The medial collateral ligament (MCL) is the most commonly injured ligament of the knee. There is a spectrum of injury severity, and injuries may be acute or chronic. The MCL is also frequently injured in conjunction with other knee structures. Clinical evaluation of the knee is important to assess the degree of surgical acuity, but magnetic resonance imaging can provide details about the injury that may not be obvious clinically. In addition to injury, MCL bursitis can occur and may be treated with needle aspiration and corticosteroid injection. This review article covers the anatomy and biome

This study presents a novel approach to revising failed medial unicompartmental knee arthroplasty into kinematically aligned total knee arthroplasty using an augmented reality (AR) system (NextAR by Medacta International). Traditional methods involve mechanically aligned total knee arthroplasty (TKA), which can be complex and often requires additional components like stems and augments. The proposed technique builds on the caliper-based manual method by integrating AR-based navigation to enhance precision during surgery, particularly in maintaining the patient's native joint line and soft tiss

PURPOSE: The concept of medial stabilizing technique total knee arthroplasty (MST-TKA) is to minimize the medial release without the superficial layer of medial collateral ligament (MCL). However, it is unclear at what stage the proper medial laxity is obtained during surgery. The purpose of this study was to investigate the implication of deep layer of MCL (dMCL) and osteophyte resection on medial laxity during MST-TKA.
METHODS: A total of 103 consecutive patients who underwent cruciate-retaining TKA using the navigation system were included. The intraoperative hip-knee-ankle (HKA) angle was

PURPOSE: A dynamic iso-height Axis (IHA) was invented based on knee flexion motion and supposed to be a target to restore knee soft tissue balance, which is critical in knee surgeries. This study aims to validate the correlation between IHA and the elongation patterns of the medial and lateral collateral ligaments (MCL and LCL).
METHODS: Twenty-six patients with unilateral MCKOA and intact contralateral knees were enrolled. Three-dimensional models of knee were built from CT scan. The TEA, GCA and footprints of superficial, deep MCL (sMCL, dMCL) and LCL were determined based on knee surface m

OBJECTIVE: To summarize the prevention and treatment of iatrogenic medial collateral ligament (MCL) injuries in total knee arthroplasty (TKA).
METHODS: The relevant literature about iatrogenic MCL injuries in TKA was summarized, and the symptoms, causes, preventions, and treatments were analyzed.
RESULTS: Preventions on the iatrogenic MCL injuries in TKA is significantly promoted. With the occurrence of MCL injuries, the femoral avulsion can be fixed with the screw and washer or the suture anchors; the tibial avulsion can be treated with the suture anchors fixation, bone staples fixation, or

Understanding the anatomy and biomechanics of the medial collateral ligament (MCL) is crucial in producing good outcomes after total knee arthroplasty. A solid grasp of the surgical techniques that address the MCL are necessary to ensure good coronal plane ligament balance. Furthermore, intraoperative injury to the MCL in total knee arthroplasty is an uncommon yet serious complication which often goes unrecognized. Loss of the integrity of the MCL can lead to instability, loosening, and accelerated polyethylene wear. There is still controversy regarding the ideal method of treatment of intraop

High-grade medial collateral ligament (MCL) injury and chronic medial knee instability can compromise both valgus and rotational control, especially in cruciate or multiligament injury. This systematic review evaluated operative treatment according to clinical phenotype rather than MCL grade alone. PubMed/MEDLINE and Web of Science Core Collection were searched from January 1, 2000, to August 10, 2026; a Cochrane CENTRAL update was completed on September 7, 2026. Eligible clinical reports included operative treatment of acute high-grade MCL/medial-complex injury or chronic symptomatic medial i

BACKGROUND: Although a medial collateral ligament (MCL) injury is associated with anteromedial rotatory instability (AMRI) and often with an anterior cruciate ligament (ACL) injury, there has been little work to develop anteromedial (AM) reconstruction to address this laxity.
PURPOSE: To measure the ability of a novel "anatomic" AM reconstruction technique to restore native knee laxity for isolated AM insufficiency and combined AM plus posteromedial insufficiency.
STUDY DESIGN: Controlled laboratory study.
METHODS: A total of 12 cadaveric knees were mounted in a kinematic testing rig that a

