Mixture of bupivacaine 0.5% and hyperbaric mepivacaine 4% for spinal anaesthesia.

Recent research shows that using more than 12.5 mg bubivacaine for spinal block may possibly be harmful. A dose of 12.5 mg isobaric bupivacain does not always establish a spinal anaesthesia sufficient for hip or lower abdominal surgery. However short acting local anaesthetics of sufficient effectiveness usually do not last long enough for those operations. We evaluated a method using the subarachnoid injection of 2 to 3 (4) ml of an equal parts mixture of isobaric bupivacain 0.5% and hyperbaric mepivacaine 4%. We generally achieved good spinal anaesthesia with quick onset and long duration of

[Subconjunctival anaesthesia using cocaine or mepivacaine for cyclocryotherapy in advanced glaucoma].

BACKGROUND: Retro- or peribulbar anaesthesia are the standard procedures for cyclodestructive surgery. Because these methods of anaesthesia may further compromise optic nerve function, especially in advanced glaucoma, subconjunctival anaesthesia was evaluated as an alternative procedure in cyclocryotherapy. PATIENTS AND METHODS: 1. To evaluate the current methods of anaesthesia a questionnaire was sent to all University Eye Clinics in Germany in 2001. 2. A prospective, consecutive study concerning cyclocryotherapy in advanced glaucoma was started using subconjunctival anaesthesia with 5 % coc

A double-blind comparison of lidocaine and mepivacaine during epidural anaesthesia.

The effects of epidural anaesthesia with plain 2% lidocaine or mepivacaine were compared in 200 patients undergoing extracorporeal shock wave lithotripsy in a double-blind manner. The onset, spread, duration and quality of analgesia were similar in both groups. The numbers of patients who needed vasoconstrictor or atropine were almost equal in both the lidocaine and the mepivacaine groups. Mild but significant hypotension continued for a longer period in the mepivacaine group than in the lidocaine group. A transient decrease in arterial oxygen saturation was seen in two patients receiving lido

[Bupivacaine and mepivacaine on artificial lipid membranes. A comparison of physicochemical parameters].

Mepivacaine and bupivacaine, frequently used local anesthetics, differ in a variety of clinical effects but show only small differences in chemical structure. In order to learn more about molecular mechanisms of action, we investigated the effects of these two local anesthetics on artificial membranes and liposomes. Methods. Artificial membranes were formed from dipalmitoylphosphatidylcholine - cholesterol or from a phospholipid mixture extracted from rabbit heart to separate into two aqueous phases (100 mM NaCl, 1 mM glycine, pH 7.4). Membrane current and potential were recorded after additio

[Mepivacaine in brachial plexus anesthesia in surgical interventions of long duration].

To evaluate the use of mepivacaine-HCl for continuous axillary brachial plexus block for surgery lasting several hours a prospective study was performed. Mepivacaine-HCl 400 mg was injected every two hours. Over a period of eight to twenty hours there was in all cases adequate analgesia and muscle relaxation for microsurgical repair of limbs. Mean serum levels of mepivacaine-HCl did not exceed the concentration of 5-6 micrograms/ml. There were no side effects caused by the local anaesthetic agent.

HPLC-DAD determination of mepivacaine in cerebrospinal fluid from a fatal case.

A fatal case involving mepivacaine-induced epidural anesthesia is described. The pathological findings were typical of cardiac shock from ischemic origin. Cerebrospinal fluid (CSF) was obtained several hours after death and mepivacaine was identified by gas chromatography-mass spectrometry (GC-MS). Its concentration was determined by high performance liquid chromatography with diode array detection (HPLC-DAD). Extraction from CSF was performed by deproteinization with acetonitrile. The mepivacaine concentration in the sample was 264 microg/mL. Concentrations of mepivacaine in CSF following epi

Transpulmonary disposition of prilocaine, mepivacaine, and bupivacaine in humans in the course of epidural anaesthesia.

The pulmonary first-pass kinetics of the amide-linked local anaesthetics prilocaine, mepivacaine and bupivacaine were studied in 33 patients after a single epidural injection. Drug concentrations were monitored before and after lung passage, i.e. in samples withdrawn simultaneously from mixed venous and arterial blood. In most cases, maximum plasma concentrations were observed 10 min after injection (range 2 to 30 min). Two min after injection the local anaesthetics were distinctly extracted by the lung (prilocaine 40%, mepivacaine 20%, and bupivacaine 12%). Prilocaine was retained by the lung

Effects of mepivacaine, prilocaine and lidocaine on the induced constriction of the maxillofacial vasculature of the rat.

