Paget-Schroetter syndrome (PSS), which is also called "effort thrombosis," is a venous variant of thoracic outlet syndrome. We report a rare case of upper-limb deep venous thrombosis (ULDVT) in a young patient who was later diagnosed as PSS. PSS is a rare cause of ULDVT, and it is usually seen in young adults who are involved in strenuous physical activity. PSS is either due to anatomical abnormality of the thoracic outlet or due to repeated microtrauma to the endothelium of the subclavian/axillary vein. Clinically, the patient usually presents with signs and symptoms of ULDVT. Noninvasive Dop

The shoulder joint, the most movable joint in our body, is exposed to a considerable load during many sporting activities. The shoulder can be injured and limited in its function not only by direct and indirect trauma but also by repetitive load. Sports with body contact may cause bruises, glenohumeral and acromio-clavicular dislocations or strains of the rotator cuff. Any repetitive microtrauma, particularly those involving sporting activities requiring repetitive overhead use of the arm, may develop lesions of tendons, glenohumeral instabilities or impingement of the deep surface of the supr

The author reviews the current concepts of the pathogenesis, clinical course and treatment of pterygium. This neoplasm can be considered as to be a manifestation of a chronic irritative process caused by solar radiation and repeated microtrauma and, to a lesser degree, by other aetiological factors which are studied successively. The clinical course of the disease consists of two phases: a conjunctival phase and a corneal or true pterygial phase. The anatomical and clinical forms studied in a population of 2,350 subjects in an endemic region of pterygium (Central Sahara) appear to be closely r

We report a case of a 45-year-old man, complaining of swelling and pain in his epigastric region for the last 3 years. According to his medical history, he had undergone various investigations and treatments for gastro-oesophageal reflux, without relief. He had had a history of chronic repeated microtraumas to his sternum during 9 years of working as a carpenter, as a result of placing wood against his anterior chest wall and pushing the former into a plank cutting machine. On examination, a tender swelling was palpable as an immobile, hard mass showing minimal protrusion under the skin on the

Symptomatic spondylolysis is a stress reaction caused by microtrauma during physical exercise, an imaging diagnostic subgroup of Adolescent Low Back Pain (ALBP), found in adolescent athletes. Early diagnosis increases the possibility of healing. Thus, it is important to divide ALBP into subgroups. The aim of this study was to evaluate clinical tests that can distinguish symptomatic spondylolysis from other forms of ALBP in order to facilitate early referral for diagnostic imaging. The investigation subjects were a prospective case series with a control group, 25 subjects with ALBP and 13 subje

A homogeneous group of 34 subjects exposed to occupational vibratory stress were studied to determine their vasodilatory response to methacoline. The methacoline was administered by iontophoresis. Methacoline is an acetylcholine derivate with the same pharmacological properties of acetylcholine. The 34 study subjects were divided into 3 groups according to their plethysmographic response to a preliminary cold test: group 1, had a typical Raynaud's response; group 2 had normal response to cold, and group 3 had an "intermediate" response. The subjects in group 1 did not respond to methacoline wi

This study aimed to critically evaluate the information provided by ChatGPT on the role of lactate in fatigue and muscle pain during physical exercise. We inserted the prompt "What is the cause of fatigue and pain during exercise?" using ChatGPT versions 3.5 and 4o. In both versions, ChatGPT associated muscle fatigue with glycogen depletion and "lactic acid" accumulation, whereas pain was linked to processes such as inflammation and microtrauma. We deepened the investigation with ChatGPT 3.5, implementing user feedback to question the accuracy of the information about lactate. The response was

BACKGROUND: Chronic inflammatory tendon diseases in athletes are frequent, and they often result from modifications in normal kinematics of a tendon associated with a patient's anatomical determinants. De Quervain stenosing tenosynovitis is an inflammatory disease of tendons of the first dorsal compartment of the wrist. There is no literature about this disease concerning professional volleyball players.
HYPOTHESIS: Limited, multiple trauma on the dorsal radial portion of the wrist, associated with long training times, can be involved in the pathogenetic process of de Quervain disease in prof

Injuries at the medial gastrocnemius-soleus junction are common among athletes and physically active individuals. They typically result from sudden overstretching or repetitive microtrauma, leading to significant calf pain and functional limitations. Diagnosis is primarily clinical, with ultrasound or MRI used to assess injury severity. Most cases are managed conservatively, including rest, cryotherapy, anti-inflammatory therapy, and structured rehabilitation focusing on gradual stretching, eccentric strengthening, and proprioceptive training. Recovery generally occurs within 6-12 weeks, with

