Recurrent or chronic dislocation of the temporomandibular joint is relatively rare and often results in facial asymmetry, impairment of function, and discomfort. Although manual reduction is the primary choice of treatment, patients presenting with recurrent or prolonged dislocations require conservative and surgical methods to limit forward movement of the mandibular condyle. This clinical report presents a 75-year-old woman with severe mandibular deviation and subsequent facial asymmetry caused by a chronic unilateral temporomandibular joint dislocation that was treated with a mandibular gui

Large mandibular defects in children are an uncommon but challenging problem for surgeons to solve. The time-honored options of autologous bone grafts are seldom a viable option, as suitable donor sites are unavailable. Osteoinductive morphogens may yet provide a solution in these cases. A large mandibular tumor in a child 10 years of age necessitated the resection of the entire dentate portion of the mandible. The defect was reconstructed at a second stage with a composite graft of human transforming growth factor-β3 (hTGF-β3), human demineralized bone matrix, and 12 g of autologous bone harv

INTRODUCTION: Segmental mandibular defects present complex reconstructive challenges and are associated with significant functional and aesthetic morbidity. While vascularized bone flaps remain the standard for reconstruction, they are associated with donor-site morbidity, prolonged operative times, and limitations in predictable dental rehabilitation. This proof-of-concept study evaluated the mechanical feasibility, in a synthetic bench-top model, of a patient-specific, 3D-printed titanium mandibular prosthesis incorporating conventional dental implants for both dental rehabilitation and biol

The aim of this study was to perform a systematic review of the literature on the temporomandibular joint (TMJ) prosthesis as a treatment option after mandibular condyle fracture. Three databases were searched (PubMed, Embase, Cochrane Library) and 2670 unique papers were identified. A total of 337 studies were included (121 case reports, 89 case series, and 127 cohort/clinical studies). In total 14,396 patients and 21,560 prostheses were described. Of the 127 cohort or clinical studies, 100 (79%) reported inclusion criteria, 54 (43%) reported exclusion criteria, and 96 (76%) reported the incl

The authors aim to present an updated protocol for mandibular reconstruction in nongrowing patients with Pruzansky/Kaban type IIb/III congenital craniofacial microsomia with customized temporomandibular joint (TMJ) prosthesis to reduce facial nerve (FN) damage and improve surgical accuracy. This is illustrated (using 3 cases) and is based on preoperative mapping of the FN using MRI for better virtual surgical planning of custom-made TMJ prosthesis. Intraoperative FN mapping and monitoring, as well as verification of the final result with intraoperative cone-beam computed tomography (CBCT) and

The conventional approach for replacing congenitally missing mandibular lateral incisors dictates the placement of either a conventional porcelain-fused-to-metal (PFM) bridge, Maryland bridge, or fiber-reinforced composite veneer bridge. However, several appearance-related disadvantages have been reported in the use of a prosthesis which incorporates a metal substructure. To address these limitations, metal-free restorative alternatives have been recently developed to expand the clinical options when fabrication of these prostheses is indicated. The learning objective of this article is to pre

BACKGROUND: Ankylosis of the temporomandibular joint (TMJ) is a pathological disorder which results in patients' limited or even complete failure of mouth opening. When TMJ ankylosis occurs during the growing age, moderate to severe micrognathia might be the proposed scenario of such cases, accompanied by obstructive sleep apnea and hypopnea syndrome (OSAHS). TMJ reconstruction using total TMJ prostheses with mandibular advancement (MA) can simultaneously improve the function and aesthetic profile of such patients. The purpose of this study was to determine whether the upper airway can be impr

OBJECTIVES: Ameloblastoma (AB) are aggressive odontogenic tumors, often treated with wide marginal resection and concurrent reconstruction. Extended total temporomandibular joint replacement (eTMJR) prosthesis is a viable treatment option for reconstruction of large defects caused by resection of AB tumors. The objective of this study was to systematically review the utilization and effectiveness of eTMJR prosthesis in treating patients with AB tumors.
METHODS: A detail search in Web of Science, Medline/PubMed, Google Scholar and Embase identified studies reporting AB patients who underwent i

