Insulation failure with externalized conductor of a Linox SD lead: a case report.

A 53-year-old man with a dual-chamber implantable cardioverter-defibrillator presented to the electrophysiology (EP) lab for an EP study in evaluation of supraventricular tachycardia despite medical therapy. Fluoroscopic imaging during placement of diagnostic catheters demonstrated insulation failure with an externalized conductor on the right ventricular defibrillator lead just distal to the proximal defibrillator coil. Device interrogation revealed normal pacing, and shock impedances, normal sensing, and stable pacing thresholds.

Ten-Year Risk of Recall of Novel Spine Devices.

STUDY DESIGN: Observational epidemiological study. OBJECTIVE: This study's primary objective was to examine the risk of recall for novel spine devices over time. Secondarily, we sought to analyze interbody fusion and vertebral body replacement (VBR) devices (corpectomy cages) as a risk factor for recall. BACKGROUND: The recall risk of a novel spine device over time has not been reported. In addition, FDA regulations were lowered for interbody fusion devices to enter the market in 2007. As well, VBR implants were recently approved by the FDA for use in the cervical spine in 2015. MATERIALS A

Recalls of Arthroscopic Equipment After Food and Drug Administration 510(k) Approval: A Twenty-Year Analysis of Causes, Trends, and Time to Recall (2004 to 2024).

BACKGROUND: Current reliance on the expedited 510(k) approval pathway has driven rapid commercial availability of novel arthroscopic devices. Despite the low complication rates of arthroscopic procedures, products from this pathway are suspected to increase the rate of recalls and device malfunctions. PURPOSE: This study aimed to characterize arthroscopic device recalls, analyze trends in recall incidence, and identify predictors of time to recall. METHODS: A 20-year, retrospective, cross-sectional study was conducted using the Food and Drug Administration (FDA) Recalls database. Recalled de

Complications from prophylactic replacement of cardiac implantable electronic device generators in response to United States Food and Drug Administration recall: A systematic review and meta-analysis.

BACKGROUND: The number of cardiac implantable electronic device (CIED) recalls and advisories has increased over the past 3 decades, yet no consensus exists on how to best manage patients with these CIEDs, partially because rates of complications from prophylactic replacement are unknown. OBJECTIVE: The purpose of this study was to establish rates of complications when recalled CIED generators are replaced prophylactically. METHODS: We searched MEDLINE and the Cochrane Controlled Trials Register for reports of prophylactic replacement of recalled CIED generators. Studies with <20 subjects we

Levonorgestrel intrauterine device with absence of threads: don't take expulsion for granted! A case series of possible errors in ultrasound evaluation, in case of "lost IUDs".

Levonorgestel intrauterine devices are safe contraceptive methods, with even medical indications. However, they may present rare, but severe complications such as perforation and abdominal dislocation. Ultrasound assessment may be limited if a medicate intrauterine device is lost, due to its particular echogenicity. We report cases complicated by loss of intrauterine devices. Based on this experience we believe that ultrasound should always be associated to an X-ray examination to prevent misdiagnosis, especially in case of loss of a levonorgestrel device. Although our conclusions are not inno

Balloon dilation complication during the treatment of subglottic stenosis: background of the FDA class 1 recall for the 18 x 40-mm Acclarent Inspira AIR balloon dilation system.

OBJECTIVES: Balloon dilation for subglottic stenosis allows for a controlled radial expansion of the airway and is considered superior to older techniques of airway dilation. We report the case of a 39-year-old woman with idiopathic subglottic stenosis who had entrapment of an inflated balloon in her subglottis due to device failure while undergoing balloon dilation of the stenotic area. METHODS: We present a case report and a review of the literature. RESULTS: As a result of this and other reported incidents, on March 13, 2012, the US Food and Drug Administration issued a class 1 recall of

Single-incision sling operations for urinary incontinence in women.

