Mandibular coronoid fractures: treatment options.

Fractures of the coronoid process are uncommon and can easily be missed. The purpose of this study was to classify the fracture patterns and explore the treatment options. This retrospective study included 39 patients with fractures of the mandibular coronoid process. Treatment protocols were developed based on the time of fracture, degree of mouth opening, location of the coronoid fracture, types of fracture, and other concomitant fractures. All patients were followed up for 12-60 months. Sixteen patients underwent conservative management and four of these patients developed progressive trism

Treatment of mandibular-condylar fractures.

Particularly with true dislocation fractures, nonoperative treatment with maxillomandibular fixation followed by physiotherapeutic exercises leads to poor results, as was proved with axiography and clinical examinations. The main reason for this is the shortening and scarring of the condyloid process and the lack of function of the lateral pterygoid muscle. The condyle with its insertion of the muscle is usually displaced medially and anterially and nearly in touch with the origin on the pterygoid process so that protrusion by the muscle is no longer possible. The physiologic relationship of t

Prospective study of mandibular fractures.

A one-year prospective study of mandibular fractures that included 253 consecutive patients with 422 mandibular fractures was completed at Charity Hospital, New Orleans. Nine major categories were studied. Patients with multiple mandibular fractures seemed to have a fracture pattern. Complications associated with mandibular fractures are influenced more by location of the fracture or fractures and the cause than by a few days' delay in treatment, the specific technique in treatment, or extraction of teeth associated with the fracture. Thorough knowledge of occlusion and the masticatory apparat

Mandibular fractures: the problem patient.

Of 110 adult patients with 163 mandibular fractures treated at University of Pennsylvania Graduate Hospital from 1964 to 1974, 17% had a history of previous mandibular fractures. All patients' medical records and X-rays were reviewed. Because of the characteristics of this patient population, it seems well suited for discussion as a problem group. Mandibular fractures in this urban area are most commonly caused by blows of a fist and the patient is often intoxicated, which impedes diagnosis, delays treatment, and probably results in further complications from injuries. Patients frequently do n

[A toddler with a mandibular fracture].

BACKGROUND: Mandibular fractures are rare in children. Symptoms of these fractures include preauricular pain and swelling, trismus and pain when opening the mouth, and they are often preceded by a fall on the chin. If the diagnosis is missed they may have serious consequences for the growth and development of the jaw. CASE DESCRIPTION: A 3-year-old boy was referred with a swelling in his left cheek, fever and vomiting. Two days earlier he had fallen on his chin. At physical examination he had severe preauricular pain on palpation and a trismus. The patient was admitted because of suspicion of

The infected mandibular fracture.

The cause of most fracture gap infections is either a lack of or inadequate primary management. It is therefore imperative that jaw fractures be handled initially by an oral and maxillofacial surgeon. Surgical revision and absolute immobilization are essential in cases of osteomyelitis following mandibular fracture. In special cases, additional deposition of gentamycin-PMMA spheres and autologous osteoplasty may be necessary.

[Treatment of mandibular fracture].

Diagnosis of mandibular fracture is usually obvious, but may in unclear cases require several imagings. Even a fracture with good position will require follow-up. Treatment can usually be conducted systematically within the following day. Surgical treatment has become more common, with a fortunately rare need for intermaxillary fixation. Severe cases require several operations, e.g., osteotomies and bone transplantations. The patient's nutrition must be taken into consideration. Treatment of dental injuries is especially lengthy and involves expensive implants and prosthetics.

Management of mandibular fractures using biphasic pins and mandibular splints.

Often the otolaryngologist is asked to manage the patient with facial trauma. Mandibular fractures make up 20 to 39% of all facial trauma cases, and thus necessitate a diversified and complete set of management tools. We have selected two seldom discussed modes of therapy for the complicated jaw fracture to present in this paper: 1. external fixation of Joe-Morrison Hall biphasic pins, and 2. mandibular dental appliances--splints. The advantage of the biphasic pin apparatus is that the fracture can be repaired and stabilized at a point remote from the infected fracture site. The high risk pati

Neonatal mandibular fractures: a scoping review.

