Median neuropathy in malunited fractures of the distal radius.

PURPOSE: Irritation of the median nerve is a well-characterized complication after acute fractures of the distal radius, but there is limited literature on median neuropathy in malunited fractures. The aims of our prospective study were to estimate the prevalence of the median neuropathy, explore the relationship between radiographic findings and the condition, and investigate whether corrective osteotomy without carpal tunnel release was a sufficient treatment. METHODS: Thirty consecutive patients, who were referred to us for treatment of symptomatic distal radial malunion, underwent nerve c

Use of palmitoylethanolamide in the entrapment neuropathy of the median in the wrist.

AIM: Carpal tunnel syndrome (CTS) is a medical condition in which the median nerve is compressed, leading to discomfort and pain. Palmitoylethanolamide (PEA) is an endogenous fatty acid amide, able to modulate inflammatory cell reactivity and pain. This study deals with the capability of PEA to normalize the electroneurographic alterations associated with moderate CTS. METHODS: Patients displaying moderate CTS were enrolled and daily PEA (600 mg or 1 200 mg/die) was administered for 30 days. Control group received no treatment. RESULTS: PEA treatment significantly improved the CTS-induced re

Upper extremity neuropathies following median sternotomy.

The status of 958 patients who underwent median sternotomy between January, 1978, and May, 1981, was analyzed. Fifty-four patients had an upper extremity neuropathy. Among 38 patients who underwent further evaluation, motor and sensory nerve conduction studies localized the injury to the level of the elbow in 13, to the brachial plexus in 10, and to both locations in 6. Ninety-two percent of these 38 patients were asymptomatic 3 months after operation.

Correlation between subclinical median neuropathy and the cross-sectional area of the median nerve at the wrist.

Although subclinical median neuropathy is not uncommon, its correlation with sonographic changes to the median nerve at the wrist has not been studied. We included 62 subjects (with 107 wrists) who reported experiencing no hand numbness. All subjects underwent nerve conduction studies (NCS) and sonography for median nerve at the pisiform level. The cross-sectional area (CSA), perimeter, long axis and short axis of median nerve were obtained off-line by manual tracing with a mouse and computed by one algorithm written in MatLab. Eighteen wrists met the inclusion criteria of subclinical median n

Pediatric median neuropathy due to pruritus in Alagille syndrome.

Median entrapment neuropathy or carpal tunnel syndrome is uncommon in children. The majority of cases are related to genetic conditions which result in skeletal dysplasia or altered connective tissue characteristics, direct injury to the median nerve caused by intensive sports or trauma, or hereditary neuropathy with liability to pressure palsies. This report describes a 10-year-old patient with Alagille syndrome who presented with poor fine motor skills because of an entrapment neuropathy of the median nerve at the wrist. This condition was probably caused by intermittent external compression

Sonographic pattern of median nerve enlargement in Hansen's neuropathy.

OBJECTIVES: Median nerve enlargement in leprosy seems to be more proximal than in carpal tunnel syndrome (CTS), but this feature has not been studied systematically. The aim of the study was to compare the sites of median nerve enlargement in patients with leprosy with that of patients with CTS. MATERIALS AND METHODS: Transverse sections of the median nerve were recorded from wrist to the mid-forearm (at distal wrist crease and at 2-cm: M1, 4-cm: M2, 6-cm: M3, 8-cm: M4 and 10-cm: M5, proximal to the distal wrist crease in the forearm) in patients with leprosy, CTS and healthy subjects using h

Role of Ultrasonography in Severe Distal Median Nerve Neuropathy.

PURPOSE: Electrodiagnostic studies do not differentiate severe lesions of the median nerve in the distal forearm from those within the carpal tunnel when compound muscle action potential over the abductor pollicis brevis and sensory nerve action potential are absent; needle electromyography showing denervation confined to the abductor pollicis brevis is presumed to suggest localization to the carpal tunnel, although the lesion may be in the forearm. Under these circumstances, the patient may undergo carpal tunnel release without benefit. This retrospective study looked at patients with clinica

Proximal median neuropathy and cervical radiculopathy: double crush revisited.

"Double crush" refers to the hypothesis that a single lesion along the course of a nerve predisposes that nerve to a second lesion further along its course. The reason for this is uncertain and indeed the existence of the double crush syndrome is itself debated. We present two cases of proximal median neuropathy (PMN) associated with cervical radiculopathy which we diagnosed in our EMG laboratory over a short period of time. Seeing these cases in relative rapid succession and considering the extreme rarity of PMN, its association in both cases with cervical root disease supports the notion tha

Papular mucinosis, destructive arthropathy, median neuropathy, and sicca complex.

