PPV) in patients treated with MAD by analyzing changes in AHI, minimum oxygen saturation, and

The aim of the present study was to evaluate whether the outcome of treatment using an intraoral mandibular advancement device in patients with obstructive sleep apnoea is associated with the mandibular morphology. The effects of the device on apnoeas and sleep were evaluated in 32 men with obstructive sleep apnoea in continuous polysomnographic sleep recordings including body position, during one night without the device and one night with it. Mandibular morphology variables were measured on cephalograms. The odds ratio for a supine apnoea-hypopnoea index of below 15 during treatment was 17 f

Severe obstructive sleep apnea (OSA) is linked to adverse cardiovascular outcomes. While continuous positive airway pressure (CPAP) is the standard treatment, mandibular advancement devices (MADs) offer an alternative. This substudy of a randomized trial compared the effectiveness of MADs versus CPAP on 24-h ambulatory blood pressure (BP), sleep-related quality of life, myocardial remodeling, ambulatory heart rhythm, and biomarkers in severe OSA. A total of 144 participants were randomized to MAD (n = 73) or CPAP (n = 71) for 12 mo. Median nightly usage was 5.4 (2.9-6.5) h for the MAD group (≥

Mandibular advancement splint (MAS) therapy emerged as an effective therapy for obstructive sleep apnoea (OSA) in the mid 1990s, and is now the leading treatment alternative for OSA. Since its inception, the field has seen a suite of revisions and advances in relation to design and customisation, fabrication, titration methods, response prediction models and the integration of data collection technology. This paper reviews these current and emerging innovations in MAS therapy and their impact upon sleep apnoea management.

INTRODUCTION: Mandibular advancement (MA) has emerged over the last decade as an alternative solution to nasal continuous airway pressure (nCPAP) for the treatment of obstructive sleep apnea syndrome (OSAS).
OBSERVATION: We report the case of a patient with history of chronic atrial fibrillation and moderate supine-dependent OSAS in whom central sleep apneas developed during treatment by a bi-bloc MA device. Central apneas increased with the level of MA and preferentially occurred in the supine position. We hypothesized that mouth opening under excessive mandibular advancement in supine posit

The cephalometric analysis of two groups: 30 sleep apnoea patients, and 30 age- and sex-matched control patients, established predictors for the orthopaedic mandibular advancement by means of the Esmarch device (ED) in treating obstructive sleep apnoea (OSA). Polysomnographic sleep and ventilation data gathered for each patient with and without mandibular advancement by means of the ED were compared. Cephalometric data and treatment efficacy for each patient were submitted to a regression analysis. Maximal efficacy is predicted by: 1. The combination of an orthognathic to prognathic maxilla (S

Ten of 12 adolescents treated with surgical mandibular advancement showed postsurgical mandibular growth, as indicated by an increase in the distance from condylion to pogonion. In all cases, the growth was expressed vertically relative to the cranial base, so that the chin did not come forward. None of the patients had significant increments of anterior maxillary growth postsurgically. Several patients had vertical maxillary growth, which was compensated by vertical mandibular growth, so that the anteroposterior position of the chin was maintained. Forward growth of the maxilla is minimal aft

This study examines the short-term stability of the mandible following mandibular advancement surgery in which skeletal suspension wires were used in addition to dental maxillomandibular fixation. Twenty adults underwent sagittal ramus osteotomies. No concomitant surgical procedures were performed. Maxillomandibular fixation consisted of wiring between the upper and lower orthodontic brackets and circummandibular wires connected to the piriform aperture or anterior nasal spine wires for eight weeks. Cephalograms were analyzed during this period to evaluate skeletal stability. A statistically i

STUDY OBJECTIVES: Catathrenia is a rare disease, classified as isolated symptoms and normal variants under sleep-related breathing disorders in the International Classification of Sleep Disorders, third edition. Because of its rarity, the research on its pathogenesis and treatment is insufficient. This study aimed to evaluate whether the mandibular advancement device (MAD) could be considered an alternative treatment trial and if so, to explore factors predicting its effectiveness.
METHODS: Thirty patients (12 men and 18 women, aged 16-67 years) with catathrenia participated in the study. The
