Emerging drugs for the treatment of progressive familial intrahepatic cholestasis: a focus on phase II and III trials.

INTRODUCTION: Progressive familial intrahepatic cholestasis (PFIC) is a group of disorders characterized by inappropriate bile formation, causing hepatic accumulation of bile acids and, subsequently, liver injury. Until recently, no approved treatments were available for these patients. AREAS COVERED: Recent clinical trials for PFIC treatment have focused on intestine-restricted ileal bile acid transporter (IBAT) inhibitors. These compounds aim to reduce the pool size of bile acids by interrupting their enterohepatic circulation. Other emerging treatments in the pipeline include systemic IBAT

Chaperone-aided in vitro renaturation of an engineered E1 envelope protein for detection of anti-Rubella virus IgG antibodies.

The envelope glycoproteins of Rubella virus, E1 and E2, mediate cell tropism, and E1 in particular plays a pivotal role in the fusion of the virus with the endosomal membrane. Both are the prime targets of the humoral immune response. Recombinant variants of the E1 ectodomain as well as E1 antigen preparations from virus lysates are commonly used to detect anti-Rubella immunoglobulins in human sera. Hitherto, recombinant E1 for diagnostic applications has been produced chiefly in eukaryotic expression systems. Here, we report the high-yield overproduction of an engineered E1 ectodomain in the

Surgery for gastroesophageal reflux disease with Gaucher disease type 2.

Gaucher disease, the most common lysosomal storage disease, is sometimes complicated with gastroesophageal reflux disease (GERD). The present patient was a 136-day-old Japanese boy with Gaucher disease type 2. Enzyme replacement therapy and chemical chaperone therapy were successful for the skin disorders, joint contractures, hepatosplenomegaly and thrombocytopenia, but he also had GERD. Accordingly, a Nissen fundoplication with gastrostomy was performed. There was no vulnerability of organs, easy bleeding or difficulty of maintaining the visual field because of hepatosplenomegaly during opera

The GM1 and GM2 Gangliosidoses: Natural History and Progress toward Therapy.

The gangliosidoses are lysosomal storage disorders caused by accumulation of GM1 or GM2 gangliosides. GM1 gangliosidosis has both central nervous system and systemic findings; while, GM2 gangliosidosis is restricted primarily to the central nervous system. Both disorders have autosomal recessive modes of inheritance and a continuum of clinical presentations from a severe infantile form to a milder, chronic adult form. Both are devastating diseases without cure or specific treatment however, with the use of supportive aggressive medical management, the lifespan and quality of life has been exte

Reconstitution of recombinant human LSm complexes for biochemical, biophysical, and cell biological studies.

Sm and Sm-like (LSm) proteins are an ancient family of proteins present in all branches of life. Having originally arisen as RNA chaperones and stabilizers, the family has diversified greatly and fulfills a number of central tasks in various RNA processing events, ranging from pre-mRNA splicing to histone mRNA processing to mRNA degradation. Defects in Sm/LSm protein-containing ribonucleoprotein assembly and function lead to severe medical disorders like spinal muscular atrophy. Sm and LSm proteins always assemble into and function in the form of ringlike hexameric or heptameric complexes whos

The C24:0 Sulfatide Isoform as an Important Molecule in Type 1 Diabetes.

Particular molecules play pivotal roles in the pathogenesis of many autoimmune diseases. We suggest that the C24:0 sulfatide isoform may influence the development of type 1 diabetes (T1D). C24:0 sulfatide is a sphingolipid with a long carbon-atom chain. A C16:0 sulfatide isoform is also present in the insulin-producing beta cells of the islets of Langerhans. The C16:0 isoform exhibits chaperone activity and plays an important role in insulin production. In contrast, the C24:0 isoform may suppress the autoimmune attacks on beta cells that lead to T1D. Sphingolipid levels are reduced in individu

[Women's experience with cesareans in Uruguay: the woman's right to be accompanied by a person of her choice and difficulties with enforcement].

The article aims to analyze how women are accompanied by a trusted person during cesareans, as guaranteed under Law 17,386 on "accompanying persons during labor and childbirth", enacted in Uruguay in 2001. The findings are part of a larger study on the experience and meaning of cesareans for Uruguayan women. An exploratory, descriptive qualitative methodology was used for this purpose. The technique involved in-depth interviews with 31 women whose cesareans were performed in their first pregnancy in the city and greater metropolitan area of Montevideo. Based on the findings, the conclusion is

Homologous chaperone-mediated soluble production of Mfp-3 and its application in atopic dermatitis therapy.

Mussel foot proteins (Mfps) are a class of specialized adhesive proteins synthesized by marine mussels. Among these, type 3 mussel foot protein (Mfp-3) has tremendous application potential as a cosmetic and medical raw material, owing to its excellent antioxidant and anti-inflammatory properties. However, the low efficiency of the natural extraction method (only 1 mg of protein can be obtained from 10,000 mussels) has hindered the industrial application. Heterologous expression via genetic recombination offers a reliable alternative. However, a major challenge is that recombinant expression fr

Moving Beyond the Androgen Receptor (AR): Targeting AR-Interacting Proteins to Treat Prostate Cancer.

Medical or surgical castration serves as the backbone of systemic therapy for advanced and metastatic prostate cancer, taking advantage of the importance of androgen signaling in this disease. Unfortunately, resistance to castration emerges almost universally. Despite the development and approval of new and more potent androgen synthesis inhibitors and androgen receptor (AR) antagonists, prostate cancers continue to develop resistance to these therapeutics, while often maintaining their dependence on the AR signaling axis. This highlights the need for innovative therapeutic approaches that aim

Exploring the efficacy and safety of Ambroxol in Gaucher disease: an overview of clinical studies.

