We reported a 26-year-old man with mitral regurgitation and sick sinus syndrome. He underwent VVI pacemaker implantation, and admitted to our hospital due to congestive heart failure. Echocardiography and cineangiography revealed severe mitral regurgitation. Mitral valvuloplasty and modified Maze procedure were performed, and a DDD pacemaker was implanted after cardiopulmonary bypass. We recognized cross stimulation 7 months after operation. The reason of the cross stimulation was the position of the implanted atrial lead, which was the low right atrium. Thus, an atrial lead of DDD pacemaker s

From January 1991 until May 1992, a total of 14 patients (mean age 48 years) underwent the maze procedure for refractory atrial fibrillation (mean duration, 7 years; mean number of antiarrhythmic medications, six). Three patients had had embolic events, one patient had had a cardiac arrest from flecainide, one had pulmonary fibrosis from amiodarone, and six of ten who were employed were temporarily disabled. Two patients underwent successful mitral valve repair in which the maze procedure was added as a secondary goal of the operation. Postoperative fluid retention was a problem in five patien

BACKGROUND: TO IMPROVE SINUS RHYTHM CONVERSION, THE COX MAZE III PROCEDURE WITH NARROW MAZES (WIDTH: ≤3.0 cm) was performed in combination with left atrial volume reduction.
METHODS: From October 2007 to April 2013, 87 patients with atrial fibrillation (paroxysmal in 3, persistent in 14, and permanent in 70) underwent the Cox maze procedure concomitant with another cardiac procedure. They were followed-up with serial electrocardiographic and echocardiographic studies. We used 24-hour Holter monitoring tests to evaluate postoperatively symptomatic patients.
RESULTS: At the mean follow-up time

It is important to explain the benefits and risks of surgery thoroughly to patients and their families. This paper describes the benefits and risks of the Maze procedure for atrial fibrillation (AF). The purpose of the Maze procedure is the restoration of sinus rhythm from AF, recovery of cardiac function by atrial contraction, and prevention of stroke. The AF cure ratio is 70-90% after the Maze procedure. It was reported that atrial function improves due to atrial contraction after surgery, and the risk of a stroke is reduced to about 2-4 events per 1,000 patients per year after surgery. On t

Atrial fibrillation secondary to long-standing mitral valve disease is very common. The maze procedure carried out along with mitral valve surgery reverts many of these patients back to sinus rhythm. Using radiofrequency ablation to carry out the maze procedure during mitral valve surgery gives results comparable to those from the Cox surgical maze procedure. We present our experience with 3 patients of rheumatic mitral valvular disease in whom radiofrequency ablation was used for the maze procedure.

BACKGROUND: Mitral valve disease is often accompanied by atrial fibrillation, which may compromise the patient even after the valvular lesion has been repaired.
METHODS: Three patients with rheumatic type mitral valve disease and chronic atrial fibrillation were treated by mitral valve replacement with cryopreserved mitral valve homograft and Maze III procedure, as a method to relieve both the valvular pathology and the rhythm disorder. The patients' clinical courses have been followed for up to 1 year after operation.
RESULTS: All patients survived operation, and all have normal sinus rhyth

Although atrial fibrillation is well tolerated by most patients, in some patients the consequences may be severe. The Maze procedure is a new open-heart operation that creates a carefully designed maze of incisions in the atrial myocardium; this maze then acts as an electrical conduit to channel atrial impulses from the sinoatrial node to the atrioventricular node. The Maze procedure has been shown to restore sinus rhythm and atrial systole (thus reducing the risk of thromboembolism), improve hemodynamics, alleviate palpitations, and eliminate the need for antiarrhythmic and anticoagulant drug

A 75-year-old woman with refractory paroxysmal atrial fibrillation and hypertrophic obstructive cardiomyopathy underwent a successful combined maze procedure and septal myectomy. Postoperative episodes of atrial fibrillation and flutter occurred only during periods of bradyarrhythmia and did not recur with atrial inhibited pacing.

BACKGROUND: Catheter ablation (CA) for long-standing persistent atrial fibrillation (LSPAF) is suboptimal, and open surgical ablation, although more successful, is too invasive to be a first-line therapy. Less invasive hybrid procedures that combine thoracoscopic surgery (TS) with CA have been only marginally more successful for LSPAF than CA alone.
METHODS: Joint hybrid procedures for LSPAF are based on the assumption that AF surgery and CA procedures can be guided by intraoperative mapping. However, intraoperative mapping is not always dependable because of the transient nature of the susta

