Implementation and expansion of inpatient and ambulatory pharmacist credentialing and privileging at an academic medical center.

PURPOSE: This article highlights one academic medical center's effort to implement a complete credentialing and privileging (C&P) process for both inpatient and ambulatory clinical pharmacists. SUMMARY: The C&P process offers a recognized method to advance pharmacy practice. Credentialing is defined as a process whereby an individual is deemed qualified in a specific subject matter area. Privileging is the process whereby an institution grants authority to an individual to perform services based on credentials. Federal guidelines permit pharmacists to obtain the same level of privileges as pr

Quality assurance of clinical transfusion practice by implementation of the privilege of blood prescription and computerized prospective audit of blood requests.

Guidelines, algorithms and recommendations have been issued in the attempt to ensure appropriateness of transfusion practice, but the results are less than satisfactory, mainly due to the difficulty to turn paper procedures into actual practice. In our hospital we have tried to overcome this difficulty through the implementation of a quality assurance programme which includes giving the privilege of nonurgent blood prescription to a limited number of physicians and a computerized prospective audit of blood requests. The latter is performed through verification of the compliance of blood reques

Coping with closure. Consideration of legal issues in hospital closing can save precious time and considerable money.

From the legal perspective, closing a hospital can be as difficult as opening one. Whether a hospital is closing or downsizing, it should become familiar with the relevant legal issues before the process begins and develop a strategy for addressing them. Hospitals planning to discontinue services often must give advance notice to federal, state, and local governmental agencies, as well as to nongovernmental organizations such as the Joint Commission on Accreditation of Healthcare Organizations and to contractors. Failure to notify the applicable state licensing agency may violate the hospital

Implementing contingency management in family medicine: A qualitative inquiry on provider and patient preferences for a low magnitude reward program compatible with buprenorphine treatment.

BACKGROUND: Contingency management (CM) is an effective yet underutilized behavioral intervention that uses rewards to improve outcomes in medication for opioid use disorder (MOUD) treatment. Prior implementation attempts have focused on specialized addiction clinics, using intensive daily treatment with methadone and high reward values (e.g. >$200 total). However, many people get MOUD from less specialized, more accessible, family medicine clinics. These clinics could also benefit from CM, yet present unique challenges for CM. Family medicine clinics typically use buprenorphine as their prima

Development and implementation of the Clinical Privileges for Dietitian Nutrition Order Writing program at a long-term acute-care hospital.

A patient nutrition care plan is of little value unless it is quickly and efficiently implemented. Recognition of the value of appropriate and timely medical nutrition intervention in improving patients' nutritional status led to the development of the Clinical Privileges for Dietitian Nutrition Order Writing (CPD NOW) program at a long-term, acute-care hospital. CPD NOW is a formalized approach specifying the types of nutrition care orders that may be written by a registered dietitian (RD), allowing nutrition recommendations to be implemented immediately. More than 95% of the facility's prima

Not what it seems: Distributed agencies and clinical narratives in electronic health records.

Medical records mediate clinical work at multiple levels. Focusing on the transition from paper-based medical records to Electronic Health Records (EHRs), we analyze how the incorporation of digital infrastructures reconfigures healthcare networks and reshapes practices of writing, reading, and narration. Our analysis draws on ethnographic research conducted between 2024 and 2025 in two psychiatric settings in southwestern Colombia. We trace how the implementation of EHRs redistributed agency within clinical documentation and altered relations among clinicians, administrative staff, software s

The National Practitioner Data Bank: coping with the uncertainties.

The National Practitioner Data Bank is a database of adverse events involving physicians and other practitioners. Querying the database is mandatory for hospitals in several situations. So too, hospitals are required to report specified adverse events. Thus, hospitals need to be able to identify incidents that are reportable events, events that require them to update the databases, and any possible liability issues that may surround the hospital's reporting duties. The author argues that the regulations are unclear in addressing these issues. Likewise, he notes that practitioners should be awa

'I can see it and I can feel it, but I can't put my finger on it': A Foucauldian discourse analysis of experiences of relating on psychiatric inpatient units.