BACKGROUND: Urinary incontinence has been shown to affect up to 50% of women. Studies in the United States have shown that up to 80% of these women have an element of stress urinary incontinence. Colposuspension and now mid-urethral slings have been shown to be effective in treating patients with stress incontinence. However, associated adverse events include bladder and bowel injury, groin pain and haematoma formation. This has led to the development of third-generation single-incision slings, also referred to as mini-slings.It should be noted that TVT-Secur (Gynecare, Bridgewater, NJ, USA) i

Expanding the Scope of Multimodality Imaging in Durable Mechanical Circulatory Support.

An increasing number of patients transition to advanced-stage heart failure refractory to medical therapy. Left ventricular assist systems (LVAS) provide a bridge to candidates awaiting heart transplantation and extended device durability allows permanent implantation referred to as destination therapy. Noninvasive imaging plays a pivotal role in the optimal management of patients implanted with durable mechanical circulatory support (MCS) devices. Several advances require an updated perspective of multi-modality imaging in contemporary LVAS management. First, there has been substantial evolut

Transvenous extraction profile of Riata leads: procedural outcomes and technical complexity of mechanical removal.

BACKGROUND: Riata (RT) and Sprint Fidelis (SF) leads were recalled by the United States Food and Drug Administration because of an increased rate of failure mainly due to conductor fracture or insulation abrasion. According to lead design and type of failure, extraction complexity may be different, potentially affecting procedural outcomes and indications. OBJECTIVE: The purpose of this study was to assess the extraction profile of RT leads with and without cable externalization in comparison to SF leads. METHODS: From January 1997 to April 2014, all consecutive RT and SF leads extracted tra

Ventilator change in children on home mechanical ventilation affected by the Philips respironics trilogy ventilator recall.

The Philips Respironics recall notification issued in June 2021 affected many of their positive airway pressure devices and mechanical ventilators including the Trilogy 100 and 200 ventilators that are often utilized in children using home positive pressure ventilation via tracheostomy (PPV-T). Optimal strategies to replace ventilators in children using home PPV-T affected by the Philips recall are unknown. We conducted a retrospective study of children using home PPV-T with recalled Trilogy ventilators who underwent inpatient ventilator change to non-recalled portable home ventilators (PHV) u

Post-market safety signals of energy-based surgical devices: a cross-specialty disproportionality analysis of the FDA MAUDE database with robotic-platform validation and regulatory triangulation, 2015-2025.

Post-market safety evidence for energy-based surgical devices is fragmented. We characterized adverse-event signals in the FDA MAUDE database, validated them on a third (robotic) platform, and triangulated across regulatory and international sources. Medical Device Reports (2015-2025) under electrosurgical and ultrasonic product codes were analyzed with reporting odds ratios (RORs), stratified by specialty and procedure, with robustness checks. All reports under the robotic-system code were retrieved as a third-platform validation cohort. Signals were triangulated with FDA recalls, 510(k) clea

Failure modes in soft-tissue surgical robots: an adjudicated FDA recall cohort, 2005-2026.

FDA recall records document corrections and removals; however, product naming, administrative root-cause descriptors, and the functional mechanisms underlying failure are not interchangeable. Accordingly, the affected assets, functional loci, failure modes, and requested actions were characterized in recalls involving soft-tissue surgical robotic systems. A retrospective cohort was constructed from the US Food and Drug Administration (FDA) Medical Device Recall Database and openFDA through 17 July 2026. Eligible product records were deduplicated by FDA product-record identifier and grouped by

FDA regulation of cardiovascular devices and opportunities for improvement.

Medical devices are of increasing importance in cardiovascular disease and have made important contributions to patient care. These devices continue to evolve with increasing complexity. FDA regulation of medical devices involves increased stringency for higher risk and novel devices. However, there are some current opportunities to strengthen the FDA's pre-approval regulatory process and improve post-marketing surveillance. This article reviews FDA regulation of cardiovascular devices and offers suggestions for strengthening the process while focusing on examples relevant to cardiac electroph