This study presents a novel way to perform the mini-maze procedure through the left atrial appendage. By this way, the usual additional incision of the intra-atrial groove is avoided, especially in patients receiving coronary artery bypass grafting (CABG) or aortic valve replacement without mitral valve disease. We retrospectively analysed 23 consecutive patients who received this novel mini-maze procedure between 2009 and 2011. In recognition of a learning curve, we divided the patients into two groups (Group 1: Patients 1-11 versus Group 2: Patients 12-23), according to the date of operation

We anesthetized three patients with refractory atrial fibrillation undergoing the new thoracoscopic mini Maze procedure. This minimally invasive surgical procedure provides isolation and ablation of the bilateral pulmonary vein without thoracotomy or cardiopulmonary bypass, which was performed first in the United States in 2003. General anesthesia was administrated with a double lumen tracheal tube for bilateral single lung ventilation, and epidural anesthesia was also administrated. We prepared transesophageal echocardiography, a pulmonary artery catheter with pacing wires, and a 20 G arteria

BACKGROUND: This study was conducted to evaluate the effectiveness of the saline-irrigated radiofrequency modified Maze operation for treatment of chronic atrial fibrillation, and to compare the results of the left and biatrial procedures.
METHODS: During a period of 11 months, 62 patients with chronic atrial fibrillation who were having concomitant cardiac surgery underwent the procedure. The mean age of the patients was 52 +/- 14 years. Patients underwent either a biatrial (group A; n = 39) or left atrial (group B; n = 23) procedure.
RESULTS: Two patients (3.2%) died early in the postopera

A 64-year-old woman presented with congestive heart failure due to severe mitral valve stenosis and chronic atrial fibrillation. A Maze III procedure was performed, and the mitral valve replaced with a mitral homograft. Postoperatively, the patient regained normal sinus rhythm, had trivial mitral regurgitation, regained her atrial transport function, and had improved myocardial function.

From May 1994 to May 1996, 20 cases of atrial fibrillation were treated by means of Maze procedure by radiofrequenncy ablation, at the same time 19 cases of these patients were complicated with rheumatic heart valve disease and valve replacement operations were perfomned, in the other case atrial septal defect was repaired. Yoshio Kosakai's operation route was adopted in radiofrequency ablation procedure. After operation 16 patients of atrial fibrillation resumed sinus rhythm (80%), in 4 casess of atrial fibrillation sinus rhythm was unsuccessfully restored, two patients remained atrial fibril

BACKGROUND: Minimally invasive Maze procedure via right mini-thoracotomy approach is reportedly a promising option for paroxysmal and non-paroxysmal atrial fibrillation (AF), although it is not widely performed. This study aimed to validate feasibility and safety of minimally invasive stand-alone Maze procedure in an institutional first case series.
METHODS: This study enrolled an institutional consecutive series of 20 cases who underwent minimally invasive Maze procedure between November 2018 and January 2021. Concomitant tricuspid annuloplasty was performed in five cases who showed moderate

We report a case of pulmonary artery catheter (PAC) injury by radio frequency device for maze procedure. A 64-year-old female with severe mitral insufficiency, tricuspid insufficiency and paroxysmal atrial fibrillation was scheduled for mitral valve repair, tricuspid annulo- plasty and maze procedure including right-sided maze. Under general anesthesia, a PAC was inserted to pul- monary artery (PA) uneventfully. After radio frequency maze procedure and mitral valve repair, PAC was removed from right atrium by the surgeon for tricus- pid annuloplasty. Thereafter, the surgeon reinserted the PAC

We performed a right atrial reduction plasty combined with the Cox-maze procedure in a patient with a giant right atrium and permanent atrial fibrillation. The reduction plasty involved excisions of the redundant atrial wall along two ablation lines of the maze procedure, and excision of the redundant thin septum primum. This procedure achieved a 60% dimension reduction and an 80% volume reduction. The reduced atrial size and sinus rhythm were maintained after 3.5 years.

BACKGROUND: Atrial fibrillation (AF) is a relatively common complication in elderly patients with an atrial septal defect (ASD). However, use of the simultaneous maze procedure for AF associated with ASD remains controversial. We examined the efficacy and risk of the use of the maze procedure in these patients.
METHODS AND RESULTS: Between March 1992 and April 1997, 26 patients underwent the maze procedure as a concomitant operation with ASD closure (maze group). Kosakai's modified maze procedure was performed in 17 patients, the modified Cox maze II or III procedure was performed in 6, and t

OBJECTIVE: Blunted sinus node response to exercise has been reported after the maze operation. We suggested the autonomic vegetative function of the heart to be disturbed after the maze procedure.
METHODS: 17 patients, mean age 63 +/- 15 years, with chronic atrial fibrillation for 49 +/- 46 months (range 5-65) underwent the maze procedure during mitral valve surgery. Bicycle stress test, 24-h electrocardiography and heart rate variability were analysed in 11 patients after three and in six after 14 +/- 3 months.
