CONTEXT: Several studies have been conducted to better understand the effect of load on the Achilles tendon structure. However, the effect of a high cumulative load consisting of repetitive cyclic movements, such as those that occur during the running of a marathon, on Achilles tendon structure is not yet clear. Clinicians, coaches, and athletes will benefit from knowledge about the effects of a marathon on the structure of the Achilles tendon.
OBJECTIVE: To investigate the short-term response of the Achilles tendon structure to running a marathon.
DESIGN: Case series (prospective).

Several studies report neurological complications such as brain injury induced by ischemia or edema following exhaustive endurance sport. We aimed to detect the frequency of acute brain lesions after a marathon race. In the prospective observational Berlin Beat of Running study, 110 experienced endurance athletes underwent 3-Tesla brain MRI exams 2-3 days prior and within 2 days after a marathon run. MRI results were compared to an age- and sex-matched control group of 68 non-athletes, including the "Age-Related White Matter Changes" (ARWMC) scale to assess white matter lesions (WML) in the br

Here we use magnetic resonance imaging to study the impact of marathon running on brain structure in humans. We show that the signal for myelin water fraction-a surrogate of myelin content-is substantially reduced upon marathon running in specific brain regions involved in motor coordination and sensory and emotional integration, but recovers within two months. These findings suggest that brain myelin content is temporarily and reversibly diminished by severe exercise, a finding consistent with recent evidence from rodent studies that suggest that myelin lipids may act as glial energy reserves

Several studies have investigated the effect of hot and humid ambient conditions on running exercise up to the marathon. However, studies on exercise longer than marathon are sparse. Events exceeding 6 h can be defined as ultra-endurance and have variable characteristics (e.g., distance, elevation profile, technical difficulty, altitude, night running) making hazardous the transposition of the current knowledge obtained in marathon to ultra-endurance running. Thus, the aim of this manuscript was to discuss the potential differences between marathon and ultra-endurance running in terms of heat

Strenuous physical activity can result in exercise-induced muscle damage. The purpose of this study was to investigate the efficacy of a lower limb compression garment in accelerating recovery from a marathon run. Twenty four subjects (female, n = 7; male, n = 17) completed a marathon run before being assigned to a treatment group or a sham treatment group. The treatment group wore lower limb compression tights for 72 hours after the marathon run, the sham treatment group received a single treatment of 15 minutes of sham ultrasound after the marathon run. Perceived muscle soreness, maximal vol

OBJECTIVES: This analysis was performed to quantitatively assess the relative risks, associated with underlying cardiovascular disease, incurred in the course of intense competitive sports.
BACKGROUND: Sudden cardiac death during athletic activities is a highly visible event, and controversy persists regarding the true risks associated with participation in sports.
METHODS: The prevalence of sudden death was assessed in two systematically tabulated groups of endurance runners competing in the annual Marine Corps (1976 to 1994) and Twin Cities (1982 to 1994) marathons, held over a cumulative

Sudden cardiac death (SCD) in young athletes during physical stress is a rare event with an incidence of 1-3 deaths per 100,000 athletes per year. A coronary anomaly is the second most common cause of death following hypertrophic cardiomyopathy. Symptomatic prodromes occur in 20% of cases prior to the SCD event. This case report describes a 35-year-old male who collapsed near the finishing line of a half marathon run. Despite immediate resuscitation attempts and initial return of spontaneous circulation (ROSC), a pulseless electrical activity (PEA) followed and the patient died 1 h after arriv

BACKGROUND: Exercise-induced hypoglycaemia is common in people with insulin-treated diabetes and if severe can provoke neurological morbidity including coma and seizures. Depending on the duration and demands of the physical activity, various strategies can be used to limit the risk of hypoglycaemia with strenuous exercise. However, metabolic events occurring in the 48 h before the exercise can influence the risk and responses to exercise-induced hypoglycaemia.
CASE REPORT: A 27-year-old man with Type 1 diabetes suffered an episode of nocturnal hypoglycaemia which provoked a tonic-clonic seiz

The pacing of a marathon is arguably the most challenging aspect for runners, particularly in avoiding a sudden decline in speed, or what is colloquially termed a "wall", occurring at approximately the 30 km mark. To gain further insight into the potential for optimizing self-paced marathon performance through the coding of comprehensive physiological data, this study investigates the complex physiological responses and pacing strategies during a marathon, with a focus on the application of Shannon entropy and principal component analysis (PCA) to quantify the variability and unpredictability

Renal functions were investigated in 29 marathon runners and in 20 runners in connection with 16-kilometre long-distance run. Body weight in runners decreased after marathon run in average of 1.3 +/- 0.5 kg and after 16-kilometre long-distance run in average of 1.4 +/- 0.4 kg. Blood pressure decreased after both runs. Total proteinuria and albuminuria significantly increased after both runs. The significant non-glomerular erythrocyturia was found in 9 runners after marathon run and in 3 runners after 16-km long-distance run. Total catalytic activity of serum creatine kinase, and its isoenzyme

Although previous studies examining treadmill or relatively flat overground running events have found relationships for running performance and flexibility, no study has examined these outcomes during downhill events, including between sexes. Therefore, the purpose of this study was to determine if a relationship exists between lower body flexibility and running performance in recreational adult male and female distance runners competing in a downhill half marathon race. Recreational (n=11 male, n=19 female) adult distance runners completed this study. On the day prior to the race, participant

Background and Objectives: Pacing behavior has been consistently associated with performance in long-distance running, yet its patterns across different race distances and population subgroups remain insufficiently understood. Therefore, this narrative review synthesizes and critically discusses available evidence on pacing in half-marathon and marathon running, considering both within- and between-race differences, with a particular focus on performance level, sex, and age. Materials and Methods: A structured, non-systematic search was conducted in PubMed, SPORTDiscus, Scopus, and Web of Scie

Marathon running or other forms of strenuous exercise have been reported as a rare cause of acute pancreatitis. Theories as to the mechanism of acute pancreatitis include microvascular ischaemia due to dehydration or repetitive trauma to the pancreas. We report a case of a healthy woman in her 30s who developed abdominal pain, nausea and vomiting after a 32 km marathon training run. She was found to have elevated lipase and inflammation of the pancreatic tail with associated pericolic and pelvic free fluid on CT scan. Workup including abdominal ultrasound and magnetic resonance cholangiopancre

In full marathons, a considerable slowdown that typically occurs after 30 km, often referred to as "hitting the wall," is well known, but the underlying biomechanical mechanism remains unclear. This study aimed to elucidate pacing patterns and kinematic changes in marathon runners. Hierarchical clustering of running pace per kilometer of 297 participants, obtained from 58 marathon races, identified 3 groups, each exhibiting different biomechanical changes tracked by wearable sensors. Two groups that experienced a slowdown showed decreases in step length (effect size [ES] = 0.40-1.43), step fre

The focus of this study was to assess exercise-induced alterations of circulating dendritic cell (DC) subpopulations and toll-like receptor (TLR) expression after marathon running. Blood sampling was performed in 15 obese non-elite (ONE), 16 lean non-elite (LNE) and 16 lean elite (LE) marathon runners pre- and post-marathon as well as 24 h after the race. Circulating DC-fractions were measured by flow-cytometry analyzing myeloid DCs (BDCA-1+) and plasmacytoid DCs (BDCA-2+). We further analyzed the (TLR) -2/-4/-7 in peripheral blood mononuclear cells (rt-PCR/Western Blot) and the cytokines CRP,
