The meniscal roots, or insertional ligaments, firmly attach the menisci to tibial plateau. These strong attachments anchor the menisci and allow for the generation of hoop stress in the tissue. The meniscal roots have a ligament-like structure that transitions into the fibrocartilagenous structure of the meniscal body. The purpose of this study was to carry out a complete analysis of the structure and tissue organization from the body of the meniscus through the transition region and into the insertional roots. Serial sections were obtained from the meniscal roots into the meniscal body in fix

Bilateral congenital absence of the anterior cruciate ligament associated with the absence of the medial meniscus is reported in a 34-year-old-woman. The simultaneous absence may be explained by a common embryological mesenchymal origin of the anterior cruciate ligament and the menisci. This absence was at the origin of an abnormal development of the condyles and especially of the tibial epiphysis which was tipped over in flexum. Treatment consisted of a central ligamentous plasty associated with a lateral plasty, a medial plasty and a closing anterior tibial osteotomy. The result was good.

To clarify the function of the menisci, dynamic contact pressure in femoro-tibial joints obtained from postmortem or surgical specimens with intact menisci, following partial and total meniscectomy was measured using transistor pressure transducers and Buldon tube pressure gauges. After meniscectomy, the peak contact pressure increased strikingly and contact area decreased markedly. The lower contact pressure after partial meniscectomy than after total meniscectomy is probably one reason why the results of partial meniscectomy are clinically much better than those of total meniscectomy. BACKGROUND: There is limited information available concerning normal equine meniscal morphology, its degeneration and role in osteoarthritis (OA).
OBJECTIVES: To characterise normal equine meniscal morphology and lesions and to explore the relationship between equine meniscal degeneration and femorotibial OA.
STUDY DESIGN: Ex vivo cadaveric study.
METHODS: Menisci were harvested from 7 normal joints (n = 14 menisci) and 15 joints with OA (n = 30 menisci). A macroscopic femorotibial OA score (cartilage degeneration and osteophytosis) was employed to measure disease severity in each compartme

BACKGROUND: The clinical and radiographic abnormalities of the lower limb in children with Blount disease are well known. However, there is controversy regarding the intra-articular morphologic changes in the involved knee joint. The purpose of our study was to evaluate the meniscal and articular surface morphology in children with Blount disease with use of magnetic resonance imaging and to compare these findings with similar measurements in a control group.
METHODS: Preoperative magnetic resonance imaging scans of the knee of children with Blount disease were compared with those of a contro

PURPOSE: This investigation arthroscopically assesses the frequency of soft tissue injury in tibial plateau fracture according to the severity of fracture patterns. We hypothesized that use of arthroscopy to evaluate soft tissue injury in tibial plateau fractures would reveal a greater number of associated injuries than have previously been reported.
METHODS: From March 1996 to December 2003, 98 patients with closed tibial plateau fractures were treated with arthroscopically assisted reduction and osteosynthesis, with precise diagnosis and management of associated soft tissue injuries. Arthro

The menisci of the knee reduce tibiofemoral contact pressures and aid in knee lubrication and nourishment. Meniscal injury occurs in half of knees sustaining anterior cruciate ligament injury and the vast majority of tears in the medial meniscus transpire in the posterior horn region. In this study, computational multibody models of the knee were derived from medical images and passive leg motion for two female subjects. The models were validated against experimental measures available in the literature and then used to evaluate medial meniscus contact force and internal hoop tension. The mode

Six anatomical specimens were used to evaluate the ligament length pattern and mean maximal ligament length change (MMLLC) of the anterior cruciate ligament (ACL) using a ligament replacement with a 6-mm diameter. Three femoral and three tibial attachment sites were selected. Nine different combinations were studied in intact knees and also in knees whose posterior capsule and arcuate ligament complex were sequentially divided, whose menisci were resected, and whose capsule and arcuate complex were repaired. The smallest ligament length change (5.6 mm) was obtained in the intact knee with the

The different types of tibial plateau fractures have similar problems. Today most of them are treated surgically to obtain the adequate reduction. Ninety-three fractures of the tibial plateau have been reviewed. The clinical and X-ray examinations were performed from 7 to 13 years after surgery. Poor results with ligamentous instability and posttraumatic arthritis seem to be related to insufficient reduction of the fracture, loose fragments in the joint, axial angulation and insufficient repair of ligaments and menisci.

