First report of presumed parasitic keratitis in Indians from the Brazilian Amazon.

PURPOSE: To describe corneal disorders in patients with systemic parasitic disease. METHODS: Cross-sectional study consisted of ophthalmologic examinations, peripheral blood tests and skin biopsies in 496 subjects (395 Indians and 101 non-indians) from the upper Negro River, Brazilian Amazon. RESULTS: Mansonella ozzardi was detected in the blood of 140 (28.23%) subjects. Seventy-six (55.07%) of them also presented multiple nummular opaque superficial lesions in the cornea. None of the examined patients presented onchocercal-related skin lesions or positive skin biopsies for filaria. CONCLUS

Mansonella ozzardi in the municipality of Tefé, Amazonas, Brazil, 60 years after the first report: an epidemiologic study.

The prevalence of mansonelliasis was studied in the municipality of Tefé, state of Amazonas, Brazil. The prevalence (thick blood smear method) was 13.6% (147/1,078), higher in the Solimões River region (16.3%) than in the Tefé River region (6.3%). In the sampled communities in the Solimões River region, a higher density of cases was observed, as indicated by a kernel analysis (odds ratio 0.34; 95% confidence interval: 0.20-0.57). Males had a higher prevalence (χ2 = 31.292, p < 0.001) than women. Mansonella ozzardi prevalence was higher in retirees and farmers (28.9% and 27%, respectively). Pre

Sustained clearance of Mansonella ozzardi infection after treatment with ivermectin in the Brazilian Amazon.

Therapy for mansonelliasis is challenging because there is no standard drug recommended for its treatment. This non-randomized study was conducted to evaluate the effectiveness of a single dose of 0.15 mg/kg of ivermectin to reduce Mansonella ozzardi microfilaraemia in infected persons. A total of 74 patients were studied within the municipality of Lábrea, which is located in Amazonas State, Brazil. The patients were treated with ivermectin after detection of the parasite by blood examination. Significant microfilaraemia reduction was observed and its residual effect was maintained for at leas

[Contribution of the PCR assay to the diagnosis of Mansonella ozzardi in endemic areas of Argentina].

Mansonella ozzardi is a tissue-dwelling parasitic nematode, the causative agent of mansonelliasis in almost all Latin American countries. It has been described along the Argentine Yungas region. The microscopic diagnosis can yield false-negative test results at low microfilaremia levels. The aim of this study was to optimize the molecular diagnostic technique and compare it with the Knott's method and standard blood smear procedures (thin blood films and thick smears) in 92 blood samples of individuals from an endemic area. The PCR technique followed by the sequencing of the amplified product

Development of Mansonella ozzardi in Simulium amazonicum, S. argentiscutum, and Culicoides insinuatus from Amazonas, Colombia.

Simulium amazonicum, S. argentiscutum and Culicoides insinuatus were collected from the Amazon region of Colombia after having blood-fed on volunteers naturally infected with Mansonella ozzardi. Dissection of the specimens revealed that these species supported the development of the microfilaria of M. ozzardi to the infective stage. Infective larvae were obtained from Simulium within 7 days at temperatures ranging from 23-30 degrees C. S. argentiscutum ingested more microfilariae and had a correspondingly higher experimental infection rate. Three of 31 C. insinuatus dissected beyond day 5 post

Development of Mansonella ozzardi in a black fly species of the Simulium sanguineum group from Eastern Vaupés, Colombia.

Development of the microfilaria of Mansonella ozzardi to the infective stage in a species of the Simulium sanguineum group from the Mitú area, Comisaría del Vaupés, Colombia is described. Development was synchronous, and by day 6 third-stage larvae were observed in the head of flies which had fed on two naturally infected volunteers. Simulium biting activity was high from January-March, during the dry season. Only 261 blood-fed Culicoides were collected from an infected volunteer; 40% of 43 dissected on day 0 had ingested microfilariae. However, only one of 129 C. caprilesi specimens dissected

No evidence of Wolbachia endosymbiosis with Loa loa and Mansonella perstans.

Endosymbiotic Wolbachia bacteria from different filarial species, including major pathogens of humans such as Wuchereria bancrofti, Brugia malayi and Onchocerca volvulus, seem to play an important role in the development, viability and fertility of these worms. Wolbachia trigger inflammatory host responses as well as adverse reactions against standard treatment regimens and are therefore under investigation as novel treatment targets. We investigated whether Wolbachia are also endosymbiotic in Loa loa and Mansonella perstans. In both male and female adult L. loa, we found no evidence of bacter

Efficacy of ivermectin in the treatment of concomitant Mansonella perstans infections in onchocerciasis patients.

