Expert opinion on drug metabolism & toxicology2025eng
INTRODUCTION: Pregnant women are customarily excluded from phase 1 and 2 clinical trials which, provide the earliest indication of drug safety and efficacy. This leads to lack of pregnancy-specific safety information for over 90% of approved drugs, including many commonly used during the first trimester.
AREAS COVERED: In this review, we highlight the importance of pregnancy-induced changes in pharmacokinetics, commonly used indices of fetal exposure to maternal drugs, and their limitations. Inter-species differences make data from animal models used in preclinical embryofetal development tox

The detection of exposure of pregnant women to toxicants in the environment is important because these compounds can be harmful to the health of the woman and her fetus. The aim of this study was to analyze for pesticides/herbicides in paired maternal hair and blood samples to determine the most appropriate matrix for detecting maternal exposure to these compounds. A total of 449 pregnant women were prospectively recruited at midgestation from an agricultural site in the Philippines where a preliminary survey indicated significant use at home and on the farm of the following compounds: propoxu

OBJECTIVE: Reports of maternal effects resulting from drug exposure during pregnancy are inconsistent. The Maternal Lifestyle Study (MLS) is a multicenter, prospective, observational study that was initiated to better define the effects of exposure to illicit drugs during pregnancy on the mother, fetus, and infant.
METHODS: Between May 1993 and May 1995, of 19,079 mother-infant dyads that were screened after delivery for cocaine and opiate exposure at four clinical centers (Brown University, University of Miami, University of Tennessee, Memphis, and Wayne State University), 16,988 (89%) met e

Plants that resprout after fire experience drastically altered conditions in post-fire environments. However, the effects of plant exposure to fire on offspring traits remain understudied. Many resprouting species are herbaceous plants that flower and fruit rapidly after fire (i.e., fire-stimulated flowering), developing their seeds under increased resource availability and modified biotic interactions, particularly enhanced pollination. We hypothesize that these changes may improve early offspring development through maternal provisioning and increased quality of floral visitation and pollen

OBJECTIVES: Premature constriction of the fetal ductus arteriosus is a clinically relevant cause of fetal cardiac compromise. Maternal exposure to prostaglandin-inhibiting analgesics is a recognized trigger. We describe two pregnancies complicated by fetal ductus arteriosus constriction temporally associated with maternal analgesic use.
CASE PRESENTATION: In the first case, ductal constriction occurred after a seven-day course of indomethacin initiated at 28 weeks' gestation for polyhydramnios. Although initial improvement followed drug discontinuation, recurrent constriction with right ventr

BACKGROUND: Babesiosis is a disease caused by parasites that infect red blood cells; in infants it can be acquired from tick bites, blood transfusions, or congenitally via vertical transmission. It can present with thrombocytopenia, fevers, and parasitemia.
CASE REPORT: A case of vertically transmitted babesiosis in an infant is described. Thrombocytopenia and parasitemia > 4% developed in this well-appearing infant. The diagnosis was made by history and blood smear in both infant and mother, and the patient recovered fully with oral antibiotics. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF

BACKGROUND: Cigarette smoking is a well-studied risk factor for orofacial clefts (OFCs). Little is known about which constituents in cigarette smoke contribute to this teratogenicity in humans. One constituent, cadmium, has been associated with OFCs in animal studies; in humans, the role of maternal cadmium exposure on OFCs, independent of cigarette smoke, is unclear. In particular, the relation between maternal occupational cadmium exposure and OFCs is largely unexplored.
METHODS: Using data from a large, population-based case-control study, we compared expert rater assessed maternal occupat

BACKGROUND: The knowledge about air pollution effects on birth weight, prematurity, and small for gestational age (SGA) in low-exposure areas is insufficient.
OBJECTIVES: The aim of this birth cohort study was to investigate whether low-level exposure to air pollution was associated with prematurity and fetal growth and whether there are sex-specific effects.
METHOD: We combined high-quality registry information on 81,110 births with individually modeled exposure data at residence for nitrogen oxides (NO(x)) and proximity to roads with differing traffic density. The data were analyzed by log

Women pass on low levels of hazardous environmental contaminants and naturally occurring substances during pregnancy and nursing. In addition to estrogenic activity, these substances exhibit antiestrogenic, antiandrogenic, and androgenic actions, and because they can affect thyroid metabolism, they are described as endocrine-active substances (EAS). Specific topics related to EAS metabolism by mothers, fetuses, and infants are discussed. There is strong evidence that the assumed risk of EAS in early human development is exacerbated by interrupting neonatal priming provided by breastfeeding. Th

