[Quality assurance in antenatal, perinatal and neonatal care. A medical audit].

Formalized quality assessment of prophylaxis, diagnosis, treatment, and care has recently been introduced in the Danish health service. A review is presented of quality assessment undertaken by the medical audit method and a model is described for quality assurance of prophylaxis, diagnosis, treatment and care during the ante-, peri and neonatal periods. The information is based partly on results from a medical audit project and also experience from work in a perinatal committee in the County of Copenhagen. The method of medical audit is reviewed, including here the specialists participating,

An audit of basic practical skills acquisition of final year medical students in a Nigerian medical school.

BACKGROUND/OBJECTIVE: Young medical graduates undertaking their housemanship are naturally expected to demonstrate reasonable competence in basic practical skills. Failure to do this may not only be a source of anxiety to the doctor but also potentially dangerous to the patient. The objective was to assess the level of exposure of final year medical students of a Nigerian medical school to basic practical skills. MATERIALS AND METHODS: This is a descriptive cross-sectional study. Structured questionnaires were distributed to a set (all) of 86 final year medical students of the University of P

An audit of regular medication compliance prior to presentation for elective surgery.

AIM: Patients presenting for elective surgery frequently omit one or more regular medications. There is evidence that withholding some medication classes may lead to harm in the peri-operative period. We aim to quantify medication non-compliance in an adult, elective surgical population and identify the reasons for this non-compliance. METHOD: Data was collected over a four-week period at an elective surgical facility on adult patients presenting for surgery. Surgery and anaesthesia type, the names of regular medications and reasons for omission were recorded. This information was matched to

An inter-district audit of the school entry medical examination in Cheshire.

An inter-district audit was carried out on the benefit to children of universal school entry medical examination, the appropriate use of professional time and to establish a baseline against which to measure future changes in services to under-fives and schoolchildren. The school health records of 1127 Cheshire schoolchildren, a random 10% sample, were scrutinised. There were significant district variations. Overall, 45% of children had problems not previously noted: 21% were serious enough to be referred; 11% could have had their problems managed by a well-trained school nurse but 9.8% would

Audit of child maltreatment medical assessments in a culturally diverse, metropolitan setting.

OBJECTIVE: Child maltreatment (CM) is a major public health problem globally. While there is evidence for the value of medical examination in the assessment of CM, little is known about the quality of clinical assessments for CM. South Western Sydney (SWS) has a large metropolitan population with many vulnerable subgroups. We aimed to describe acute presentations of CM in SWS over a 3-year period-with a focus on the quality of the clinical assessments. We wanted to determine whether the cases assessed fulfilled established minimum standards for clinical assessment of CM and whether the assessm

A Structured, Auditable Taxonomy for Pharmacist-Identifiable Medication-Related Risk Signals in Routine Admission Data: A Retrospective Medication Safety Audit in a High-Risk Inpatient Cohort.

Background Hospital pharmacists collect rich admission data, i.e., medication history, brought-in medicines, prescription and laboratory records, and vital signs, yet the medication-related problems these data contain are surfaced by ad hoc, non-auditable judgment. Existing drug-related problem classifications code a problem after it has been recognized; they do not specify which admission data element to examine or which decision rule to apply. The prespecified aims of this study were to develop a taxonomy of pharmacist-identifiable medication-related risk signals with explicit operational de

Two year audit of perinatal mortality at Kathmandu Medical College Teaching Hospital.

INTRODUCTION: Perinatal mortality rate is a sensitive indicator of quality of care provided to women in pregnancy, at and after child birth and to the newborns in the first week of life. Regular perinatal audit would help in identifying all the factors that play a role in causing perinatal deaths and thus help in appropriate interventions to reduce avoidable perinatal deaths. AIMS AND OBJECTIVES: This study was carried out to determine perinatal mortality rate (PMR) and the factors responsible for perinatal deaths at KMCTH in the two year period from November 2003 to October 2005 (Kartik 2060

Antibiotic use review in ambulatory care using computer-assisted medical record audit.

