Consequences and management of nasal airway obstruction in the dentofacial deformity patient.

INTRODUCTION: At the time of orthognathic surgery the maxillofacial surgeon has an opportunity to further contribute to a patient's quality of life by simultaneously addressing long-standing breathing and sinus drainage difficulties that may coexist with the jaw deformity. Breathing difficulties may range from isolated symptomatic nasal airway obstruction to obstructive sleep apnea syndrome and should not be overlooked. The impact of the combined occurrence of nasal obstruction and maxillary deformity has been recognized and discussed in the literature. RECENT FINDINGS: Studies have establish

[Possibilities and limitations for the correction of bony, dental and mucosal tissue loss in children using a maxillofacial prosthesis].

The maxillofacial prosthesis for surgical treatment of loss of bony, dental or mucosal substance in children has the advantage of allowing postponement of therapy until growth is terminated or any other favorable period. In addition, the temporary reconstitutions possible by their use often provide, apart from esthetic results, an effective action physically and mentally on the mainly labile growth.

A syndrome of hemimaxillary enlargement, asymmetry of the face, tooth abnormalities, and skin findings (HATS).

Hemimaxillofacial dysplasia and segmental odontomaxillary dysplasia appear to be the same syndrome, having the common features of unilateral abnormalities of bone, teeth, gums, and skin. Oral manifestations are the hallmark of this condition. Those affected are generally recognized in childhood and may have partial anodontia, abnormal spacing of the teeth, delayed eruption, and gingival thickening of the affected segment. Reported cutaneous manifestations include facial asymmetry, Becker's nevus, "hairy nevus," lip hypopigmentation, discontinuity of the vermilion border, depression of the chee

[Etiopathogenic and clinical aspects of asymmetry].

Complex dysmorphies, asymmetries can affect the various elements of the stomatognathic system: cranial base, maxilla, mandible, musculature and dental arches, in the three dimensions of space. Their etiopathogenesis, as well as their clinical forms, vary considerably. Dysembryopathies, growth disorders, TMJ damages, postural or functional disorders and dental anomalies can create, according to their severity and their moment of appearance, severe or minor facial asymmetries, or asymmetries of the arches. [Stickler syndrome: maxillo-facial abnormalities].

A case of Stickler syndrome (hereditary arthro-ophthalmo-dystrophy) with maxillo-facial abnormalities and colon atresia is reported. The authors emphasize that in Stickler syndrome, (a chondrodystrophy with congenital alteration of type II collagen) the maxillofacial abnormalities are consecutive to prosencephalic neural crests dysneurulation, which caused mesethmoidal dysneurulation (and consequently of frontonasal-premaxillary structures); eye and colon abnormalities are consecutive to rhombo-mesencephalic neural crests dysneurulation. The Authors hypothesize that genic mutation, responsible

Prenatal diagnosis and treatment of craniomaxillofacial anomalies.

Many advances in health care are built on the evolution of technology. An entirely new patient has emerged in fetal medicine, with these advances in prenatal imaging allowing one to see and diagnose disease not previously appreciated. Clinicians can better plan for the delivery of the neonate, with identified anomalies being optimally managed and the impact on the neonate's health minimized. The oral and maxillofacial surgeon offers expertise in the management of craniomaxillofacial anomalies, including congenital tumors, facial clefts, craniosynostosis, micrognathia, and other congenital abno

[Maxillofacial reflection of choanal atresia. Apropos 11 cases].

The authors study the maxillofacial repercussions of choanal imperforation based upon 11 cases (8 unilateral and 3 bilateral). Skeletal abnormalities are defined using teleradiographic and tomographic techniques. The origin of these abnormalities is sought: malformations, deformities or sequelae of surgical treatment.

Multidetector Row Computed Tomography in Maxillofacial Imaging.

Multidetector row CT (MDCT) offers superior soft tissue characterization and is useful for diagnosis of odontogenic and nonodontogenic cysts and tumors, fibro-osseous lesions, inflammatory, malignancy, metastatic lesions, developmental abnormalities, and maxillofacial trauma. The rapid advances in MDCT technology, including perfusion CT, dual-energy CT, and texture analysis, will be an integrated anatomic and functional high-resolution scan, which will help in diagnosis of maxillofacial lesions and overall patient care.

Secondary Rhinoplasty in Binder Syndrome: Considerations and Management of Complex Problem With Heterologous Bone Graft.

