Mechanisms of improvement in generalized anxiety disorder: A mediation and moderated mediation analysis from a randomized controlled trial.

BACKGROUND: Generalized anxiety disorder (GAD) is associated with the lowest treatment response rate among all anxiety disorders. Understanding mechanisms of improvement may help to develop more effective and personalized treatments. AIM: The objective of the study was to investigate different improvement mechanisms in the treatment of individuals diagnosed with GAD. DESIGN: We reported data from a randomized controlled trial that evaluated three different GAD treatments (mindfulness-based intervention, BMT; fluoxetine, FLX; and an active comparison group, QoL) for 8 weeks. METHOD: Mediatio

Non-Parametric Mediation Analysis of Non-Markov Illness-Death Model.

The illness-death model is widely used to characterize disease progression over time. Previous work focuses either on estimation without causal interpretation or on causal interpretation under a strong Markov assumption where the terminal event depends on the status but not the timing of the intermediate event. To bridge the research gap, we propose a new definition of counterfactual hazard that relaxes the Markov assumption by considering the entire history of the intermediate event. We derive an identification formula that involves an integral with respect to the probability density function

Breakroom Culture and Nurse Burnout: A Multilevel Moderated Mediation Analysis.

AIM: To examine whether perceived organisational support (POS) mediates the association between breakroom culture and nurse burnout (low psychological well-being and disengagement) and whether unit-level teamwork engagement moderates this mediation. BACKGROUND: Nurse burnout threatens care quality and workforce retention globally. Informal social resources embedded in the everyday work environment, such as breakroom culture (shared supportive peer interactions during rest periods), remain underexplored-particularly in nonWestern healthcare contexts. Grounded in the Job Demands-Resources theor

Continuous-time mediation analysis for repeatedly measured mediators and outcomes.

Mediation analysis aims to decipher the underlying causal mechanisms between an exposure, an outcome, and intermediate variables called mediators. Initially developed for fixed-time mediator and outcome, it has been extended to the framework of longitudinal data by discretizing the assessment times of mediator and outcome. Yet, processes in play in longitudinal studies are usually defined in continuous time and measured at irregular and subject-specific visits. This is the case in dementia research when cerebral and cognitive changes measured at planned visits in cohorts are of interest. We th

Pregnancy intentions and teenage pregnancy among Latinas: a mediation analysis.

CONTEXT: The extent to which pregnancy intentions mediate the relationship between individual, familial and cultural characteristics and adolescent pregnancy is not well understood. The role of intentions may be particularly important among Latina teenagers, whose attitudes toward pregnancy are more favorable than those of other groups and whose pregnancy rates are high. METHODS: Prospective, time-varying data from 2001-2004 were used to investigate whether two measures of pregnancy intentions, wantedness and happiness, mediated associations between risk factors and pregnancy among 213 Latina

Sleep duration, depressive symptoms, and postoperative delirium: a mediation analysis.

PURPOSE: The mechanism linking postoperative sleep duration to postoperative delirium (POD) remains unclear. This study examined the potential indirect pathway involving depressive symptoms (assessed via the PHQ-9) in the association between postoperative sleep duration and POD. METHODS: We included 500 patients aged ≥ 65 years undergoing unilateral total hip or knee arthroplasty under general anesthesia. The associative pathways among sleep duration, depressive symptoms, and POD were assessed using covariate-adjusted three-step linear probability models with heteroscedasticity-consistent (HC

Mediation analysis using incomplete information from publicly available data sources.

Our work was motivated by the question whether, and to what extent, well-established risk factors mediate the racial disparity observed for colorectal cancer (CRC) incidence in the United States. Mediation analysis examines the relationships between an exposure, a mediator and an outcome. All available methods require access to a single complete data set with these three variables. However, because population-based studies usually include few non-White participants, these approaches have limited utility in answering our motivating question. Recently, we developed novel methods to integrate sev

Mediation analysis with multiple mediators under unmeasured mediator-outcome confounding.

It is often of interest in the health and social sciences to investigate the joint mediation effects of multiple post-exposure mediating variables. Identification of such joint mediation effects generally require no unmeasured confounding of the outcome with respect to the whole set of mediators. As the number of mediators under consideration grows, this key assumption is likely to be violated as it is often infeasible to intervene on any of the mediators. In this article, we develop a simple two-step method of moments estimation procedure to assess mediation with multiple mediators simultaneo

Air pollution, environmental perceptions, and citizen satisfaction: A mediation analysis.

Air pollution is more than an environmental or health issue. Our findings suggest that Chinese citizens exposed to more industrial sulfur dioxide (SO2) and dust emissions are less likely to be satisfied with environmental administration. Chinese citizens living with poorer air quality also tend to be dissatisfied with law enforcement. Last, perceptions of air pollution are affected by measured industrial emissions and shape citizen satisfaction. In sum, this paper provides evidence that air pollution corrodes citizen satisfaction via environmental perceptions. This improves understanding of th

Metacognition, perseverative thinking, and pain catastrophizing: A moderated-mediation analysis.

