Microvascular decompression surgery: surgical principles and technical nuances based on 4000 cases.

BACKGROUND: As an etiological treatment of trigeminal neuralgia (TN) and hemifacial spasm (HFS), microvascular decompression (MVD) has been popularized around the world. However, as a functional operation in the cerebellopontine angle (CPA), this process can be risky and the postoperative outcomes might not be good enough sometimes. OBJECTIVE: In order to obtain a better result with less complication, this surgery should be further addressed. METHODS: With experience of more than 4000 MVDs, we have gained knowledge about the operative technique. Through abundant intraoperative photos, each s

Value-based neurosurgery: measuring and reducing the cost of microvascular decompression surgery.

OBJECT: Care providers have put significant effort into optimizing patient safety and quality of care. Value, defined as meaningful outcomes achieved per dollar spent, is emerging as a promising framework to redesign health care. Scarce data exist regarding cost measurement and containment for episodes of neurosurgical care. The authors assessed how cost measurement and strategic containment could be used to optimize the value of delivered care after the implementation and maturation of quality improvement initiatives. METHODS: A retrospective study of consecutive patients undergoing microvas

Characteristics and management of the offending veins in microvascular decompression surgery for trigeminal neuralgia.

The optimal technique of microvascular decompression (MVD) for trigeminal neuralgia (TN) caused by venous conflict remains unclear. The objectives of this study are to characterize the offending veins identified during MVD for TN and to evaluate intraoperative technique applied for their management. From 2007 till 2019, 308 MVD surgeries were performed in 288 consecutive patients with TN, and in 58 of them, pure venous conflict was identified. In 44 patients, the offending vein was interrupted, as was done for small veins arising from the cisternal trigeminal nerve (CN V) or its root entry zon

Current neurosurgical management of glossopharyngeal neuralgia and technical nuances for microvascular decompression surgery.

Glossopharyngeal neuralgia (GPN) is an uncommon facial pain syndrome often misdiagnosed as trigeminal neuralgia. The rarity of this condition and its overlap with other cranial nerve hyperactivity syndromes often leads to a significant delay in diagnosis. The surgical procedures with the highest rates of pain relief for GPN are rhizotomy and microvascular decompression (MVD) of cranial nerves IX and X. Neurovascular conflict at the level of the root exit zone of these cranial nerves is believed to be the cause of this pain syndrome in most cases. Vagus nerve rhizotomy is usually reserved for c

Gamma Knife Surgery for Recurrent Trigeminal Neuralgia in Cases with Previous Microvascular Decompression.

BACKGROUND: Microvascular decompression (MVD) and Gamma Knife surgery (GKS) are the primary treatments for trigeminal neuralgia (TN). However, many patients require further surgical treatment after initial surgery for recurrent TN. The aim of this study was to evaluate efficacy and safety of GKS for recurrent TN cases with prior MVD. METHODS: From October 2008 to June 2015, 658 patients at West China Hospital underwent GKS as the only surgical treatment, and 42 patients underwent GKS with prior MVD. The single 4-mm isocenter was located at the cisternal portion of the trigeminal nerve in all

Exposure of Titanium Mesh after Cranioplasty for Microvascular Decompression Surgery: Two Case Reports.

Two cases of exposed titanium mesh occurred after implantation for cranioplasty after repeated procedures for microvascular decompression (MVD). Case 1 was a 62-year-old female who underwent MVD for left hemifacial spasm followed by repair of cerebrospinal fluid leak after the surgery, and Case 2 was a 75-year-old female who underwent MVD twice for right trigeminal neuralgia. Both patients visited our hospital again with complaints of postauricular lesion. Titanium mesh was visible through the operative scar and was successfully removed with no complication in both cases. Both patients were un

Effectiveness and safety of microvascular decompression surgery for treatment of trigeminal neuralgia: a systematic review.

Microvascular decompression has been now accepted worldwide as a reasonable treatment for trigeminal neuralgia, yet, as a functional operation in the cerebellopontine angle, this process may be risky and the postoperative outcomes might not be good enough sometimes. To assess the effectiveness and safety of microvascular decompression for treatment of trigeminal neuralgia, we conducted a systematic review. Keyhole retrosigmoid approach for microvascular decompression surgery: systematic review and single-arm meta-analysis.

Microvascular decompression (MVD) surgery is traditionally performed via a retrosigmoid craniotomy. Recently, neurosurgeons have increasingly adopted smaller approaches; however, the available evidence has not yet been systematically compiled. This is the first systematic review and meta-analysis on the efficacy and safety of ≤ 2-cm-keyhole retrosigmoid MVD. PubMed and Embase databases were searched for studies involving adults with trigeminal neuralgia (TN), hemifacial spasm (HFS), or glossopharyngeal neuralgia (GPN), who underwent first-time retrosigmoid keyhole MVD (diameter ≤ 2 cm). Primar

Pontine Microtubular Signal Intensity in Hemifacial Spasm: Association with Outcome After Microvascular Decompression Surgery.

BACKGROUND: We aimed to investigate the prevalence and clinical significance of pontine microtubular signal intensity (MSI), presumed dilated perivascular or perineural spaces, in patients with hemifacial spasm (HFS) using high-resolution MRI using proton density-weighted imaging (HR-PDI). METHODS: We retrospectively analyzed 438 patients with unilateral HFS who underwent microvascular decompression (MVD) and preoperative HR-PDI. MSI was defined as a linear or curvilinear hyperintense lesion along the presumed course of the intraparenchymal facial nerve fascicles within the pons on HR-PDI. Th

Use of fenestrated aneurysm clips in microvascular decompression surgery. Technical note and case series.

/The standard techniques of microvascular decompression (MVD) surgery in which implant materials such as shredded Teflon felt are used may be inadequate in some complex cases. The authors evaluated the use of fenestrated aneurysm clips to maintain transposition of culprit vessels in patients with trigeminal neuralgia (TN) and hemifacial spasm (HFS). The authors conducted a retrospective review of MVD operations in which the culprit vessel was transposed and then maintained in position with a fenestrated aneurysm clip secured in position by suturing it to the dura mater. Microvascular decompression after gamma knife surgery for trigeminal neuralgia: intraoperative findings and treatment outcomes.

OBJECT: The authors sought to determine whether the results of trigeminal microvascular decompression (MVD) are influenced by prior gamma knife surgery (GKS). METHODS: Gamma knife surgery is an established procedure for treating medically intractable trigeminal neuralgia but failures do occur. The authors assessed six patients (two men and four women; mean age 52 years) who experienced pain recurrence after GKS and elected to undergo trigeminal MVD via retrosigmoid craniotomy. Three patients underwent a single GKS to a maximal dose of 80 Gy, whereas three others underwent a second GKS to tota
