Surgical techniques remain the gold standard to diagnose and staging lung and pleural tumours. Non-invasive techniques have become more accurate but actually they are not enough to plan and evaluating prognosis of lung and pleural tumours. In some cases, we need to explore the pleural cavity and the mediastinal lymph node status to confirm or rule out tumour dissemination. The combination of video-assisted mediastinoscopic lymphadenectomy (VAMLA) and thoracoscopy through a single transcervical incision allows the surgeon to widen the range of the exploration and to improve the staging for lung

BACKGROUND: Extended thymectomy is indicated for children with myasthenia gravis (MG) when drug-resistance or dependence is seen. We have employed a technique for mediastinoscopic extended thymectomy (MET) on children with MG.
METHOD: A total of 14 children underwent MET at Kanagawa Children's Medical Center between 2005 and 2013. A mediastinal operation field was made by a V-shaped hook infrasternally to extirpate the thymus with adipose tissue around the thymus.
RESULTS: The operation time and the amount of blood loss were 182±44 minutes and 34±43 ml, respectively. Postoperative complicati

Mediastinal cysts account for 20% of all mediastinal masses. Symptomatic cysts need treatment, and surgical excision is the treatment of choice. One approach to resect them is video-assisted mediastinoscopy. An 80-year-old man with cough and dyspnea at rest was referred for evaluation. Chest computed tomography revealed a mediastinal cyst. Because of his symptoms, he was a candidate for surgery, and the cyst was resected by video-assisted mediastinoscopy through a cervical incision. Video-assisted mediastinoscopy is a safe and effective approach for complete resection of mediastinal cysts.

BACKGROUND: A few medical centers have used the "inflatable mediastinoscope" technology in radical esophagectomy for esophageal cancer, and its safety and short-term efficacy have been limitedly verified. However, this technology has some defects. Therefore, we developed a new method for radical resection of esophageal cancer: the expanded mediastinoscopic combined with laparoscopic radical esophagectomy (EMLE).
METHODS: From June 2019 to June 2022, we successfully performed 176 cases of radical resection of esophageal cancer using the new EMLE technique and recorded the baseline and perioper

BACKGROUND: Mediastinoscopy was originally applied for lymph node biopsy and mediastinal tumor resection. Improved video imaging with spreadable working channels enabled mediastinoscopy for inspection and tissue biopsy in the superior mediastinum but it is rarely used in minimally invasive esophageal cancer surgery. In this prospective trial, the practicability and security of spreadable video-assisted mediastinoscopic combined with laparoscopic transhiatal esophagectomy (VAME) with video-assisted thoracoscopic esophagectomy (VATE) were compared.
METHODS: A total of 200 eligible patients with

We report a 57-year-old man who was found to have a left ventricular apical mass during a routine check-up. He previously had neither any subjective symptom nor hemodynamic problem. The mass was located deeply in the left ventricular cavity. With exploration using a mediastinoscope, we successfully resected the tumor, which was a yellowish lipoid mass that adhered to the trabeculae near the apex. Histopathologic examination of the tumor was consistent with a lipoma, a very rare benign primary cardiac tumor. The mediastinoscope can be useful for surgical resection of tumors located deeply in th

BACKGROUND: Endoscopic thymectomy is commonly used for treatment of myasthenia gravis (MG) patients due to its relatively low invasiveness. However, the long-term effects of endoscopic thymectomy have not been fully evaluated.
OBJECTIVE: To assess the long-term effects of extended infrasternal mediastinoscopic thymectomy (IMT) in MG patients and compare them with those of extended transsternal thymectomy (TT).
METHODS: Among 24 MG patients without thymoma who underwent thymectomy in our Institute between January 1997 and December 2000, 14 patients who received IMT and 10 who received TT were

There are no definite criteria for the indication of surgery in lung cancer with mediastinal lymph node involvement. During the past 20 years, 100 patients (76 patients with adenocarcinoma and 24 patients with squamous cell carcinoma) have undergone thoracotomy for lung cancer with mediastinoscopic positive lymph nodes at our hospital. Of these, relatively curative resection was performed on 13 patients. The 5-year survival rate in these 13 patients was 28%, which was significantly higher than the 0% in 42 patients with relatively non-curative resection and the 0% in 26 patients with absolutel

Mediastinoscopic esophagectomy (ME) for esophageal cancer, first reported by Tangoku et al. in 2004, has evolved into a standardized procedure incorporating radical lymphadenectomy. The surgery is performed using a mediastinoscope from the neck and a laparoscope from the abdomen, creating a connected operative field within the mediastinum. ME avoids thoracotomy and one-lung ventilation, preserving respiratory muscles and significantly reducing postoperative pneumonia, while maintaining pulmonary function and quality of life. Meta-analyses have shown that although recurrent laryngeal nerve pals