BACKGROUND: Axial Spondyloarthritis (axSpA) is a chronic, inflammatory rheumatic disease that affects the axial skeleton, causing pain, stiffness, and fatigue. Genetics and environmental factors such as microbiota and microtrauma are known causes of disease susceptibility and progression. Murine models of axSpA found a decisive role for biomechanical stress as an inducer of enthesitis and new bone formation. Here, we hypothesize that muscle properties in axSpA patients are compromised and influenced by genetic background.
OBJECTIVES: To improve our current knowledge of axSpA physiopathology,

Diabetic non‑healing wounds pose a clinical challenge due to persistent inflammation, dysregulated immunity, and poor integration of dressings with the skin. Conventional hydrogel dressings have proven effective in providing physical barriers, moisture retention, and local drug delivery. Further optimization is still feasible for precise immune regulation and dynamic skin interfacial matching. In non-healing wound therapy, enantiomerically defined chiral hydrogels have emerged as a promising platform for targeted immune remodeling. By selectively steering macrophage polarization from pro-infla

PURPOSE OF REVIEW: Ehlers-Danlos syndrome (EDS) is a congenital connective tissue disorder that is often marked by chronic pain and considerable functional impairment. Pain in EDS is typically multifaceted, involving musculoskeletal, neuropathic, autonomic, and centralized pain components. The present investigation, therefore, aims to examine common pain phenotypes and mechanisms in EDS and to discuss strategies for interdisciplinary pain management briefly.
RECENT FINDINGS: Recent studies have identified multiple sources of pain etiologies in EDS, including joint instability, repetitive micr

Vascular compression syndromes are caused by the entrapment of vessels between rigid or semirigid surfaces in a confined anatomic space. Chronic entrapment may lead to arterial ischemia and embolism, venous stasis and thrombosis, and hematuria. These syndromes are usually seen in otherwise healthy young patients, among whom underdiagnosis is common. Most occurrences of vascular compression are associated with an underlying anatomic abnormality. In a small percentage of cases, other contributing factors, including repetitive microtrauma, may cause pathologic changes leading to the onset of pain

A 61-yr-old female equestrian presented after 2 wks of left medial thigh pain, which developed suddenly while exiting a car. She denied any history of recent trauma or falls. On examination, she was found to have tenderness at the left distal medial thigh with a palpable region of decreased tissue volume at the gracilis myotendinous junction. Point-of-care ultrasound and magnetic resonance imaging confirmed a high-grade partial thickness tear of the left distal gracilis at the myotendinous junction, as well as pes anserine bursal distention. She received physical therapy and underwent a 1-time

Increased physical activity in childhood has resulted in a large number of sports-related injuries. Although there is overlap between the sports-related injuries seen in pediatric and adult patients, important differences exist in the injury patterns of pediatric patients. These differences are related to the continuous changes in the developing skeleton and its relationship with adjacent soft tissues. The imbalance in strength between the growing bones and the nearby tendons and ligaments makes the bones prone to acute and chronic injuries. Acute injuries are more common in contact sports suc

In the competitive athlete, there are many causes of anterior knee pain, one of which is patellar tendinitis. Repetition of explosive movements can cause microtrauma to the tendon and its insertion, resulting in patellar tendinitis and occasional tearing, either partial or total. Due to its refractory nature, the treatment of this disorder can be quite frustrating to all involved. A 20-year-old collegiate football player with patellar tendinitis was treated conservatively for more than 2 years. Despite aggressive training regimens, including quadriceps stretching, eccentric strengthening, and

BACKGROUND: Osteochondritis dissecans (OCD) has been defined as a localized process in which a focus of subchondral bone and adjacent articular cartilage separates from the surrounding bone. With the knee being the most common location for OCD development and the propensity for this lesion to be found in those who participate in sports, a repetitive microtrauma hypothesis for its cause has gained favor. However, the cause of OCD remains controversial, as does the most appropriate treatment for the varying degrees of OCD lesions.
CASE DESCRIPTION: We present a unique case of three OCD lesions

PURPOSE: Tuberculosis of the osteoarticular system usually manifests as joint arthritis. There is no available English literature on the tubercular involvement of the enthesis (tendon-bone junction).
METHODS: We performed a retrospective analysis on 14 patients with tuberculosis of the tendon-bone junction. Patients presenting with a sinus with or without presence of radiological evidence of bone destruction around the enthesis, and pain unresponsive to a trial of analgesics and physical therapy, were evaluated by closed or open biopsy for tuberculosis. A staging system is proposed for biopsy

Quadriceps muscle strains are common sporting injuries, but occasionally a tear of the rectus femoris muscle can appear as a soft tissue mass of the anterior thigh with or without a significant history of trauma. Between 1992 and 1996, seven patients were referred to the Orthopaedic Oncology Unit at Walter Reed Army Medical Center with an unexplained soft tissue mass of the thigh. Three were active duty soldiers, three were military dependents, and one was a retired serviceman. All patients were men, and the mean age was 32 years (range, 15 to 73). A palpable, mildly tender mass was confirmed