As part of an ivermectin dose-ranging study of onchocerciasis patients in Togo, 55 onchocerciasis patients with concomitant mansonelliasis received single oral doses either of ivermectin (100 to 200 micrograms/kg body weight) or placebo. As expected, Onchocerca volvulus microfilariae in the skin were greatly reduced in number soon after drug treatment, but microfilariae of Mansonella perstans reacted differently. Microfilarial densities of M. perstans were assessed with a filtration technique both before, and 4 times after, treatment. In untreated patients microfilarial densities were stable u

[The efficacy and tolerance of ivermectin (Mectizan) prescribed for the patient with multiple filarial infections (Loa loa/Onchocerca and/or M. perstans)].

Administration of ivermectin (Mectizan) in Loa loa filariasis induces a significant decrease of microfilaria load within 3 to 15 days. The excellent tolerability of the drug in patients infested simultaneously by O. volvulus and Loa loa allows to use ivermectin in mass treatments in geographical areas where the same patients can be affected by several filariasis. There is almost no efficacy against Mansonella perstans. A second administration one month after the first one in Loa loa affected patients has not led to a complete disappearance of microfilaria; the tolerability has still been good.

Culicoides variipennis, a laboratory vector for the amazon form of Mansonella ozzardi.

The colonized midge, Culicoides variipennis, is shown to be a competent laboratory host for the Amazon form of Mansonella ozzardi. It raises the possibility that certain Culicoides spp. in the Amazon region may support development of the parasite and possibly play a role in the natural transmission cycle.

Inflammatory and regulatory CCL and CXCL chemokine and cytokine cellular responses in patients with patent Mansonella perstans filariasis.

Mansonella perstans (Mp) filariasis is present in large populations in sub-Saharan Africa, and to what extent patent Mp infection modulates the expression of immunity in patients, notably their cellular cytokine and chemokine response profile, remains not well known. We studied the spontaneous and inducible cellular production of chemokines (C-X-C motif) ligand 9 (CXCL9) [monokine induced by interferon (IFN)-γ (MIG)], CXCL-10 [inducible protein (IP)-10], chemokine (C-C motif) ligand 24 (CCL24) (eotaxin-2), CCL22 [macrophage-derived chemokine (MDC)], CCL13 [monocyte chemotactic protein-4 (MCP-4

Mansonella ozzardi: a neglected New World filarial nematode.

Mansonella ozzardi (Nematoda: Onchocercidae) is an understudied filarial nematode, originally described by Patrick Manson in 1897, that can be transmitted by two families of dipteran vectors, biting midges (most of them members of the genus Culicoides) and black flies (genus Simulium). With a patchy geographic distribution from southern Mexico to northwestern Argentina, human infection with M. ozzardi is highly prevalent in some of the Caribbean islands, along riverine communities in the Amazon Basin, and on both sides of the border between Bolivia and Argentina. There is no clinical entity un

Environmental factors and the distribution of mansonelliases in southern Venezuela.

The distribution of mansonelliases and their relation to various quantitative criteria were determined through the study of 1,057 subjects in 17 localities in ten regions of Amazonas State and Bolívar State. The total prevalence among the blood samples, determined through the Knott technique, was 18.54%. 11.26% were parasited by Mansonella perstans, 9.93% by Mansonella ozzardi, and 2.63% by both species. The average of microfilaremia was 48.19 mf/mL of blood in M. perstans and 13.79 mf/mL in M. ozzardi. In the regions studied, M. ozzardi has a wider area of distribution than M. perstans. Preva

Phase III Clinical Trial to Evaluate Ivermectin in the Reduction of Mansonella ozzardi infection in the Brazilian Amazon.

The treatment of mansonelliasis is still a challenge because there are few clinical trials for the treatment of the disease. This double-blind, randomized, placebo-controlled study (phase III clinical trial) was conducted to evaluate the effectiveness of a single oral dose of ivermectin (0.15 mg/kg) in the reduction of the Mansonella ozzardi microfilaraemia and the occurrence of adverse effects in infected people compared with the control group treated with placebo. [A group of African students parasitized with Mansonella perstans in Cuba (a non-endemic area) from 1979 to 1982].

Sixty cases of Manzonella perstans were studied during a three year period by clinical and blood parasitologic studies. It was observed that 31.2 was negative and that mean microfilaremia decreased from 49.3, at the beginning of the study, to 32.9 at the end of it, in those cases yet positive. According to the persistent microfilaremia, which means a relative longevity of adult parasites, the surveillance of individuals suffering it is recommended at their arrival to a non-endemic area where the vectors of such disease are present.
