PURPOSE: This study evaluated the functional and cosmetic results of mandibular basal osteotomy, introducing new indications and fixation alternatives.
MATERIALS AND METHODS: Ten patients 19 to 46 years old (mean, 32.3 years) were treated by orthognathic surgery, including mandibular basal osteotomy for correction of deficiency, excess, or asymmetry of the inferior mandibular border. An intraoral approach based on 3 incisions to the anterior and posterior mandible was used to maintain and protect soft tissues surrounding the mental nerve. After marking the bone inferior to the nerve, a recipr

With the development of craniofacial surgery, today sagittal split mandibular osteotomies are frequently used for the treatment of mandibular deformities with modifications and different fixation techniques. In clinics, 57 patients are evaluated in this study. Of these patients, 35 were male, 22 were female, and their ages were between 17 and 47 years (on average, it was 23 years). Thirty-seven of these patients were treated with bilateral sagittal split mandibular osteotomy (SSMO), 12 of them with unilateral SSMO, 6 of them with bilateral SSMO + Le Fort I osteotomy, and 2 of them with bilater

Following a brief literature review of treatment modalities for mandibular prognathism the authors propose a simplified method for vertical ramus osteotomy. The method was successfully used in 24 patients with mandibular prognathism (alone or in combination with open bite). Postoperative complications (suppuration of the operative wound) were found in 2 patients (8%), and postoperative recurrences--in 5 patients (20.8%). No complications, usually associated with sagittal and vertical intraoral osteotomies (hypoesthesia of the trigeminal nerve, Frey's syndrome, dissecting aneurysm of the intern

INTRODUCTION: A malformed mandible and an abnormally positioned mandibular foramen make it difficult to plan an ideal osteotomy line for mandibular distraction. In addition, there have been reports of such complications as nonunion, damage and stretch injury of the inferior alveolar nerve and tooth germ damage when conventional osteotomy or corticotomy are used for mandibular distraction. The authors utilized the original sagittal split ramus osteotomy for mandibular distraction.
PATIENTS AND METHODS: Five patients (three unilateral hemifacial microsomia, one bilateral hemifacial microsomia,

A 21-yr-old female suffering from osteogenesis imperfecta was anesthetized for correction of maxillary and mandibular deformities that had restricted her chewing. Preoperative assessment revealed a difficult intubation, restrictive lung disease secondary to bony deformities, and multiple repairs of fractures. Management of anesthesia for this operation--which is very rarely carried out in this disorder--is described along with a review of the problems of anesthesia associated with osteogenesis imperfecta.

Bilateral sagittal split osteotomy (BSSO) is a surgical procedure to treat mandibular malpositions. However, this surgery is associated with a risk of recurrence. Osteosynthesis techniques may be the cause of early recurrences. This study aims to demonstrate the efficiency and stability of osteosynthesis with two bicortical screws placed with a transjugal approach. We included patients who benefited from a BSSO alone, between May 2020 and August 2021. We performed the osteosynthesis using two bicortical screws placed with a transjugal approach. All the patients underwent lateral X-rays before

BACKGROUND: Dexmedetomidine may be suitable for postoperative sedation of patients with mandibular osteotomy.
METHODS: Twenty patients were sedated with dexmedetomidine (group D) employing loading infusion at 1.0 microg x kg(-) x hr(-1) and then continuous infusion at 0.7 mg x kg(-1) x hr(-1). Other twenty patients were sedated with midazolam 0.1 mg x kg(-1) (group C). Ramsay score was recorded at 3 hours and 12 hours after infusing sedative drugs. Then, we questioned patients, nurses and doctors.
RESULTS: Ramsay score in the group D was higher than that in the group C (P < 0.01). Hypotensio

The sagittal split ramus osteotomy is probably the most frequently used procedure for correction of mandibular skeletal dentofacial deformities. Despite numerous improvements in the technique in the 30 years since the procedure was introduced, a number of troublesome complications still occur. These include unfavorable fracture during surgery, paresthesia, and relapse. The purpose of this study was to determine where fusion of the buccal and lingual cortical plates occurs in the upper mandibular ramus, as it is thought that placement of the horizontal medial osteotomy above the point of fusion

