[Breast reconstruction using a skin expander prostheses following modified radical mastectomy].

In the period from September 1983 to May 1986, 116 breast reconstructions were performed at Heidelberg University Gynecological Clinic with the aid of skin expander prostheses, following modified radical mastectomy (89 primary and 27 secondary reconstructions). In 18% of all cases, complications occurred due to the expanders, the most common being loss of prosthesis material. A total of 82 women have meanwhile had the expander prosthesis replaced by a definitive gel prosthesis in a "second reconstruction phase". Reconstruction of the nipple and areola has been performed in six cases in a third

Wound complications following modified radical mastectomy: an analysis of perioperative factors.

To assess the effect of perioperative factors on the incidence of postoperative wound complications, the authors retrospectively analyzed 101 patients who had undergone modified radical mastectomy. These factors included age, body weight, operative techniques, estimated blood loss, wound-catheter drainage, extent of axillary dissection, nodal involvement, and length of hospital stay. Complications included seroma (10.9%), wound infection (8.9%), wound necrosis (5%), hematoma (3%), lymphedema (2%), and pneumothorax (1%). The logistic regression method was used to analyze the data. When electroc

A Comparison of Modified Radical Mastectomy, Breast Conservation Therapy, and Nipple-sparing Mastectomy With Immediate Implant Reconstruction.

BACKGROUND: This study evaluated and compared the perioperative outcomes and quality of life associated with modified radical mastectomy (MRM), breast-conserving surgery (BCS), and nipple- and areola-sparing subcutaneous mastectomy with immediate breast reconstruction (NSM-IBR). METHODS: This retrospective cohort study included 105 patients with breast cancer, stratified into MRM (n = 35), BCS (n = 35), and NSM-IBR (n = 35) groups. The perioperative outcomes (operative times, intraoperative blood loss, and recent complications) were subsequently compared across the 3 groups. Differences in pa

Efficacy of octreotide in preventing lymphedema after radical, modified radical, and partial mastectomy with axillary lymphadenectomy.

BACKGROUND: Postoperative octreotide is known to reduce lymphorrhea after axillary node dissection in breast cancer and other gynecological malignancies with varying efficacy. However, the effect of octreotide on the development of lymphedema after axillary node dissection has not been explored. We sought to evaluate the role of octreotide in preventing lymphedema in patients who underwent axillary lymphadenectomy. METHODS: This retrospective review of the TriNetX Analytics Network queried the Global Collaborative Network with over 170 million patients to identify patients undergoing complete

Serratus anterior plane block in modified radical mastectomy surgery: a case series.

BACKGROUND: Postoperative breast pain may lead to poorer outcome if left untreated. Common analgesia modalities for postoperative breast pain include opioids and regional anesthesia. However, both of these modalities can cause significant side effects or complications. Serratus anterior plane (SAP) block is a new procedure that is relatively easier to perform and safer, compared with other modalities. Previous studies have reported its usefulness in reducing the need for both intraoperative and postoperative opioids. CASE: We reported 2 patients that underwent SAP block combined with general

[Cytologic examination of the axillary lavage fluid obtained during modified radical mastectomy].

The axillary dissection is part of the breast cancer surgical protocol. We have cytologically analyzed the axillary lavage fluid obtained during 67 modified radical mastectomy. Our results showed good correlation between the node status, histological grading and the cytological diagnosis of the axillary lavage fluid. As some other dissemination assessment techniques showed important prognostic value, the cytologic analysis of the axillary lavage fluid also seems to be a prognostic parameter.

Role of increased arterial inflow in arm edema after modified radical mastectomy.

Chronic arm edema is a common finding after modified radical mastectomy and its pathophysiology is unclear. In a prospective study the value of increased arterial inflow and venous abnormalities after mastectomy was evaluated. Arterial and venous blood flow in axillary vessels of 39 patients with arm swelling and 16 patients without swelling were investigated by Doppler ultrasound. In patients with arm edema the arterial flow on the surgical treated side was 689.73+/-44.6 (mean+/-sem) ml/min and 427.73+/-30.8 ml/min on the contralateral side (p<0.05). In those without swelling the flow was 447

Locoregional recurrences after 649 modified radical mastectomies: incidence and significance.