The use of a computer-assisted medical record audit (CAMRA) for reviewing antibiotic use in ambulatory patients was evaluated. A random sample of 40 medical records documenting treatment of streptococcal pharyngitis, otitis media, or acute, uncomplicated urinary tract infections at two family practice clinics was used to evaluate the accuracy and efficiency of computerized prescription screening and CAMRA relative to medical record audit (MRA) alone. Accuracy was the ability to correctly classify antibiotic therapy as appropriate or inappropriate. The initial computerized prescription screenin

Education and audit can improve the identification of excessive drinkers among medical inpatients.

This study investigated the effect of education and audit on the identification and management of medical inpatients with excessive alcohol consumptions by use of a cross-sectional survey of medical and nursing notes, compared with historical controls. We report the proportion of medical and nursing notes containing qualitative and quantitative drinking histories, the use of the CAGE questionnaire, and notification of alcohol consumption in discharge summaries. A total of 1,979 out of 2,680 (74%) notes of eligible patients were examined. The number of medical notes containing quantitative alco

Medical audit and formulary management: a policy for rational use of thrombolytic drugs.

In this article we report on the development, introduction, and maintenance of a policy to promote rational use of thrombolytic drugs by hospital doctors. The work was undertaken within the framework of the voluntarily operated Riverside East drugs guide (formulary) management system (FMS). The policy was introduced in October 1988 and revised in November 1989 to coincide with the launch of the new, expensive thrombolytic drugs, alteplase (rt-PA, Actilyse) in 1988 and antistreplase (APSAC, Eminase) in 1989. Streptokinase was recommended as the first-line drug for patients who had not received

The influence of medical audit on the management of epistaxis in three district general Hospitals.

The implementation of audit in most hospital departments in the UK has been instigated by the guidelines set out by the Royal Colleges. This paper aims to demonstrate the results of regular subregional audit meetings in the ENT departments involving three District Hospitals in East Anglia. We report the effects of audit meetings in improving the management of epistaxis, whereby a protocol for treatment and discharge was established, the duration of the nasal pack left in situ was reduced, and the material of the pack was changed.

A private breast imaging practice: medical audit of 25,788 screening and 1,077 diagnostic examinations.

During 1989 and 1990, 25,788 screening and 1,077 diagnostic breast imaging examinations were performed. Audit was performed in 6-month intervals to allow comparison of performance over time. Sensitivity, positive predictive value, and stage of disease were determined for each radiologist, for patients over and those under 50 years of age, and for patients with and for those without a suspect palpable breast abnormality. In the screening portion of the study, 1,539 of 25,788 (5.9%) patients were asked to return for diagnostic breast imaging, 119 of 188 (63%) cancers were stage 0 or stage 1 dise

Diagnosis and Treatment of Acute Heart Failure: A Retrospective Observational Study and Medical Audit.

Background: Acute Heart Failure (AHF) is a leading cause of hospitalizations and remains a significant socioeconomic burden. Despite advances in medical care, mortality and rehospitalization rates remain high. Previous AHF audits have revealed regional differences and a poor adherence to guidelines. This study aimed to assess guideline adherence in a public teaching hospital to identify areas for improvement. Methods: This retrospective observational study examined clinical routine data of patients hospitalized for AHF at a Swiss public teaching hospital between 2018 and 2019. AHF management w

Does medical audit lead to explicit standards? Experience with thrombolysis in four UK hospitals.

The setting of explicit standards against specific criteria is a recognized part of the medical audit cycle, but often in practice it has been neglected, implicit judgements being used instead. The conduct of a study to evaluate audit among physicians in four UK district general hospitals provided an opportunity to encourage the setting of explicit standards and observe the results. The subject chosen for audit by the participating physicians was the extent of use of intravenous thrombolysis in patients with suspected acute myocardial infarction (AMI). Standard setting was requested at initial
