Psychotherapeutic Competencies in Blended Care Psychotherapy: Development of a Competence Framework.

BACKGROUND: Blended care psychotherapy (BCP) combines face-to-face psychotherapy with digital interventions, such as videoconferencing, apps, or virtual reality, which can be incorporated in various ways and at different stages of the therapeutic process. Despite its increasing implementation, conceptual and terminological inconsistencies continue to impede a shared understanding of the competencies required for its effective delivery. OBJECTIVE: This theoretical paper introduces a framework of psychotherapeutic competencies in BCP, designed to provide conceptual clarity. METHODS: The framew

Psychoanalytic education and teleanalysis: The role of the frame in the formation of psychoanalytic identity.

In this manuscript, I describe my experience of the evolving place of teleanalysis in psychoanalytic practice and education, focusing on the central role of the analyst's internal frame in sustaining analytic work across settings. Drawing on clinical, supervisory, teaching, and personal analytic experience, I argue that the usefulness of psychoanalysis depends less on the external setting than on the analyst's capacity to function analytically in the face of absence, uncertainty, and loss. I suggest that a priori views of teleanalysis as "less real" or "less effective" often reflect defensive

Health Care Professionals' Perspectives on Conversational Mental Health Chatbots: Protocol for a Systematic Review.

BACKGROUND: Mental health disorders (MHDs) represent a growing global challenge and pose a significant risk to public health. Alongside developments in the field of large language models (LLMs), conversational mental health chatbots (CMHBs) have emerged and are increasingly being used by individuals in self-directed and independent ways for mental health support. Although users' perspectives on CMHBs have been extensively examined and systematically synthesized, relatively little research has focused on how health care professionals (HCPs) perceive these tools. To develop a more comprehensive

Digital mental health: innovations in assessment and intervention.

Depression and anxiety together are ranked among the leading causes of disability, affecting hundreds of millions of individuals worldwide (GBD 2019 Mental Disorders Collaborators, 2022). Yet, psychiatric care faces persistent structural barriers: limited specialist availability, geographic inequities, diagnostic subjectivity, and the stigma that deters help-seeking behaviors (Patel et al., 2018). Digital technologies, including mobile health platforms, internet-delivered interventions, and artificial intelligence (AI)-driven analytic tools, are beginning to address these constraints by extend

Informatics-Based Psychotherapeutic and Psychiatric Interventions in Dermatology: Scoping Review of Impacts on Skin Disease Severity and Mental Health Outcomes.

BACKGROUND: Chronic dermatologic conditions such as psoriasis, atopic dermatitis, and hidradenitis suppurativa are associated with a high burden of psychiatric comorbidities, including depression, anxiety, and suicidality. Despite growing awareness of the psychosocial impact of skin diseases, mental health needs remain underaddressed in dermatologic care. Digital technologies (including teledermatology, mobile health apps, and internet-delivered psychotherapies) offer promising avenues for integrating psychotherapeutic and psychiatric interventions into dermatology. However, the scope, effecti

Quantifying the spatiotemporal associations of flooding on mental health using telehealth call data.

Floods have increased in both frequency and severity in recent decades. National surveys within England have linked flooding to elevated anxiety, depression, and Post-Traumatic Stress Disorder (PTSD). This study analyses daily telephone helpline calls related to mental health problems to assess the spatiotemporal patterns of flood risk and the 2015/2016 winter floods. Spatial analysis categorised postcode districts by flood risk (High, Medium, Low, Very Low) and proximity (Flood, <1 km, 1-5 km, 5-10 km). Interrupted time series and generalised linear models evaluated trends, adjusting for conf

Telemental Health Assessment and Implications for Intimate Partner Violence in Rural Settings.

Violence and abuse in rural America are exacerbated by limited access to support services for victims of intimate partner violence (IPV) due to family connections with people in positions of authority, geographic isolation, transportation barriers, stigma of abuse, lack of available shelters and affordable housing, poverty as a barrier to care, and other challenges. Victims of abuse who live in small communities may be well-acquainted with local health care providers and often voter-elected law enforcement officers. Individuals may fear being seen walking into a mental health clinic, with subs

Therapy chatbots and emotional complexity: do therapy chatbots really empathise?

Internet-based cognitive behavioural therapy (iCBT) and, more specific, therapy chatbots are at the heart of this article. We review studies about therapy chatbots in terms of risks, benefits, and 'empathetic design'. Research shows that these chatbots can relieve symptoms of stress, depression, and anxiety, by tracking emotions over time, encouraging self-reflection, and helping to gain insight into cognitive biases, among other things. However, several studies focus on the short term, have limited samples, and include self-report. While there are certainly benefits, risks such as privacy, da

Early feasibility of telemedicine-based mental health wellbeing centers: an implementation study in district and sub-district health facilities in Bangladesh.

