We report two patients of recurrent breast cancer with carcinomatous pleurisy well controlled pleural effusion. One patient is a 49-year-old woman. She underwent radical mastectomy for right breast cancer in September 1993. She suffered from multiple liver metastases in June 2000, so CEF therapy contained hepatic arterial infusion chemotherapy and extended right lobectomy of the liver were performed in December 2001. Two methods of filling breast defects after radical resection for nodular cancer located in inner and low quadrants were developed allowing to perform axillary lymphadenectomy without additional axillary approach (patents No. 2125408 and No. 2127552). The methods are based on transposition of breast skin flaps. The authors discuss ten cases of occult carcinoma of the breast which presented initially as the axillary node enlargement. All cases were proven as adenocarcinoma. Several months after left radical mastectomy without irradiation therapy for breast cancer, a 74-year-old woman developed severe edema on the homolateral arm extending to the axilla. Ten years later, purplish to brownish blotch and nodules accompanied with heating sensation and pain appeared and increased in size gradually on the left forearm. The patient was treated by irradiation therapy under the clinical and histopathologic diagnosis of Stewart-Treves syndrome and almost all of the skin lesions and symptoms disappeared after irradiation of 6450 rads.

In situ lobular carcinoma, intraductal carcinoma and papilliferous non-invasive carcinoma are three non-invading cancers that present clinically as the other malignant tumors of the breast. Between January 1, 1973 and August 31, 1976, ten patients were operated at the institut Bordet for such tumors. Surgical treatment varied between radical mastectomy according to Madden and partial mammectomy. Follow-up reveals no recurrence over periods extending from zero to three years. The choice of treatment depends on the histology and the probability of evolution of the actual cancer.

Thoracic endovascular aortic repair of the ascending aorta remains challenging. We have reported the case of an 81-year-old woman with ascending aortic injury who underwent a life-saving hybrid repair. The management of intraductal carcinoma of the breast at the present time is necessarily diverse because there is difficulty in detecting it, as well as understanding its basic biology and natural history. We describe herein a case of primary leiomyosarcoma of the breast in a 59-year-old woman. Preoperative clinical examination and cytology findings indicated a benign breast tumor, but intraoperative histopathological examination of the tumor revealed malignant spindle cells, and a modified radical mastectomy was performed. Immunohistochemical analysis subsequently confirmed a diagnosis of leiomyosarcoma. Patterns of axillary lymph node metastases were analyzed in 1228 recently performed modified, radical, and extended radical mastectomies. In these specimens the position or level of lymph nodes was designated intraoperatively by the surgeon. Primary osteogenic sarcoma (POS) of the breast is very rare. Only eleven cases have been previously reported in Japan. In this paper, we report an additional case of POS.
