Same-Day Discharge Sleeve Gastrectomy at a Military Treatment Facility: Protocol and Initial Outcomes.

INTRODUCTION: The laparoscopic sleeve gastrectomy (LSG) has become the most common bariatric surgery. Due to its relative safety and low complication rate, it has been more commonly performed as an ambulatory surgery. "Outpatient" or discharge from postanesthesia care unit within 23 h has been well studied in the civilian sector and has been shown to result in reduced costs, hospital-acquired infections, and improved patient satisfaction. Although there are numerous benefits to outpatient LSG, the military health system has been slow to adopt this practice. This study represents the first and

A Case Series of Ocular Syphilis Cases at Military Treatment Facility From 2020 to 2021.

Syphilis is a sexually transmitted infection, caused by the spirochete Treponema pallidum, that can lead to multi-organ involvement. In 2020, over 138,000 cases were reported in the United States equating to a case report of 40.8 per 100,000 people. Ocular syphilis is a rare manifestation and is defined as the clinical presentation of ocular disease in a person with laboratory-confirmed syphilis infection of any stage, with estimated incidence of 0.6-2% of all cases. Syphilis is known as "The Great Imitator," and can present as nearly any form of ocular disease, though the most common manifest

Emergency contraception knowledge, attitudes, practices, and barriers among providers at a military treatment facility.

The safety and efficacy of postcoital administration of oral contraceptives, commonly called "emergency contraception pills" (ECPs), have been well documented; nonetheless, this method of preventing pregnancy is underused in the United States. Through the use of surveys, the current study assessed providers' knowledge, prescribing practices, attitudes, and barriers regarding ECPs at a military treatment facility in the southwest United States. Results showed a lack of knowledge of ECP provision, with 34% believing treatment had to be initiated in 48 or fewer hours, and 60% believing two doses

Outpatient evaluation, recognition, and initial management of pediatric overweight and obesity in U.S. military medical treatment facilities.

BACKGROUND AND PURPOSE: As childhood obesity is a concern in many communities, this study investigated outpatient evaluation and initial management of overweight and obese pediatric patients in U.S. military medical treatment facilities (MTFs). METHODS: Samples of 579 overweight and 341 obese patients (as determined by body mass index [BMI]) aged 3-17 years were drawn from MTFs. All available FY2011 outpatient records were searched for documentation of BMI assessment, overweight/obesity diagnosis, and counseling. Administrative data for these patients were merged to assess coded diagnostic an

Effects of modified foodservice practices in military dining facilities on ad libitum nutritional intake of US army soldiers.

BACKGROUND: Modifying foodservice practices in military dining facilities could influence ad libitum nutritional intake patterns of soldiers. OBJECTIVE: We aimed to determine how changes in foodservice operations consistent with 2005 Dietary Guidelines for Americans affected soldiers' ad libitum nutritional intake in military dining facilities (DFACs). DESIGN: Ten DFACs participated, and the intervention was implemented in five DFACs in an independently sampled, partial crossover design. Nutrient intake of diners was assessed during a test meal using digital photography, and customer satisfa

A Comparison of Combat Casualty Outcomes after Prehospital Versus Military Treatment Facility Airway Management.

BACKGROUND: Airway obstruction is the second leading cause of potentially survivable death on the battlefield. Previous studies demonstrate casualties undergoing airway interventions have worse outcomes when the procedure occurs in the prehospital setting versus the military treatment facility (MTF) setting. We compare outcomes between casualties undergoing airway management in these 2 settings using the Department of Defense Trauma Registry (DODTR). METHODS: This is a secondary analysis of a previously described dataset from the DODTR. We included US military casualties with at least 24 hour

Preventing Surgical and Postpartum Hemorrhage in an Active Duty Patient With an Undiagnosed Coagulopathy at a Military Treatment Facility.

Factor VII (FVII) deficiency is a hereditary bleeding disorder that significantly increases the risk of hemorrhage during the intrapartum and postpartum periods as well as during surgery. Management often requires careful pre- and post-operative planning, a multi-disciplinary approach, and management at a tertiary center. Most cases in the literature utilized recombinant FVII for treatment. We present a case of a young active duty female who had an undiagnosed FVII deficiency that became apparent during her expedited delivery for fetal distress. Our patient was admitted for delivery while unde

Beyond the Leash: Exploring the Public Health Dimension of Facility Dog Handler Experiences in Military Graduate Education.

Facility dogs are increasingly incorporated throughout higher-education institutions as integrative conduits for supporting students by reducing stress, anxiety, and emotional exhaustion. While these programs are designed to enhance student well-being, little is known about the experiences of the handlers who train, care for, and work with these dogs-specifically within military-affiliated programs. The handlers' role necessitates a unique dynamic of caregiving, emotional labor, and professional responsibility and management that can create role conflict and additional stress among those gradu

Musculoskeletal Complaints Among Active Duty Service Members Seeking Treatment at a Navy Military Treatment Facility.