BACKGROUND: Mental health disorders pose a critical burden globally, 400 million incident and 1.1 billion prevalent cases; and in Bangladesh, 10.38 million incident and 24.05 million prevalent cases in 2023. Despite the mental health burden in Bangladesh, the majority of the people (> 90%) do not receive required services, due to the concentration of service providers in big cities, leaving rural and marginalized people disproportionately affected. Therefore, to address these barriers, the Non-Communicable Disease Control (NCDC) programme established tele-mental health services in government f

Meta-Analysis on the Effectiveness of mHealth Interventions for Mental Health: A 10-Year Update.

Mobile health (mHealth) interventions offer scalable ways to deliver mental health support via smartphone technology. This updated meta-analysis builds on Lindhiem et al., synthesizing findings from 79 randomized controlled trials identified through PsycINFO. Eligible studies included clinical samples, randomized designs, and at least one mHealth intervention group. Three comparisons were examined: mHealth + treatment versus control treatment, mHealth versus control treatment, and mHealth versus no treatment. Results showed small to moderate effects, with the strongest outcomes when mHealth in

Association Between Care Utilization and Anxiety Outcomes in an On-Demand Mental Health System: Retrospective Observational Study.

BACKGROUND: Anxiety is an extremely prevalent condition, and yet, it has received notably less attention than depression and other mental health conditions from a research, clinical, and public health perspective. The COVID-19 pandemic has only exacerbated growing concerns about the burden of anxiety due to the confluence of physical health risks, economic stressors, social isolation, and general disruption of daily activities. OBJECTIVE: This study examines differences in anxiety outcomes by care modality (coaching, teletherapy and telepsychiatry, and combined care) within an on-demand menta

Treatment of Obsessive-Compulsive Disorder via Mental Health Technology.

Obsessive-compulsive disorder (OCD) is a debilitating neuropsychiatric disorder that remains chronic unless intervened with evidence-based intervention. Cognitive behavioral therapy (CBT) with exposure and response prevention is the gold-standard psychological intervention for OCD, but many individuals do not receive this intervention due to barriers to accessing treatment. Mental health technology tools such as telehealth, computerized programs, internet-delivered CBT, and mobile applications have been adopted to expand the accessibility of CBT. An up-to-date summary of the evidence base of t

Ethics in Telepsychiatry.

This article reviews contemporary issues in telepsychiatry and telepsychotherapy. The authors examine ethics within a larger social context, particularly access to care and cost of care. They consider the evolving scientific validation of telepsychiatry and the ethical significance of telepsychiatry in various practice settings. They also focus on the doctor-patient relationship, assessments, and provision of psychiatric treatments such as medication management and psychotherapy. The authors consider concerns about malpractice liability and medicolegal constraints on telepsychiatry. They do no

Psychotherapy and Teletherapy in U.S. Latinx Populations: A Narrative Review.

Latinx patients possess a rich cultural background and shared heritage; they also possess culturally distinct medical beliefs and practices, including beliefs around mental health and interactions with mental health systems. Most forms of psychotherapy were not developed for Latinx populations, and the efficacy of existing and novel forms of psychotherapy in Latinx patients as a group has received precious little study. In this study, we review psychotherapy and telepsychotherapy interventions for Latinx patients with mental health disorders. We conducted a literature review applying Preferred

Extending the reach of inpatient consultation-liaison psychiatry: A systematic review of telepsychiatry in U.S. general hospitals.

BACKGROUND: Psychiatrist shortages and rural hospital gaps limit inpatient consultation-liaison (C-L) psychiatry in U.S. general hospitals. Telepsychiatry has expanded rapidly, yet implementation models, outcomes, and challenges of inpatient C-L telepsychiatry remain incompletely and inconsistently described. METHODS: We conducted a systematic review following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We searched major medical, nursing, and psychological databases and ClinicalTrials.gov (1990-November 2025) for English-language studies of adul

From research to practice: a qualitative study examining the integration of telehealth-based suicide prevention in HIV care in Tanzania.

Despite the life-saving potential of mental health interventions, their scalability and integration within healthcare systems present significant challenges, particularly in low-resource global settings. This study investigated scaling IDEAS for Hope, a telehealth counseling intervention designed to reduce suicide risk and improve HIV care engagement among people living with HIV in Kilimanjaro, Tanzania, and identified actionable implementation strategies for successful scale-up. We conducted 20 semi-structured interviews, between May and July of 2024, with purposively sampled stakeholders fro

Barriers and limitations to the development of a telemental health service for workers in Peru- A user-centered approach.

INTRODUCTION: Over the past decade, the surge in digital healthcare services has transformed traditional healthcare, requiring multidisciplinary engagement to adapt to the digital realm. The rise of telehealth services, particularly amid COVID-19, has been widely embraced globally, but its implementation in Peru faces unique challenges, including infrastructure issues and economic constraints. Therefore, this research aims to identify the barriers and limitations in developing a telemental health service for screening, evaluation, and timely referral of vulnerable occupational groups.