The changing distributions of collagens and glycosaminoglycans have been studied at the attachments of the medial collateral ligament during postnatal development. The ligament is of particular interest because it has a fibrocartilaginous attachment to the femoral epiphysis, but a fibrous one to the tibial metaphysis. Ligaments were examined in rats killed at birth and at 2, 4, 6, 8, 10, 20, 30, 45, 60, 90 and 120 days after birth. Cryosections were immunolabelled with monoclonal and polyclonal antibodies against types I and II collagen, chondroitin 4 and 6 sulfate, dermatan and keratan sulfat

Medial knee pain is common in clinical practice and can be caused by various conditions. In rare cases, it can even be by calcific bursitis of the medial collateral ligament (MCL). Treatment of calcific bursitis and/or calcification of the MCL classically includes observation, local injections, shockwave therapy and surgical resection. We report a case of nontraumatic medial knee pain poorly responsive to conservative treatments. Ultrasound (US) imaging revealed a massive lobed hyperechoic formation with partial acoustic shadow in the MCL context compatible with calcific bursitis, and magnetic

Injuries involving the medial collateral ligament (MCL), or combined injuries to both the anterior cruciate ligament (ACL) and MCL, are common and present considerable diagnostic and therapeutic challenges. Better understanding of MCL anatomy and biomechanics, especially in combined ACL-MCL injuries, has driven the need for updated clinical examination and treatment approaches. The MCL complex comprises the superficial MCL (sMCL), deep MCL (dMCL), and the posteromedial complex (PMC), which includes the posterior oblique ligament (POL). Additional contributors to medial knee stability include t

BACKGROUND: Optimal treatment for intraoperative injury to the medial collateral ligament (MCL) during primary total knee arthroplasty remains controversial. While some advocate primary ligament repair and a period of bracing, others suggest conversion to a knee prosthesis with increased intrinsic constraint. The purpose of this study was to characterize the outcomes of primary repair followed by bracing.
METHODS: We performed a retrospective review of consecutive primary total knee arthroplasties to identify patients with intraoperative MCL laceration or avulsion treated with primary repair.

AIM: To evaluate the effectiveness of a novel injection technique in the management of recalcitrant medial collateral ligament (MCL) injuries of the knee.
MATERIALS AND METHODS: The injection, comprising 10 ml local anaesthetic with 25-50 mg hydrocortisone, is directed beneath the periosteal attachment of the MCL. Twenty-eight patients who received the intervention were asked to complete a questionnaire, a visual analogue scale (VAS) and the International Knee Documentation Committee (IKDC) subjective knee form to quantify symptoms pre-injection and at follow-up. Data were assessed using desc

PURPOSE: Medial collateral ligament (MCL) release is one of the essential steps toward the achievement of ligament balancing during the total knee arthroplasty (TKA) in patients with varus deformity. When the varus deformity is severe, complete release of the MCL until balanced is often required. However, it is believed that complete MCL release may lead to catastrophic laxity. The purpose of this prospective study is to compare the medial joint gap opening in postoperative valgus stress radiograph in patients with complete MCL release against patients with partial release.
METHODS: Out of 20

PURPOSE: Kinematically aligned total knee arthroplasty (KA TKA) targets restoration of patient-specific alignment and soft tissue laxity. However, whether KA TKA reproduces native soft tissue strain remains unclear. This cadaveric study tested the hypothesis that KA TKA would better restore the quantitative strain and strain distribution of medial collateral ligament (MCL) to the native healthy knee compared to mechanically aligned (MA) TKA.
METHODS: Twenty-four fresh-frozen cadaver knees (12 pairs) were mounted on a customized knee squatting simulator to measure MCL strain during flexion. Fo

Superficial medial collateral ligament (MCL) injury is an occasional intraoperative complication during total knee arthroplasty (TKA) that can lead to failure. Although previous studies have recommended complex repair or conversion to a constrained implant, the authors evaluated results of superficial distal MCL reapproximation using bone staples. Records of 31 patients who underwent staple reapproximation for superficial MCL avulsion from the tibial attachment during primary TKA from 2005 to 2015 were reviewed. They were compared with 685 patients who underwent uncomplicated TKA (primary cont

This article describes an all-arthroscopic knotless suture-bridge technique for medial collateral ligament repair. Its key steps involve placing a medial row anchor and a lateral row anchor, connected via sutures passed through the torn ligament, thereby applying the suture-bridge technique to achieve secure knotless fixation. Compared with conventional open knotted repair, this method offers a fully arthroscopic approach that avoids extensive dissection and eliminates knot-related complications. The technique effectively restores knee joint stability, providing a minimally invasive surgical o