Three-dimensional models created by milling machines and stereolithography from the three-dimensional computed tomograms (CTs) have become important in the diagnosis and preoperative planning of some craniomaxillofacial irregularities. In this study three-dimensional milled models were used to verify operative results after patients had undergone bilateral sagittal split osteotomies of the mandible. Fifteen models of patients with mandibular prognathism and 31 with retrognathism were created preoperatively and 6 weeks postoperatively to examine the changes of the osseous anatomical structures

PURPOSE: Mandibular corpus distraction is conventionally carried out by fixing miniaturized intraoral distractors to the body of the mandible after vertical osteotomy posterior to the last tooth. This procedure is not only technically difficult but also has high chances of damaging the inferior alveolar neurovascular bundle. The third molars are also required to be extracted. To overcome these problems, bilateral sagittal split ramus osteotomy was used and the distractor was fixed. The study was carried out to compare the results of the 2 osteotomies for mandibular corpus distraction.

Bone healing across the osteotomy gap after bilateral sagittal split osteotomy (BSSO) determines whether osteosynthesis material can be safely removed, but is traditionally assessed by subjective visual inspection of cone-beam computed tomography (CBCT) images. This study aimed to develop and validate a deep learning-based model for automated segmentation of BSSO osteotomy gaps on postoperative CBCT scans and to quantify the effect of training dataset size on segmentation performance. Seventy-two osteotomy gaps from 36 patients were manually segmented. Six patients (12 osteotomy sites) were ra

Mandibular symphysis segmentation for the correction of transverse deformities has been recently described in the literature, highlighting its potential in the management of dentofacial deformities. However, the use of osteosynthesis systems in this context has not yet been fully standardized, mainly due to the difficulty of obtaining standard plates with specific designs for these procedures. The aim of this report is to present a solution to this limitation using a standard plate that provides adequate spacing and 3-dimensional stability. This approach optimizes the osteosynthesis stage, red

Surgical navigation systems are extensively employed to enhance the accuracy and safety of traditional surgeries. However, current single-mode systems, using either optical navigation or electromagnetic (EM) navigation, suffer from either occlusion or magnetic interference when simultaneously tracking the mandible and surgical saw in mandibular angle osteotomy (MAO). To resolve this, we propose a hybrid optical and EM navigation system (HOENS), utilizing optical navigation to track the surgical saw and EM navigation to track the mandible. A distance-based automatic hybrid navigation strategy i

BACKGROUND: Anecdotal experience demonstrates the existence of patients with superiorly located carotid stenosis, neoplasms, or aneurysms where the mandible obstructs effective surgical access using standard techniques. As carotid pathology extends anatomically beyond the limits of standard operative technique, additional exposure becomes paramount to safely and effectively address the lesion. Double mandibular osteotomy (DMO) is one of several techniques to obtain additional exposure to high-carotid pathology; however, there is no large series to address the outcomes of patients undergoing th

INTRODUCTION: Mandibular incisor axis correction can be challenging in non-extraction orthodontic treatment planning. Mandibular anterior segmental subapical osteotomy (MASSO) is a surgical approach, which can be used to reposition incisor axis, improve occlusal function and enhance the soft tissue profile.
METHODS: Twenty-eight patients, mean age 22.6 years (16-39, SD=6.69), were treated for MASSO for correction of Class II dentoskeletal deformity, in addition to other orthognathic procedures. Retrospective analysis was performed on lateral cephalogram measurements, at the end of presurgical

The purpose of this study was to investigate the growth and remodeling changes of the mandible following superior anterior surgical repositioning (Le Fort I) of the maxilla in adolescent Macaca fascicularis monkeys. Eight adolescent monkeys served as controls, and seven monkeys underwent surgical procedures. All monkeys received tantalum implants on both sides of certain facial bones for stereometric and conventional cephalometry. The animals were followed up to 24 months postoperatively. Analysis of cephalometric head films taken at monthly intervals shows that both the maxillas and the mandi

Intraoral vertical ramus osteotomy (IVRO) is an effective surgical procedure that is used for the correction of mandibular prognathism. However, application of IVRO for mandibular advancement has been limited because of the instability of the proximal segments caused by the gap between the distal and proximal segments. We report a case of unilateral mandibular advancement with bilateral IVRO for the correction of facial asymmetry. This case shows possible application of bilateral IVRO for unilateral mandibular advancement without any means of fixation.