While uncommon, trauma-induced fractures to the neonatal mandible require unique consideration. A scoping review of neonatal mandibular fractures (NMF) was performed, in accordance with the PRISMA-ScR guidelines. The PubMed, Embase, Cochrane Library, Scopus, and Web of Science databases were comprehensively searched, covering the period 1990 to March 2025. All studies that reported NMF (age <28 days) were considered eligible. Due to the nature of the studies found, only a descriptive analysis was performed. Seven articles reporting seven cases of NMF were included. Four cases were birth trauma

Incidence neurosensory deficit in mandibular fractures.

Sensory disturbances in the inferior alveolar nerve (IAN) prior to treatment can be attributed to various factors, including the site, type of fracture, and fracture displacement. Therefore, it is of interest to assess the incidence of inferior alveolar nerve injuries associated with mandibular fractures before and after surgical treatment. Group A consisted of patients with inferior alveolar nerve paresthesia before treatment, while Group B consisted of patients with inferior alveolar nerve paresthesia after treatment. A significant difference was observed between the two groups, with a p-val

Postoperative wound dehiscence in mandibular fractures.

OBJECTIVE: To clarify the occurrence and causes of non-infection-related surgical wound dehiscence (SWD) in intraorally treated mandibular fractures. MATERIAL AND METHODS: Patients with one or two fractures of the dentate part of the mandible treated surgically via an intraoral approach were included in this retrospective study. The primary outcome variable was SWD. Associations between patient-, fracture- and surgery-related variables and SWD were evaluated. RESULTS: Altogether 232 patients with 270 mandibular angle, body, symphysis and/or parasymphysis fractures were included in the analys

A new classification of mandibular fractures.

There is no accepted method of reporting mandibular fracture that reflects incidence, treatment and outcome for individual cases. As most series include anatomical site only for all fractures, the aim was to establish a new method to report fractures based on a systematic review of the literature and an internal audit. The classification proposed is: Class I; condyle, II; angle, IIc; II+condyle, III; body/symphysis, IIIc; III+condyle, IV; multiple fractures not including condyle, IVc; IV+condyle, V; bilateral condyle±other fracture(s). A total of 10,971 adult and 914 paediatric cases were anal

Mandibular fractures associated with endosteal implants.

PURPOSE: The purpose of this study is to report four cases of mandibular fractures associated with endosteal implants and to discuss prevention and treatment of these types of fractures. DISCUSSION: To evaluate whether the patient's anatomy allows insertion of implants, radiological exams that demonstrate the height and the labial-lingual width are needed. To reduce the potential fracture problem, the mandible can be restrengthened with bone grafting techniques. The treatment of a fracture in an atrophic mandible is always a challenge because of the diminished central blood supply, the depres

Lag screw fixation of mandibular fractures.

The lag screw is the simplest means of rigid internal fixation. By virtue of its ability to compress bone fragments without the use of a bone plate, it has wide application and achieves great stability. This article presents techniques to apply lag screws to fractures of the mandibular symphysis, the mandibular body, and the mandibular angle. Results are reviewed in studies of 315 patients whom the author has treated with lag screw fixation. The advantages of the lag screw technique over bone-plate fixation, such as a more rapid application, obviation of the need to adapt a bone plate, minimal

Mandibular fractures in the pediatric patient.

Seventy-three patients were admitted to the Children's Hospital of Philadelphia (Pa) between January 1979 and June 1989 with a diagnosis of mandibular fracture. Data were obtained through a retrospective review of these cases. The cases were divided into three age groups that reflected the developing structure of the mandible and the maturation of the dentition. A trend toward a greater number of fractures and a predominance of males is shown with increasing age. Child abuse is a relatively frequent cause of fractures throughout all groups. Associated injuries are more common in young children
