Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2003eng
This report describes problems arising after an arthroscopic meniscal refixation using the biodegradable Clearfix screw system (Innovasive Devices, Marlborough, MA). A serious problem arose in the form of migration of a meniscal screw that had been inserted in accordance with the manufacturer's instructions. Dislocation of the screw led to pain on the medial aspect of the knee joint and irritation of the infrapatellar branch of the saphenous nerve. The case reported here is a first analysis of the potential complications associated with the use of this meniscal refixation system.

A discoid medial meniscus is an extremely rare anomaly. We present 4 cases of symptomatic discoid medial meniscus. Furthermore, magnetic resonance imaging (MRI) of the unaffected knee was obtained in 3 cases, and 1 patient had bilateral discoid medial menisci as well as a unilateral discoid lateral meniscus proven by MRI. Another patient had bilateral discoid medial menisci. In one of the other 2 cases, an MRI of the unaffected knee was not obtained. However, in the involved knees of both cases, medial and lateral menisci were discoid. The incidence of bilateral discoid medial menisci is unkno

PURPOSE: A degenerative meniscus lesion is a slowly developing process typically involving a horizontal cleavage in a middle-aged or older person. When the knee is symptomatic, arthroscopic partial meniscectomy has been practised for a long time with many case series reporting improved patient outcomes. Since 2002, several randomised clinical trials demonstrated no additional benefit of arthroscopic partial meniscectomy compared to non-operative treatment, sham surgery or sham arthroscopic partial meniscectomy. These results introduced controversy in the medical community and made clinical dec

PURPOSE: Meniscus tears are one of the most frequent orthopedic knee injuries, which are currently often treated performing meniscectomy. Clinical concerns comprise progressive degeneration of the meniscus tissue, a change in knee biomechanics, and an early onset of osteoarthritis. To overcome these problems, meniscal transplant surgery can be performed. However, adequate meniscal replacements remain to be a great challenge. In this research, we propose the use of a decellularized and sterilized human meniscus allograft as meniscal replacement.
METHODS: Human menisci were subjected to a decel

The lateral and medial meniscus play vital roles in load distribution across the knee joint and in the prevention of knee osteoarthritis. As such, every effort should be made to save and repair the meniscus when possible, as partial and total meniscectomies have been shown to impair the meniscus' ability to effectively dissipate shock. Recently, an algorithm-coined the "ABCs of meniscal repair"-has emerged as a blueprint for optimizing the success of a meniscal repair. The "ABCs of meniscal repair" refer to anatomic reduction, biologic augmentation, and circumferential compression across the m

The aim of this study was to evaluate the diagnostic performance of shear wave elastography (SWE) in human meniscus degeneration, with histology serving as the standard of reference. This comparative in vivo and ex vivo study was performed in 15 medial and 15 lateral menisci from 13 patients undergoing total knee arthroplasty (TKA) for primary osteoarthritis with varus deformity. Patients underwent in vivo coronal measurement with SWE for meniscus before surgery. Then, ex vivo assessment of meniscus stiffness with SWE was performed with the tissue obtained after TKA. SWE measurements were made

Background Meniscus root tears (MRTs) are radial tears located near the anterior or posterior meniscotibial attachment, which are often underdiagnosed and associated with accelerated knee osteoarthritis (OA). Medial meniscus posterior root tears (MMPRTs), frequently observed in middle-aged women, lead to altered knee biomechanics and joint degeneration if untreated. While historically managed with meniscectomy, the modern approach emphasizes arthroscopic repair to restore joint stability and delay OA progression. Objective This study evaluates the clinical outcomes of single-tunnel root repair

PURPOSE: Meniscal surgery is one of the most common orthopaedic surgical interventions. Total meniscus replacements have been proposed as a solution for patients with irreparable meniscal injuries. Reliable fixation is crucial for the success and functionality of such implants. The aim of this study was to characterise an interference screw fixation system developed for a novel fibre-matrix-reinforced synthetic total meniscus replacement in an ovine cadaveric model.
METHODS: Textile straps were tested in tension to failure (n = 15) and in cyclic tension (70-220 N) for 1000 cycles (n = 5). The

Discoid lateral meniscus is an intra-articular knee disorder that typically presents in children and adolescents. The natural history depends on the type of anomaly and the nature and presence of symptoms. Management of this disorder should be directed toward resolution of the symptoms while preserving meniscal tissue and function. Modern surgical techniques make suturing and preservation of meniscal tissue feasible. In the present article, the clinical manifestations, diagnostic criteria and practical management considerations are reviewed.

