Current treatments of medulloblastoma.

PURPOSE OF REVIEW: The biological knowledge and the new biopathological classification of medulloblastoma subtypes have dramatically changed the therapeutic indications, taking into account not only age and staging but also biopathological risk criteria. This review covers the multidisciplinary approach including surgery, radiation oncology and medical treatments. RECENT FINDINGS: The neurosurgical management of tumor-related hydrocephalus has been modified by the introduction of third ventriculostomy. The initial complete excision is no longer always the first choice, to preserve neurologica

Adult hemispheric cerebellar medulloblastoma.

BACKGROUND: Medulloblastoma is an embryonal neoplasm and accounts for 1% of all adult intracranial tumors. It is associated with many familiar cancer syndromes, but there is no known cause for medulloblastoma. Many studies have documented differences between childhood and adult medulloblastomas in terms of location, proliferation, and apoptotic indices. There are four histological groups - classic and the variant forms (desmoplastic/nodular, anaplasic, and large cell). There are four major subgroups according to molecular configuration: wingless (WNT), sonic hedgehog (SHH), group 3, and group

Adult medulloblastoma: multiagent chemotherapy.

In this study, the records of 17 adult patients with medulloblastoma treated with craniospinal radiation and 1 of 2 multiagent chemotherapy protocols were reviewed for progression-free survival, overall survival, and toxicity, and the patients were compared with each other and with similarly treated children and adults. Records of patients treated at 3 institutions were reviewed. Seventeen medulloblastoma patients (11 female, 6 male) with a median age of 23 years (range, 18-47 years) were treated with surgery, craniospinal radiation (CSRT) plus local boost, and 1 of 2 adjuvant chemotherapy reg

Late recurrence in medulloblastoma.

The authors present 3 cases of recurrence of medulloblastoma at 6 years, 6 years 7 months, and 7 years 4 months disease-free intervals from the time of diagnosis. Two of these cases have violated Collins' rule of survivorship; i.e., an interval longer than the patient's age plus 9 months being equivalent to cure. Two of the cases were biopsy proven recurrences of medulloblastoma and in the third, the recurrence was diagnosed on the basis of clinical and roentgenographic data. Other than for these late recurrences, our series is similar to other series reported in the literature.

Medulloblastomas in Pediatric and Adults.

Medulloblastoma is the primary malignant embryonic tumor of the cerebellum and the most common malignant tumor of childhood, accounting up to 25% of all CNS tumors in children, but is extremely rare in adults. Despite the fact that medulloblastomas are one of the most malignant human tumors, it is worthy to note that a great breakthrough has been achieved in our understanding of oncogenesis and the development of real methods of treatment. The main objective of surgical treatment is a maximum resection of tumor with minimal impairment of neurological functions, in order to reduce the volume, r

Chemotherapy for children with medulloblastoma.

BACKGROUND: Post-surgical radiotherapy (RT) in combination with chemotherapy is considered as standard of care for medulloblastoma in children. Chemotherapy has been introduced to improve survival and to reduce RT-induced adverse effects. Reduction of RT-induced adverse effects was achieved by deleting (craniospinal) RT in very young children and by diminishing the dose and field to the craniospinal axis and reducing the boost volume to the tumour bed in older children. PRIMARY OBJECTIVES: 1. to determine the event-free survival/disease-free survival (EFS/DFS) and overall survival (OS) in chi

Craniosynostosis in association with medulloblastoma.

A case of a 13-month-old boy with craniosynostosis associated with medulloblastoma was presented. Sagittal suture was prematurely closed, and coronal and lambdoid sutures were widely separated. A large infiltrative tumor in the midcerebellar region was partially removed and treated by irradiation and chemotherapy. A review of the current literature failed to reveal a similar report.

Supratentorial dissemination of medulloblastomas.

The present paper reports on 13 cases of medulloblastoma with supratentorial dissemination. With reference of these cases the authors discuss the pathogenetic probabilities of the dissemination of brain tumors, their evidence and treatment, emphasizing the importance of cerebral scintigraphy formed periodically in all patients previously operated of medulloblastoma. When dissemination is detected radiotherapy may give good results. If the symptomatology is very accentuated the secondary supratentorial tumour has to be removed.

Cicatricial fibromatosis mimics metastatic medulloblastoma.

Cicatricial fibromatoses usually occur in the anterior abdominal wall or in the extremities, but rarely in the scalp or the soft tissues of the neck. We report a case of desmoid fibromatosis that developed in a 15-year-old boy 8 months after surgery for cerebellar medulloblastoma.