Tracheal injury during mediastinoscopic esophagectomy is a life-threatening complication that is challenging to manage. However, no precise treatment has been defined. An 80-year-old male patient with upper esophageal cancer underwent a mediastinoscopic esophagectomy and gastric tube reconstruction through the posterior mediastinal route. When the esophagus was separated from the trachea using a bipolar vessel sealing system, the left side of the membranous trachea incurred a 3-cm defect 7 cm below the sternal notch. We successfully repaired the tracheal injury not by directly suturing the def

Video-assisted mediastinoscopy (VAM) is the most commonly used invasive method for the preoperative mediastinal staging of lung cancer and for the diagnosis of other mediastinal diseases. However, VAM has the risk of causing life-threatening bleeding consequent to the specific mediastinal anatomy. We adopted the ultrasonic technique for VAM biopsies that can easily distinguish the lymph nodes from the surrounding great vessels and thus makes the procedure easier and safer.

Bronchogenic cysts are congenital lesions from the primitive foregut, mostly located in the mediastinum. Surgical excision in symptomatic cases is often challenging. Video-assisted mediastinoscopy offers a safe and effective approach with less morbidity and shorter hospital stay compared to open sternotomy or posterolateral thoracotomy. We describe a case of a young female with a large symptomatic cyst located in the superior mediastinum. The cyst was completely removed through a video-assisted cervical mediastinoscopy.

BACKGROUND: Mediastinoscopic esophagectomy is a minimally invasive approach that avoids thoracotomy and single-lung ventilation. We previously performed this procedure using hand-assisted laparoscopic surgery (HALS) and recently established a totally laparoscopic transhiatal approach. This retrospective, non-randomized study reported laparoscopic technique and compared the feasibility and surgical outcomes of these approaches.
METHODS: We retrospectively reviewed 262 patients who underwent radical mediastinoscopic esophagectomy using left cervical and transhiatal approaches at the Kyoto Prefe

We herein describe a single-port mediastinoscopic method for upper mediastinal dissection in esophageal cancer surgery. After the left cervical incision and lymphadenectomy, a Lap-Protector (Hakko, Tokyo, Japan) was inserted into the wound and an EZ Access port (Hakko) was attached. Esophageal mobilization with en bloc lymphadenectomy along the left recurrent laryngeal nerve was then performed using a port-in-port technique with conventional flexible laparoscopy. Carbon dioxide insufflation expanded the intramediastinal space, and minute structures in the deep mediastinum around the aortic arc

The main prerequisite for the unimpaired wound healing after resection of tracheal stenosis is its tension-free end-to-end anastomosis. This is only achievable when the tracheal ends can be adequately mobilized. Several mobilization techniques have been described in the literature. Here, the authors describe a case of tracheal mobilization under video mediastinoscopic assistance that displayed a considerable advantage over blunt mobilization and a length gain of over 3 cm.

The treatment of bronchogenic cysts (BCs) is still controversial. Many authors advocate the complete surgical excision of cysts [by video-assisted thoracoscopic surgery (VATS) or thoracotomy] to prevent their high rate of recurrence. Nevertheless, some recent works have attracted attention to a less invasive endoscopic management of benign mediastinal cysts. Here, we report a novel, safe, effective and minimally invasive mediastinoscopic technique used in the complete resection of two mediastinal BCs. We believe that this approach can be applied, with some restrictions: lesions located in the

Acute hemorrhage during mediastinoscopy is a life-threatening complication. Although rare, iatrogenic damage of the thoracic great vessels is probable during mediastinoscopic biopsy. We report two cases of iatrogenic massive mediastinoscopic bleeding from the aortic arch and innominate artery managed initially by simple packing and controlled finally by cardiopulmonary bypass and repair under total circulatory arrest.

We report on a case of thymic cyst which was successfully treated with mediastinoscopic resection combined with a sternum lift technique. A 62-year-old man was admitted to our hospital with a thymic cyst, which was causing chest discomfort and was increasing in size. The tumor was located above the pulmonary artery in the anterior mediastinum. He underwent mediastinoscopic resection while lifting the inferior portion of the sternum with a Laparolift. The cyst was resected sharply and bluntly with the Harmonic Scalpel without rupture. The patient was discharged on the 6th postoperative day in a

INTRODUCTION: Esophageal cancer with a right aortic arch (RAA) is rare and technically demanding because anomalous vascular anatomy and altered recurrent laryngeal nerve (RLN) courses complicate safe dissection. While thoracoscopic approaches have been reported, this is the first report of mediastinoscopic esophagectomy for thoracic esophageal cancer with an RAA and a Kommerell's diverticulum (KD).
CASE PRESENTATION: A 74-year-old woman who had undergone endoscopic submucosal dissection (ESD) for lower esophageal cancer (T1a-EP) 6 months prior required balloon dilation every 2 weeks for post-