Virtual surgical planning for CAD/CAM mandibular reconstruction by titanium prosthesis was recently reported for resected cases. Even if some advantages are evident, difficulties that may arise for TMJ function after reconstruction originate from prosthesis contamination through oral mucosa dehiscence. In these two cases reported of mandibular reconstruction after resection of ameloblastoma by custom-made CAD/CAM titanium prosthesis, the procedures were aimed to preserve the TMJ glenoid cavity and articular disc avoiding functional problems for hemi-mandibular resections that included the cond

OBJECTIVES: An alternative option was proposed regarding the prosthetic rehabilitation of a fully edentulous mandible using only four implants. The aim was to reduce the stiffness of the prosthetic framework. To that end, the alternative option consists of a prosthetic framework optimized with a porous structure. Mechanical differences were analyzed between non-prosthetic mandible and restored mandible either with a conventional bulk titanium framework or with this alternative option. The non-prosthetic mandible corresponds to the mandible in its natural state, without prosthesis. This will be

Patients undergoing mandibular resection often have facial asymmetry and cosmetic disfigurement, which include a retruded, deviated mandible, motor and sensory deficiencies, and abnormal intermaxillary relationship and occlusion. An implant-supported fixed prosthesis can be an optimal treatment modality. However, there is a problem in creating a repeatable, stable plane of occlusion and maxillomandibular relationship. This report describes the treatment sequence and considerations to rehabilitate a patient undergoing mandibular resection and radiotherapy with an implant-supported fixed prosthe

Patients who undergo hemimandibulectomy have multiple functional impairments, and early initiation of guidance therapy has been associated with an improved definitive occlusal relationship. This article describes a digital workflow to determine the angle and form the shape of the flange of the mandibular guide plane prosthesis (MGPP), and to create an MGPP before mandibular resection surgery. A virtual articulator was used to achieve the movements of the maxilla relative to the mandible, and the shape of the MGPP was adjusted by dynamic occlusion. The MGPP and mandibular case were printed usin

PATIENTS: This report describes the digital workflow for the design and fabrication of a telescopic prosthesis that incorporates a polyetherketoneketone (PEKK) primary crown and PEKK framework to restore multiple teeth in the mandible of a 56-year-old female patient. The process includes intraoral scanning, computer-aided design, digital milling of the telescopic crown and PEKK framework, and 3D printing of the casts. Good retention, mastication, aesthetics, and biological outcomes are observed in the four-year follow-up.
DISCUSSION: Two clinical outcomes are observed. (1) A PEKK telescopic p

BACKGROUND: Accurate occlusal registration is critical for successful dental implant treatments. Traditional mechanical articulation provides insight into occlusal relationships but cannot replicate complex mandibular movements accurately. CAD/CAM advancements, including virtual articulators, enable dynamic occlusion analysis using either average-value settings or individualized mandibular movement data, depending on the system used. Patient Specific Motion (PSM) technology, using intraoral scanners to record actual mandibular movements, offers enhanced functional occlusal design. Despite thes

BackgroundConventionally, vector guidance for internal distractor placement during cleft maxillary distraction osteogenesis (DO) is done using vector guidance splint. Vector guidance splint cannot be fabricated if occlusion is not decompensated for model surgery.SolutionUsing mandibular occlusal plane as anatomical reference, vector guidance oral prosthesis is fabricated to guide distractor placement and vector determination, in cases where model surgery is not possible. What we do that is newVector guidance oral prosthesis is a new prototype produced to guide maxillary DO without the need of m

OBJECTIVE: This study aimed to compare the biomechanics of implant prostheses and peri-implant bone among 6 different mandibular reconstruction models based on patient data involving the use of an upper free-end double-barrel fibula.
METHODS: This study was an observational study. Five models were reconstructed using fibular-supported and implant-supported partial dentures. Two of these models were double-barrel fibula models with different gaps between the upper and lower segments. The other 2 were single-layered fibular models (the fibula was placed at the inferior or superior mandibular bo

Carcinomas of the mandibular buccogingival region being aggressive rapidly destroy the supporting bone and adjacent soft tissues, making it mandatory for the oncosurgeon to resect substantial amounts of involved hard and soft tissues. Segmental mandibulectomy -followed by immediate bony and soft tissue reconstruction is the most acceptable approach. However, immediate reconstruction may not always be feasible or possible due to high recurrences potential of lesion, inaccessible residual pathology, or systemic -conditions that pose a risk for graft inosculation. In certain cases, where large po
