Hypoxic ventilatory response in successful extreme altitude climbers.

A very high ventilatory response to hypoxia is believed necessary to reach extreme altitude without oxygen. Alternatively, the excessive ventilation could be counterproductive by exhausting the ventilatory reserve early on. To test these alternatives, 11 elite climbers (2004 Everest-K2 Italian Expedition) were evaluated as follows: 1) at sea level, and 2) at 5,200 m, after 15 days of acclimatisation at altitude. Resting oxygen saturation, minute ventilation, breathing rate, hypoxic ventilatory response, maximal voluntary ventilation, ventilatory reserve (at oxygen saturation = 70%) and two ind

Respiratory control during nipple feeding in preterm infants.

The present study was designed to investigate the development of respiratory control during feeding in premature infants. Cardiorespiratory disturbances during feeding were evaluated with polygraphic monitoring in 24 premature infants within 1 week of beginning nipple feeds. During the initial study, 15 infants exhibited one or more episodes of short apnea (greater than or equal to 10 sec) and three infants exhibited prolonged apnea (greater than or equal to 20 sec). Bradycardia developed in seven infants; apnea and decreases in oxygen saturation invariably preceded the development of bradycar

Open biopsy for chronic diffuse infiltrative lung disease: clinical, roentgenographic, and physiological correlations in 502 patients.

Clinical, physiological, roentgenographic, and histological data concerning 502 patients who had open biopsy for chronic "interstitial" lung disease were reviewed. Mortality was 0.3%, the rate of complications was 2.5%, and the diagnostic yield was 92.2%. A modified Chamberlain approach in the second interspace is preferred for easy access to all lobes and mediastinum. Brief tube drainage is mandatory. Atelectasis and hemorrhage in the specimen are prevented by avoiding palpation and clamps, by delineating the wedge during full inflation, and by instant fixation. Customary biopsies of the tip

Environmental exposure to tobacco smoke and lung function in young adults.

The relationship between lung function and environmental exposure to tobacco smoke (passive smoking) was studied in 293 nonsmoking young men and women, 15 to 35 yr of age. A self-administered mailed questionnaire was used to assess the lifetime environmental exposure to cigarette smoke at home and at work for each subject. Lung function information used here had been gathered in the course of a previous study of the determinants of lung function in early adulthood. In men, maximal midexpiratory flow rate (FEF25-75) decreased in relation to an index of cumulative lifetime environmental exposure

Adverse pulmonary responses to aspirin and acetaminophen in chronic childhood asthma.

Because aspirin (ASA) is often reported to have an adverse effect on pulmonary function in children with chronic asthma, acetaminophen is commonly used as an ASA substitute in these children. To study acetaminophen effects on pulmonary functions, double-blind, oral challenges of ASA (600 mg), acetaminophen (600 mg), or lactose were administered on separate days to 25 chronic asthmatics, ten boys and 15 girls, ranging in age from 8 to 18 years (mean age +/- 1 SD: 12.5 +/- 2.8 years). No patient had a past history of adverse reactions to either drug. Forced expiratory volume in 1 second (FEV1),

[Pulmonary ventilation indices of patients following the surgical correction of mitral stenosis].

The method of spiroveloergometry was used in 97 patients after operation of mitral commissurotomy for studying reserves of pulmonary ventilation and its relation to the level of physical working capacity. The results obtained have shown that in patients of worse functional classes the ventilation potency of the lungs deteriorated which resulted in the restricted physical working capacity. It should be necessarily taken into consideration when making programs for the rehabilitation of cardiosurgical patients.

Control of respiratory therapy in flail chest.

Pulmonary mechanics and oxygenation were measured in 24 consecutive patients with posttraumatic flail chest requiring continuous mechanical ventilation. The mean duration of mechanical ventilation was fourteen days. Mortality was 38% for all patients, 29% if deaths from head injury are excluded. Pneumonia occurred in 4 patients (17%) and pneumothorax in 1 (4%). Vital capacity and maximal inspiratory force measurements were useful in assessing chest wall stabilization. Total lung compliance correlated negatively with fatal outcome from respiratory failure. The alveolar-arterial oxygen gradient

The effects of the neck stabilization exercise on the muscle activity of trunk respiratory muscles and maximum voluntary ventilation of chronic stroke patients.

BACKGROUND: Unstable neck posture, muscle imbalance, and segmental instability can cause neck pain and decrease the respiratory function. OBJECTIVE: To examine effects of a neck stabilization exercise on respiratory muscle activity and maximal voluntary ventilation (MVV) in patients with a chronic stroke. METHODS: A total of 40 patients with a chronic stroke participated in this study. They were randomly divided into two groups (20 in each group). However, ten patients dropped out of the experiment (5 in each group). The experimental group (n= 15) performed a 15-minute neck stabilization exe

[Treatment of subcutaneous and mediastinal emphysema with cuirass ventilation in a patient with Pompe disease undergoing non-invasive positive pressure ventilation].

We report a case of subcutaneous and mediastinal emphysema in a 39-year-old woman with late-onset Pompe disease who was undergoing non-invasive positive pressure ventilation (NPPV). Although the patient had a history of pneumothorax, she did not present with pneumothorax at the time of admission. She had not undergone adequate respiratory rehabilitation, which resulted in decreased respiratory compliance. We speculated that the emphysema had been caused by an increase in the airway pressure due to NPPV. Decrease in inspiratory pressure of NPPV from 14 cm H2O to 9 cm H2O made the patient dyspne

Ingestion of an antihistamine does not affect exercise performance.