A continuous series of 649 patients, treated by modified radical mastectomy for primary breast carcinoma, is analyzed after a median follow-up of 92 months. 'True isolated' locoregional recurrences (LR), defined as LR not preceded or followed by distant metastases within 6 weeks, appeared with a cumulative actuarial incidence rate of 6%, 14% and 19% after 1, 5 and 7 years respectively, whereas the respective figures for distant metastases (M1) were 10%, 37% and 48%. The main initial parameters, predicting both the LR-free and the M1-free interval, are presented by statistical analyses in the f

Modified radical mastectomy for level III axillary lymph node clearance: a case report.

While modified radical mastectomy with level I and level II axillary lymph node clearance is a typical operating method in breast surgery, level III axillary lymph node clearance is necessary in some cases such as those involving apical axillary nodes. Level III dissection can provide accurate postoperative staging and essential guidance for postoperative adjuvant therapy. Although it is often difficult to expose the subclavian region and dissect level III axillary lymph nodes, in this case, the author split the pectoralis major muscle 2 cm inferior to the collarbone and performed a skeletoniz

Modified Pectoral Nerve Block versus Serratus Block for Analgesia Following Modified Radical Mastectomy: A Randomized Controlled Trial.

BACKGROUND: Modified pectoral nerves (PECSII) and serratus blocks have been recently used for analgesia in breast surgery, but evidence comparing their analgesic benefits is limited. This prospective randomized, controlled study aims to examine the analgesic efficacy and safety profile of ultrasound-guided PECSII versus serratus blocks in patients undergoing modified radical mastectomy (MRM) for breast cancer. PATIENTS AND METHODS: One-hundred and eighty adult females scheduled for MRM were randomly allocated to three groups. PECS group patients received a PECSII block with 30mL of bupivacain

Ultrasound-Guided Erector Spinae Plane Block Compared to Modified Pectoral Plane Block for Modified Radical Mastectomy Operations.

BACKGROUND: Modified radical mastectomy (MRM) is the most common surgery for cancer breast that is associated with marked postoperative pain. Effective control of this pain suppresses the surgical stress response and decreases the opioids and general anesthetics' needs. This study compared the erector spinae plane (ESP) block and the pectoral plane (PECS) block effects on the opioid consumption postoperatively, stress response, fentanyl needs intraoperatively, pain scores, and incidence of complications in female patients subjected to MRM surgery. PATIENTS AND METHODS: Fifty patients were all

Fibrin Glue Sealant: An Effective Modality to Mitigate Postoperative Seroma After Modified Radical Mastectomy.

Introduction Modified radical mastectomy is a common modality used in treating breast cancer and often presents a series of post-operative challenges. Seroma formation is the most common complication with no single method shown to be reliably effective against it. Fibrin glue is one such modality. There is a paucity of studies describing the utility of fibrin glue to prevent seroma formation. Aim To evaluate the effect of intraoperative application of fibrin glue sealant on drain output and seroma formation in patients undergoing modified radical mastectomy by comparing outcomes between the tw

Effect of collagen powder on lymphorrhea after modified radical mastectomy. A randomized controlled trial.

AIM: Postoperative lymphorrhea is a major complication of axillary lymphadenectomy. The aim of our study was to evaluate the impact of type I collagen in postoperative lymphorrhea in mastectomy patients. METHODS: Eighty patients that underwent modified radical mastectomy for breast cancer were randomized in two groups. In group A (collagen group, n = 42) collagen type I (Cellerate RX powder) was applied in the axillary cavity after lymphadenectomy while in group B (control group, n = 38) lymphadenectomy was performed in the standard fashion without the use of a sealant. Suction drains remaine

Comparison of partial and modified radical mastectomy in the community setting--"10 years later".

Conservative surgery followed by postoperative radiation is considered equivalent to a modified radical mastectomy (MRM) for the treatment of early breast cancer. It cannot be assumed that results from selected academic centres are equivalent to those obtained in the general community setting, because there may be differences in patient selection or surgical or radiotherapy techniques that may adversely affect outcome. A quality-control study of women who were seen at the British Columbia Cancer Agency and were treated by partial mastectomy (PM) was begun in 1983. Eighty-four women who underwe

Effects of Fibrin Glue on Postoperative Complications After Modified Radical Mastectomy for Breast Carcinoma.

Background Seroma formation is a common postoperative complication following modified radical mastectomy (MRM) for breast cancer, contributing to delayed wound healing, increased morbidity, prolonged hospitalization, and delay in adjuvant treatment. Fibrin glue, a biological adhesive, is proposed to reduce seroma formation by sealing lymphatic channels and reducing dead space. This study evaluated the effectiveness of fibrin glue combined with closed suction drainage in reducing postoperative complications following MRM compared to suction drainage alone. Methodology A prospective observationa
