The role of psychodynamic understanding in the treatment of the medication-refusing schizophrenic patient: a clinical report.

This paper describes the psychoanalytically informed treatment of a schizophrenic patient. The author discusses elements of a comprehensive treatment and illustrates how psychodynamic understanding can help avert impasses in therapy. In this case, for example, psychodynamic understanding helped the hospital staff overcome a sense of frustration and impotence in dealing with a patient who needed to isolate herself in order to overcome feelings of panic. Rehabilitative work was important, not only to recover lost or lessened capacities, but also to modify the patient's feelings towards external

Alcoholism treatment and evaluation: in search of methods. II. Collation of two-year follow-up studies.

This study collated 23 articles reporting outcome of alcoholism treatment efforts over a 2-year follow-up period. Broad conclusions reached by Costello (1975) in a similar collation of studies reporting on a 1-year follow-up interval found general support. Similar consistencies were noted across studies within groups described as having good outcome profiles as was noted by the previous collation. Good outcome results from matching relatively better prognostic cases wtih programs with broad treatment resources. Multiple baselines were offered as guidelines for program evaluation. Baselines wer

Neurocognitive disorders seen in HIV disease.

Despite dramatic improvements in the therapeutic management of clients with human immunodeficiency virus (HIV) disease, cognitive disorders still appear as clinical manifestations of the illness trajectory. The development of neuro-cognitive impairment is associated with high levels of HIV activity and the resultant severe degree of immunosuppression. Although many clinicians almost exclusively associate HIV-related cognitive dysfunction with HIV encephalopathy and dementia, the etiologic conditions are numerous and include not only HIV infection affecting the brain, but also infections and ne

Low-dose neuroleptic therapy for children with childhood-onset pervasive developmental disorder.

An open study of the therapeutic effects of low-dose neuroleptic treatment was performed on 12 consecutively hospitalized children, aged 7-11, who satisfied DSM-III criteria for childhood-onset pervasive developmental disorder. While receiving an average dose of 0.04 mg/kg per day of haloperidol or fluphenazine hydrochloride, the patients exhibited significant reductions in hyperactivity and aggressive symptoms and significant improvement in peer relations. This dose of neuroleptic was associated with minimal side effects. The authors conclude that low doses of potent neuroleptics may be effec

[Empirically based early intervention programs for children with autistic disorders - a selective literature review].

Autistic Disorders (AD) are characterized by impairments in social interaction and communication, as well as by stereotyped behaviors and interests. Early intervention programs in AD aim to improve several aspects of the child's abilities: joint attention, play abilities, language development, and especially social interaction and communication. In this review article based on a selective literature search, the relatively best empirically based early intervention programs will be discussed with a focus on the proven efficacy of these interventions.

[Treatment of young transsexuals in the Netherlands].

For more than ten years transsexual adolescents have been diagnosed and treated psychologically at the department of Child and Adolescent Psychiatry, University Medical Centre in Utrecht, the Netherlands. The medical part of the treatment takes place at the Academic Hospital of the Free University of Amsterdam. Diagnosis is done in two phases: the first diagnostic phase and the 'real life test'. In this second phase the ability to live in the opposite gender role is tested. Gender dysphoric non-transsexual adolescents are offered psychological or psychiatric interventions. For transsexual adol

Psychiatric hospital treament of suicidal children.

Three phases of psychiatric hospital treatment of children, ages 6 to 12 years, admitted to the hospital because of suicidal threats or suicidal attempts are described. Suicidal behavior in young children must always be taken seriously. Although suicidal behavior usually ceases upon hospitalization, suicidal ideas may continue to be expressed in play fantasy. In addition to individual and milieu therapy, treatment must also include concomitant assistance of the parents. The treatment procedures are illustrated by suicidal case examples of a depressed girl, an acting-out boy, and a psychotic bo

Initial assessment of hospital treatment by patients with paranoid schizophrenia: a predictor of outcome.