The effects of a single oral dose of a sedating and nonsedating H1 receptor antagonist on exercise performance and tolerance were examined in a double-blind, randomized study. Twelve healthy, physically active subjects were tested under a placebo condition and two antihistamine conditions (diphenhydramine hydrochloride (50 mg) and terfenadine (60 mg)). The following treadmill exercise tests were administered: maximal aerobic power (MAX), submaximal steady state (SS), and high-intensity, intermittent exercise (HI). The MAX test consisted of incremental treadmill running to volitional exhaustion

Effects of head-out water immersion on cardiorespiratory responses to maximal cycling exercise.

Our objectives were to determine effects of head-out immersion (HOI), scuba breathing, and water temperature on cardiorespiratory responses to maximal aerobic work. Measurements of VO2, VE, and heart rate (HR) were obtained on seven men (27 yr, 177 cm, 67 kg) as they performed the same upright bicycling exercise to exhaustion (4-5 min) in 23 degrees C air and 30 degrees C water. Maximal oxygen uptake (VO2 max) during HOI was 3.18 liters - min-1, which was not statistically different from the mean of 3.29 liters- min-1 in air. When compressed air was breathed via scuba during HOI, VO2 max was 3

Effect of the Y-piece of the ventilation circuit on ventilation requirements in extremely low birth weight infants.

OBJECTIVE: Volutrauma caused by high tidal volumes contributes considerably to the development of bronchopulmonary dysplasia. Yet high tidal volumes are required to overcome dead space. In an experimental arrangement we tested whether reduction of dead space might reduce ventilation requirements and thus reduce volutrauma in preterm infants. MATERIALS AND METHODS: The time required to eliminate CO2 by standardized mechanical ventilation from a preterm infant's test lung flooded with CO2 was measured. Four different Y-pieces and flow sensor combinations were tested with and without a device fo

[A comparison of the effects of PAV, PSV and IPPV on cardiopulmonary function in patients with acute respiratory failure].

OBJECTIVE: Comparing the effects of proportional assist ventilation (PAV), pressure support ventilation (PSV), and intermittent positive pressure ventilation (IPPV) on cardiopulmonary function in patients with acute respiratory failure, in particular, evaluating the clinical significance of PAV. METHODS: Ten patients with acute respiratory failure were firstly ventilated with IPPV. Elaslance (Ers) and Resistance (Rrs) were measured and calculated. Then PSV mode was adapted. Based on the parameters of IPPV, inspiratory positive airway pressure was adjusted to maintain the same tidal volume (V(

Changes in respiratory muscle function after neostigmine injection in patients with myasthenia gravis.

In sixteen patients with myasthenia gravis, the weakness of respiratory muscles, especially of expiratory, caused some disturbances in lung function. Thirty minutes after injection of neostigmine, general improvement of muscle function was accompanied by an increase of strength and endurance of respiratory muscles. The mean value of maximal inspiratory mouth pressure (PImax) increased by 33%, maximal expiratory mouth pressure (PEmax) by 23.5% and maximal voluntary ventilation (MVV) by 21%. As a consequence of these changes amelioration of lung function indices was found, vital capacity increas

Conscious volunteers developed hypoxemia and pulmonary collapse when breathing air and oxygen at reduced lung volume.

Ventilation at maximal voluntary reduction of lung volume caused significant desaturation in some healthy subjects breathing air. Saturation rapidly returned to control levels when normal lung volume was regained. These changes are probably due to reversible airway obstruction. During the inhalation of oxygen, ventilation at maximal voluntary reduction of lung volume caused, in one subject, a reduction of arterial Po2 of 243 mmHg. Normal arterial Po2 was not immediately restored on regaining normal lung volume. Chest radiographs showed extensive atelectasis, which persisted for several hours i

Influence of sex, menstrual cycle, and oral contraceptives on the cerebrovascular response to paced deep breathing.

PURPOSE: Deep breathing assesses autonomic function; however, many researchers/clinicians do not account for hyperventilation, brain blood flow or blood pressure. METHODS: Men and women (with/without oral contraceptives) participated. women participated during low and high hormone phases of the menstrual cycle. Blood pressure, end-tidal carbon dioxide, middle cerebral artery velocity and cerebrovascular resistance were assessed. RESULTS: Deep breathing decreased end-tidal carbon dioxide and middle cerebral artery velocity while increasing cerebrovascular resistance in all participants; blood

Cardiac and respiratory responses to exercise in adolescent idiopathic scoliosis.

Twenty girls, aged 11-15 years, with adolescent idiopathic scoliosis were studied before spinal fusion was performed. Each underwent a range of lung function tests and a standardised progressive exercise test on a bicycle ergometer. The resting lung function tests showed reduced peak flow rates, lung volumes, and maximum voluntary ventilation. The maximum oxygen uptake was slightly diminished but maximum exercise ventilation was normal. The latter was achieved by using a greater than normal fraction of the vital capacity in tidal breathing while exercising. Mild hyperventilation during submaxi

Regulation of respiration in Friedreich's ataxia.

Friedreich's Ataxia (F.A.) is a degenerative disease which commonly leads to premature death of cardiorespiratory origin. To explain the early death of these patients, previous investigations have established the existence of 1) a cardiomyopathy in nearly 100% of cases, 2) a restrictive pulmonary syndrome of scoliotic origin and 3) a mild hypoxemia associated with slight respiratory alkalosis and a normal oxyhemoglobin dissociation curve. To further assess the cause of early death in patients with such neuromyopathy, we evaluated, in eleven F.A. patients, the sensitivity of the respiratory cen