Mutism after cerebellar medulloblastoma surgery.

The case of a 9-year-old boy is presented, who developed transient mutism after removal of a medulloblastoma in the region of the cerebellar vermis. The mutism disappeared within 6 months. Neither reduction of consciousness nor disturbances of caudal cranial nerves or phonation ever appeared. The case is discussed with regard to its phenomenology, pathogenesis, and etiology, corresponding case reports are also taken into consideration.

Sudden death due to medulloblastoma.

Intracranial tumours can be accompanied with intracerebral and intracerebellar non-traumatic haemorrhage; in some cases, this might be their first clinical sign. We report a case of a man who suffered sudden death due to medulloblastoma, with intratumoral haemorrhage; his tumour had a hemispheric cerebellar positioning. The cerebral oedema seen at autopsy reflected the immediate cessation of cerebral vascular circulation, leading to sudden death. Acute obstructive hydrocephaly is a mechanism widely known in the literature for causing sudden death.

Ophthalmic complications of childhood medulloblastoma.

BACKGROUND: To study the ophthalmic complications and sequelae in children surviving medulloblastoma in order to assess the need for therapeutic intervention by the ophthalmologist. PROCEDURE: We identified all children attending the Leeds General Infirmary and St. James' University Hospital for treatment of medulloblastoma in the period January 1990 to March 1997, and the notes of all surviving patients were reviewed. Those patients who had not had an ophthalmic assessment within the last 6 months were recalled for examination. RESULTS: Twenty-four surviving patients underwent full ophthalm

Hyperfractionated craniospinal radiation in medulloblastoma.

From 1986 to 1991, 13 patients at Northwestern Memorial Hospital were entered onto a pilot study designed to test the feasibility of treating children with medulloblastoma (11 patients) or primitive neuroectodermal tumors of the cerebral hemispheres (2 patients) with hyperfractionated craniospinal radiotherapy (HFxRT). Follow-up times ranged from 10 to 96 months with a median of 53 months. The patients were prospectively divided among three treatment arms depending on prior treatment history, if any, and degree of surgical resection. The 3 patients in group I had undergone gross total resectio

Brain scintigraphy in recurrent medulloblastoma.

Twenty-three patients with previously treated medulloblastoma had brain scintigrams revealed posterior fossa recurrence in 15 patients and supratentorial metastases in 9; only 1 of the latter was clinically suspected. Two subdural hematomas and one calvarial metastasis were also detected. Comparison with other diagnostic evidence shows agreement in 13 out of 14 cases (93%) for posterior fossa recurrence and 8/10 (80%) for supratentiorial masses. Brain scintigraphy appears to be a sensitive and specific test for the detection of recurrence and metastasis in patients with treated medulloblastoma

Revealing the Ferroptotic Phenotype of Medulloblastoma.

The interaction of iron and oxygen is an integral part of the development of life on Earth. Nonetheless, this unique chemistry continues to fascinate and puzzle, leading to new biological ventures. In 2012, a Columbia University group recognized this interaction as a central event leading to a new type of regulated cell death named "ferroptosis." The major feature of ferroptosis is the accumulation of lipid hydroperoxides due to (1) dysfunctional antioxidant defense and/or (2) overwhelming oxidative stress, which most frequently coincides with increased content of free labile iron in the cell.

Epigenetics and immune cells in medulloblastoma.

Medulloblastoma (MB) is a highly malignant childhood tumor of the cerebellum. Transcriptional and epigenetic signatures have classified MB into four molecular subgroups, further stratified into biologically different subtypes with distinct somatic copy-number aberrations, driver genes, epigenetic alterations, activated pathways, and clinical outcomes. The brain tumor microenvironment (BTME) is of importance to regulate a complex network of cells, including immune cells, involved in cancer progression in brain malignancies. MB was considered with a "cold" immunophenotype due to the low influx o

Mutations in SUFU predispose to medulloblastoma.

The sonic hedgehog (SHH) signaling pathway directs the embryonic development of diverse organisms and is disrupted in a variety of malignancies. Pathway activation is triggered by binding of hedgehog proteins to the multipass Patched-1 (PTCH) receptor, which in the absence of hedgehog suppresses the activity of the seven-pass membrane protein Smoothened (SMOH). De-repression of SMOH culminates in the activation of one or more of the GLI transcription factors that regulate the transcription of downstream targets. Individuals with germline mutations of the SHH receptor gene PTCH are at high risk