BACKGROUND: Pain catastrophizing is linked to a range of negative health and treatment outcomes, although debate continues about how best to define and treat it, since most interventions produce only modest benefit. This study aimed to contribute to theory-driven development of these treatments by exploring the role of perseverative thinking in pain catastrophizing, along with the higher order beliefs, called metacognitions that might shape it. METHODS: An Internet sample of 510 people with chronic pain (≥3 months), who mostly (54.9%) had clinical levels of catastrophizing, completed self-rep

Associations between childhood maltreatment and adult depression: a mediation analysis.

BACKGROUND: There is ample evidence showing that childhood maltreatment (CM) is a risk factor for the development of depression in adulthood. However, little is known about the psychological processes mediating this relationship. This study used a large community sample to investigate the mediating role of emotional, cognitive and/or interpersonal dysfunctions on the one hand and posttraumatic stress disorder symptoms on the other hand. METHODS: One thousand twenty seven participants of a community sample filled out an online survey. Mediation analyses were computed via linear structural equa

Chronic Obstructive Pulmonary Disease Influence on Lung Cancer Risk Through Blood Metabolite Mediation: A Two-Sample Mendelian Randomisation and Mediation Analysis.

PURPOSE: Lung cancer (LC) risk is increased by chronic obstructive pulmonary disease (COPD) through metabolic and inflammatory processes. In addition to identifying possible inflammatory or blood metabolite mediators, this study explores the causal link between LC and COPD. PATIENTS AND METHODS: To determine whether COPD and LC are causally related, as well as whether inflammatory variables and blood metabolites mediate this relationship, we used two-sample and two-step Mendelian randomization (MR) analyses based on summary data from published genome-wide association studies (GWAS). Mediation of Adverse Pregnancy Outcomes in Autoimmune Conditions by Pregnancy Complications: A Mediation Analysis of Autoimmune Conditions and Adverse Pregnancy Outcomes.

OBJECTIVE: Autoimmune conditions are associated with an increased risk of adverse pregnancy complications and outcomes, suggesting that pregnancy complications may mediate the excess risk. We performed a causal mediation analysis to quantify the mediated effects of autoimmune conditions on adverse pregnancy outcomes. METHODS: We queried a California birth cohort created from linked birth certificates and hospital discharge summaries. From 2,963,888 births, we identified women with rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), psoriasis, and inflammatory bowel disease (IBD). P

Strengthening Workplace Health Promotion Through Emergency Preparedness: A Normative Mediation Analysis.

Workplace emergency preparedness is increasingly recognized as a cornerstone of occupational health promotion and prevention. Understanding the mechanisms through which institutional arrangements are associated with employees' willingness to provide emergency assistance is essential for advancing prevention science and strengthening organizational safety culture. Guided by the Theory of Planned Behavior (TPB), this study tested a dual-mediation structural model using survey data from 147 frontline employees in a private-sector organization engaged in high-risk operational tasks. Perceived work

Confirmatory Person-Oriented Mediation Analysis in Categorical Variables-Options and Limitations.

In this article, we propose three new approaches to analyzing person-oriented mediation hypotheses. First, we propose using relevance logic instead of classical logic. Relevance logic (1) rejects the ex falso sequitur quodlibet ("from a falsehood anything follows") rule and (2) requires that the premise and the outcome of an implication exhibit variable sharing, that is, possess a substantive link. Propositions are declared irrelevant when the premise is false, when there is no substantive link, or both. Second, we discuss, based on relevance logic, alternative concepts of mediation. Specifica

Exploring pathways between residential greenness and antepartum stillbirth: A mediation analysis.

BACKGROUND: Antepartum stillbirth is an important public health issue. This study aimed to identify mediation pathways from the vegetation greenness surrounding the maternal residence to antepartum stillbirth, through stillbirth risk factors. METHODS: This study is part of the FetRisks project-a population-based case-control study of stillbirth and live births in 14 public hospitals in the Municipality of São Paulo, Brazil (December 2019-December 2023). We estimated vegetation greenness using the median value of the normalized difference vegetation index (NDVI) obtained from satellite images

Preterm birth etiological pathways: a Bayesian networks and mediation analysis approach.

BACKGROUND: The aim of this study was to determine the mediating effect of spontaneous preterm birth (PTB) main predictors that would allow to suggest etiological pathways. METHODS: We carried out a case-control study, including sociodemographic characteristics, habits, health care, and obstetric data of multiparous women who gave birth at a maternity hospital from Tucumán, Argentina, between 2005 and 2010: 998 women without previous PTB who delivered at term and 562 who delivered preterm. We selected factors with the greatest predictive power using a penalized logistic regression model. A da

Birth weight, biological maturation and obesity in adolescents: a mediation analysis.

This study was aimed to investigate associations between birth weight and multiple adiposity indicators in youth, and to examine potential mediating effects by biological maturation. This was a school-based study involving 981 Brazilian adolescents aged between 10 and 17 years. Birth weight was reported retrospectively by mothers. Maturation was estimated by age of peak height velocity. Adiposity indicators included body mass index (BMI), waist circumference and percent body fat estimated from triceps and subscapular skinfolds. Multilevel mediation analyses were performed using the Sobel test,