The value of schizophrenic inpatients' initial global assessments of treatment in the prediction of outcome was investigated. Within 3 days of admission, 31 patients with an acute paranoid schizophrenic psychosis according to ICD-10 rated on a visual analog scale to what extent they believed the treatment they were receiving was right for them. Outcome criteria were overall clinical changes measured on the Brief Psychiatric Rating Scale and the Intentionality Scale. The patients' initial global assessments of treatment were significantly correlated to both outcome criteria, indicating that pat

Regenerative treatment of zinc-based nanoenzyme incorporated injectable hydrogel with mesenchymal stem cells alleviates spinal cord injury.

Mesenchymal stem cells (MSCs)-derived therapy has surfaced as a viable approach for addressing spinal cord injury (SCI). The unfavorable milieu of SCI significantly impairs the paracrine and survival function of implanted MSCs, hence greatly restricting the therapeutic efficacy of this method. We present a zinc-based nanoenzyme (ZnNZs)-combined in situ forming thermoresponsive hydrogels (ZnNZ-HGL) that facilitate dual improvement of MSCs paracrine and viability effects, resulting in highly effective spinal cord reconstruction. The sol-gel transitions characteristic of ZnNZ-HGL at the physiolog

Effective psychiatric day treatment: historical lessons.

This paper describes a successful day treatment program that provides four months of intensive psychodynamic group-oriented milieu treatment for patients with long-standing personality disorders, who commonly experience recurrent major depression, dysthymia, and excessive anxiety. The authors outline three factors that have contributed to the success of the program--optimal treatment-patientmatching, judicious use of authority in milieu therapy, and careful attention to maintaining close working relationships with referral sources. They provide an overview of the historical evolution of day tr

Optimizing the host substrate environment for cardiac angiogenesis, arteriogenesis, and myogenesis.

The diseased host milieu, such as endothelial dysfunction (ED), decreased NO bioavailability, and ischemic/inflammatory post-MI environment, hamper the clinical success of existing cardiac regenerative therapies. Area covered: In this article, current strategies including pharmacological and nonpharmacological approaches for improving the diseased host milieu are reviewed. Specifically, the authors provide focus on: i) the mechanism of ED in patients with cardiovascular diseases, ii) the current results of ED improving strategies in pre-clinical and clinical studies, and iii) the use of biomat

Orally Administrable H2 S-Scavenging Metal-Organic Framework Prepared by Co-Flow Microfluidics for Comprehensive Restoration of Intestinal Milieu.

Intestinal milieu disorders are strongly related to the occurrence of inflammatory bowel diseases (IBDs), which results from mucosa destruction, epithelium disruption, and tight junction (TJ) proteins loss. Excess of H2 S in the intestinal milieu produced by the sulfate-reducing bacteria metabolism contributes to development of IBDs via epithelial barrier breakdown. Conventional interventions, such as surgery and anti-inflammatory medications, are considered not completely effective because of frequent recurrence and other complications. Herein, a novel oral delivery system, a hydroxypropyl me

The protective role of the microenvironment in hairy cell leukemia treatment: Facts and perspectives.

Hairy cell leukemia (HCL) is an incurable, rare lymphoproliferative hematological malignancy of mature B cAlthough first line therapy with purine analogues leads to positive results, almost half of HCL patients relapse after 5-10 years, and standard treatment may not be an option due to intolerance or refractoriness. Proliferation and survival of HCL cells is regulated by surrounding accessory cells and soluble signals present in the tumor microenvironment, which actively contributes to disease progression. In vitro studies show that different therapeutic approaches tested in HCL impact the tu

The clinical effectiveness of a multisensory therapy on clients with developmental disability.

Many clients in Hong Kong with developmental disabilities stay in mental hospitals because of mental disorders and behavioural problems. There is a need to identify strategies that promote psychological well-being and reduce problem behaviours in this group of clients. This study evaluates the impact of multisensory therapy on participants' emotional state, level of relaxation, challenging behaviour, stereotypic self-stimulating behaviour (SSB) and adaptive behaviour (AB). Using an experimental design, 89 participants were recruited from a developmental disability unit in a hospital in Hong Ko