Meniscus lesions are among the most frequent injuries in orthopaedic practice and they will inevitably lead to degeneration of the knee articular cartilage. The fibro-cartilage-like tissue of the meniscus is notorious for its limited regenerative capacity. Tissue engineering could offer new treatment modalities for repair of meniscus tears and eventually will enable the replacement of a whole meniscus by a tissue-engineered construct. Many questions remain to be answered before the final goal, a tissue-engineered meniscus is available for clinical implementation. These questions are related to

Meniscus root tears are radial tears in the region of the posterior insertion zones. Medial root injuries usually occur in individuals > 50 years of age without adequate trauma and are associated with obesity and varus deformities. The root lesion leads to a loss of ring tension, which results in extrusion of the meniscus and a strong increase in joint pressure that is biomechanically equivalent to a complete meniscectomy. When indicating arthroscopic transosseous refixation of the medial root lesion, factors such as accompanying cartilage damage, osteoarthritis, obesity and varus deformity mu

Meniscus repair over resection, when feasible, should be strongly considered in an effort to preserve meniscus integrity and function, especially in younger patients. Currently, a number of techniques and implants may be used to achieve a successful result. Although all-inside meniscus repair devices have evolved significantly since their introduction and have become the repair technique of choice for many surgeons, the classic inside-out repair technique is still very useful to have in one's armamentarium. Though less popular because of the ease of current-generation fixators, the inside-out

The existence of abnormal-shaped menisci has been long recognized. The presence of discoid menisci in the human knee is considered to be a congenital malformation with a very low rate of incidence except in Asian populations. Since the publication of Watanabe's Atlas, three types of lateral meniscal abnormalities are generally accepted: the complete and incomplete discoid, as well as the Wrisberg-ligament type meniscus. The present case is the second description of a ring-shaped meniscus on the lateral side of the knee and we propose that this variant be included as a fourth variant in a futur

In chest roentgenograms, the pulmonary meniscus sign is a crescent-shaped inclusion of air surrounded by consolidated lung tissue. The common cause is aspergilloma. A hydatid cyst is, however, the most common cause in endemic areas. In this study, 10 cases with meniscus sign are presented and the pertinent literature is reviewed.

Meniscus surgery is one of the most commonly performed orthopedic procedures worldwide. Modifiable risk factors for meniscus injury include body mass index, participation in athletics and occupation. Nonmodifiable risk factors include age, sex, lower extremity alignment, discoid meniscus, ligamentous laxity, and biconcave tibial plateau. Conditions commonly associated with meniscal injury are osteoarthritis, anterior cruciate ligament injury, and tibial plateau fractures. Tear type and location vary by patient age and functional status. Surgical management of meniscus injury is typically cost-

Meniscus root tears biomechanically disrupt normal joint loading and lead to joint overload with the possible development of spontaneous osteonecrosis of the knee and early-onset osteoarthritis. Proper identification and treatment of meniscal root tears has been proven to restore joint loading and improve patient outcomes.

The knee is one of the major weight-bearing joints and is relatively exposed to trauma. Capsuloligamentous structures are essential to provide joint stability and -- in turn -- persistent instability bears a risk for osteoarthritis that needs timely and comprehensive diagnosis. Using MRI it may be beneficial to routinely apply (T)SE sequences in all three major planes as a basic protocol and to add additional sequences according to the clinical information available and imaging findings in the basic protocol. Especially fat-suppressed sequences (STIR, T2w/PDw FS TSE) are very useful because th