Infusions of mepivacaine 5 or 50 micrograms.min-1 and prilocaine 50 micrograms.min-1 for 10 min into the maxillofacial arterial vasculature of the anaesthetized rat curtailed significantly rises in infusion pressure induced by the administration of adrenaline via the same route. The effect of mepivacaine was significantly greater than that of prilocaine. Such anti-constrictory action following intraluminal infusion, also shown by lidocaine in a previous study, contrasts with the variable (dilatory or constrictory) consequences of perivascular injections of the same local anaesthetics. Moreover

Blood concentrations of lidocaine, mepivacaine and bupivacaine during caudal analgesia in children.

The venous blood concentrations of lidocaine, mepivacaine and bupivacaine were measured in children following the caudal epidural administration of these local anaesthetic solutions with adrenaline. The mean maximum concentrations of lidocaine and mepivacaine were 2.20 micrograms/ml and 2.53 micrograms/ml, respectively, which occurred at 45 min after injection of lidocaine or mepivacaine 11 mg/kg. By combined administration of 5.5 mg/kg equally of lidocaine and mepivacaine, the blood concentration of mepivacaine was significantly higher than that of lidocaine. The mean maximum concentration of

Brain tissue levels in a fatal case of neonatal mepivacaine (Carbocaine) poisoning.

A fatal case of mepivacaine poisoning in a newborn infant is reported. Regional brain tissue concentrations of mepivacaine were determined by a sensitive and specific gas chromatographic-mass spectrometric method. The brain tissue levels in this patient were higher than those previously reported, possibly due to alkalosis occurring several hours prior to the infant's death.

Spinal Anesthesia in Same-Day Discharge Total Hip Arthroplasty: Chloroprocaine versus Mepivacaine.

PURPOSE: Chloroprocaine spinal anesthetic may facilitate quicker postoperative recovery in total hip arthroplasty (THA) than mepivacaine due to its shorter duration of action and rapid metabolization, which could reduce time to ambulation and discharge. Given the increasing emphasis on same-day discharge (SDD) protocols in THA to improve efficiency and reduce healthcare costs, evaluating the impact of chloroprocaine on discharge timing and postoperative outcomes is critical. This study compared the clinical outcomes and safety of chloroprocaine compared to mepivacaine in SDD primary THA. MATE

Transient neurological symptoms after spinal anaesthesia with 4% mepivacaine and 0.5% bupivacaine.

Several studies have reported transient neurological symptoms after spinal anaesthesia with 5% lignocaine. In order to evaluate the role of concentrated solutions of local anaesthetic in the development of transient neurological symptoms, 200 ASA I or II patients undergoing minor orthopaedic or rectal surgery under spinal anaesthesia were allocated randomly to receive 4% mepivacaine 80 mg or hyperbaric 0.5% bupivacaine 10 mg. All patients were interviewed by an anaesthetist approximately 24 h after spinal anaesthesia, and after 1 week patients were asked to return a written questionnaire. The

Allergic reactions to local anesthetic mepivacaine in dental procedures: a case report.

Local anesthetics are an essential part of pain control during dental treatment. Despite its effectiveness and safety, patients should constantly be aware of potential adverse effects, including allergic reactions. Allergic reactions to amide-type local anesthetics (LAs), such as lidocaine and mepivacaine, are rare compared to those to ester-type LAs. Herein, we report the case of a patient with a history of allergy to lidocaine and mepivacaine, with symptoms of itching, diffuse erythema of the wrists and hands, dizziness, and pectoralgia. This case report emphasizes the importance of collecti

Spinal procaine is less neurotoxic than mepivacaine, prilocaine and bupivacaine in rats.

BACKGROUND AND OBJECTIVES: Lidocaine has been reported to be more neurotoxic than other local anesthetics. Alternatives to lidocaine with lower toxicity and shorter duration of action are desirable. Therefore, we compared the histologic and functional changes induced by intrathecal injection of prilocaine, mepivacaine, procaine, and bupivacaine in rats. METHODS: Rats (n = 184) randomly received via an intrathecal catheter 0.12 microL/g body weight of 2%, 10%, 16%, or 20% prilocaine, mepivacaine, or procaine; 0%, 0.5%, 2.5%, 4%, or 5% bupivacaine in distilled water; or distilled water or 15% g

Tramadol added to mepivacaine prolongs the duration of an axillary brachial plexus blockade.

UNLABELLED: Tramadol is an analgesic drug that is antagonized by alpha2-adrenoceptor antagonists, as well as opioid antagonists. We hypothesized that tramadol might produce effects on an axillary brachial plexus blockade similar to those of clonidine. We designed a prospective, controlled, double-blinded study to assess the impact of tramadol added to mepivacaine on the duration of an axillary brachial plexus blockade. After institutional approval and informed consent, 60 patients (ASA physical status I or II) scheduled for forearm and hand surgery after trauma under brachial plexus anesthesia