A patient with papular mucinosis (scleromyxedema) developed an erosive seronegative rheumatoid-like arthropathy, carpal tunnel syndrome, sicca complex, and marked increase in TG(OKT8) suppressor/cytotoxic circulating T-cells akin to that reported in scleroderma. Sclerodactyly, acrolysis and stiff digits were striking but other features of scleroderma, i.e., Raynaud's and esophageal hypoperistalsis, were absent. The diagnosis of papular mucinosis, a pseudoscleroderma syndrome, should be considered in a patient with atypical arthritis, median neuropathy, myopathy, and/or sclerodactyly and a papu

Median nerve entrapment neuropathy: a review on the pronator syndrome.

Pronator syndrome is a compression neuropathy of the median nerve within the anatomical structures of the elbow and forearm. It presents with neuropathic pain, numbness, and weakness of the forearm and hand, which are often exacerbated by repetitive pronation-supination movements. Patient presentation may mimic the signs and symptoms of carpal tunnel syndrome. Diagnosis requires comprehensive clinical assessment, employing provocative examination along with electrophysiological and imaging studies for accurate evaluation. Treatment strategies encompass conservative measures such as activity mo

Isolated neuropathy of the recurrent motor branch of the median nerve.

Unlike carpal tunnel syndrome, isolated neuropathy of the recurrent motor branch of the median nerve is a rare condition as a cause of thenar muscle wasting. A focused neurophysiological approach is mandatory to reach the proper diagnosis and start specific treatment early enough to prevent possible irreversible hand function disabilities. This is a description of 16-year-old female patient who presented with isolated thenar muscle wasting for 3 years. A focused neurophysiologic approach using abductor pollicis brevis and second lumbrical muscles for recording of median nerve motor conduction

Bilateral median and ulnar neuropathy at the wrist in a parquet floorer.

Many cases of work related compression neuropathy of the ulnar and median nerves at the wrist have been described. This report presents a case of bilateral distal neuropathy of the median and ulnar nerves in a parquet floorer, who laid wooden block flooring by hand and used the palms and volar surface of both hands to hit the blocks into place. He also used an electric sander and polisher. Bilateral numbness and paraesthesias in all fingers had been present for about one year. Clinical examination was normal; the neurological assessment indicated slight impairment in response to tactile, heat,

Median-radial sensory nerve comparative studies in the detection of median neuropathy at the wrist in diabetic patients.

OBJECTIVE: Median-ulnar comparative studies (MUCS) play an important role in the electrodiagnosis of carpal tunnel syndrome, but in diabetes concomitant involvement of Guyon's canal (ulnar nerve compression at the wrist) would reduce the sensitivity of MUCS. This study tested the utility of median-radial comparative studies (MRCS) in diabetic patients. METHODS: Anti-dromic MUCS and MRCS were prospectively performed in 120 patients with diabetes, and 64 normal controls. In 28 diabetic patients, latent addition using threshold tracking was performed in superficial radial sensory axons to estima

Incidence of Median and Ulnar Neuropathy Following Nonupper Extremity Surgery.

PURPOSE: Carpal tunnel syndrome (CTS) and cubital syndrome (CuTS) are common neuropathies typically associated with upper-extremity surgeries or systemic risk factors. This study investigates the incidence of CTS and CuTS following anterior cervical discectomy and fusion (ACDF), total hip arthroplasty (THA), and total knee arthroplasty (TKA) to evaluate whether postoperative upper-extremity neuropathy is linked to perioperative systemic responses, patient-specific risk factors, or a combination of the above. METHODS: Patients who underwent ACDF, primary THA, or primary TKA at a single institu

Most Carpal Tunnel Releases Address Moderate or Severe Median Neuropathy.

BACKGROUND: The role of surgery in normal or very mild median neuropathy can be questioned given that surgery in the absence of pathophysiology may offer only nonspecific effects that can be achieved without surgery, which raises ethical concerns. It's also important to avoid misdiagnosis: given that mild median neuropathy is prevalent and generally well-accommodated, notable symptoms from mild median neuropathy can signal unhelpful thoughts and feelings of distress that are a more pressing health priority. METHODS: We identified 38 studies that categorized electrodiagnostic (EDX) measured me

Isolated Proximal Median Neuropathy after Aortic Dissection Repair: Case Report.

Surgery-related isolated proximal median neuropathy is a rare complication. Brachial plexus injury is a possible complication after major cardiac surgery; however, isolated mononeuropathy is less frequently documented. We present an unusual case of isolated proximal median neuropathy after aortic dissection repair surgery in a 39-year-old man. Electrodiagnostic study and ultrasound examinations helped in localizing the lesion to the axillary region. Serial follow-ups showed improvement in electrodiagnostic parameters, which were compatible with clinical symptoms. Partial recovery was achieved