Binder Syndrome occurs in less than 1 per 10,000 live births. Mean features of the syndrome include arhinoid face, abnormal position of the nasal bones, intermaxillary hypoplasia with associated malocclusion, reduced or absent anterior nasal spine, atrophy of the nasal mucosa and absence of the frontal sinus. Treatment of these facial deformities is obviously surgical. In the present work, the authors describe, step by step, their technique in secondary rhinoplasty in a 36-years-old patient affected by Binder Syndrome. In this case, the authors used autologous cartilage graft and heterologous

Congenital unilateral maxillo-mandibulo-zygomatic fusion (syngnathia): a case report in an 8-year-old boy.

Congenital causes of limited mouth opening involving fusion of maxilla and mandible (syngnathia) are rare anomalies that often present as part of syndromes such as Van der Woude and popliteal pterygium syndrome. Surgical management involves division of the bony fusion or breakdown of the adhesions in the first few days of life. The authors present a case of maxillo-mandibulo-zygomatic fusion (syngnathia) in an 8-year-old boy, who underwent surgical intervention to achieve adequate jaw function.

Ellis-Van Creveld syndrome: dental management considerations and description of a new oral finding.

Ellis-Van Creveld is a rare syndrome with characteristic dental and orofacial findings. Dental management of patients with Ellis-Van Creveld syndrome can be complicated by the associated skeletal and cardiac abnormalities. Here, we present the dental and orofacial findings in a patient with Ellis-Van Creveld syndrome, describe a new oral finding, and discuss the dental management considerations.

[Dental findings in a case of Chondrodysplasia Punctata].

Chondrodysplasia Punctata is a disease belonging to the metaphysia syndrome. This disease is characterized by endochondral punctata calcification of epiphysis or apophysis, short legs and hands, arthrogryposis, saddle nose, ichthyosis, cataract and mental retardation. This report concerns a case of Chondrodysplasia Punctata (Rhizomeric form) in a 4 year 1 month old girl. Observations were made from the dental point of view, and the findings were as follows: 1) The mesio-distal and bucco-lingual lengths of all primary teeth were smaller than the standard size. 2) Measuring the dental cast, the

Bimaxillary osteotomies in correction of dentofacial deformities.

In 342 patients with dentofacial deformities cephalometric analyses were preoperatively performed. A study of occlusion on cast models as the simulated new position of the facial soft tissues was done on the photographs. Surgical treatment using bimaxillary osteotomies for correction of dentofacial deformities was done in 38 patients: 31 with prognathism and maxillary hypoplasia, 5 with facial asymmetry and laterognathism and in 2 with otomandibular dysostosis. The LeFort I type osteotomy was used for the upper jaw and Obwegeser technique for the lower jaw. Intermaxillary and circumzygomatic i

Malformations of the zygomatic and maxillary regions.

The maxillofacial region develops during 3 to 8 weeks in an embryo. The process involves neural crest cell migration and proliferation as well as facial protrusion jointing and fusion. The maxillofacial region is one of the predilection sites of congenital malformations. We treated a 5-year-old Chinese boy with abnormal development of the left maxillofacial region. We describe in detail the patient's characteristics, diagnosis, and treatment processes and try to explain the possible causes of the disease.

Maxillofacial manifestations of Proteus syndrome: a systematic review with a case report.

OBJECTIVES: Proteus syndrome (PS) is an extremely rare disorder with asymmetric and disproportionate bone overgrowth. Craniofacial abnormalities in PS are less frequent than skeletal abnormalities. Although there are recognized oral and maxillofacial manifestations of PS, few case reports describing these manifestations are available. Thus, the objective of this systematic review and case report is to describe oral and maxillofacial manifestations of PS and to report a PS case. METHODS: A 31-year-old male presented with restricted mouth opening and pain during mastication. A panoramic radiogr

Radiologic screening for midfacial fractures: a single 30-degree occipitomental view is enough.

BACKGROUND: The standard radiologic screening views for maxillofacial trauma in the accident and emergency department have been the Waters (30-degree occipitomental [OM]) view, the Caldwell (posteroanterior) view, and the lateral view. We wanted to see if a single 30-degree occipitomental view would be sufficient to screen for maxillofacial trauma (excluding nasal fractures). METHODS: Over a 1-year period, each of 730 patients had three radiologic views obtained for maxillofacial trauma at our emergency department. A consultant emergency physician read the single OM view for injury, and then