Pregnant albino rats were exposed to nitrogen dioxide in concentrations of 0.05, 0.10, 1 and 10 mg/m3 for 6 hr each day throughout gestation. Postnatal viability, growth, physical maturation, neuromotor development, and biochemical parameters indicative of NO2 oxidizing effects were studied in progeny up to the age of 3 months. It was found that although viability and physical development were little affected (only at 10 mg/m3), dose-dependent neurobehavioral deviations (such as disturbances in early neuromotor development, coordination deficits, retarded development of locomotion, reduced act

BACKGROUND: Previous studies suggest that periconceptional maternal occupational exposure to solvents and pesticides increase the risk of oral clefts in the offspring. Less is known about the effect of occupational exposure to metals, dust, and gases and fumes on development of oral clefts.
METHODS: This case-malformed control study used data from a population-based birth defects registry (Eurocat) of children and foetuses born in the Northern Netherlands between 1997 and 2013. Cases were defined as non-syndromic oral clefts. The first control group had chromosomal/monogenic defects, and the

Sulfur dioxide (SO2) is one of the commonly encountered environmental contaminants. Experiments were carried out to test for neonatal behavioral alterations associated with maternal SO2 exposure. Pregnant CD-I mice were exposed to 0, 32, or 65 PPM SO2 in environmental chambers from gestation day 7 to 18 (plug = day 1). The SO2 air flow was set at 500 ml/min. Food and water were available at all times. The dams were allowed to deliver and neonatal behavioral development of the pups was studied. Maternal SO2 exposure did not affect the mean number of live pups born/litter; however, the exposure

Acute and chronic intubations of ethanol to pregnant rats produced changes in spontaneous fetal behavior four hours later. Fetuses from mothers in intermediate alcohol groups (4 and 6 g/kg) were substantially less active than controls (0 g/kg), but fetuses from low (2 g/kg) and high (8 g/kg) alcohol groups showed little indication of behavioral suppression. Circulating levels of alcohol in maternal blood, fetal homogenate and amniotic fluid at the time fetuses were observed confirmed that fetuses were exposed to alcohol in utero, but the measured concentrations of alcohol were not predictive o

The public health crisis of pregnant women being exposed to drugs of abuse and of its impact on their unborn children continues to grow at an alarming rate globally. The state of pregnancy is unique, with physiological changes that can lead to changes in the way drugs are handled by the body in both pharmacokinetics and response. These changes place the pregnant woman, fetus, and newborn infant at risk, as many of these drugs can cross the placenta and into breast milk. The substances most commonly linked to harmful effects include alcohol, tobacco, cannabis, stimulants, and opioids. The pharm

BACKGROUND: Daily changes in aeroallergens during pregnancy could trigger early labor, but few investigations have evaluated this issue. This study aimed to investigate the association between exposure to aeroallergens during the week preceding birth and the risk of early delivery among preterm and term pregnancies.
METHODS: We identified data on 225,234 singleton births that occurred in six large cities in the province of Ontario, Canada, from 2004 to 2011 (April to October) from a birth registry. We obtained daily counts of pollen grains and fungal spores from fixed-site monitoring stations

Although the link between chronic stress and the development of cardiovascular and metabolic diseases of adulthood has been known for some time, there is growing recognition that early environmental influences may result in developmental programming via epigenetic mechanisms, thereby affecting the developmental trajectory of disease progression. Previous studies support the idea that offspring of Holocaust survivors may have been subjected to early developmental programming. We evaluated the relationship between parental exposure to the Holocaust and self-reported health ratings and disorders

The effects of intake of acetaldehyde, the proximal metabolite of ethanol, were studied in two groups of Fischer strain rats. Virgin rats were mated at 3 months of age or at 8 months of age. The acetaldehyde intake group (AcH) was given a 2% aqueous solution of acetaldehyde for the first time on the first day of pregnancy. The solution was then given once a day, oral net acetaldehyde 240 mg/kg b.w. through gestation, labor and lactation. The control group was not exposed to acetaldehyde. Comparative observations were made on both maternal rats and their offspring. 1) Maternal body weight gain

Data on exposure of 10,879 pregnant women to various household and agricultural chemicals were obtained by interview; 6,267 of these women were seen both antenatally and postnatally, enabling a genuine prospective study of the association between chemical exposure and congenital abnormalities in the infant. The remaining 4,612 women were privately booked and were first seen postnatally. Statistical analysis of the 2 groups revealed a strong association of malformations with chemical exposure in both groups, even after other known risk factors were taken into account.

Prior studies have suggested the reproductive effects of nickel; however, few epidemiological studies have investigated the associations of maternal exposure to nickel with preterm delivery. To investigate prenatal exposure to nickel as a risk factor for preterm delivery (< 37 weeks) in a large birth cohort. A total of 7291 pregnant women participated in the study were recruited between September 2012 and October 2014 in the longitudinal Healthy Baby Cohort (HBC) in Wuhan, China. Inductively Coupled Plasma Mass Spectrometry was employed to examine levels of nickel in urine from pregnant women