INTRODUCTION: Musculoskeletal injuries (MSIs) pose a significant problem for active duty service members (ADSMs). MSIs may compromise readiness and are one of the main reasons for separation from the Navy and long-term disability. Implementation of optimal treatment strategies rests on understanding the characteristics of MSI complaints in a given population. This study reports on the frequency and nature of MSI complaints of ADSMs seeking care at a military treatment facility (MTF). MATERIALS AND METHODS: As part of a larger quasi-experimental study, data on MSIs from ADSMs reporting to an M

Thinking Outside the Blood Box: Reimagining the Military Treatment Facility as a Power Projection Platform for Blood Logistics.

Future large-scale combat operations threaten to outpace and overwhelm forward blood supply chains, exposing a critical vulnerability in combat casualty care. Although the emergency use of fresh whole blood and "buddy transfusions" drawn from active-duty service members is a proven, common practice in deployed environments, the military currently underutilizes a massive, untapped resource: the healthy, non-mobilized active-duty population stationed in-garrison. This article proposes a conceptual shift to reimagine stateside military medical treatment facilities as medical power projection plat

Head Face and Neck Surgical Workload From a Contemporary Military Role 3 Medical Treatment Facility.

INTRODUCTION: Previous analyses of head, face, and neck (HFN) surgery in the deployed military setting have focused on the treatment of injuries using trauma databases. Little has been written on the burden of disease and the requirement for follow-up care. The aim of this analysis was to provide the most comprehensive overview of surgical workload in a contemporary role 3 MTF to facilitate future planning. METHOD: The operating room database and specialty surgical logbooks from a U.S.-led role 3 MTF in Afghanistan were analyzed over a 5-year period (2016-2020). These were then matched to the

Retrospective Assessment of the Efficacy of Modified Progressive Tinnitus Management Skills Education in a Military Medical Treatment Facility.

INTRODUCTION: The number-one service-connected disability is tinnitus. Tinnitus currently has no cure, but the functional impact of tinnitus has been shown to be mitigated by Progressive Tinnitus Management (PTM), a multi-level management approach. The duration of PTM Level 3 skills education (PTM-SE) and the inclusion of mental health providers have been identified as barriers to implementation of PTM-SE in Department of Defense (DOD) medical treatment facilities. The goal of this study was to determine if a version of PTM-SE modified for use in DOD medical treatment facilities resulted in po

A QuEST for Nursing Clinical Activity Exposure: Comparison of the Military Treatment Facility and a Civilian Level I Trauma Center.

INTRODUCTION: Most military nurses work day-to-day in Military Treatment Facilities (MTFs), caring for a low volume of non-trauma patients until tasked to deploy. Military-civilian partnerships (MCPs) were established to increase opportunities for trauma exposure and high acuity patient care that are often lacking at MTFs but are essential for deployment readiness. In a near-peer conflict, deployed medical teams can expect to hold patients for days before evacuation and must be proficient in prolonged casualty care. Current Air Force nurse readiness checklists do not quantify expeditionary ski

Response to active tuberculosis within a maternal newborn unit-military treatment facility, Alaska, April-August 2025.

Tuberculosis (TB) disease exposure within maternity units is a significant concern due to increased risk of infection among pregnant and post-partum women and their newborns. During April-August 2025, personnel from several military health authorities were deployed to assist a military treatment facility in Alaska after a labor and delivery nurse was diagnosed with pulmonary TB disease. A total of 282 individuals were identified through contact tracing. Staff contacted the affected families, answered questions, and facilitated evaluation and treatment. 263 (92.9%) of the 283 contacts were test

Comparison of Mental Health Visits at a Military Treatment Facility Emergency Department Pre- and Post-COVID-19 Pandemic.

INTRODUCTION: There is a paucity of quantitative research regarding the effect of coronavirus 2019 (COVID-19) on Emergency Department (ED) visits in the United States, and specifically mental health-related ED visits. The small existing body of research describes an overall decline in ED visits worldwide; however, there are anecdotal reports that psychiatric complaints to the ED have increased during the pandemic. The primary objective of this study was to describe the volume of mental health ED visits at a single ED during the COVID-19 pandemic compared to previous years. MATERIALS AND METHO

Qualitative Detection Toward Military and Improvised Explosive Vapors by a Facile TiO2 Nanosheet-Based Chemiresistive Sensor Array.

A facile TiO2 nanosheets-based chemiresistive gas sensor array was prepared to identify 11 kinds of military and improvised explosive vapors at room temperature. The morphology of TiO2 nanosheets was well-controlled by adjusting the concentration of HF applied during the preparation. Owing to the morphology difference, the TiO2 nanosheet-based sensors show different response values toward 11 kinds of explosives, which is the basis of the successful discriminative identification. This method owes lots of advantages over other detection techniques, such as the facile preparation procedure, high