OBJECTIVES: Research has shown interpersonal relationships influence experiences of inpatient psychiatric services. This study explored inpatient staff and service users' talk about relating, and consequences on available/limited social actions. DESIGN: A Foucauldian discourse analysis was used to analyse transcribed semi-structured interviews and focus groups with current inpatient staff members and members of a service-user involvement group. METHOD: Two focus groups (service users n = 10; staff n = 6) and five interviews (service users n = 2; staff n = 3) were held, with participants resp

Comparison of the frequency of behavioral disinhibition on alprazolam, clonazepam, or no benzodiazepine in hospitalized psychiatric patients.

Several case reports have suggested that treatment with the benzodiazepine alprazolam can result in behavioral disinhibition. To address this question, the authors reviewed the medical records (blinded to all pharmacologic treatments the patients received) of 323 psychiatric inpatients treated with alprazolam (108 patients), clonazepam (111 patients), or no benzodiazepine (104 patients) between January 1989 and June 1990. During benzodiazepine treatment, there were no significant differences among the three groups on the following measures: (1) acts of self-injury (alprazolam, 1.9%; clonazepam

219 vascular fellows' perception of the future of vascular surgery.

In an attempt to identify the fellows' concerns about the future of the field of vascular surgery, we conducted a survey consisting of 22 questions at an annual national meeting in March from 2004 to 2007. In order to obtain accurate data, all surveys were kept anonymous. The fellows were asked (1) what type of practice they anticipated they would be in, (2) what the new training paradigm for fellows should be, (3) to assess their expectation of the needed manpower with respect to the demand for vascular surgeons, (4) what were major threats to the future of vascular surgery, (5) whether they

Characteristics of psychiatrists who perform ECT.

OBJECTIVE: Use of ECT is highly variable, and previous study has linked its availability to the geographic concentration of psychiatrists. However, less than 8% of all U.S. psychiatrists provide ECT. The authors analyzed the characteristics of psychiatrists who use ECT to understand more fully the variation in its use and how changes in the psychiatric workforce may affect its availability. METHOD: Data from the 1988-1989 Professional Activities Survey were examined to investigate the influence of demographic, training, clinical practice, and geographic characteristics on whether psychiatrist

Welfare for Elites: The Student Health Center at Tokyo Imperial University and the Paradox of Medicare in Japanese Meritocracy.

How did the Japanese establish a medical welfare system? In answering this question, historians of modern Japan have accentuated the assertive role of state bureaucrats, especially from those of the Home Ministry (naimushō). Historians of Japanese medicine also emphasized the role of the state. William Johnston, in his pioneering work on tuberculosis in Japan, explored the rise of a hygiene administration on this disease as a state enterprise. In the medical history of Japan, scholars highlighted the significance of the wartime period in the birth of this system. The emphasis on the Japanese w

Trends in emergency department antibiotic prescribing for acute respiratory tract infections.

BACKGROUND: Injudicious use of antibiotics is associated with the reported rise in antibiotic-resistant bacteria. With an estimated 26 million antibiotics being prescribed annually in the emergency department (ED), the ED represents an important setting for targeting interventions. OBJECTIVE: To provide national estimates of potentially inappropriate antibiotic prescribing during ED visits for acute respiratory tract infections (ARTIs) and examine associations between patient, provider, visit characteristics, and antibiotic prescribing patterns. METHODS: A cross-sectional study was conducted

Healthcare and social responsibility. The revised directives clarify the Catholic position.

The Catholic Church participates in the U.S. healthcare system by reason of its contribution to the common good of society. To facilitate this, the Ethical and Religious Directives for Catholic Health Care Services set forth certain normative principles. Catholic healthcare is dedicated to promoting human dignity and the sacredness of life; it has an "option for the poor"; it seeks the common good, cooperating with other providers toward that end; it prohibits abortion, in vitro fertilization, contraceptive sterilization, and assisted suicide procedures in free-standing Catholic healthcare ins

From 'Little Things' to 'Seagulls': A Mixed Methods Exploration of Patients' Experiences of Person-Centred Compassionate Care in General Medical and Surgical Wards.

BACKGROUND: Person-centred compassionate care (PCCC) shapes the quality of relationships between health care professionals (HCPs) and patients, influences how healthcare is experienced, and ultimately can impact patient outcomes and HCP satisfaction. There is limited evidence about patient perspectives of multidisciplinary PCCC in acute general medical and surgical wards. OBJECTIVE: This study aimed to explore patients' perceptions of PCCC to inform co-design of an intervention to enhance PCCC in general medical and surgical wards. METHODS: Sixty (60) inpatients on a general medical or gener