➢ Meniscal preservation has become the central management principle of meniscal tears. Biomechanical evidence has demonstrated that meniscal resection increases joint contact stress, accelerates osteoarthritis progression, and worsens long-term outcomes compared with repair and nonoperative management. ➢ Treatment decisions should be individualized based on tear morphology, tissue quality, and patient-specific factors. ➢ Repair technique selection (all-inside, inside-out, or outside-in) should be dictated by the tear location and pattern. ➢ Adjunct treatment strategies, such as biologic augmen

BACKGROUND: Discoid meniscus (DM) is a rare variant of regular knee anatomy. Compared to standard meniscus it is thicker and abnormal in shape; these characteristics make it more prone to tear. It is a congenital defect whose correct etiology is still debated and far from being clarified. The purpose of this systematic review is to evaluate evidences of DM in human fetuses in order to assess whether embryological development may have a role.
METHODS: A systematic review was performed on PubMed, Scopus, and Embase with different combinations of the keywords "discoid meniscus", "embryology", "f

Varus misalignment of the hip-knee-ankle angle causes greater loads on the medial compartment of the knee and increases the risk of developing knee osteoarthritis. High tibial osteotomy is a surgical method where the load-bearing axis is shifted laterally. The purpose of this study is to define a subject-specific three-dimensional multibody model of the knee to investigate the effect of osteotomy on cartilages and menisci during the stance phase of gait. It is assumed that osteotomy transfers load-bearing to the lateral parts of the knee. Magnetic resonance images of a patient with varus align

BACKGROUND: Soft tissue injuries may co-occur with tibial plateau fractures. These injuries may include medial or lateral ligament ruptures, peroneal nerve lesions, anterior cruciate ligament ruptures, and meniscus tears.
OBJECTIVE: The aim of this study was to investigate the frequency of meniscus tears in lateral tibial plateau fractures and to evaluate the clinical and radiological results of meniscus repairs.
MATERIALS AND METHOD: The study included 19 patients who underwent surgery for a closed lateral tibial plateau fracture. Anteroposterior and lateral radiographs of the knee, followe

The topographic relation of the popliteus bursa to the peripheral rim of the lateral meniscus is described with particular attention to the arthroscopic appearance. The arthroscopic findings are compared to sections of knee joints obtained after amputation. As a constant result we found a peripheral separation of the lateral meniscus in the posterior part which extends for 2-3 cm. In consequence of these physiological defects in the superior and inferior attachments to the joint capsule the popliteus bursa communicates with the central part of the joint cavity on the femoral and in the majorit

PURPOSE: Meniscus allograft transplantation (MAT) is a possible treatment for patients suffering with pain after meniscectomy. Here, peracetic acid (PAA) sterilised meniscus transplants were investigated on whether they would provide an adequate alternative to fresh-frozen transplants in their viscoelastic and mechanical properties.
METHODS: In this analysis, 31 menisci donors (26 male and 5 female) were included. The average donor age was 49.87 years, ranging from 32 to 65 years. Menisci of matched pairs of knees underwent chemical sterilisation while counterparts were left fresh-frozen. Sti

We examined the relationships between the density of the subchondral bone of the proximal end of the tibia and the location and condition of the overlying articular cartilage and menisci. We took coronal sections of the tibial plateaus and of the overlying articular cartilage and menisci from human knee joints that were grossly free of osteoarthritis. The specimens were examined for the presence of cartilage fibrillation using the India-ink technique of Meachim. Bone density was determined using the Quantimet picture-point analyzer. Fibrillation of articular cartilage was observed only in the

As a result of research supporting meniscus preservation, evidence shows that internationally, there has been a rise in rates of meniscus repair, especially in younger population, and a decline in rates of partial meniscectomy. The decline in partial meniscectomy has been rather rapid and has outpaced the rise in meniscal repair, likely due to increasing evidence against routine partial meniscectomy in degenerative tears with coexistent arthritis. However, despite the rise in meniscus repair, close to 95% of meniscus surgeries are still partial meniscectomies. The partial meniscectomy to menis

BACKGROUND: Pediatric patients with a discoid meniscus often present with peripheral rim instability. Following saucerization, evaluation of peripheral rim instability is routine. However, the normal mobility of pediatric menisci when probed has yet to be studied. The purpose of this study is to establish normal age-based arthroscopic meniscal mobility and meniscal rim width parameters to aid the treating surgeon in identifying an unstable peripheral rim and determining the appropriate amount of saucerization.
METHODS: Forty fresh-frozen pediatric cadaveric knee specimens under the age of 12
