Many U.S. military veterans lack health insurance and are ineligible for care in Veterans Administration health care facilities. Using two recently released national government surveys--the 2004 Current Population Survey and the 2002 National Health Interview Survey--the authors examined how many veterans are uninsured (lacking health insurance coverage and not receiving care from the VA) and whether uninsured veterans have problems in access to care. In 2003, 1.69 million military veterans neither had health insurance nor received ongoing care at Veterans Health Administration (VHA) hospitals

INTRODUCTION: Although breast cancer deaths have declined, the mortality rate among women from medically underserved communities is disproportionally high. Screening mammography is the most effective tool for detecting breast cancer in its early stages, yet many women from medically underserved communities do not have adequate access to screening mammograms. Mobile mammography may be able to bridge this gap by providing screening mammograms at no cost or low cost and delivering services to women in their own neighborhoods, thus eliminating cost and transportation barriers. The objective of thi

BACKGROUND: People without medical insurance often lack access to medical care, increasing their risk of chronic diseases and reducing life expectancy. This disparity likely extends to end-of-life care, when patients are especially vulnerable. The main research question was whether outpatient and inpatient palliative care facilities regularly treat uninsured patients. Based on the results, the study initiated an ethical reflection on the circumstances of uninsured palliative patients in Germany.
METHODS: The study ran for 15 months from 2024 to 2025 1494 inpatient hospices, outpatient hospice

CONTEXT: Uninsured adults receive less appropriate care and have more adverse health consequences than insured adults. Longitudinal studies would help to more clearly define the effects of health insurance on health care and health.
OBJECTIVE: To assess the differential effects of gaining Medicare coverage on use of basic clinical services and medications by previously insured and uninsured adults.
DESIGN AND SETTING: Household survey data from the nationally representative Health and Retirement Study were used to analyze differences in receipt of basic clinical services by adults in 1996 an

Many transgender Americans continue to remain uninsured or are underinsured because of payers' refusal to cover medically necessary, gender-affirming healthcare services-such as hormone therapy, mental health counseling, and reconstructive surgeries. Coverage refusal results in higher costs and poor health outcomes among transgender people who cannot access gender-affirming care. Research into the value of health insurance coverage for gender-affirming care for transgender individuals shows that the health benefits far outweigh the costs of insuring transition procedures. Although the Affordab

BACKGROUND: Depression, one of the most frequent mental disorders, affects more than 350 million people of all ages worldwide, with China facing an increased prevalence of depression. Childhood depression is on the rise; globally, and in China. This study estimates the hospitalization costs and the financial burden on families with children suffering from depression and recommends strategies both to improve the health care of children with depression and to reduce their families' financial burden.
METHODS: The data were obtained from the hospitalization information system of 297 general hospi

AbstractMedical deportation, or the forced removal of severely injured and chronically ill, low-income, uninsured migrants to their countries of origin, remains a practice implemented by hospitals throughout the United States. This practice has been rightfully highly criticized by immigration advocates and human rights organizations. In 2019, the U.S. government eliminated a deferred action program allowing migrants without documentation to avoid deportation while they or their relatives were undergoing lifesaving medical treatment, forcing patients with highly complex medical needs to give up

There is a growing dissatisfaction with the American health care system that results primarily from the problem of the uninsured. This problem threatens not only the health of the uninsured, but also the health of our hospitals and ultimately of all Americans. There is a growing support for a universal health care plan, but less support for new taxes to pay for such coverage. One method to pay for universal health care may be to use the proceeds of a national lottery. In addition to possibly being more politically acceptable, there are a number of specific benefits that may follow from such a

BACKGROUND: There is limited research on screening rates among uninsured cancer survivors. Uninsured cancer survivors are at higher risk of poorer health outcomes than the insured due to limited access to preventative screening for secondary cancers. This study examines the rates of surveillance and screening of uninsured cancer survivors and compares to uninsured patients without a cancer history seen in free clinics.
METHODS: Data were collected retrospectively from electronic medical records and paper charts of patients from 10 free clinics between January 2016 and December 2018 in the Tam

BACKGROUND: Cervical cancer shows a growing incidence and medical cost in recent years that has increased severe financial pressure on patients and medical insurance institutions. This study aimed to investigate the medical economic characteristics of cervical cancer patients with different payment modes within a Grade A tertiary hospital to provide evidence and suggestions for inpatient cost control and to verify the application of Gamma model in medical cost analysis.
METHODS: The basic and cost information of cervical cancer cases within a Grade A tertiary hospital in the year 2011-2016 we

BACKGROUND: Patients with public health insurance have greater difficulty obtaining orthopaedic care than their privately insured counterparts because of lower reimbursements. However, the relationship between insurance status and financial burden for patients and treating institutions is unknown. We compared patient medical debt and uncompensated hospital costs by insurance type for pediatric patients who received nonoperative treatment for distal radius fractures (DRFs).
METHODS: We reviewed medical records of 100 pediatric patients (above 18 y) treated nonoperatively at our US academic hos

Health insurance coverage is important for accessing prenatal care, yet the association of pre-conception gaps in health insurance coverage with prenatal care initiation is unknown. Experiencing uninsurance before pregnancy could discourage use of preventive care, including prenatal care, due to difficulties with finding a provider, negative experiences with seeking care while uninsured, or medical debt accumulation. We used 2013-2023 data from a longitudinal US panel study to analyze how pre-pregnancy gaps in coverage (part-year uninsurance) and year-round lack of coverage were associated wit

IMPORTANCE: The US 340B Drug Pricing Program enables eligible hospitals to receive substantial discounts on outpatient drugs to improve hospitals' financial sustainability and maintain access to care for patients who have low income and/or are uninsured. However, it is unclear whether hospitals use program savings to subsidize access as intended.
OBJECTIVE: To evaluate whether the 340B program is associated with improvements in access to hospital-based services and to test whether the association varies by hospital ownership.

Adults with part-year or year-round uninsurance are less likely to receive recommended preventive services. Sex differences in preventive care use have been documented, but it remains unclear whether sex moderates the association between insurance coverage disruptions and preventive care use. The current study analyzed data from 37,659 adults aged 18-64 in the 2019 and 2023 National Health Interview Survey. Insurance coverage over the past 12 months was categorized as continuous private, continuous public, coverage gap, or continuously uninsured. Outcomes included blood pressure and cholestero

PURPOSE OF REVIEW: To define, describe, and identify potential solutions for health disparities in the uninsured and underinsured with migraine in the USA.
RECENT FINDINGS: Uninsured and underinsured patients with migraine experience health disparities in diagnosis and treatment of migraine. Migraine patients have higher healthcare costs and higher employment disability, which contribute to a higher likelihood of uninsured or underinsured status. Uninsured or underinsured status, combined with factors such as race, socioeconomic status, geographic location, and care location, are correlated w

AIM: In Mexico, many pharmacies sell oral contraceptives (OCs) over the counter (OTC); however, little is known about the background characteristics of OTC pill users. The primary objective of this study was to understand the characteristics of OTC OC users in Mexico, including whether there were differences by age, urbanicity, and insurance status. This information is instructive as other countries explore allowing OTC access to OCs.
METHODS: We analysed the nationally representative 2014 Mexican National Survey of Demographic Dynamics (ENADID) among a sample of OC users aged 15-54 years (n=

There is growing concern in Massachusetts that rising health care costs will derail the state's move to universal health coverage. A special state panel has recommended moving from fee-for-service to a new system of global payments as the best way to reduce unnecessary care and expenses, while improving the health of patients. The commission points to specific examples in the state where global payments seem to be working. But other providers are pushing back.