Gaucher disease (GD) is mainly caused by glucocerebrosidase (GCase) enzyme deficiency due to genetic variations in the GBA1 gene leading to the toxic accumulation of sphingolipids in various organs, which causes symptoms such as anemia, thrombocytopenia, hepatosplenomegaly, and neurological manifestations. GD is clinically classified into the non-neuronopathic type 1, and the acute and chronic neuronopathic forms, types 2 and 3, respectively. In addition to the current approved GD medications, the repurposing of Ambroxol (ABX) has emerged as a prospective enzyme enhancement therapy option show

Recent advances in treatment approaches to Gaucher disease.

Gaucher disease is inherited as an autosomal recessive disorder. The absence of β-glucocerebrosidase whose purpose is to cleave the glucose from ceramide results in accumulation of glucocerebroside; storage of this glycolipid results in Gaucher disease. There is tremendous clinical heterogeneity: prediction of onset of symptoms (if at all), which organs will be affected, and the degree of severity of the signs and symptoms are areas of current research. Lysosomal storage diseases may be treatable by enzyme replacement therapy. Enzyme replacement for Gaucher disease has been attempted intermitt

Label-free quantitative proteomic analysis of Panax ginseng leaves upon exposure to heat stress.

BACKGROUND: Ginseng is one of the well-known medicinal plants, exhibiting diverse medicinal effects. Its roots possess anticancer and antiaging properties and are being used in the medical systems of East Asian countries. It is grown in low-light and low-temperature conditions, and its growth is strongly inhibited at temperatures above 25°C. However, the molecular responses of ginseng to heat stress are currently poorly understood, especially at the protein level. METHODS: We used a shotgun proteomics approach to investigate the effect of heat stress on ginseng leaves. We monitored their phot

Long-term efficacy of low-dose perampanel for progressive myoclonus epilepsy in a patient with Gaucher disease type 3.

PURPOSE: We report the case of a patient with progressive myoclonus epilepsy due to Gaucher disease type 3 whose seizures and ability to perform activities of daily living were significantly improved after starting low-dose perampanel therapy. CASE: Our patient's generalized tonic-clonic seizures and myoclonus did not improve despite the administration of multiple antiseizure medications and enzyme replacement therapy. The myoclonus reduced following pharmacological chaperone therapy, but this effect was temporary, and the generalized tonic-clonic seizures continued to occur. However, the gen

A case of latent heterozygous Fabry disease in a female living kidney donor candidate.

A 52-year-old woman had been found to have hematuria at her annual checkup 5 years in a row. She hoped to donate her kidney to her husband, so we performed a percutaneous kidney biopsy at our department. It was difficult for us to detect apparent abnormalities under a light microscopic examination, and she was determined to meet the eligibility criteria for living kidney transplantation. However, the sample for electron microscopy was not evaluated before kidney donation. She subsequently underwent living kidney transplantation as a donor. A 1-h biopsy revealed swelling and obvious vacuolation

Therapeutic potential of proteasome inhibitors in congenital erythropoietic porphyria.

Congenital erythropoietic porphyria (CEP) is a rare autosomal recessive disorder characterized by uroporphyrinogen III synthase (UROS) deficiency resulting in massive porphyrin accumulation in blood cells, which is responsible for hemolytic anemia and skin photosensitivity. Among the missense mutations actually described up to now in CEP patients, the C73R and the P248Q mutations lead to a profound UROS deficiency and are usually associated with a severe clinical phenotype. We previously demonstrated that the UROS(C73R) mutant protein conserves intrinsic enzymatic activity but triggers prematu

Screening for health-related quality of life and its determinants in Fabry disease: A cross-sectional multicenter study.

BACKGROUND: Fabry disease (FD) is a rare X-linked lysosomal storage disorder caused by α-galactosidase A (α-Gal A) deficiency. The progressive accumulation of globotriaosylceramide results in life-threatening complications, including renal, cardiac, and cerebrovascular diseases. In order to improve health care of FD-patients, knowledge of its predictors is important. The aim of our study was to evaluate health-related quality of life (HrQol) in FD and to identify its independent determinants by exploring a wide range of demographic, social and clinical parameters. RESULTS: In this cross-secti

Variants in ABCB4 (MDR3) across the spectrum of cholestatic liver diseases in adults.

The ATP binding cassette subfamily B member 4 (ABCB4) gene on chromosome 7 encodes the ABCB4 protein (alias multidrug resistance protein 3 [MDR3]), a P-glycoprotein in the canalicular membrane of the hepatocytes that acts as a translocator of phospholipids into bile. Several variants in ABCB4 have been shown to cause ABCB4 deficiency, accounting for a disease spectrum ranging from progressive familial cholestasis type 3 to less severe conditions like low phospholipid-associated cholelithiasis, intrahepatic cholestasis of pregnancy or drug-induced liver injury. Furthermore, whole genome sequenc

Challenges in the conduct of community-based research.

Community-based research is an excellent opportunity for medical students to learn outside the boundaries of a medical college, where health-related conditions and factors can be studied in a larger social context. It also brings a variety of other educational and social learning with it. The research question is usually a community-driven issue. There are multiple ways to arrive at a research question. Sampling poses a unique challenge, especially in terms of availability of a sampling frame. However, accessing electoral rolls, census data and approaching community leaders of voluntary agenci
